  • Although the long-term success of osseointegrated endosseous implants for the support of fixed dental prostheses has been reported, the increasingly widespread use of implant-supported prostheses has led to problems associated with their structural integrity. The most common biomechanical complications observed in dental implant treatment are fracture and screw loosening. The nature of loosening or fracture of dental implant components is complex, since it involves fatigue, fitness, and varied chewing patterns and loads. To assess the service life of the components of the prosthetic system, a knowledge of the loads transmitted through the system is necessary. Design of the final restoration and occlusion in relation to the geometry of a prosthetic restoration has a great influence on the mechanical loading of the implant. It is proposed that control of force in oral cavity may play a larger role in failures than previously believed. Based on theoretic consideration and clinical experiences with dental implant, this article gives simple guidelines for controlling these loads.

St. Jude Medical 기계판막의 임상적 연구 (Clinical Analysis of St. Jude Medical Prosthesis)

  • Result of St. Jude Medical valve replacement are presented in 106 patients who underwent operation from January 1985 through July 1989. The patient were 52 males and 56 females. Total 136 of St. Jude Medical valves were implanted; 91 in mitral position, 45 in aortic position. The hospital mortality rate was 5.7%o[6 patients] and the late mortality rate was 2.0%[2 patients]. The causes of death were low cardiac output in 5, iatrogenic right ventricular rupture in 1, heart failure in 1, ventricular arrhythmia in l. And, the causes of valve related complication were anticoagulant related hemorrhage in 5 patients[0.03% /patient-year] and thromboembolism[0.01% /patient-year] in 2 patients. In conclusion, the performance of the St. Jude Medical valve compare most favorably with other artificial valves. But it remains still hazards of mechanical prosthesis such as thromboembolism and anticoagulant related hemorrhage.

  • Prosthetic valve thrombosis is rare but it is one of fatal complication after heart valve surgery. Improvements of the valve design and the material have decreased the frequency of thrombosis but have not eliminated completely. And some cases of prosthetic valve thrombosis during pregnancy were reported inspite of adequate anticoagulation therapy.Urgent surgical intervention is indicated for prosthetic valve thrombosis but it is associated with high operative risk, therefore medical thrombolytic therapy such as urokinase or streptokinase therapy is regarded as an alternative therapy. This is a case report of the successful thrombolytic therapy for valve thrombosis in a pregnant patient after mechanical mitral valve replacement.

  • The hand is a very specialized organ that functions to obtain information and to execute motor acts essential to human interaction with the environment. Loss of hand function through infections affects the mechanical tasks that the hand performs and psychological adjustments to their disability. Infection is a disastrous complication of hand injuries and adequacy of circulation is of greatest importance to prevention of infection. Careful debridement, incision, and adequate drainage and antibacterial treatment are of great importance. Optimal care of the infected hand demands that carefully surgical care, early postoperative exercises and other therapy. Hand rehabilitation has grown as a specialty area of both physical and occupational therapy. It is essential that the surgeon and therapist work together, and communicate freely-all of which generally require daily contact. Treating the psychological loss suffered by the patient with a hand infections is an integral part of the rehabilitation therapy as well. Treatment techniques, Whether thermal modalities or specifically designed exercises, are used as a bridge to reach a further goal of returning to functional performance.

Bjork-Shiley monostrut 판막의 임상 치험 (Long-term Experience with the Bjork-Shiley Monostrut Valve)

  • One hundred and fourty-one Bjork-Shiley Monostrut valves were implanted in 105 consecutive patients from November 1983 to February 1990. There were 61 male and 44 female patients with a mean age of 33.6 years at the surgery. The cummurative follow-up was 370.3 patient-years with a mean of 44.0 months per patient. The operative mortality rate was 3.8%, and the linealized late mortality was 1.18%/patient-year. The incidence of major complications were 4.59%/patient-year, and the actuarial rate of freedom from valve-related morbidity was 80.2$\pm$4.4% at 7 years. The 7-year survival rate was 90.5$\pm$4.1%, and the actuarial rate of freedom from thromboembolism at 7 years was 93.3$\pm$2.3%. The linealized annual rates of complication were ; structural valve failure 0.27% /patient-year, non-structural valve failure 0.54% /patient-year, thromboembolism 1.62% /patient-year; bleeding 1.62% /patient-year; endocarditis 0.54% /patient-year. On the basis of our experience, we judged the Bjork-Shiley Monostrut valve reliable, with similar incidence of valve-related morbidity of other mechanical prosthetic valves.

