The Knowledge, Need, and Usage of Medical Terminology in Clinical Nursing Practice (임상 간호 실무에서의 기초 의학용어에 대한 지식, 필요도 및 활용 정도)

  • Purpose: The purpose of this study was to identify levels of knowledge, need, and usage of medical terminology in clinical nurses. Methods: Participants for this study consisted of 127 clinical nurses who work at one general hospital located in Seoul in Korea. The data was collected from April 1st to the 22nd in 2014. Participants responded to self-administered questionnaires about their knowledge, need, and usage of English medical terminology, Chinese character medical terminology, and Korean medical terminology. The data was analyzed using an independent t-test, one-way ANOVA, Duncan's test as post hoc and Pearson's correlation. Results: The mean scores of knowledge and needs with English medical terminology were high. English medical terminology and Chinese character medical terminology were more useful tools for disease than Korean medical terminology. Knowledge of medical terminology was positively correlated between needs and correct use with the clinical nursing situation. Conclusion: Based on these results, it can be concluded that medical terminology education for bio-nursing curriculum is important for clinical nurses and standardization of essential medical terminology is needed.

Development of Medical Service Scale (의료서비스 척도 개발)

  • Objective : The purpose of this study is to establish a concept called as 'medical service' newly and to develop an reliable and valid scale that measures medical service. Methods : In order to accomplish these objectives, both conceptual study and empirical study were performed. In a conceptual study, 5 core factors(doctor, medical team, medical facilities, medical process, and hospital) and 24 sub factors were extracted with assistant by 7 experts, based on antecedent studies about components of medical service. In an empirical study, data was taken from 244 adults living in Seoul and Daejeon with the conceptually founded medical service scale. Results : Results of empirical study showed reliability and validity of the medical service scale. Internal consistency coefficient showed high score of .97 and, in accordance with the conceptual study, "medical treatment factor", "appearance factor", "personal factor", "trust factor" and "dissatisfaction factor" were extracted from the data as the five main factors. Conclusions : Finally, limitations of this study and directions for further studies are discussed. Acknowledgement : This work was supported by the Daejeon University Research Grant.

CLINICAL STUDIES OF LAG -SCREW OSTEOSYNTHESIS IN CONDYLAR FRACTURES OF THE MANDIBLE (하악 과두 골절 환자에서 Lag-screw를 이용한 관혈적 정복 후 임상적 연구)

  • Purpose : This study evaluated clinical and radiographic results after open reduction and lag-screw fixation of mandibular condyle fractures. Patients and Methods : 26 patients had been treated by lag-screw fixation for madibular condyle fracures via submandibular approach and follow up for over 6 months. The lag-screw used in this study was designed by Eckelt. Results : Radiographically, resorption of the condyle head found in 4 cases (15%) and minimal displacement of the condyle head was found in 3 cases (12%). Post-operative infection were not found in all cases. Clinically, All patiens had a stable occlusion and normal mouth opening (over 40mm). TMD problems with crepitus or some discomfort encountered in 4 patients but tolerable to the patients. Conclusion : Lag screw fixation can be a good option especially high level condylar fractures, however this procedure must be used in cautiously because of slipping of the fragments and possible to resorption of the fragments.

Factors Related to Unmet Medical Care Needs in Adult Diabetes Patients in Korea (우리나라 성인 당뇨병 환자의 미충족의료 관련 요인)

  • Background: This study was conducted to investigate factors related to unmet medical needs of medical care in adult diabetes patients and to suggest factors related to unmet medical in Korea. Methods: This study used data from the Korea National Health and Nutrition Examination Survey (KNHNES) 2014-2017. The subjects of the study were conducted on patients with unmet medical needs experience among the patients and analyzed using the IBM SPSS ver. 25.0 program (IBM Corp., Armonk, NY, USA). Results: Overall, 10.9% of patients had unmet medical needs. Being female, less educated, and lower medical aid were related to unmet medical needs. And subjects with poor subjective health and higher stress level were more likely to report unmet medical needs. Conclusion: Although comprehensive health insurance coverage, 10.9% of people with diabetes experienced unmet healthcare needs. The results of this study suggest that socioeconomic factors such as low education and medical aid were associated with unmet medical needs.

Comparative Study of Dermatoscopic and Histopathologic Results in Facial Basal Cell Carcinoma and Melanocytic Nevi

  • Background: Dermatoscopy can be applied to diagnose pigmented skin lesions. The aim of the present study was to compare dermatoscopic and histopathologic results in basal cell carcinoma (BCC) and melanocytic nevus of theface. Materials and Methods: In an analytical-descriptive study, 61 patients suspected of BCC or melanocytic nevi of face were randomly selected. The skin lesions of patients were evaluated with dermatoscopic method from February 2012 to February 2014 and results were compared with pathological features of samples. Results: In this study, mean age of patients was $49.5{\pm}18.9$. Some 25 (41%) were men and 36 (59%) were women. In 27 cases (44.3%) there was diagnosis of melanocytic nevus, in 28 cases (45.9%) diagnosis of BCC, and in 3 cases (4.9%) there was mixed diagnosis. The relationship between patients' gender and dermatoscopic diagnosis of the patients was statistically significant (P=0.001). For BCC the sensitivity and specificity of dermatoscopic method were 100% and 97% respectively and for melanocytic nevi 96.4% and 97%. Conclusions: Dermatoscopic study not only can be helpful in improving clinical diagnosis while guiding missed malignant lesions to pathologic evaluations, but also could be useful in evaluating further suspicious or recurrent cases.

