색전용 금속 코일 개발과 단백질 처리 후 혈전성에 대한 연구 (Study of the capability of metallic coils immersed with protein solution as embolic masterials)

  • In this study, we designed a vascular occlusion device based on the principle that slowing blood flow would induce thrombosis. Tungsten, stainless steel and platinum were tested for finding a good embolic metallic coil. The primary coil and the second coil were done with heat treatment or different time. The pitch distance and the shape retention capability of second coil were characterized by SEM. To increase thrombogenicity, we tried to treat different proteins on steel coils: thrombin, gelatin and both gelatin and thrombin. To verify protein materials treated on coils, the surface of coil treated with different proteins were characterized using Laser Raman Spectrophotometer. After observation of embolic coils, the peptides bonds on theirs' surface were found. In order to compare the thrombogenicity of different embolic agents, we measured whole blood clotting time.

체중지지 훈련이 만성 뇌졸중 환자의 체중지지능력 및 균형에 미치는 영향 (Effect of Weight Bearing Exercise on Weight Bearing and Balance for Patients with Chronic Stroke)

  • Purpose: In this study, the following experiment was conducted to see how to apply the sprinter pattern in proprioceptive neuromuscular facilitation (PNF) and functional weight bearing exercise affected balance capabilities and weight bearing among chronic stroke patients. Methods: The subjects included 27 subjects who had been diagnosed with hemiplegia due to stroke. A total of 12 sessions was held, 3 sessions a week over 4 weeks, were provided to the groups. The control group received general rehabilitation program, and the experimental group performed sprinter pattern in PNF or functional weight bearing exercise. The weight bearing capability and static balance capability test was implemented by using Good-Balance System and Frailty and Injuries Cooperative Studies of Intervention Techniques, while the dynamic balance capability test was carried out by using Four Square Step Test and Timed "Up and Go". Results: The paretic/nonparetic side weight bearing by application of each exercise showed that there were no significant differences in variation among each groups on before exercise, after exercise, 2 weeks after exercise. Static balance test by application of each exercise showed that there were significant differences after exercise and 2 weeks after exercise. Dynamic balance test by application of each exercise showed that there were significant differences after exercise and 2 weeks after exercise. Conclusion: To put the results together, the application of sprinter pattern and functional weight bearing exercise was effective in improving static and dynamic balance capabilities. Therefore, these exercises are helpful to improve balance in patients with stroke.

횡복직근피판술을 이용한 유방재건 후 복벽의 운동능력에 관한 전향적 연구 (Evaluation of Abdominal Wall Function after TRAM Breast Reconstruction: A Prospective Study in 375 Consecutive Cases)

  • Purpose: TRAM flap surgery has settled down as a common method for breast reconstruction after mastectomy. We investigated how TRAM flap surgery influences on the patients' physical movement capability by observing their capability of sit-ups as well as exercises they usually enjoy. Methods: A total of consecutive 375 patients were investigated who had breast reconstruction with unilateral pedicled TRAM flap surgery at Asan Medical Center from July 2001 to August 2005. The patients were asked to sit up right before the surgery and do it again 6 months later and 1 year later. 221 patients were followed up 6 month after the surgery. And 132 patients were followed up 1 year after the surgery. In addition, 155 patients who used to exercise before the surgery were also asked to show us the change in their physical movement capability one year after their surgery. Results: 139 patients showed decrease in the counted number of sit-ups, 48 increase, and 34 showed no change between 6 months in the first group of 221 patients. Among the second group of 132 patients, 64 showed decrease, 39 increase, and 29 no change a year later. There was a statistical significant decrease in the number of sit-ups between pre-operation and six months later and between pre-operation and one year later. According to the research on the exercise that 155 patients participated, 3 of them showed improvement in athletic ability, 7 showed decrease, while the rest, 145 patients, showed no change at all. Conclusion: Considering no difference in usual exercise ability, some patients' increase in the number of sit-ups and the effect of anticancer treatment, we found that the loss of abdominal wall function on this research is not too serious to exclude TRAM flap surgery in the field of breast reconstruction

Sealing capability and marginal fit of titanium versus zirconia abutments with different connection designs

