임상시험 및 대상자보호프로그램의 운영과 현황에 대한 설문조사 연구(2019) (Survey of Operation and Status of the Human Research Protection Program (HRPP) in Korea (2019))

  • Purpose: The purpose of this study is to assess the operational status and level of understanding among IRB and HRPP staffs at a hospital or a research institute to the HRPP guideline set by the Ministry of Food and Drug Safety (MFDS) and to provide recommendations. Methods: Online survey was distributed among members of Korean Association of IRB (KAIRB) through each IRB office. The result was separated according to topic and descriptive statistics was used for analysis. Result: Survey notification was sent out to 176 institutions and 65 (37.1%) institutions answered the survey by online. Of 65 institutions that answered the survey; 83.1% was hospital, 12.3% was university, 3.1% was medical college, 1.5% was research institution. 23 institutions (25.4%) established independent HRPP offices and 39 institutions (60.0%) did not. 12 institutions (18.5%) had separate IRB and HRPP heads, 21 (32.3%) institutions separated business reporting procedure and person in charge, 12 institutions separated the responsibility of IRB and HRPP among staff, and 45 institutions (69.2%) had audit & non-compliance managers. When asked about the most important basic task for HRPP, 23% answered self-audit. And according to 43.52%, self-audit was also the most by both institutions that operated HRPP and institutions that did not. When basic task performance status was analyzed, on average, the institutions that operated HRPP was 14% higher than institutions that only operated IRB. 9 (13.8%) institutions were evaluated and obtained HRPP accreditation from MFDS and the most common reason for obtaining the accreditation was to be selected as Institution for the education of persons conducting clinical trial (6 institutions). The most common reason for not obtaining HRPP accreditation was because of insufficient staff and limited capacity of the institution (28%). Institutions with and without a plan to be HRPP accredited by MFDS were 20 (37.7%) each. 34 institutions (52.3%) answered HRPP evaluation method and accreditation by MFDS was appropriate while 31 institutions (47.7%) answered otherwise. 36 institutions answered that HRPP evaluation and accreditation by MFDS was credible while 29 institutions (44.5%) answered that HRPP evaluation method and accreditation by MFDS was not credible. Conclusion: 1. MFDS's HRPP accreditation program can facilitate the main objective of HRPP and MFDS's HRPP accreditation program should be encouraged to non-tertiary hospitals by taking small staff size into consideration and issuing accreditation by segregating accreditation. 2. While issuing Institution for the education of persons conducting clinical trial status as a benefit of MFDS's HRPP accreditation program, it can also hinder access to MFDS's HRPP accreditation program. It should also be considered that the non-contact culture during COVID-19 pandemic eliminated time and space limitation for education. 3. For clinical research conducted internally by an institution, internal audit is the most effective and sole method of protecting safety and right of the test subjects and integrity for research in Korea. For this reason, regardless of the size of the institution, an internal audit should be enforced. 4. It is necessary for KAIRB and MFDSto improve HRPP awareness by advocating and educating the concept and necessity of HRPP in clinical research. 5. A new HRPP accreditation system should be setup for all clinical research with human subjects, including Investigational New Drug (IND) application in near future.

의료기관 인증제도가 구성원 만족도와 병원운영 효과에 미치는 영향 : 비 인증병원과 인증병원 비교 (The Effects of the Healthcare Accreditation on Hospital Employees' Satisfaction Level and Hospital Management Performance)

  • 본 연구에서는 의료기관인증을 받은 병원과 받지 않은 병원의 내부 구성원을 비교 분석하여 의료기관인증제도가 구성원 만족도와 병원운영효과에 어떤 영향을 미치는지 파악하고자 하였다. 인증제도에 따른 직원만족도 차이는 인증을 받은 병원에서 인센티브 지급, 본인업무 자부심 등에서와 병원운영효과는 업무표준화로 업무수행의 정확성 등의 문항에서 통계적으로 유의미한 차이로 정(+)적 영향을 미치는 것으로 나타났다. 인증을 획득한 병원에서 구성원 만족이 높으며, 만족도가 높을수록 병원운영효과에 영향을 미치는 것으로 나타났다. 인증을 획득한 의료기관에 국가가 의료수가의 차별화나 의료장비를 지원하면, 인증을 받지 않은 병원들의 자발적인 인증 평가를 적극적으로 참여시킬 수 있는 활성화 방안이라고 사료된다.

우리나라 전공의 교육 및 관련 인증기관에 대한 통합적 관리계획 (Integrated Management Plan for Graduate Medical Education and Accreditation Bodies in Korea)

  • Graduate medical education is the most important phase among the three stages of medical education. It links basic medical education and continuing professional development. It is also a critical period in acquiring meaningful knowledge, skills and professionalism. The residents should be able to develop the core common competencies on top of their specialozeds field's expertise, in order to function as independent and qualified physicians. Despite the obvious importance of graduate medical education, the system in Korea has been designed and executed to meet the needs of the hospitals in the perspective of manpower management, rather than to educate and empower the residents. As a result, graduate medical education in Korea lacks clear educational objectives and systemic, resident-centered curriculums. Yet, we have no accreditation body to evaluate graduate medical education programs. In order to normalize graduate medical education, an integrated and unified institution that manages the whole process of the graduate medical education is desperately needed. Special attention should be given to the role of medical schools in educating the core common competencies. The Korean Association of Medical Colleges, the Korean Institute for Medical Education and Evaluation, and the Korean Academy of Medical Sciences should cooperate intimately to establish a new organization for the systemic management and development of graduate medical education.

