  • Background: Liu-Yuan-Lei(陸淵雷) said that a medical record is both the marks of treatments and arts made by a excellent practitioner and the essence of TCM(Traditional Chinese Medicine). Jiang-Guan(江瓘) also said that reading medical records is one of the best way to develop one’s abilities If curing a disease without perfect clinical practice. Objectives: study on the special treatment about metrostaxis(崩漏) based on the Yi-Da-Gan(易大艮)’s medical records. and study on the differentiation of abnormal symptoms and signs about cold syndrome with pesudo-heat(眞寒假熱) based on the Yu-Chang(喩昌)'s medical records. Methods: First, read and study the medical records on metrostaxis(崩漏) of that made a profound study by Yi-Da-Gan(易大艮) and cold syndrome with pesudo-heat(眞寒假熱) of that made a profound study by Yu-Chang(喩昌). The next, write a paper on results and conclusions. Results and Conclusions: First, Yi-Da-Gan(易大艮) insist that must control the Qi under the blood disease conditions, taking the case of metrostaxis(崩漏). Secondly, we must study more on estimating the changing condition of Qi and the blood as time goes by, also study on the pulse and pulse condition in the four seasons(四時脈). Thirdly, Yu-Chang(喩昌) insist that be more careful in differentiation of symptoms and signs, taking the case of cold syndrome with pesudo-heat(眞寒假熱). Fourthly, Yu-Chang(喩昌) give an example that in condition of cold syndrome with pesudo-heat(眞寒假熱), sometimes, the pulse and pulse condition can be strong.

전문간호사가 주도하는 급성대동맥증후군 환자의 병원간 다이렉트 전원(direct interhospital transfer)의 임상 결과 (Clinical Outcomes of Direct Interhospital Transfers of Patients with Acute Aortic Syndrome Led by Advanced Practice Nurses)

  • Purpose : This study aimed to evaluate the clinical outcomes of direct interhospital transfers (IHTs) of patients with acute aortic syndrome (AAS) led by advanced practice nurses (APNs). Methods : From September 2014 to June 2017, the study retrospectively investigated 183 patients with AAS who were transferred to a high-volume tertiary hospital. Results : One hundred forty-eight (81%) patients were admitted through direct IHTs, and 35 (19%) patients were admitted through non-direct IHTs. The direct IHT group had a significantly shorter time from symptom onset to hospital arrival than the non-direct IHT group (11.4 vs. 32.1 h, p=.043). There were no significant differences in other clinical outcomes, such as peri-transfer status, mortality, hospital length of stay, and readmission, between the two groups. In the direct IHT group, 55% of transfers were led by APNs. There was no significant difference in outcomes between APN- and physician-led transfers. Conclusions : Implementation of direct IHTs markedly shortened the time from symptom onset to hospital arrival in patients with AAS. Finally, direct IHTs can potentially improve the outcomes of patients with AAS, a condition with time-dependent mortality and morbidity. In addition, APNs can effectively lead the direct IHT of patients with AAS.

윤동리(尹東里)의 가계(家系)와 『초창결(草窓訣)』 중(中) 「운기연론(運氣衍論)」에 관(關)한 연구(硏究) (A Study on Family line of Yoondongri(尹東里) and Ungiyeonron(運氣衍論) in Chochangkyeul(草窓訣))

  • Objectives : Yoondongri is a famous Korean medical doctor in the 18th century. He wrote Chochangkyeul, a special medical book which records his treatment based on the theory of Ungi. Nevertheless, his life and his book have not received due research it deserves. Methods : The paper studied the family tree as revealed in the Papyeong Yoon clan and Chochangkyeul. Further study was done on Yoon's medical theories and clinical features as discovered in Chochangkyeul and Ungiyeonron. Results & Conclusions : Very little is known about Yoondongri other than that he is the 27th generation from the progenitor of the Yoon clan of Papyeong Yoon clan, and that he is the great grandchild of Choongheon-gong Yoon Jeon. One of the important elements of his Ungi theory is Owoon Habgi, which illustrates the integration of Gabsin, Muui, Byeonggye, Gyungjeong, and Yimgi, thereby forming one Ilki. Ilki signifies that mechanisms of disease are inter-connected and one in essence. As the core of his clinical practice, he explains the 25 different types of diseases that can afflict those who are born in the years Gabsin, Muui, Byeongye, Gyungjeong, and Yimgi when they are met with their corresponding year. Yoon described for each script the mechanism of disease, symptom, treatment, and prescription, all of which showcases his status as a great clinical practitioner. His application of Ungi theory is uncomplicated and accessible. His method was also highly effective, the likes of which were unseen even in China and Japan. His prescriptions are mainly based on the Ungi treatment found in Jinmutaek's Saminang, and on Donguibogam.