  • PURPOSE. Limited data is available regarding the differences for possible microleakage problems and fitting accuracy of zirconia versus titanium abutments with various connection designs. The purpose of this in vitro study was to investigate the effect of connection design and abutment material on the sealing capability and fitting accuracy of abutments. MATERIALS AND METHODS. A total of 42 abutments with different connection designs [internal conical (IC), internal tri-channel (IT), and external hexagonal (EH)] and abutment materials [titanium (Ti) and zirconia (Zr)] were evaluated. The inner parts of implants were inoculated with $0.7{\mu}L$ of polymicrobial culture (P. gingivalis, T. forsythia, T. denticola and F. nucleatum) and connected with their respective abutments under sterile conditions. The penetration of bacteria into the surrounding media was assessed by the visual evaluation of turbidity at each time point and the number of colony forming units (CFUs) was counted. The marginal gap at the implant- abutment interface (IAI) was measured by scanning electron microscope. The data sets were statistically analyzed using Kruskal-Wallis followed by Mann-Whitney U tests with the Bonferroni-Holm correction (${\alpha}=.05$). RESULTS. Statistically significant difference was found among the groups based on the results of leaked colonies (P<.05). The EH-Ti group characterized by an external hexagonal connection were less resistant to bacterial leakage than the groups EH-Zr, IT-Zr, IT-Ti, IC-Zr, and IC-Ti (P<.05). The marginal misfit (in ${\mu}m$) of the groups were in the range of 2.7-4.0 (IC-Zr), 1.8-5.3 (IC-Ti), 6.5-17.1 (IT-Zr), 5.4-12.0 (IT-Ti), 16.8-22.7 (EH-Zr), and 10.3-15.4 (EH-Ti). CONCLUSION. The sealing capability and marginal fit of abutments were affected by the type of abutment material and connection design.

Participation in and perceptions of antibiotic stewardship behaviors among nurses at a children's hospital in South Korea: a descriptive study

  • Purpose: This study investigated participation in and perceptions of antibiotic stewardship among nurses at a children's hospital. Methods: This descriptive study included 125 nurses working in the inpatient ward, intensive care unit and emergency room of a single tertiary children's hospital. The study measured 14 factors influencing antibiotic stewardship behaviors using the theoretical domains framework. Each factor was analyzed by categorizing it into components (capability, opportunity, and motivation) that have been proposed as influencing factors in the COM-B model of behavior. One-way analysis of variance and Pearson correlation coefficients were used to explore differences in antibiotic stewardship behaviors and influencing factors according to general characteristics and the correlation between antibiotic stewardship behaviors and COM-B components. Results: No statistically significant difference in antibiotic stewardship behaviors was found based on the experience of antibiotic stewardship education or the nursing department. However, significant differences were observed in the perception levels of factors related to antibiotic stewardship behaviors according to the experience of antibiotic stewardship education in skill (physical) (p=.042), knowledge (p=.027), intentions (p=.028), and social influences (p=.010). Additionally, significant differences were observed in perception levels according to the sub-components of the COM-B model, specifically physical capability (p=.042), psychological capability (p=.027), and social opportunity (p=.010). Conclusion: To expand nurses' involvement and roles in antibiotic stewardship, nurses should acknowledge the significance of appropriate antibiotic use, aiming to enhance the quality of medical care and ensure patient safety. In pursuit of this objective, tailored education aligning with the specific needs and practices of nurses is essential.

편마비 환자의 재활치료 효과를 위한 파크골프 도입에 대한 고찰 (A Study on Park-Golf for Rehabilitation training of the Patients with Hemiplegia)

  • The purpose of this study suggest that Park Golf for rehabilitation training of the patients with hemiplegia. The increase of the aged caused various problems. Among the problems, chronic illnesses the old people suffer from charge their family with mental, physical, social and economic burden as well as the person. Especially, the more old people are the more risks and attacks of cerebral vascular accident(CVA) exist. Therefore, the increase of the aged means the increase of cerebral vascular accident(CVA) patients. The cerebral vascular accident(CVA) patient get into difficulties to secure their livelihood due to troubles and disoders, so medical care facilities and rehabilitation training are really necessary to help them return to their normal life with mental, physical and social capability. The rehabilitation of patient with hemiplegia is ongoing process. It may possibly accomplish throughout the whole life because it takes a lot of time and effort. In addition, an ultimate aim of the rehabilitation is returning and adaptation to society. In this way, it is hardly to expect a satisfied results of the rehabilitation without motive. Therefore, patient with hemiplegia rehabilitation motive will is the most important. If interesting of park golf the patient with hemiplegia can be absorbed in easily are applied to the rehabilitation training, the patient are will to participate in the training without troubling.

