반사 지점의 개수와 조건이 맥파에 미치는 영향 (Effect of Number and Condition of Reflection Site on Pulse Wave)

  • To develop cardiovascular simulator capable of implementing pulse waves similar to the human body, accurate information about reflection wave is required. However, the conclusion is still not clear and various discussions are underway. In this study, the pulse wave velocity of the tube used in the experiment was first determined by measuring the pressure waves at two points in a single tube system with the experimental device to implement the pulse wave transmission of blood vessels, and the superposition time and characteristics of the reflection wave were confirmed. After that, an air chamber was set at the reflection site, and the effect of the change of air volume on the reflection wave was investigated. Finally, the effect of the number of branches connected to a single tube on the reflection wave was investigated. The superposition time of the reflection wave can be controlled by the air volume of the air chamber, and the magnitude of the reflection wave is influenced by the number of reflection sites that generate the reflection wave. The results of this study may be of practical assistance to simulator researchers who want to implement pulse wave similar to clinical data. It is expected that the more results similar to clinical are provided, the greater the scope of the simulator's contribution to clinical cardiovascular research.

교육용 의료방사선 시뮬레이터 시스템 개발 및 연구 모델 제안 (Development of a Medical Radiation Simulator System for Education and Proposal of a Research Model)

  • 첨단 기술의 발전으로 디지털 방사선영상 장비들이 많이 개발되어 정확한 진단과 치료에 많은 도움을 받고 있으며, 이를 안전하고 효과적으로 사용하기 위하여 전문적인 지식을 습득한 인력양성이 매우 중요하다. 진단용 X선 장비를 활용한 촬영 실습 교육에 있어서 학생들은 방사선 피폭의 위험성에 노출되어 있고, 일부 교육기관에서는 원자력안전법에 따른 관리의 어려움으로 X선 장비를 사용하지 않는 경우도 발생하고 있다. 이에 대한 해결책으로, 본 연구에서는 비전 센서와 자체 개발한 소프트웨어를 활용하여 방사선이 발생하지 않는 교육용 의료방사선 시뮬레이터를 개발하였고, 이를 통해 교육기관은 법에 따른 행정 이행 사항의 부담을 줄일 수 있고, 학생들은 방사선 피폭이 없는 건강한 실습환경에서 높은 수준의 교육 효과를 얻을 수 있게 하였다.

  • Robotic devices have been widely used in many medical applications due to their accuracy and programming ability. One of the applications is a virtual reality medical simulator, which trains medical personnel in a computer generated environment. In this paper, we are going to present an application, an epidural anesthesia trainer. Because performing epidural injections is a delicate task, it demands a high level of skill and precision from the physician. This trainer uses a robotic device and computer controlled solenoid valve to recreate interaction forces between the needle and the various layers of tissues around the spinal cord. The robotic device is responsible for generation of interaction forces in real time and can be used to be haptic guidance that allows the user to follow a previous recorded expert procedure and feel the encountered forces.

  • 사람들의 건강상태를 모니터링하기 위해 뇌파신호에 관련된 많은 연구가 진행되고 있다. 특히, 병원에 상주하는 환자들은 뇌경색, 간질 등과 같은 위급상황에 대비하여 모니터링할 필요가 있다. 뇌파 네트워크 서비스의 안정성을 요구하는 효율적인 네트워크 토폴로지가 필요하며 본 실험을 위해 OPNet 시뮬레이터를 활용하였다. 따라서, 환자들의 뇌파는 네트워크에 있는 센서장치로부터 읽어들인다. 네트워크의 성능을 비교하기위해 두 가지의 센서 네트워크 토폴로지를 제안하고 시뮬레이션하였다. 하나는 지역화된 네트워크이고 다른 하나는 비지역화된 네트워크이다. 옵넷시뮬레이터를 이용하여 시뮬레이션을 수행하였다.

