Electric Leakage Point Detection System of Underground Power Cable Using Half-period Modulated Transmission Waveform and Earth Electric Potential Measurement (반주기 변조된 송신파형과 대지전위 측정을 이용한 지중 케이블 누전 고장점 탐지 시스템)

  • The precise detection of electric leakage point of underground power cable is very important to reduce cost and time of maintenance and prevent electric shock accident through expedite repair of electric leakage point. This paper proposes a electric leakage point detection system underground power cable using of half-period modulated transmission waveform and earth electric potential measurement. The developed system is composed of transmitter to generate the wanted pulse waveform, receiver to measure and display earth electric potential by the transmitted pulse in electric leakage point and PC Software program to display of GPS coordinate on detection cable line. The performance of the electric leakage point detection system was tested in the constructed underground cable leakage detection test bed. The test results on signal generation voltage precision of signal transmitter, mean detection earth voltage, mean detection leakage current and electric leakage point detection error showed the developed system can be used in electric leakage point detection underground power cable.

Correlation between contrast leakage period of procedural rupture and clinical outcomes in endovascular coiling for cerebral aneurysms

  • Objective: Intraprocedural rupture (IPR) is a fatal complication of endovascular coiling for cerebral aneurysms. We hypothesized that contrast leakage period may be related to poor clinical outcomes. This study aimed to retrospectively evaluate the relationship between clinical outcomes and contrast leakage period. Methods: Data from patients with cerebral aneurysms treated via endovascular coiling between January 2010 and October 2018 were retrospectively assessed. The enrolled patient's demographic data, the aneurysm related findings, endovascular treatment and IPR related findings, rescue treatment, and clinical outcome were analyzed. Results: In total, 2,859 cerebral aneurysms were treated using endovascular coiling during the study period, with IPR occurring in 18 (0.63 %). IPR occurred during initial frame coiling (n=4), coil packing (n=5), stent deployment (n=7), ballooning (n=1), and microcatheter removal after coiling (n=1). Tear sites included the dome (n=14) and neck (n=4). All IPRs were controlled and treated with coil packing, with or without stenting. Flow arrest of the proximal balloon was not observed. Temporary focal neurological deficits developed in two patients (11.1%). At clinical follow-up, 14 patients were classified as modified Rankin Scale (mRS) 0, three as mRS 2, and one as mRS 4. The mean contrast leakage period of IPR was 11.2 min (range: 1-31 min). Cerebral aneurysms with IPR were divided into late (n=9, mean time: 17.11 min) and early (n=9, mean time: 5.22 min) control groups based on the criteria of 10 min of contrast leakage period. No significant between-group differences regarding clinical outcomes were observed after IPR (p=1). Conclusions: In our series, all patients with IPR were controlled with further coil packing or stenting without proximal balloon occlusion within 31 min of contrast leakage. There was no difference in clinical outcomes when the long contrast leakage period group and short contrast leakage period group were compared.

  • We give new definition of the effective-leakage and the signal to effective-leakage plus noise ratio (SELNR) to consider receiver combining motivated by the leakage. We propose a method to find jointly beamforming vector and combining vector for the two linear receivers (maximal ratio combining (MRC) receiver and minimum mean square error (MMSE) receiver) based on the SELNR.

Laparoscopic Reinforcement Suture (LARS) on Staple Line of Duodenal Stump Using Barbed Suture in Laparoscopic Gastrectomy for Gastric Cancer: a Prospective Single Arm Phase II Study

  • Purpose: Laparoscopic gastrectomy is accepted as a standard treatment for patients with early gastric cancer in Korea, Japan, and China. However, duodenal stump leakage remains a fatal complication after gastrectomy. We conducted a prospective phase II study to evaluate the safety of the new technique of laparoscopic reinforcement suture (LARS) on the duodenal stump. Materials and Methods: The estimated number of patients required for this study was 100 for a period of 18 months. Inclusion criteria were histologically proven gastric adenocarcinoma treated with laparoscopic distal or total gastrectomy and Billroth II or Roux-en-Y reconstruction. The primary endpoint was the incidence of duodenal stump leakage within the first 30 postoperative days. The secondary endpoints were early postoperative outcomes until discharge. Results: One hundred patients were enrolled between February 2016 and March 2017. The study groups consisted of 65 male and 35 female patients with a mean age (years) of 62.3. Of these, 63 (63%) patients had comorbidities. The mean number of retrieved lymph nodes was 38. The mean operation time was 145 minutes including 7.8 minutes of mean LARS time. There was no occurrence of duodenal stump leakage. Thirteen complications occurred, with one case of reoperation for splenic artery rupture and one case of mortality. Conclusions: Based on the results of this prospective phase II study, LARS can be safely performed in a short operation period without development of duodenal stump leakage. A future randomized prospective controlled trial is required to confirm the surgical benefit of LARS compared to non-LARS.