Educational Program Evaluation System in a Medical School (일개 의과대학 교육프로그램 평가체제에 대한 연구)

  • A systematic educational program evaluation system for continuous quality improvement in undergraduate medical education is essential. Monitoring and evaluation (M&E) are two distinct but complementary processes referred to in an evaluation system that emphasizes formative purpose. Monitoring involves regular data collection for tracking process and results, while evaluation requires periodic judgment for improvement. We have recently completed implementing an educational evaluation using the M&E concept in a medical school. The evaluation system consists of two loops, one at the lesson/course level and the other at the phase/graduation level. We conducted evaluation activities in four stages: planning, monitoring, evaluation, and improvement. In the planning phase, we clarified the purpose of evaluation, formulated a plan to engage stakeholders, determined evaluation criteria and indicators, and developed an evaluation plan. Next, during the monitoring phase, we developed evaluation instruments and methods and then collected data. In the evaluation phase, we analyzed results and evaluated the criteria of the two loops. Finally, we reviewed the evaluation results with stakeholders to make improvements. We have recognized several problems including excessive burden, lack of expertise, insufficient consideration of stakeholders' evaluation questions, and inefficient data collection. We need to share the value of evaluation and build a system gradually.

A study on the database structure of medical records - Focusing on Yakazudōmei's medical records - (의안(醫案)의 데이터베이스 구조화 연구 - 시수도명의 의안을 중심으로 -)

  • Objectives : The contents of the literature associated with the medical records were entered into the database. We want to find the structure and search methods for efficient utilization of the database. Methods : The contents were entered into the database using the 'Access 2014 of the MS'. The Query Sentences were created and utilized for a search. Results : We could find information about the prescriptions, medical records and patients by the herbs and symptom combinations using the single table named 'Integrated Knowledge' and queries. Integrated Knowledge is a table that gathered patient information, prescription information and symptom information together. Conclusions : If you store patient, prescription and symptom information on a single table, you could search and use the results by various combinations of the various elements included in the table. These results could help curing patients on the basis of evidence-based treatment at the clinics.

Effects of Oriental Medical Treatment on ADHD - A retrospective clinical survey - (ADHD에 대한 한방치료의 효과에 대한 후향적 관찰 연구)

  • Objectives: This case report aimed to evaluate whether oriental medical treatment was effective on ADHD through a records review of a local oriental medical clinic. Methods: We reviewed clinical charts of a local oriental medical clinic between December 2007 and September 2010 to select ADHD patients. Among those patients, for comparison before and after treatment, we selected cases which had at least twice Stroop test results and 2 months of treatment. Baseline, 3-month and 6-month Stroop test results were compared. Oriental medical treatment consisted of 2 months' herbal medicine, periodic acupuncture and exercise. Results: 1. 3-month data showed that subjects were 12 and improved word score and word-color score of Stroop test. 2. 6-month data showed that subjects were 20 and improved word score, color score, and word-color score of Stroop test. Conclusions: 1. Relatively short term herbal medication and exercise are effective on the word score and word-color score of the Stroop test. 2. After short term herbal medication, continuous exercise maintained the effects of short term oriental medical treatment. 3. We need to consider the balance of left and right as an important point of exercise, but further study is needed.

A Study on the Analysis of Korean Medical Services using Latent Dirichlet Allocation Topic Modeling : Focusing on online reviews by medical consumers (Latent Dirichlet Allocation 토픽모델링을 이용한 한방 의료 서비스 분석에 관한 연구 : 의료 소비자의 온라인 리뷰를 중심으로)

  • Objective : This study aims to understand the consumer's needs for Korean medicine medical service using online review analysis of medical consumers. Methods : We analyzed the purpose and satisfaction factors of medical service use using LDA (Latent Dirichlet Allocation) topic modeling. The data used in the study was 120,727 screened reviews written by medical consumers registered on Naver. The analyzed results were compared with the "2020 Korean Medicine Utilization Survey". Results : From 2018 to 2021, the five most frequently used terms were "kindness", "treatment", "doctor", "Korean medicine", and "acupuncture". The main purpose of visiting Korean medicine medical clinic and hospital was to treat "traffic accidents" in 2018, "waist(back) pain" in 2019, "musculoskeletal pain" in 2020 & 2021. Based on the rating, reviewers were satisfied with "explanation of treatment" and "treatment attitude", and dissatisfied with "accessibility to the institution". Conclusion : We concluded that the main purpose of use of Korean medicine institution was to treat musculoskeletal disorders. Based on the results of this study, it is expected that it will be used to improve Korean medicine medical service in the future.
