The Results of the Endoscopic Decompression for the Treatment of Haglund's Disease (Haglund씨 병에서 시행한 내시경적 감압술의 결과)

  • Purpose: Haglund's disease represents a painful heel caused by mechanically induced inflammation of the retrocalcaneal bursa and insertional Achilles tendinosis may coexist. Traditional open surgery can cause complications such as skin breakdown, painful scar and altered sensation around the heel. Endoscopic treatment offers the advantages that are related to minimally invasive procedure and we evaluate the clinical results and operative techniques of endoscopic decompression of retrocalcaneal space for Haglund's disease. Materials and Methods: Our retrospective study included seven heels in six consecutive patients for which nonoperative treatment had failed and endoscopic decompression was performed. The mean age was forty-one years (range, 28 to 53 years). All of the patients had typical complaints of inflammation of the retrocalcaneal bursa and Fowler-Philip angle of more than $75^{\circ}$ and positive parallel pitch lines were present on the lateral calcaneal radiograph. The endoscopic procedure consists of the resection of inflamed retrocalcaneal bursa and enough bone to prevent impingement of the bursa between the calcaneus and Achilles tendon. All patients were evaluated with radiologic angle, visual analogue scale (VAS) for pain and Ogilvie-Harris functional score. The mean follow-up was 18 months (range, 15 to 21 months). Results: The mean operation time was 61 minutes (range, 50 to 85 minutes). VAS for pain and Fowler-Philip angle were decreased from preoperative 8.7 and $82^{\circ}$ to postoperative 2.3 and $57^{\circ}$, respectively. One patient with ankylosing spondylitis had a fair result, 2 patients had good results and the remaining 3 patients had excellent results according to Ogilvie-Harris functional score. There were no surgical complications such as infection, Achilles tendon avulsion or abnormal heel sensation. Conclusion: The endoscopic decompression for Haglund's disease was demonstrated to have several advantages including low morbidity, allowance of functional rehabilitation, short recovery time and quick sports resumption. However a comparative study is needed to determine the value of endoscopic decompression and particular caution should be exerted for the enthesiopathy.

Drag Reduction of a Circular Cylinder With O-rings (O-ring을 이용한 원주의 항력감소에 관한 실험적 연구)

  • The flow around a circular cylinder was controlled by attaching O-rings to reduce drag force acting on the cylinder. Four experimental models were tested in this study; one smooth cylinder of diameter D (D=60mm) and three cylinders fitted with O-rings of diameters d=0.0167 D, 0.05D and 0.067 D with pitches of PPD=2D, 1D, 0.5D and 0.25D. The drag force, mean velocity and turbulence Intensity profiles in the near wake behind the cylinders were measured for Reynolds numbers based on the cylinder diameter in the range of Re$_{D}$=7.8$\times$10$^3$~1.2$\times$10$^{5}$ . At Re$_{D}$=1.2$\times$10$^{5}$ , the cylinder fitted with O-rings of d=0.0167D in a pitch interval of 0.25D shows the maximum drag reduction of about 5.4%, compared that with the smooth cylinder. The drag reduction effect of O-rings of d=0.067D is not so high. For O-ring circulars, as the Reynolds number increases, the peak location of turbulence intensity shifts downstream and the peak magnitude is decreased. Flow field around the cylinders was visualized using a smoke-wire technique to see the flow structure qualitatively. The size of vortices and vortex formation region formed behind the O-ring cylinders are smaller, compared with the smooth cylinder.der.

  • A design system for power transmission bevel gears(straight, zerol, and spiral) is developed, in which the strength and durability of bevel gears can be estimated and the size of bevel gears can be minimized by introducing optimal techniques. The size of bevel gear pair as the object function to be minimized is the volume of equivalent spur gear pair at mean normal section, and the design variables to be determined are considered as the number of teeth, face width, diametral pitch, and spiral angle in spiral bevel gear. The strength(bending strength, pitting resistance) according to the AGMA standards, geometrical quantities, and operating characteristics(interference of pinion, contact ratio, etc.) are considered as the constraints in design optimization. The optimization with these constraints becomes nonlinear problem and that is solved with ALM(Augmented Lagrange Multiplier) method. The developed design method is applied to the example designs of straight, zerol, and spiral bevel gears. The design results are acceptable from the viewpoint of strength and durability within the design ranges of all other constraint, and the bevel gears are designed toward minimizing the size of gear pair. This design method is easily applicable to the design of bevel gears used as power transmitting devices in machineries, and is expected to be used for weight minimization of bevel gear unit.

