Purpose: This study aimed to present an analysis model evaluating evacuation performance considering patient types and procedural evacuation in the medical facility. The user group of the medical facility, including users challenged in evacuation behavior, entails the risk of many casualties. Therefore, it is necessary to plan an evacuation procedure that considers the evacuation characteristics of users. Methods: Through the review of precedent studies, the evacuation procedure of the medical facility, the classification of patient types, and the evacuation procedure was set as conditions and variables for the analysis. The result caused by a variety of conditions and variables were explored. Results: 1) The total evacuation completion time and congestion time were shortened at the procedural evacuation. Moreover, it derived many users from evacuating at the initial phase. 2) The proposed model can provide a basis for proposing a space planning direction that considers the possibility of not carrying out the evacuation plan. 3) It supports safe evacuation by identifying variables that reduce overcrowding by comparing the congestion time of overcrowded spaces. 4) The analysis model can identify the overcrowded space through the evacuation route and suggest the basis for architectural improvements that reduce overcrowding. Implications: The study results can be used to analyze the performance of evacuation procedures and support the establishment of evacuation procedures and building plans for safe evacuation for medical facilities.
Background In Japan, there is a large regional disparity in plastic surgery availability. In order for plastic surgery to be widely available for all citizens, it is essential for at least one plastic surgery facility to be located in each secondary medical zone. Methods Using the Japan Society of Plastic and Reconstructive Surgery homepage and some databases, we extracted data on secondary medical zones that do not have a plastic surgery facility. The national and regional coverage rates were calculated. The coverage rate for each group divided by the degree of population concentration was also calculated. Results We found that 147 of 344 secondary medical zones did not have a plastic surgery facility, and the area coverage rate was found to be 57.27% nationwide. The coverage rate in terms of population was 87.07% (correlation coefficient of area and population coverage = 0.983). The area coverage rates in Hokkaido-Tohoku, Kanto, Chubu, Kansai, Chugoku-Shikoku, and Kyushu-Okinawa districts were 47.46, 72.15, 76.47, 62.79, 52.08, and 32.81%, respectively. The corresponding population coverage rates were 79.92, 91.62, 94.27, 90.59, 80.68, and 69.54%, respectively. The area coverage rates in metropolitan areas, provincial cities, and rural areas were 98.08, 75.90, and 15.87%, respectively. In contrast, the area coverage rate of dermatology was 62.79% and that of orthopaedics was 97.09%. Conclusion Unfortunately, it is estimated that more than 40% of secondary medical zones are underserved by plastic surgery, and 13% of the population is not able to fully benefit from this specialty in Japan.
As a society advances and develops more rapidly, there is a corresponding increase in an aging population; and therefore, transportation and transportation needs develop in a complicated way. In these times, medical rehabilitation services are required and the demands for these services will increase as time passes. Moreover, the scope of disability as defined by the government has been expanded owing to the welfare policy for the disabled. Therefore, the number of disabled who require medical services is on the rise. Along with an increase in medical rehabilitation demands, the necessity for medical rehabilitation increases as well and its role becomes more diverse on a wider spectrum. However, architectural studies on rehabilitation facilities have been insufficient and there are many problems currently in facilities operations. This study aims to present a spatial organization system along with required space that would form the basis for the most effective spatial plan and facility operation for rehabilitation hospitals.
With hope and concern, the first Korean proton therapy facility was introduced to the National Cancer Center (NCC) in 2007. It added a new chapter to the history of Korean radiation therapy. There have been challenging clinical trials using proton beam therapy, which has seen many impressive results in cancer treatment. Compared to the rapidly increasing number of proton therapy facilities in the world, only one more proton therapy center has been added since 2007 in Korea. The Samsung Medical Center installed a proton therapy facility in 2015. Most radiation oncology practitioners would agree that the physical properties of the proton beam provide a clear advantage in radiation treatment. But the expensive cost of proton therapy facilities is still one of the main reasons that hospitals are reluctant to introduce them in Korea. I herein introduce the history of proton therapy and the cutting edge technology used in proton therapy. In addition, I will cover the role of a medical physicist in proton therapy and the future prospects of proton therapy, based on personal experience in participating in proton therapy programs from the beginning at the NCC.
Purpose: Tuberculosis(TB) care unit in public health center should be carefully considered to be re-designed as an infection safety environment for both patient and healthcare workers. So, for the enhancement, this study analyses the facility requirements for co-using the screening clinic as a TB and other respiratory disease care unit. Methods: Not only screening clinic facility guidelines from "A Study for Standard Triage Design and Construction Document" but also the guidelines of TB care and related medical facility are reviewed; KDCA, CDC, ECDC and WHO as a TB care, and FGI and NHS for facility. The facility requirements are summarized space, approach, and mechanical requirement in order. By comparing the summary and screening clinic facility guidelines, supplementations are proposed for TB care unit setting. Results: The result of this study shows that both the space program and mechanical requirement of the screening clinic and that of TB care unit are almost identical and could be share, which include direct airflow or negative air pressure in an exam room. To increase functional and economical efficiency, however, it is necessary to consider a multi-functional negative pressured room, So care process may be re-designed based on a room type; face-to-face room or glass wall inbetween. Implications: The facility guidelines for TB care unit of a public health center should be developed to build a safe environment for infection control by reflecting its medical plan and budget.