  • 중심정맥 삽관은 술기의 안정성과 효용성으로 인해 날로 증가하는 추세이나 그에 의한 합병증도 빈발하게 나타난다. 본 증례는 혈관 천공 등의 여타 합병증 없이 당뇨와 천식을 가진 74세 남자 환자에서 중심정맥 삽관 후 발생한 종격동염을 수술을 통해 성공적으로 치료한 예이다.

Heterotopic Pancreas Presented as Duodenal Tumor with Obstruction

  • Heterotopic pancreas (HP) is defined as pancreatic tissue lacking anatomic and vascular continuity with the main body of the pancreas. Most are asymptomatic, but can cause ulcer, bleeding, intussusception, and mechanical obstruction. Herein, we presented one case of HP presented as duodenal tumor causing duodenal obstruction. A 7-year-old girl visited the emergency room for abdominal pain with vomiting for 24 hours. Computed tomography and upper gastrointestinal series revealed a polypoid mass with short stalk in the 2nd portion of duodenum. We attempted an endoscopic removal. However, the lumen was nearly obstructed by the mass and the stalk was too broad and hard to excise. The mass was surgically removed via duodenotomy. It was confirmed as a HP with ductal and acini components (type 2 by Heinrich classification). Postoperatively, the patient has been well without any complication and recurrence.

  • Results of St. Jude Medical valve replacement are presented in 171 patients who underwent operation from January 1983 through June 1989. The patients were 79 males and 92 females with ages ranging from 12 to 71 years. Total 211 of St. Jude Medical valves were implanted: 148 in mitral position, 57 in aortic position, 6 in tricuspid position. The follow-up was from 2 to 76 months with a cumulative period of 375 patient-year. The actuarial survival at 1 year, 3 year and 5 year were 92.1 %, 87.6 % and 86.3% respectively. The linearlized incidences of valve failure, thromboembolism, thrombotic obstruction, anticoagulation related hemorrhage and all valve related complication were as follows: 0.5 %/pt-yr, 0.5 %/pt-yr, 0.5 %/pt-yr, 1.1 %/pt-yr, and 2.4 %/pt-yr, respectively. The performance of the St. Jude Medical valve compares most favorably with other artificial valves. But it remains still hazards of mechanical prostheses such as thromboembolism and thrombotic obstruction.

  • PHILOS plate fixation in osteoporotic proximal humerus fracture of old age is well-known for high complication rate, especially metal failure, providing various augmentation techniques, such as calcium phosphate cement, allogenous or autologous bone graft. We report a case of polymethyl methacrylate augmentation to provide appropriate reduction with a significant mechanical support. This can be a treatment option for displaced unstable osteoporotic proximal humerus fracture with marked bony defect.

  • Laryngomicrosurgery is common procedure applying to benign laryngeal lesion. Suspension of the laryngoscope is a vital component of Laryngomicrosurgery. Suspension laryngoscopy allows for bimanual surgery and a stable operating platform. Little information is known about oropharyngeal & vocal fold complications of suspension laryngoscopy. Because laryngomicrosurgery is dependent upon suspension laryngoscopy, surgeons should fully understand the risks of suspension laryngoscopy to properly educate and care for patients undergoing suspension laryngoscopy. That is problem to allow otolaryngologist is embarrassing, for voice restoration surgery are not satisfied with the results. The authors reviewed mechanical and phonological complications after laryngomicrosurgery.