Estimation of Large Amplitude Motions and Wave Loads of a Ship Advancing in Transient Waves by Using a Three Dimensional Time-domain Approximate Body-exact Nonlinear 2nd-order BEM (3 차원 시간영역 근사비선형 2 차경계요소법에 의한 선체의 대진폭 운동 및 파랑하중 계산)

  • A three-dimensional time-domain calculation method is of crucial importance in prediction of the motions and wave loads of a ship advancing in a severe irregular sea. The exact solution of the free surface wave-ship interaction problem is very complicated because of the essentially nonlinear boundary conditions. In this paper, an approximate body nonlinear approach based on the three-dimensional time-domain forward-speed free-surface Green function has been presented. The Froude-Krylov force and the hydrostatic restoring force are calculated over the instantaneous wetted surface of the ship while the forces due to the radiation and scattering potentials over the mean wetted surface. The time-domain radiation and scattering potentials have been obtained from a time invariant kernel of integral equations for the potentials which are discretized according to the second-order boundary element method (Hong and Hong 2008). The diffraction impulse-response functions of the Wigley seakeeping model advancing in transient head waves at various Froude numbers have been presented. A simulation of coupled heave-pitch motion of a long rectangular barge advancing in regular head waves of large amplitude has been carried out. Comparisons between the linear and the approximate body nonlinear numerical results of motions and wave loads of the barge at a nonzero Froude number have been made.

  • In the present paper, an infinite-depth unified theory is applied to the computation of the linear and second-order hydrodynamic forces on slender bodies. No forward speed is assumed, which is valid for some types of ships, like FPSOs and shuttle tankers. Strip theory solution, which is essential for the extension to theory is extended to unified theory, was obtained using NIIRD program developed at MIT. The linear theory is extended to the computation of the second-order mean-drift forces and moment. Furthermore, Aranha's formular is applied to the prediction of wave drift damping coefficients. From this study, it is proved that unified theory provides an accuracy comparable with 3D panel method for the second-order forces as well as the linear solution with much less computational effort.

The Effects of Intralaryngeal Needle Technique in Intracordal Cyst (성대내낭종에서 성대내바늘기법의 효과)

  • Background and Objectives : Surgery is considered the primary treatment for intracordal cyst. However, patients who had undergone surgery are still subject to recurrence and continued voice changes. Intracordal cysts naturally disappear in some patient population. Cyst does not always recur in patients who had received partial surgical removal, too. Contradicting results raises a question whether complete surgical removal of intracordal cyst is necessary and demonstrate need for better treatment. Herein, the author proposes novel surgical method technique intralaryngeal needle technique (INT), a technique using surgical needle for not only injection but also for aspiration and excision of cyst. This study aims to examine the potential of intralaryngeal needle technique in treating intracordal cysts. Materials and Methods : Surgical procedures were done on in-patients diagnosed with intracordal cyst. 23 patients received follow-up screening after the surgery for one year. Patients' subjective satisfaction levels, acoustic measures, aerodynamic measures, laryngeal stroboscopic results were compared before and after the treatment. Results : Overall patients were satisfied with novel surgical excision method. In terms of aerodynamic measures, maximum phonation time, mean air flow rate improved after the surgery. In terms of acoustic measures, Jitter, Shimmer, NHR, and voice pitch changes after the treatment showed statistically significant differences. Laryngeal stroboscopy results showed significant decreases in cyst sizes. Post-surgery patients had improved mucosal waves and amplitudes values. Conclusion : The results show the validity of intralaryngeal needle technique in reducing intracordal cyst size by excision, aspiration, and injection. The author believes this novel technique can be used as an alternative surgical method for intracordal cysts.

The Study on Korean Prosody Generation using Artificial Neural Networks (인공 신경망의 한국어 운율 발생에 관한 연구)

  • The exactly reproduced prosody of a TTS system is one of the key factors that affect the naturalness of synthesized speech. In general, rules about prosody had been gathered either from linguistic knowledge or by analyzing the prosodic information from natural speech. But these could not be perfect and some of them could be incorrect. So we proposed artificial neural network(ANN)s that can be trained to team the prosody of natural speech and generate it. In learning phase, let ANNs learn the pitch and energy contour of center phoneme by applying a string of phonemes in a sentence to ANNs and comparing the output pattern with target pattern and making adjustment in weighting values to get the least mean square error between them. In test phase, the estimation rates were computed. We saw that ANNs could generate the prosody of a sentence.