중증 만성폐쇄성 폐질환 환자로 살아가기 -살얼음판 위를 걸어가는 삶- (Living as Severe COPD Patient - Life of Stepping on the Thin Ice)

  • Purpose: The purpose of this study was to explore the life experiences of patients with a severe Chronic Obstructive Pulmonary Disease (COPD). Methods: The data were collected through in-depth interviews of six patients suffering from severe COPD. The interviewed data were audio-recorded and transcribed verbatim and checked for accuracy. The Giorgi method of phenomenology was used for analyzing data. Results: Eight themes forming the, units of meaning, were: Repeated and Unpredictable Suffering of Dyspnea, Confidence Loss/Exhaustion Life due to non-efficient breathing, Gradually Deprived Liberty, Absolute Being to Sustaining my life, Source of Burden but Significant Person I am in the Family, Endless Tug-of-War-Capability/Endeavor to Breath, Longing for my Life, and Dead-end of breathing. Conclusion: The study results provide an in-depth understanding of life experiences of patients suffering from severe COPD. The findings will be useful to nurses caring for this population.

Development Brief of A Body Area Network for Ubiquitous Healthcare : An Introduction to Ubiquitous Biomedical Systems Development Center

  • The fusion technology of small sensor and wireless communication was followed by various application examples of the embedded system, where the social infrastructural facilities and ecological environment were wirelessly monitored. In addition, this technology represents the primary application area being extended into the healthcare field. In this study, a body area network for ubiquitous healthcare is presented. More specifically this represents a wireless biomedical signal acquisition device characterized by small size, low power consumption, pre-processing and archiving capability. Using this device, a new method for monitoring vital signs and activity is created. A PDA-based wireless sensor network enables patients to be monitored during their daily living, without any constraints. Therefore, the proposed method can be used to develop Activities of Daily Living (ADL) monitoring devices for the elderly or movement impaired people. A medical center would be able to remotely monitor the current state of elderly people and support first-aid in emergency cases. In addition, this method will reduce medical costs in society, where the average life expectancy is increasing.

Evaluation of the rat tissue reaction to experimental new resin cement and mineral trioxide aggregate cement

  • Objectives: New resin cement (NRC) has been developed as a root repairing material and the material is composed of organic resin matrix and inorganic powders. The aim of this study was to compare the rat subcutaneous tissue response to NRC and mineral trioxide aggregate (MTA) cement and to investigate the tissue toxicity of both materials. Materials and Methods: Sixty rats received two polyethylene tube-implants in dorsal subcutaneous regions, MTA and NRC specimens. Twenty rats were sacrificed respectively at 1, 4 and 8 wk after implantation and sectioned to 5 ${\mu}m$ thickness and stained with Hematoxylin-Eosin (H-E) or von-Kossa staining. The condition of tissue adjacent to the implanted materials and the extent of inflammation to each implant were evaluated by two examiners who were unaware of the type of implanted materials in the tissues. Data were statistically analyzed with paired t-test (p < 0.05). Results: In specimens implanted with both NRC and MTA, severe inflammatory reactions were present at one wk, which decreased with time. At eighth wk, MTA implanted tissue showed mild inflammatory reaction, while there were moderate inflammatory reactions in NRC implanted tissue, respectively. In NRC group, von-Kossa staining showed more calcification materials than MTA group at eighth wk. Conclusions: It was concluded that the calcium reservoir capability of NRC may contribute to mineralization of the tissues.

한의사 직능수행을 위한 대학교육에서 추구하는 한의사 인재상에 대한 논의 (Concept of Talent on the Doctor of Korean Medicine to Pursue University Education in Vocational Performed for Korean Medical Doctor)

  • Apparently, in the field of Traditional Korean Medicine(TKM), lots of papers have been published and they have helped TKM grow quantitatively. But from the inside, there has been a lot of problems like the gap of theory and practice, doubts about necessity of basic theory, and lack of internal communication. There could be many reasons for it, but it could be considered due to the lack of ideal talent that should be goal of teaching and performing TKM. In the field of TKM, the absence of ideal talent of Korean medical doctors(KMD) and inferior social position of KMD than Medical Doctors(MD) has beeb derived from the laws promulgated in Japanese Occupation. These days, KMD should have ability for diagnosis of diseases as primary care and capability for diagnosis and treatment based on TKM theory, therefore careful study of basic theory of TKM is necessary.
