병원모바일앱 품질요인이 이용자의 지속이용의도에 미치는 영향: 정보시스템성공모형과 기대일치모형의 통합적 접근 (The Effect of Hospital Mobile App Quality Factors on Users ' Continuous Use Intention: An Integrated Approach of Information Systems Success and Expectation-Confirmation Models)

  • 정보통신기술 기반의 '스마트병원'과 '디지털 헬스케어'가 의료분야의 화두가 되면서 병원모바일앱이 주목받고 있지만, 홍보 부족, 불안정한 시스템, 늦은 업데이트 등의 이유로 이용률은 저조한 편이다. 이러한 상황에서 병원모바일앱의 이용률을 높이기 위한 체계적인 연구가 필요하지만, 관련 연구는 다소 부족한 실정이다. 이에, 본 연구에서는 기술적 관점의 정보시스템성공모형과 인지적 관점의 기대일치모형을 통합하여, 병원모바일앱 지속이용의도에 대한 영향 메커니즘을 실증하고자 한다. 이를 위해 병원모바일앱 이용 경험이 있는 국내 성인 181명을 대상으로 온라인 설문조사를 실시하였다. 구조방정식모형 분석 결과, 대부분의 품질요인이 기대일치와 인지된 유용성 및 만족도에 유의미한 영향을 미치는 것으로 나타났으며, 기대일치는 인지된 유용성과 만족도에 유의한 영향을 주는 것으로 나타났다. 또한, 인지된 유용성과 만족도는 병원모바일앱의 지속이용의도에 유의한 영향을 미친다는 사실을 밝혀냈다. 본 연구는 기대일치모형과 정보시스템성공모형을 통합하여 의료분야, 그 가운데에서도 이용률이 저조한 병원모바일앱 사용 맥락에 적용한 연구라는 점에서 의의가 있으며, 연구결과를 통해 병원모바일앱의 이용률 증대 및 효과적·효율적인 서비스 운영을 위한 실무적 시사점을 제시하였다.

구강건조증 환자의 임상적 특징에 관한 연구 (A Study on the Clinical Characteristics of Patients with Dry Mouth)

  • Advances in medical procedures and utilization of medication have resulted in expanding aged population, which leads to increased aged patients with salivary hypofunction and its associated symptoms in dental clinic. The purpose of this study was to investigate clinical characteristics of patients with dry mouth and its correlation with their salivary flow rate. Forty dry mouth patients (7 males, 33 females, mean age 42.0 years) whose flow rate of unstimulated whole saliva was less than 0.15 ml/min were included and their gender- and age-matched controls (7 males, 33 females, mean age 42.9 years) who did not report any complaints, suggestive of salivary gland dysfunction and had the flow rate of greater than 0.20 ml/min were included for comparison. The salivary flow rate was measured in both unstimulated and stimulated conditions. Dry mouth-related clinical information including history, dry mouth associated symptoms, exacerbating and relieving factors, drugs, systemic diseases, and family history was investigated using questionnaires. The differences in distribution of patients and control subjects to each question and their relation to the salivary flow rate were analyzed and we came to following conclusions. 1. There were statistically significant differences in the distribution of patients and controls to the following questions: the period and frequency of suffering from dry mouth; severity of dry feeling during a meal; severity of discomfort during swallowing; necessity of sipping liquids during swallowing dry foods, severity of discomfort in usual life due to dry feeling; self-assessment of residual salivary volume; taking medications. 2. The patients had more stress-related medical histories including indigestion, insomnia, and gastritis compared with controls. The patients took many kinds of medications to control their systemic diseases. 3. There were statistically significant differences in the salivary flow rate between different groups of patients to following questions: severity of dry feeling during a meal; severity of discomfort during swallowing; necessity of sipping liquids during swallowing dry foods. The difference was more significant in the case of stimulated salivary flow rate rather than unstimulated one. 4. The salivary flow rate of patients taking medications was significantly less than that of patients who did not take medications. The difference was more significant in the case of stimulated salivary flow rate rather than unstimulated one.