Leakage and scattered radiation from hand-held dental x-ray unit (이동용 치과 X선 발생장치의 누설 및 산란 선량에 관한 연구)

  • Purpose: To compare the leakage and scattered radiation from hand-held dental X-ray unit with radiation from fixed dental X-ray unit. Materials and Methods: For evaluation we used one hand-held dental X-ray unit and Oramatic 558 (Trophy Radiologie, France), a fixed dental X-ray unit. Doses were measured with Unfors Multi-O-Meter 512L at the right and left hand levels of X-ray tube head part for the scattered and leakage radiation when human skull DXTTR III was exposed to both dental X-ray units. And for the leakage radiation only, doses were measured at the immediately right, left, superior and posterior side of the tube head part when air was exposed. Exposure parameters of handheld dental X-ray unit were 70 kVp, 3 mA, 0.1 second, and of fixed X-ray unit 70 kVp, 8 mA, 0.45 second. Results: The mean dose at the hand level when human skull DXTTR III was exposed with portable X-ray unit $6.39{\mu}Gy$, and the mean dose with fixed X-ray unit $3.03{\mu}Gy$ (p<0.001). The mean dose at the immediate side of the tube head part when air was exposed with portable X-ray unit was $2.97{\mu}Gy$ and with fixed X-ray unit the mean dose was $0.68{\mu}Gy$ (p<0.01). Conclusions: The leakage and scattered radiation from hand-held dental radiography was greater than from fixed dental radiography.

ELECTROCHEMICAL STUDY ON THE SEALING EFFECT OF CALCIUM HYDROXIDE-BASED SEALERS (수종 수산화칼슘 Sealer의 근관폐쇄효과에 관한 전기화학적연구)

  • The purpose of this electrochemical study was to determine and compare the sealing effect of three commonly used calcium hydroxide-based sealers (Sealapex, Apexit, and CRCS) with that of Roth 801 which is a ZOE-based sealer. 64 single rooted teeth were used in this experiment. After removing the crown, the teeth were devided into 4 groups of 15 in each and obturated with gutta-percha and experimental sealers. 4 teeth were as controls(2 positive and 2 negative). The results were as follows : 1. During the observation period, all sealers showed varying degree of microleakage. 2. The mean leakage currents according to the electrochemical study of each group showed $0.418{\pm}0.006mV$ for Roth 801, $2.03{\pm}0.035mV$ for Sealapex, $3.33{\pm}0.069mV$ for Apxit and $6.48{\pm}0.097mV$ for CRCS group. The positive control group showed 600mV. 3. There were statistically significant difference in mean leakage among experimental groups. ZOE-typed Roth 801 sealer was the lowest, and Sealapex, Apexit, CRCS group in that order showed increased leakage(P<0.05). 4. Roth 801, Sealapex and Apexit group showed increasing leakage with time, but CRCS group showed stable or decreasing leakage tendency.

EFFECTIVENESS OF CUSTOMIZED MASTER CONE ON APICAL SEALING IN VARIOUS APICAL SIZE OF PREPARED ROOT CANALS (근단부 크기에 따른 customized master cone의 치근단 밀폐효과에 관한 연구)

  • The purpose of this study is to evaluate the effectiveness of customized master cone on apical sealing in various apical size of prepared root canals, that is MAF(Master Apical File) and to know at which apical size the apical leakage is to be significantly reduced using customized master cone. 120 extracted single rooted premolars were divided into four groups according to their apical size(MAF), #30, 40, 50 and 60. And then, each group was subdivided into three in accordance with three obturation methods, lateral condensation with standardized master cone, lateral condensation with chloroform-dipped customized master cone, and continuous wave of obturation technique. Resorcinol-formaldehyde resin was used for the microleakage test of this study. Teeth were sectioned horizontally at 1.5mm(Level 1), 2.5mm(Level 2), and 3.5mm(Level 3) from the anatomical root apex using low speed microtome. All sections were examined under $\times$40 magnification with a stereomicroscope, photographed, and then scanned. With the scanned images, resin-infiltrated area presenting the microleakage was calculated using SigmaScan/Image, and the ratio of leakage to the total root canal area of each group was analyzed statistically(one way ANOVA). The results were as follows ; 1. In groups of MAF #30, there was no significant difference of mean leakage ratio among three obturation methods at all three levels. 2. In groups of MAF #40, the group using lateral condensation with customized master cone had the low-est mean leakage ratio at all three levels, but there was no significant difference among three obturation techniques. 3. In groups of MAF #50, the mean leakage ratio of the group using lateral condensation with standard master cone was the highest among those of three obturation techniques at level 1, and this difference was statistically significant(p<0.05). 4 In groups of MAF #60, the groups using lateral condensation with standard master cone had also the highest mean leakage ratio at all levels, but there was no significant difference at level 1 and 2. At level 3, the leakage of the group using lateral condensation with standard master cone was significantly higher than that of the group using continuous wave of obturation(p<0.05). The results of this study suggested that the obturation method using customized master cone or the continuous wave of obturation is more effective for apical sealing than that using standardized master cone when MAF is larger than #50.