신장도(腎臟圖)의 형태요소와 그 변이(變異)에 대한 연구 (Factors of Variation in Diagrams and Location of Kidney)

  • This study examines the variation in diagrams of the kidney, and the effects on location of the Life Gate as a result. My study analyzes diagrams of kidney form and location, such as appearance, the vertebral spine, ShenXi, and "Life Gate", and also explores the causes for such variations. The kidney is commonly described as having the shape of a bean, which throughout medical history has showed almost no variation. The spine was initially described with a total of 21 vertebrae, and later with a range of 19-25 vertebrae. Regarding the height of kidney in relationship to the spine, it was initially described as beginning at the 14th vertebrae, and later changed to the 15th or 17th vertebrae. However, there have been no changes in the perception of the height of kidney. Initially, the location of the Life Gate could not be identified. Three different suggestions of its location were found throughout historical literature, including: (1) at or within the right kidney; (2) between two kidneys; and (3) between 14th-15th spine. There were also variations noted in the process of copying diagrams of kidney in many books not only because the work of copying was not precise, but also because each medical practitioner had a different interpretation of the kidney's form and function in Traditional Korean and Chinese Medicine. It appears that some practitioners may have been influenced by their understandings of theory of Life Gate.

의약분업 전후 의원의 건강보험 진료비 분포변화 및 결정요인분석 (Distributional changes in Physicians' Medical Care Expenses from the National Health Insurance and its Determinants After the Separation of Prescription and Dispensing)

  • The National Health Insurance Expenditure has been increased rapidly since the introduction of the separation of prescription and dispensing in 2000, and this trend of rapid growth in overall spendings rate has been observed predominantly among medical practitioners. This study was conducted to investigate the growth rate and distributional changes in private medical practitioners' expenses from 1999 to 2002 and its determinants using the National Health Insurance claims data. The total increasing rate of all medical practitioners' expenditure paid by the National Health Insurance between 1999 and 2002 was $41.71\%$, which exceeding that of general hospitals by $20\%$p. But the income distribution among each practitioner was improved as the changes in Gini coefficient(from 0.40 to 0.38) and decile distribution ratio(from 0.25 to 0.29) during the same period showed. However, this improvement in distributional patterns is not enough since even in 2002 it turned out that the highest $10\%$ income group earned 33times more than the lowest $10\%$ income group did. Also, higher Gini coefficient was observed in larger cities and some department like plastic surgery, obstetrics and gynecology. The major causes of this differentials in medical practitioners' expenses were factors related to medical demand like proportion of old population, residential economic status in a given area. In addition, providers' economic incentives also played an important role in determining their income distribution. The large income differentials among physicians may imply a skewed distribution of patients and thus long waiting time, inefficient utilization of resources and potential inadequate quality of care. In this sense, unreasonable distributional gaps should be reduced, so effective measures as well as ongoing monitoring would be necessary to correct current distributional problems.

The Effects of Myrtle (Myrtus communis) and Clindamycin Topical Solution in the Treatment of Mild to Moderate Acne Vulgaris: A Comparative Split-Face Study

  • Objectives: Although Acne vulgaris is a chronic skin disease, which its standard treatment causes therapeutic limitations and some common adverse effects, medicinal plants can be effective in treatment with low adverse effects as combination therapy. Myrtle (Myrtus Communis) has some beneficial properties, which has been administered topically and orally for some skin diseases in Persian medicine. This study aimed to compare the efficacy and safety of Myrtle formula and 1% clindamycin topical solution. Methods: This was a split-face clinical trial that was done on 55 patients with mild to moderate acne vulgaris for 16 weeks. The patients received topical Myrtle solution to the right side of the face (group 1) and clindamycin solution to the left side (group 2) twice daily for 12 weeks. All participants were examined for the acne severity index (ASI) and total acne lesions counting (TLC) at certain times during the study. Then, they stopped using them for four weeks. They also did not take the drug in the final four weeks of the study. Results: Forty-eight patients completed the study for 16 weeks; 40 (83.2%) patients were female and the rest of them were male. The mean age and standard deviation were 25.62 ± 7.62 years. After 12 weeks, the percentage changes of comedones, inflammatory lesions, ASI and TLC were significantly reduced in both groups (p < 0.001). The percentage change of inflammatory lesions and ASI decrease was significantly higher in the group 1 (p = 0.03). There was no significant difference in the incidence of side effects between the two groups. There was a more significant decrease in sebum percentage change in the group 1 (p = 0.003). Conclusion: Myrtle lotion was effective and safe for the treatment of mild to moderate acne vulgaris.