A Study on the Management Innovation of KORAIL and Military Application -Focusing on the Direction of Innovation in the Military Medical Institution-

  • This study aims to analyze the characteristics of the management situation of the Korea Railroad Corporation(KORAIL) through the management innovation process of the KORAIL and to suggest its implications for military application. Despite stable demand, the railway passenger industry had the limitation of not being able to abolish deficit routes due to public service obligations. In addition, the launch of the Suseo High-Speed Line has introduced a competitive system, posing a threat to corporate management. KORAIL wanted to overcome this crisis by innovating its management through the utilization of big data, improvement of the freight business, decentralization of demand, the introduction of tourism railroads, and development of station influence areas. By utilizing big data, KORAIL was able to optimize the railway fare system while reducing fixed costs spent on railway maintenance. It also drastically reduced the station of cargo and created a base station to pursue economies of scale. On the other hand, the existing exclusive station system was abolished to solve the chronic saturation of the downtown area, and the railway demand was moved to Gwangmyeong Station and Suwon Station to optimize the passenger supply. In particular, it developed a new business model called the tourism railway by developing the mountain Byeokjin Line, which was a chronic deficit line, and sought to improve liquidity through the development of the station influence area. Such a process of innovation at KORAIL suggests an appropriate direction in seeking ways to innovate the military medical institutions. First of all, the necessity of improving organizational immersion through the development of a personnel structure suitable for the compulsory organization, while expanding the facilities of the division and corps, and reducing the time required for medical treatment and waiting through the establishment of a data-based medical system was suggested. Next, it was also discussed to integrate the National Health Medical College, which received accreditation as a medical facility through the designation of advanced general hospitals and is ultimately under discussion with the Medical Institution. Through this, we hope that the military medical institutions, which are facing various challenges, will overcome existing limitations and be re-lighted as innovative institution that provides comprehensive public health services.

의료기관인증이 병원 근로자가 지각하는 환자안전과 질, 직무만족 및 조직몰입에 미치는 효과 -일개 종합병원을 대상으로- (The impact of healthcare accreditation on patient safety and quality, job satisfaction and organizational commitment: about a general hospital)

  • 본 연구는 의료기관인증이 병원 근로자가 지각하는 환자안전과 질, 직무만족 및 조직몰입에 미치는 효과를 평가하기 위한 단일집단 사전 사후 검사 설계연구이다. 대상자는 서울시내 일개 종합병원에서 1년 이상 근무한 405명을 대상으로 하였다. 시험처치는 2012년 4월부터 준비를 시작하여 2012년 11월 의료기관평가인증원으로부터 조사를 받은 의료기관인증이며, 자료 분석은 SPSS(version 18.0) 프로그램을 사용하여 paired t-test와 One-Way ANOVA로 검정하였다. 연구결과, 연구결과, 의료기관인증 후 병원근로자가 지각하는 환자안전과 질(t=7.28, p=.000), 직무만족(t=7.13, p=.000) 및 조직몰입(t=3.44, p=.001)은 통계적으로 유의하게 증가하였다. 따라서 의료기관인증은 환자안전과질, 직무만족 및 조직몰입을 증진시키는 데 효과가 있다고 볼 수 있다.

한의학교육평가인증 내적타당화에 대한 고찰 (A Study on the Internal Validity of Korean Medicine Education Evaluation and Accreditation)

  • 본 연구는 제2주기 한의과대학 평가인증기준을 개발하여 타당화하는 과정에서 현장에서 직접적으로 요구되는 한의학교육의 지표는 무엇인지 고찰하고, 한의학교육계에 지표를 제시하고자 하였다. 이를 위하여 6인의 한의과대학 교육전문가를 대상으로 델파이 조사를 수행하였으며, 전문가 3인에게 내용타당도 검증 및 공청회를 거쳐 제2주기 평가인증기준을 개발하였다. 본 연구의 연구결과를 바탕으로 다음의 제언을 둔다. 첫째, 한의학교육기관의 지원을 고려해야 한다. 한의학교육기관은 각 기관명의 한방병원을 운영하고 있다. 병상 수와 학교의 지원을 고려하여 평가준거를 고려해야 한다. 둘째, 2주기 평가 당시 평가위원은 6인 모두 한의학과 교수를 대상으로 이루어졌으나, 향후 평가위원의 한 모둠당 1인은 외부 교육과정 및 평가전문가로 구성하여 교육에 초점을 둔 평가를 모색할 필요가 있다. 셋째, 교육프로그램 및 기관 평가인증기준을 개발 단계부터 교육과정 및 평가전문가 포함될 필요가 있다. 넷째, 한의학대학 교육과정 개발진으로 교육과정 전문가가 포함되어 구성되어야 한다. 본 연구는 한의학교육의 질적 향상을 위한 연구로서 그 의의가 있다.