Results of Kidner Procedure Combined with Medial Displacement Calcaneal Osteotomy for the Symptomatic Accessory Navicular with Hindfoot Valgus (후족부 외반을 동반한 증상이 있는 부주상골 환자에서 시행한 내측 전위 종골 절골술과 Kidner 술식을 동시에 시행한 결과)

  • Purpose: The purpose of this study is to evaluate the results of Kidner procedure combined with medial displacement calcaneal osteotomy (MDCO) in patients with the symptomatic accessory navicular with hindfoot valgus. Materials and Methods: From January 2014 to January 2019, fifteen patients (15 cases) who had undergone a Kidner procedure combined with MDCO for symptomatic accessory navicular with hindfoot valgus were included. Their mean age was 36.3 years old (19~61 years old) and there were 6 males and 9 females. The clinical results were evaluated using visual analogue scale (VAS), American Orthopaedic Foot and Ankle Society (AOFAS) midfoot score, and postoperative subjective satisfaction. The radiographic results were evaluated using the talonavicular coverage angle and the anteroposterior talo-first metatarsal angle, the lateral talo-first metatarsal angle, the calcaneal pitch angle, and the hindfoot alignment angle. The postoperative complications were also evaluated. Results: The VAS and AOFAS midfoot scores continuously improved until 12 months after surgery. Subjective satisfaction after surgery was excellent in 10 cases and good in 5 cases. The hindfoot alignment angle significantly changed after surgery. Pain due to lateral impingement disappeared in five patients, and persisted in one patient. Five patients complained of irritation caused by their fixation devices, and all the symptoms improved after removal of the fixation devices. Conclusion: Kidner procedure combined with MDCO in patients with the symptomatic accessory navicular with hindfoot valgus showed good clinical results with satisfactory correction of hindfoot valgus. In particular, the clinical results showed continuous improvement until 12 months after surgery.

Fundamental Frequencies in Korean Elderly Speakers (한국 정상 노인 음성의 기본주파수)

  • Multiple physical changes of the larynx and its components occur with age. Vocal pitch, commonly expressed through measures of fundamental frequency (Fo) relate to physical conditions of the larynx. Available data is lacking for the senescent voice, and should be applied to the of changes of elderly speakers' Fo characteristics. The purpose of this study was to investigate the Fo of normal elderly speaker's voice. A total of 406 normal elderly speakers (207 males and 199 females) participated in this experiment. Age ranged from 60 years to 89 years. The subjects were asked to produce sustained corner vowels (/a/ /i/ /u/) three times each and the data were analyzed using the MDVP of CSL. According to the results of this study, the mean Fo from the ages of 60's to 80's shows 143.95Hz(SD 13.94) for men and 185.42Hz (SD 15.29) for women. For men, a significant change is found as a function of age in the Fo (F=16.181, p<.05). A post-hoc Scheffe test revealed significant differences between the Fo data of subjects aged 60's and 70's, 60's and 80's. For women, a significant change is found as a function of age in the Fo (F=49.013, p<.05). A post-hoc $Scheff'{e}$ test revealed significant differences between the Fo data of subjects in their 60's and 70's, 70's and 80's, 60's and 80's. The Fo of men goes up from their 60's to 80's gradually, whereas the Fo of women goes down gradually until their 70's, and after their 70's it again increases. It has been known that diminishing estrogen levels in women in old age may be a factor in lowering Fo, whereas diminishing testosterone levels in men may contribute to a rising Fo. This result may be used as some meaningful guideline and lead the basic data to differentiate between normal aged voice and aged voice disorders.

  • The purpose of this study is to assess the ability of balance control in virtual moving surround stimulation using head mount display (HMD) device and force platform in patients with brain injury. Fifteen patients with stroke (mean age 54.47 yrs) and fifteen healthy normal persons participated. COP parameters were obtained total path distance, frequency of anterior-posterior and medial-lateral component by FFT analysis, weight-spectrum analysis in the two different conditions; (1) during comfortable standing with opened or closed eyes, (2) during virtual moving surround stimulation delivered using HMD with four different moving pattern. Moving patterns consisted of close-far, superior-inferior lilting (pitch) , right-left tilting (roll) and horizontal rotation (yaw) movement. In all parameters, the test-retest reliability was high. Also, the construct validity of virtual moving surround stimulation was excellent (p<0.05). A posturographic balance assessment system equiped with virtual moving surround stimulation using HMD is considered clinically useful in evaluation of balance control in patients with brain injury.
