  • This study was carried out to provide the essential information in improving the graduate medical education in Korea. For the study, a survey targeting the directors of GME of nationwide teaching hospitals was performed with a questionnaire asking the questions such as the director's perception on the quality of GME, trainees' salary level, trainees' specialty selection tendency, training system and its duration. The collected data were analyzed using t-test, ANOVA, and $x^2$-test. The results were as follows: 1. The survey were executed on 240 teaching hospitals in Korea and the response rate was 66.2% (159 hospitals replied). 2. The bigger a hospitals is the better in Quality of education. Larger hospitals tend to have better status in all items including medical specialists' experience, contents of medical curriculum, general environment for medical education and medical trainees's salary level. The result supported the general perception on the positive relationship between hospital size and Quality of GMA. 3. Providing convenience for medical trainees who prepares for the medical specialist Qualifying examination didn't affect the results of the examination. 4. The directions of GME have a perception that the trainees give positive impact on financial performance of their hospitals. This seems to be one of the reasons that hospitals try to retain as many trainees as possible. 5. The directors of GME considered medical trainees as an educate, and most of them responded positively on the need of governmental supports for the education cost and the trainee's salary. Considering above results, it seems that GME would get more social attention and the trainees' impact on hospitals operation would be increased more than before. In response to these trends, hospitals would find out the ways to lower dependency on trainees, and this change of attitude of hospitals on the GME would cause problems in operation of hospitals and GME itself. In order to prevent these problems the policy on GME should be directed in following ways. 1. The contents of Qualifying examination for specialist should be improved. 2. The curriculum of GME should be strictly followed. 3. The status of trainee in a hospital has to be defined as eductee. 4. Government has to support a half of the education cost and salary of trainee. 5. The distribution of the trainee among the hospital group have to be based on total available. 6. The financial support and welfare of trainee should be improved gradually and systematically.

  • This study is made for the survey of the effectiveness of the auricular reflexology, now accepted as a part of alternative medicine, applied to the patients of musculoskeltal disease patients, and data about its cognition and the success rate of the therapy. 106 patient was employed in this study who works in various part in a company in Jun-ju in Junrabukdo. From them, data such as, musculoskeletal symptoms by questionnaire examination, and data from ear inspections and interviews were collected and used for the basal data. Auricular reflexology therapies were applied to them at the areas that reflect the musculoskeletal system, for five times, each time with intervals of three or four days. The data about the effectiveness were collected by questionnaire examination, including descriptive answers. The results are as follows; cognition to the auricular reflexology was 62.3%, and acceptance rate of this method was 81.1%. From this results, I think that this therapy can be easily acceptable. The success rates of this therapy were 32.1% after 1 time application, and 84.9% after 5 times applications. This result shows that auricular reflexology therapy has therapeutic effect in musculoskeletal diseases, and the success rates increase according to the application times. Between sex, its success rate were 80% in male patients, and 88.5% in female patients, so this therapy is rather more effective in female patients. According to the body weight, the success rate were 85.8% in normal weight patients, and 71.5% in over- weight patients, so this therapy is more effective in normal weight patients. Comparisons according to other factors, such as age, marriage state etc. shows no definite differences. Conclusively, I think that auricular reflex therapy is easy and useful preventative and treatment method in our usual life. With more investigation, we can apply this method to other musculoskeletal diseases and to the diseases of other systems in the body, I hope so.

  • Background: This study used pre-hepatitis A vaccination era data in U.S. to study the relationship between serum hepatitis A antibody positivity with pancreas cancer mortality in adults. Patients and Methods: Public use National Health and Nutrition Examination Survey (NHANES III) data were employed. NHANES III uses complex probabilistic methods to sample nationally representative samples. Household adult laboratory and mortality data were merged. Sample persons who were available to be examined in the Mobile Examination Center (MEC) were included in this study. All results were obtained by using specialized survey software taking into account the primary sampling unit and stratification variables and the weights assigned to the sample persons examined in the MEC. Thus they are representative of the U.S. population. Results: The mean risk (95%CI) of death in the study population for pancreas cancer was 0.0014 (-0.000069 -.0029); their mean age (95%CI) at the mobile examination center (MXPAXTMR) was 473.43 (463.85-482.10); the follow up in months from their medical examination (permth_exm) was 170.12 (164.17-176.07). The odds ratios (S.E.) of the statistically significant univariables were: age, 1.007 (1.005-1.009); serum anti-hepatitis antibody status, 0.038 (0.004-0.376); and drinking hard liquor, 1.014 (1.004-1.023). The coefficients (S.E.) of the statistically significant variables after multivariate analysis were 0.006 (0.002-0.010) for age and -2.528 (-4.945--0.111) for serum anti-hepatitis A antibody negativity (using serum anti-hepatitis A antibody positivity as a reference). Conclusion: Serum hepatitis A antibody positivity correlates with higher pancreas cancer mortality in adults.

  • According to the analysis of special medical examination and work environment monitoring data, the rate of C1 and D1 on noise hazard exceeded 90 % among those of total hazardous factors. The rate of company exceeding noise exposure limit was also more than 90 %. The analysis result shows that main ages diagnosed with C1 and D1 was age of 50s. The majority scale company having workers diagnosed with C1 and D1 was the companies employing 5~49 workers. Types of industries which have a large number of companies exceeding noise exposure limit were automobile and trailer manufacturing, metal processing industry and primary metal manufacturing. A large number of work processes exceeding noise exposure limit were forming and processing work, cutting and bending work and grinding. To reduce the number of company exceeding noise exposure limit, the reduction counterplan should be focused on the type of industry and the work process which exceeded noise exposure limit frequently. However, the reduction counterplan is preemptively necessary to the type of industry and the work process which exceeded noise exposure limit consecutively if the purpose of reduction counterplan is not to merely reduce the number of company exceeding noise exposure limit but to abate workers' suffering from noise.

  • The purpose of this study is to provide the general history of fostering dentists in Japan and introducing their new roles. This research was conducted based on the government policy report on dentists and the information published by each educational institution. Based on the collected data, the official websites were used to represent the latest statistics of the institutions. The number of dentists in Japan has increased. The government established the National Examination for Dentists to guarantee the quality of dentists. After the standards for developing questions for the national examination were established in 1985, the contents of the examination have been appropriately improved by revising the standards every four years. This improvement has required dental students to study a variety of subjects for six years at dental school. Since dentists in Japan are required to respond to various demands from the nation; the Model Core Curriculum for Dental Education was developed to teach medical ethics and abilities to ensure that dentists conduct themselves professionally. Recently, the roles of dentists have been changing in Japan. When providing dental services to older patients over the age of 65, dentists and other dental professions focus on maintaining oral functions, such as saliva secretion, bite force, tongue movement, and masticatory/swallowing functions. However, oral function-related services for children are different. In addition to providing essential dental services, dental practitioners also provide special treatment, such as oral muscle training, myofunctional therapy, health guidance, and space retainers to the child patients with developmental insufficiency in oral functions. Dentistry in Japan has undergone numerous changes over the years and has continued to offer high-quality dental health services. Thus, information gained from the Japanese experience may be helpful to dental professions in other developed countries for planning oral health measures.