도진우(都鎭羽)의 『동서의학요의(東西醫學要義)』에 대한 연구 (A Study of Do Jinwoo's Dongseo uihak youi (東西醫學要義))

  • This paper analyzes the historical context, the author, and the organization of contents of Dongseo uihak youi (Essentials of Eastern and Western Medicines), which was written by Do Jinwoo. In the colonial situation of the early twentieth century, the tradition of Korean medicine faced crises and challenges in many ways. Members of the Korean medicine community were simultaneously faced with continuing the tradition of Korean medicine and becoming healthcare providers with a specific role within the healthcare system of the time. Dongseo uihak youi is the result of the collective and official efforts of the Association of Korean Medicine of the time to maintain its tradition where only Western medicine was officially allowed to be taught and tested after the promulgation of the Rules of the Medical Student (ŭisaeng). Dongseo uihak youi was the first Korean medicine book to precisely describe and compare the names of diseases in Eastern and Western medicines. Dongseo uihak youi contained not only medical theories and prescriptions but also laws and forms, in that the purpose of the book was not simply to cultivate clinical skills but also to demarcate the boundary of medical knowledge and activities required of a practitioner of Korean medicine in the modern colonial health care system of the time.

『동서의학요의(東西醫學要義)』 간행으로 본 1920년대 한의학 정체성 변화에 관한 고찰 (A Study on the Identity Formation of Korean Medicine in the 1920s: Focusing on the publication of Dongseo uihak youi)

  • This paper describes the transformation of the knowledge system of Korean medicine in the early 20th-century colonial context of the 1920s in terms of 'identity formation'. At the time, newly introduced Western medicine was the dominant form of medical knowledge due to strong support from the colonial government but had did not enjoy popular support from the general public especially when compared to Korean medicine. Furthermore, the Japanese colonial government needed to utilize Korean medicine practitioners' labor due to a serious shortage of Western medicine doctors. In this context, Dongseo uihak youi (Essentials of Eastern and Western Medicines) provides an overview of the role of Korean medicine practitioners in the colonial healthcare system of the time. The book contains a figure of a 'modern' Korean medicine practitioner working within a healthcare system influenced by colonial modernity. The association of Korean medicine doctors at that time not only published Dongseo uihak youi but also attempted to establish a school specializing in both Eastern and Western medicines or integrated Korean medicine, which would produce "the Chosŏn doctors" (Chosŏn ŭisa) on a par with doctors trained in Western medicine. Although their attempts did not materialized, they provide a clue as to how and in what direction Korean medicine pursued its identity in the 1920s.

안구건조증의 병인(病因)에 대한 문헌적 고찰 (The literatual Study on the etiology of dry eye syndrome)

  • Objective : The object of this literatual study is to investigate the etiology of dry eye syndrome Methods : We have selected data related to etiology of dry eye syndrome on the literature of Oriental Medicine. And we analyzed the study. Results : Treatment and etiology concerning dry eye syndrome derived diverse opinion according to medical era and practitioner. It was a good opportunity to speculate the etiology and similar symptoms of dry eye syndrome. Conclusions : Through literatual Study, we basically understood the etiology of dry eye syndrome. In the study of the etiology, it seems that further clinical studies on treatment of dry eye syndrome via oriental medicine.

관절 모빌리제이숀에 있어서 관절운동의 기본개념 (Basic Concepts of Joint Motion in Peripheral Joint Mobilization)

  • Joint mobilization refers to techniques that are used to treat joint dysfunction such as when there is stiffness reversible joint hypomobility, or pain. Currently there are several schools of thought and treatment techniques that are popular in the United States. and leading practitioners and educators are attempting to blend common points to yield more uniform treatment from the various approaches. In order to effectively use joint mobilization for treatment, the practitioner must know and be able to evaluate the anatomy, arthrokinematics, and pathology of the neuromusculoskeletal system and to recognize when the techniques are indicated or when other stretching techniques would be more effective for regaining lost motion. Indiscriminate use of joint mobilization techniques when tot indicted could lead to potential harm to the patient's joints.