의학교육에서 의과대학생들의 개인 책무성 (Personal Accountability of Medical Students in Medical Education)

  • Producing graduates with sufficient practical competency is the main mission of every educational institution. Following the accreditation of the Korean Institute of Medical Education and Evaluation, medical schools have been stepping up efforts to establish curriculum that reflects the practical value of medical education and the importance of adapting to the practice of graduates in order to increase the accountability of medical education in Korea each year. To this end, all medical schools have recently made efforts to develop diverse policies to strengthen the social accountability of medical education along with the transition to a competency-based curriculum. In line with this trend, the institutional accountability of medical education as well as the personal accountability of students, the main subjects of learning, should be highlighted, and educational activities to foster accountability need to be specified. Personal accountability in medical students involves recognizing their social accountability as future doctors and understanding and practicing student accountability. To achieve this, medical schools should provide programs that support and teach practical application of skills, and students need to define and attempt specific activities to strengthen their accountability.

일개 종합병원 간호사의 의료기관 인증평가 인지, 직무스트레스, 이직의도, 안전관리 인지 변화 -의료기관 인증평가 전·후 비교- (Awareness, Job Stress, Turnover Intention, Safety Management Perception Change of Nurses in a General Hospital -Before and After Medical Institution Certification System-)

  • 본 연구는 일개 종합병원 간호사의 의료기관 인증평가 인지, 직무스트레스, 이직의도, 안전관리인지 변화에 대해 의료기관 인증평가 전 후를 비교하여 분석하였다. 자료수집은 2016년 6월1일부터 7월17일까지 실시 하였고 수집된 자료는 SPSS 21.0 version을 이용하여 평균, 표준편차, paired t-test를 분석하였다. 본 연구결과 의료기관인증 인지, 이직의도, 안전관리인지도의 인증평가 전 후 유의한 차이는 없었으나 인증평가 후 평균점수는 높게 나타났고 직무스트레스는 인증평가 전 후 유의한 차이를 보였다(t=2.825, p=<.005). 즉, 인증평가 전이 인증평가 후 보다 직무스트레스가 높은 것으로 나타났다. 그러므로 인증평가를 준비하는 간호사의 직무스트레스를 감소시키기 위해 업무 부담감을 줄일 수 있는 의료기관의 적극적이고 다양한 인적 물적 지원이 필요하다.

의료기관의 러시아 의료관광객 유치 준비실태 (A Study on the Preparation of Medical Institutions for Attracting Russian Medical Tourists)

  • The purpose of this study is to examine the preparation of medical institutions for Russian medical tourists. The data for this study include 44 cases which were selected from Korea Health Industry Development Institute list. Data were collected through the e-mail survey from Feb. 24 to Jul. 1 in 2014. The results of the study are as follows. Overall, tertiary hospitals were well prepared in hardware resources and structure(e.g. the number of coordinators and marketers, accreditation)compared to hospitals and clinics. On the other hand, hospitals were well prepared in service process protocol.

의료기관 인증 후 환자안전 및 질 관리 변화와 경영활동 변화 간의 관계: 정신병원과 요양병원 대상 (Relationship between Changes of Patient Safety & Medical Service Quality and Changes of Management Activity after Medical Institution Accreditation: Mental Hospitals and Geriatric Hospitals)

  • 본 연구는 의료기관 인증제의 의무 대상인 정신병원과 요양병원을 대상으로 환자안전 및 질 변화와 경영활동의 변화 간의 관계에 대해 의료기관 인증을 받은 5개 정신병원과 5개 요양병원에 근무하는 종사자들을 대상으로 조사 연구한 결과, 정신병원과 요양병원 종사자들의 인증업무 참여유무는 환자안전 및 질 관리 변화 측정내용 중 '안전보장활동 수행정도' '환자를 위한 양질의 의료서비스 제공정도' '환자의 권리와 책임에 대한 존중정도' '감염발생 위험예방 위한 감염관리활동 수행정도'의 향상 변화에 유의하게 긍정적인 상관관계를 보였다. 그리고 환자안전 및 질 관리 내용 중 안전보장활동 지속적인 질 향상 활동 환자의 권리와 책임에 대한 존중의 지속적인 발전적 변화를 추구하기 위해서는 교육훈련을 통해서 조직구성원들의 역량을 강화시키는 경영활동이 필요하며, 진료전달 체계와 평가의 관리 환자를 위한 양질의 의료서비스 제공의 지속적인 발전적 변화를 위해서는 고객지향성 업무프로세스를 사회 환경의 변화에 맞추어 추구하는 경영활동이 필요함을 알 수 있었다.