Purpose: General hospital in korea is getting large-scaled, specialized and upgraded. So, nursing unit of race track type has been advanced along with a spatial organization, building equipment and environmental approach. This study is to search the guidelines for renovation of nursing unit with race track type in the case of K university hospital in Daejeon. Methods: 9 cases of recently opened general hospital has been analyzed for spatial and environmental design solution. Questionnaires and interviews about user needs of nursing staff, patient and visitors, have been conducted for a nursing care system and facility of nursing units in K university hospital. Results: The user needs are represented as followings. Center core public zone in each floor plan and center core nursing station and supporting areas in nursing unit are good for an adjacency and separation of spatial organization. Toilet of group patient room is necessary to equip for easy, safe and infective aspects, and so toilet install will decrease the patient number and increase the nursing care service. Hall type of station front is good for visibility and observation. It is appropriate to renovate into south facing group patient room for privacy, enough space for nursing care facility and supporting area, enough width of door to move portable medical equipment, room for medical doctor and practical student, noise absorbing of day room. Implications: Upgrading the nursing care service and facility equipment is necessary to reflect the user needs and cooperate with hospital management.
The Outflow facility of eye ball provides crucial informations in the diagnose of ophthalmological glaucoma. Tn this study an ophthalmic outflow facility measuring device is developed and experimentally tested. This system employs pressure sensors and linear positioning device to regulate solution reservoir height, the pressure of eye ball. The experimental result show that the system works well even in harsh environment so that it can be implemented in ophthalmic practice as well as in biomedical research.
Objectives : This study aims to systematically examine the determinants of the intention to take herbal medicine among individuals with experience in Korean medicine. Methods : This study utilized the 3,245 respondents from the 2020 Korean Medicine Utilization and Herbal Medicine Consumption Survey who reported having used Korean medical services; the responses were selected through a complex sample analysis, and analytics techniques including frequency analysis, Rao-scott chisquare test, and logistic regression were used to analyze the responses. Results : The results of data analysis reveal that the intention to take herbal medicine in the future among these individuals was significantly influenced by factors such as enrollment in private health insurance, the facility environment of Korean medical institutions, and treatment outcomes. Conclusions : Therefore, it is imperative for Korean medical institutions to continuously try reasonable action strategies, including improving medical facilities/environments and building institutional mechanisms among medical staff and members to enhance the quality of Korean medical services. Additionally, the Ministry of Health and Welfare needs to make policy efforts to reduce patients' medical expenses, such as reducing the co-insurance rate in the pilot project of applying health insurance to herbal medicine.
The purpose of this study was to investigate these major factors on patient satisfaction, and to examine the affecting level of major factors in. The subjects in this study was 70 hospitals that were surveyed the hospital evaluation program containing the survey of patient satisfaction by KHIDI(Korea Health Industry Development Institute) from 1997 to 1999. The collected data was analysed SPSS for Windows(Ver 10.0). On basically, frequency analysis, t-test, and ANOVA was performed and, for more analysis, correlation analysis, factor analysis, multiple regression analysis, logistic regression analysis was utilized. According to this study, the major factors of inpatient satisfaction are divided 3 types facility factor, manpower factor, and service factor. And the major factors of outpatient satisfaction are analyzed 5 types; facility factor related direct medical service, facility factor related indirect medical services, manpower factor, pharmacy factor, and facility factor related utilization convenience. The importance of this study lies in the identification of major factors on hospital patient satisfaction.
The research was to investigate the effect of quality cognition of the web site of the facility and relationship commitment on customer loyalty. The respondents were 190 patients and caregivers. Data were collected from March 1st to 31st, 2010 at C university hospital in G city. The effect of facility web site of quality cognition and relationship commitment on customer loyalty showed 43.9%(F=35.806) in model 1, 58.4%(F=51.113) in model 2, and 58.7%(F=28.185) in model 3. Also, medical information & usage, aesthetics was significance in model 1, medical information & usage, relationship commitment in model 2. In relationship between web site quality cognition and customer loyalty, moderate effect of relationship commitment were not considerable in model 3. As a result, customer loyalty is the leading fact for quality cognition of the web site more than relationship commitment. Therefore, customer loyalty leaves its possibility of increment when facility web site is designated depending on medical information and aesthetics.