Burden Assessment of Thyroid cancer in Iran from 1990 to 2010: Lessons Obtained from Global Burden of Disease Report 2010

  • Background: Thyroid tumors are generally regarded as rare malignancies. Nowadays, however, their global incidence is growing continuously partially due to western life style and utilization of more sensitive methods of early detection. It is approximately three times more prevalent in females than in males. Most cases of thyroid cancer are asymptomatic nodules or just have local cervical symptoms or adenopathy in early stages. Materials and Methods: The Global Burden of Diseases report 2010 study (released 3/2013) profited from 100 collaborators worldwide and used a vast network of data on health outcomes, vital registries, and population surveys. It shared many of the Global Burden of Diseases 1990 principal databases such as all available data on injuries, diseases, risk factors, as well as comparable metrics, and used different scientific approved methods to estimate important health status data like: death rate, life expectancy, healthy adjusted life expectancy, disability-adjusted life years (DALY), years of living lost due to premature death and years of life with disabilities. Results: DALY as thyroid cancer burden per 100,000 Iranian populations had increased by about 14% during 1990 to 2010 in all ages; from 6.1 (95% UI 4.2-9.74) years in 1990 to 6.95 (95% UI 5.06-9.18) years in 2010 in both sex. The 2010 peak age-group was estimated at 45-49 years in males and 40-45 years in females.

일부 급성질환의 지역간 입원의료이용 변이에 관한 연구 -위장질환과 충수염질환을 중심으로- (A Study on Small Area Variations of Hospital Services Utilization in Some Acute diseases -Focused on Gastric Diseases and Acute Appendicitis)

  • 본 연구는 2008년 환자조사 자료를 이용하여 위장질환과 충수염질환을 대상으로 지역간 입원의료이용의 변이정도와 양상을 파악하고, 변이 발생에 영향을 미치는 관련 요인을 분석한 후, 이를 기초로 변이를 최소화하여 의료 이용을 적정화 시킬 수 있는 방안을 모색하기 위해 수행하였다. Extremal Quotient(EQ)와 변이계수(CV) 지표를 사용하여 변이수준을 파악하였고, 다중회귀분석을 통해 입원의료이용의 차이에 영향을 미치는 주요 요인들을 분석하였다. 주요 연구결과는 다음과 같다. 첫째, 위장질환의 소규모 지역별 성 연령 표준화에 따른 입원율의 경우 EQ 15.1, CV 0.4의 변이를 보였으며, 특히 농어촌 지역에서 변이 10대 상위군이 높은 분포를 나타냈다. 또한, 인구만명당 병상수, 의사수, 특수의료장비수에 따라 입원의료이용량이 통계적으로 유의한 양의 관계를 보였다. 둘째, 충수염질환에서는 EQ 12.41, CV 0.4를 나타냈고, 회귀분석을 통해 입원의료이용의 차이에 영향을 미치는 요인을 분석한 결과 통계적으로 유의하지 않았다. 본 연구 결과의 정책적인 함의로는 급성질환자의 입원의료이용의 형평성을 제고할 수 있도록 지역별 병상의 적정한 공급 관리와 효율적인 자원배분 및 의료서비스의 질적 차이를 감소시킬 수 있는 다각적인 정책 방안이 추진되어야 함을 시사해 준다.

Locally Advanced Breast Cancer in Jamaica: Prevalence, Disease Characteristics and Response to Preoperative Therapy

  • Breast cancer is the most common cancer in Jamaican women. Locally advanced breast cancer (LABC) is associated with aggressive biology and poor prognosis, and has a predilection for African-American women. In this retrospective review, we assessed the prevalence of LABC as a breast cancer presentation in a population of mainly Afro-centric ethnicity, and determined disease characteristics and response to pre-operative chemotherapy. LABC was prevalent (20%), and had a low pathological response rate to pre-operative chemotherapy, with a high risk of disease recurrence. Increased utilization of breast cancer screening may help detect cancer at less advanced stages, and optimizing pre-operative chemotherapy is recommended to improve response rates and ultimately survival.

  • Machine learning has been applied to medical imaging and has shown an excellent recognition rate. Recently, there has been much interest in preventive medicine. If data are accessible, machine learning packages can be used easily in digital healthcare fields. However, it is necessary to prepare the data in advance, and model evaluation and tuning are required to construct a reliable model. On average, these processes take more than 80% of the total effort required. In this study, we describe the basic concepts of machine learning, pre-processing and visualization of datasets, feature engineering for reliable models, model evaluation and tuning, and the latest trends in popular machine learning frameworks. Finally, we survey a explainable machine learning analysis tool and will discuss the future direction of machine learning.

