A Gastroesophageal Cyst in the Posterior Mediastinum; A Report of Case (후종격동에 발생한 위식도 낭종)

  • The gastroesophageal cyst is rare variety of benign developmental cysts in the mediastinum and it arises from sequestrations of nodules of forgut in the developing embryo.The patient was 23 year old man with complaint of right chest pain. Simple chest X-ray and chest CT scan showed a huge homogeneous cystic mass in the posterior mediastinum. The resected cystic mass showed combining of portion of esophagus and stomach. The cyst was confirmed as gastroesophageal cyst.

  • Thymoma is a common anterior mediastinal mass, although thymomas have occasionally been found in the neck, pulmonary hillus, or posterior mediastinum. But a thymoma within the middle mediastinum has rarely been reported. We report a thymoma arising in the middle mediastinum with a review of the literature.

  • Extraskeletal osteogenic sarcoma is a rare malignant tumor of soft tissue, and its predilection sites are the extremity, retroperitoneum, trunk, and the head and neck area. To our knowledge 5 cases of primary involvement of the mediastinum have been reported. Because of its rarity and difficulty in exact diagnosis preoperatively, we report an extraskeletal osteogenic sarcoma in the anterior mediastinum. The patient was a thirty eight old male. He complained of cough and sputum over 2 months. The chest roentgenogram and the chest MRI[magnetic resonance image] were done and showed anterior mediastinal mass with calcification. Excision of the mass was done under the preoperative impression of thymoma, and the pathologic report was extraskeletal osteogenic sarcoma of the mediastinum.

Clinical Review of Primary Mediastinal Tumors 4 Cysts (원발성 종격동 종양 및 낭종의 임상적 고찰)

  • We have experienced 47 cases of primary mediastinal tumors & cysts in the Department of Thoracic and Cardiovascular Surgery, Chonbuk National University Hospital, from September, 1979 to November, 1990. The results were as follows: 1. The age distribution was from 6 months to 69 years old and the mean age was 32.3 years old. Of all 47 primary mediastinal tumors and cysts, 21 patients were male and 26 patients were female. 2. The neurogenic tumors[11 cases, 23.4%] were most frequently encountered and followed by thymoma[10 cases. 21.3%] and teratodermoid[9 cases, 19.1%]. 3. The anterosuperior mediastinum[59.6%] was most common tumor location, and followed by posterior mediastinum[25.6%], middle mediastinum[14.9%]. 4. The most common tumors were thymoma and teratodermoid at anterosuperior mediastinum, benign cyst at middle mediastinum, and neurogenic tumor at posterior mediastinum 5. The malignant tumors were 10 cases[21.3%] of the 47 cases and they were all symptomatic. 6. The most common chief complaint at admission was chest pain or discomfort[34%], and followed by coughing[23.4%], and dyspnea[17%]. 7. The successful complete removal was done in 37cases of benign tumors and cysts. In malignant cases, the surgical intervention was done in 3 cases, and 6 cases were treated with irradiation and chemotherapy, and 1 case was only biopsied. 8. The postoperative complications were developed in 7 cases[14.6%]. There was no case of hospital mortality.

  • Embryonal carcinoma of the mediastinum is a very uncommon disease. This is a report of an embryonal carcinoma in the mediastinum found in a 25 years old Korean male patient who had been suffering from chest pain and intractable coughing for 6 months. 5 weeks prior to this admission hemoptysis and high fever were followed. Right exploratory thoracotomy was performed under the impression of a mediastinal tumor, but found to be unresectable. Irradiation therapy was tried, but no response was observed. Patient expired on 78th day postoperatively.

  • Fine needle aspiration has been effectively being applided to pediatric tumors since it renders a rapid diagnosis with minimal intervention. This measure is especially required for the large pediatric mass, which needs preoperative chemotherapy or radiotherapy to shrink the tumor to an operable size. A case of neuroblastoma of mediastinum, stage IV diagnosed by CT-guided FNA is described.

  • Authors experienced a case of malignant melanoma of the mediastinum, At the time of first visit to the hospital, the female patient, aged 48, and had chief complaints of right shoulder pain that was radiated to its forearm and right Horner`s syndrome. In chest X-ray and CT, extrapulmonary circumscribed mass in the right apicomedial aspect of mediastinum was found. She had no nevus and no melanoma on entire skin area. We excised the tumor and confirmed it as malignant melanoma by histopathology.

  • We report rare cases of myxoid chondrosarcoma and meningomyelocele mimicking as neurogenic tumor in the posterior mediastinum. This lesions clinically mimicked neurogenic tumor due to its location and dumbbell shape appearance. The histogenesis of myxoid chondrosarcoma is discussed as skeletal origin from the thoracic vertebrae, and meningomyelocele is ectopic harmatoma lesion of C.N.S. or meningx. This lesions is concluded that distinguished for the differential diagnosis among Neurogenic tumor arising in the posterior mediastinum.