Evaluation of alveolar crest bone loss via premolar bitewing radiographs: presentation of a new method

  • Purpose: This study aimed to assess the interdental bone level in premolar bitewing radiographs while retracting the cheeks. Methods: Seventy-two horizontal bone defects were created on dried mandibles and maxillae. The distance from the bone level to the cement-enamel junction of premolars was detected by a modified digital caliper (considered the gold standard). The reliability of all radiographs was assessed by intraclass correlation coefficient (ICC), and the validity was compared to the gold standard using the analysis of variance test. P-values less than 0.05 were considered statistically significant. Results: This study showed that the reliability of radiographs without a cheek simulator and with 0.16 second exposure time was significantly higher than that of the two other groups (ICC=0.96 compared to 0.93 and 0.88, respectively). The results from the radiographs without a cheek simulator and with 0.16 second exposure time were more similar to the gold standard measures than those of the two other groups, although the difference was not statistically significant. Conclusions: Retracting the buccal soft tissue plays an important role in increasing the accuracy of radiographs in detecting the interdental alveolar bone level and produces more accurate results than increasing the exposure time, although it does not have a significant role in reliability of results.

  • 대부분의 포탈영상이 그에 상응하는 시뮬레이터 영상을 참조 영상으로 하여 방사선치료 계획을 수행하고 있다. 이것은 선형가속기의 높은 에너지 X선으로서 얻어지는 포탈 영상의 물리적 특성 때문에, 구조적으로 대단히 불량한 포탈 영상의 개선과 잃어버린 영상 정보의 복원에 시뮬레이터 영상 자체에서의 영상정보를 이용할 수 있다는 가능성을 보여주고 있는 것이다. 본 연구에서는 최대 퍼지 엔트로피를 평가함수로 이용한 유전자 알고리듬을 사용하여 영상에서의 퍼지 영역을 자동적으로 결정하고, 그것을 멤버쉽 함수에서 적용하여 퍼지영상 개선 기법으로서 포탈 영상과 시뮬레이터 영상을 개선한 후, 잡음이 중첩된 시뮬레이터 영상들로서 연관기억장치를 학습시키고 여기에 퍼지 방법으로 개선시킨 포탈 영상을 입력하여 기존의 영상기법으로 처리된 영상보다 좋은 포탈 영상을 얻을 수 있었다.

Validation of a Cognitive Task Simulation and Rehearsal Tool for Open Carpal Tunnel Release

  • Background Carpal tunnel release is one of the most common surgical procedures performed by hand surgeons. The authors created a surgical simulation of open carpal tunnel release utilizing a mobile and rehearsal platform app. This study was performed in order to validate the simulator as an effective training platform for carpal tunnel release. Methods The simulator was evaluated using a number of metrics: construct validity (the ability to identify variability in skill levels), face validity (the perceived ability of the simulator to teach the intended material), content validity (that the simulator was an accurate representation of the intended operation), and acceptability validity (willingness of the desired user group to adopt this method of training). Novices and experts were recruited. Each group was tested, and all participants were assigned an objective score, which served as construct validation. A Likert-scale questionnaire was administered to gauge face, content, and acceptability validity. Results Twenty novices and 10 experts were recruited for this study. The objective performance scores from the expert group were significantly higher than those of the novice group, with surgeons scoring a median of 74% and medical students scoring a median of 45%. The questionnaire responses indicated face, content, and acceptability validation. Conclusions This mobile-based surgical simulation platform provides step-by-step instruction for a variety of surgical procedures. The findings of this study help to demonstrate its utility as a learning tool, as we confirmed construct, face, content, and acceptability validity for carpal tunnel release. This easy-to-use educational tool may help bring surgical education to a new- and highly mobile-level.