Ginsenoside Rb$_1$ Reduces Spontaneous Bursting Activity in Thalamocortical Slices of the Rat

  • Spontaneous bursting activity was studied in rat thalamocortical slices using extracellular field potential recording to test the potential utilization of ginsenoside Rb$_1$ in controlling overactivated neural systems. In order to induce bursting activity, slices were perfused with Mg$\^$2+/-free artificial cerebrospinal fluid (ACSF). Two major types of spontaneous bursting activity, simple thalamocortical burst complexes (sTBCs) and complex thalamocortical burst complexes (cTBCs), were recorded in Mg$\^$2+/ -free ACSF. Ginsenoside Rb$_1$ selectively suppressed cTBCs. Duration and occurrence rate of cTBCs were reduced by 87.3${\pm}$10.2% and 85.3${\pm}$ 14.7% in the presence of 90 ${\mu}$M ginsenoside Rb$_1$ respectively, while amplitude and intraburst frequency were slightly changed by ginsenoside Rb$_1$. In contrast, ginsenoside Rb$_1$was much less effective in reducing duration and occurrence rate of sTBCs. We also tested effects of ginsenoside Rb$_1$ on bursting activity in the presence of a GABA$\sub$A/ receptor antagonist, bicuculline methiodide (BMI). Ginsenoside Rb$_1$ had no effect in suppressing BMI-induced bursting activities. These results suggest that ginsenoside Rbi may be useful in controlling seizure-like bursting activity under pathological conditions.

종양 영상을 위한 PET 방사성의약품 (PET Radiopharmaceuticals for Tumor Imaging)

  • Early and accurate diagnosis of tumors using positron omission tomography (PET) has been the focus of considerable interest due to its high metastasis and mortality rates at late detection. PET radiopharmaceuticals-which exhibit a high tumor-to-background uptake ratio, and appropriate metabolic characteristics, and pharmacokinetics-are attractive tools for tumor imaging. Tumor imaging by these radiopharmaceuticals are based on metabolic and receptor imaging. The former is based on accelerated metabolism in tumor tissue compared to normal tissue and the rate roughly corresponding to the rate of growth of tumors. Radiopharmaceuticals for this purpose include radiolabeled sugars, amino acids, and nucleosides which detect increased glucose utilization, protein synthesis, and DNA synthesis, respectively. Tumor receptor imaging is based on the proliferation of tumor cells regulated by many hormones and growth factors, which bind to the corresponding receptors and exhibit the biological responses Radiopharmaceuticals used to image the tumor receptor systems may be ligands for the specific receptors and antibodies for the growth factor receptors. Some antitumor agents have been labeled with radionuclides and used to study in vivo biodistribution and pharmacokinetics in humans. This overview describes typical PET radiopharmaceuticals used for tumor imaging based on their uptake mechanisms.

민간의료보험 시장 규모 변동에 영향을 미치는 개인 특성 (Individual Characteristics Associated with the Market Size Change of Private Health Insurance Premium in Korea)

  • This study examined market size of private health insurance premium and individual characteristics associated with the market size change in Korea, using wave 1 (2008) and wave 2 (2009) of Korea Health Panel. The market size was 24.4 trillion Korean won in 2008 and 26.9 trillion in 2009. The increase rate of private health insurance premium among those who were the elderly, single, or the poor was higher than that among their counterpart respectively. Health status and utilization were insignificant in determining the increase rate of private health insurance premium. These findings were more obvious among the uninsured in 2008 than among the insured in 2008. The increase of private health insurance premium in Korea imply the increase of willingness-to-pay for health risk through private sector. The authors suggest policy intervention for accessability to health care for the underprivileged and weak through enlargement of Korean social health insurance benefit.

일 종합병원 병동 간호단위의 간호효율성 평가-자료포락분석의 적용 (Evaluation of the Efficiency of General Nursing Units using Data Envelopment Analysis (DEA))

  • Purpose: The aim of this study was to evaluate the efficiency of nursing units in a hospital using DEA. Methods: Data were collected using the medical information system of a general hospital in a city. Input variables were number of nurses, number of nurse-aides, number of beds, and overhead costs. Output variables included number of admitted patients, rate of bed utilization, satisfaction of discharged patients, and prevention rate of safety accidents and sores. EMS Window version 3.1 was used to measure the efficiency score and descriptive statistics were applied to analyze the general characteristics of variables. Results: The average efficiency score of 18 general nursing units was approximately .99. Nine nursing units had a 1.00 efficiency score. Conclusion: Our findings reveal that nursing units operated very efficiently. To increase efficiency of inefficient nursing units, we recommend results of the DEA slack analysis as a benchmark of the most efficient nursing unit.