가상 운전 시뮬레이터를 이용한 족동 조향 시스템의 운전 성능 평가 (Driving Performance Evaluation Using Foot Operated Steering System in the Virtual Driving Simulator)

  • The aim of this study was to evaluate driving performance of normal subjects for controlling the steering wheel by using foot operated steering devices in the driving simulator. Many people with complete bilateral loss or loss of use of upper limbs but with normal lower limbs are frequently left without use and/ or control of their hands, arms, or the upper extremities of their bodies. As a result, persons disabled in this manner have problems in operation an automobile because they cannot grasp and manipulate a conventional steering wheel. Therefore, if foot operated steering devices are used for controlling the vehicle on in people with disabilities, the disabled people could improve their community mobility by driving a car safely. Ten normal subjects were involved in this research to evaluate steering performance by using three types of steering devices(conventional steering wheel, pedal type foot steering, circular type foot steering) in driving simulator. STISim Drive 3 program was used for testing the driving performance in two road scenarios: straight road and curved road at low and high speed of vehicle (40 km/h and 80 km/h). This study used two-way ANOVA to compare the influences of two factors(type of foot steering device and road scenario) in the three dependent variables of steering performance(standard deviation of lateral position, the lateral position of vehicle and the number of line crossing). The average values of the three dependent variables(standard deviation of lateral position, lateral position and the number of line crossing) of driving performance were significantly smaller for conventional steering wheel or pedal type foot steering than circular type foot steering.

환자시뮬레이터활용교육에서의 자기주도적 학습능력과 집단효능감의 변화 (Effect of Human Patient Simulator-based Education on Self-directed Learning and Collective Efficacy)

  • 본 연구는 환자시뮬레이터활용교육에 따른 간호대생의 자기주도적 학습능력과 집단효능감 변화를 파악하고자 시도한 단일군 전후 실험연구이다. 대상자는 환자시뮬레이터활용교육을 받은 적이 없는 2학년 전체 학생 중 통합실습과목에 등록하고 연구에 동의한 92명이었다. 환자시뮬레이터활용교육은 팀활동학습, 술기훈련, 환자시뮬레이터를 이용한 팀시뮬레이션, 디브리핑으로 구성되어 12주간 운영되었다. 사전 자료수집은 2010년 8월 23일에서 27일까지 이루어졌고 사후 자료수집은 12주간의 환자시뮬레이터수업을 수행한 후 2010년 11월 29일에서 12월 3일 까지 시행되었다. 환자시뮬레이터활용교육 전 후로 자기주도학습능력의 증가(t=4.24, p=0.000)는 통계적으로 유의미하였으나 집단효능감의 증가는 유의한 효과를 나타내지 못하였다. 이러한 결과에 근거하여 환자시뮬레이터활용교육은 간호대생의 자기주도적 학습능력 향상에 영향을 미치는 교육전략임을 알 수 있다.

p38 MAPK을 이용한 항염증 효능 규명 프로토타입 개발 (Development of Prototype for Screening Anti-Inflammation Effects concerning p38 MAPK Signal Pathway)

  • Objectives : The purpose of this study was to develop a simulator which can analyze the anti-inflammatory effects of medical herbs based on e-cell concerning p38 MAPK signal pathway. Methods : We collected data concerning medical herbs with anti-inflammatory effects and the active compounds to provide as a fundamental databse and to validate the newly developed algorithm. At this time, we used the target database as pubmed and gathered the data by data mining tool, pathway studio. Also we have developed the web-based search system for confirming database related to anti-inflammation. We researched the mechanism of actions of proteins in p38 MAPK signal pathway when active compound has been inserted into the network. We reduced total network into TAK-MKK3-p38 and made the two types of mathematical model about active compounds' interaction. Results & Conclusion : We constructed the database which have 69 cases of medical herbs, 71 cases of active compounds, about 8,000 cases of URL(Uniform Resource Locator) related to papers and reports. We designed the ordinary differential equations for response of TAK, MKK3, p38 in e-cell's cytosol and nucleus. We used this formular as measure whether an active compound of medicinal plants which is inputted by an user would have an anti-inflammation effects. We developed the visualization program which could show the change of concentration over time.