Acidic Pelvic Drainage as a Predictive Factor For Anastomotic Leakage after Surgery for Patients with Rectal Cancer

  • Purpose: To demonstrate the value of sequential determinations of pelvic drainage in the identification of increased risk of anastomotic leakage (AL) after anterior resection for rectal cancer with a double stapling technique. Patients and Methods: Between January 2004 and December 2011, data for the daily postoperative pH of pelvic drainage fluid in 753 consecutive patients with rectal cancer who initially underwent anterior resection with a double stapling technique were reviewed. All patients experienced a total mesorectal excision. Patients with anastomotic leakage (Group AL, n=57) were compared to patients without leakage (Group nAL, n=696). Patients with perioperatively abdominopelvic implants that were likely to affect pH value (determined at $25^{\circ}C$) other than leakage were excluded. Mean postoperative values were compared. Results: Anastomotic leakage was noted in 57 (7.6%) of 753 patients with rectal cancer. The diagnosis of AL was made between the $6^{th}$ and $12^{th}$ postoperative day (POD; mean $8^{th}$ POD). There was no significance of the daily average values of pH on POD1 & 2 in group AL while a significantly sharp declining mean pH value reached its diagnostic point of AL (p<0.001) on POD3. A cut-off value of 6.978 on the $3^{rd}$ POD maximized the sensitivity (98.7.0%) and specificity (94.7%) in assessing the risk of leakage. Conclusion: According to these results, an early and persistent declining of pH value of pelvic drainage fluid after rectal surgery with anastomosis, is a marker of AL. A cut-off value of 6.798 determined at $25^{\circ}C$ on POD3 maximizes sensitivity and specificity.

Filtration efficiency and Manikin-based Total Inward Leakage Study of Particle Filtering Mask Challenged with Silver Nanoparticles (은나노입자에 대한 방진마스크 포집효율 및 총누설율)

  • Objectives: The production and use of nanoparticles have been increased. In 2014 Workplace Survey Results, 335 companies produce and treat nanoparticls. However, lack of data on nano-toxicity and a method for risk management and regulation on nanoparticles and the standard test method are not sufficient. Protective equipment selection guidelines for nanoparticles are not established. It is required to carry out respirator efficiency test against nanoparticles. This study was performed to evaluate filtration efficiency and manikin-based total inward leakage of particle filtering mask using in Korean country challenged with silver nanoparticles. Methods: We investigated filtration efficiency and total inward leakage of 7 respirator with silver nanoparticle. Results: The geometric mean diameters of Silver nanoparticles were 30 nm and number concentration were about $10^6{\sharp}/cm^3$. Filtration efficiency of six of the seven particle filtering masks was more than 98% and one particle filtering masks filtration efficiency was 94.9%. The filtration efficiency of particle filtering masks to 20 nm silver nanoparticels was highest. Artificial breathing machine with manikin based total inward leakage were 7.6% ~ 42.3%. Conclusions: The results of this study nano-silver filter efficiency was high but the total inward leakage was higher than filter penetration. Therefore, education on how to wear a respirator should be demanded. Especially for workers handling nanoparticles and toxic material, user seal checking and fit test must be performed.

Leakage and Scattered Radiation from X-ray Unit in Radiography (영상의학과 엑스선 발생장치의 누설 및 산란선량 측정)

  • The purposed of this study were measured the radiation exposure of patients and workers by generators, and the protection state for radiation facilities. The subject of the study by X-ray generators in university hospitals of capital area, we measured the maximum irradiation condition of 80 kVp, 200 mA, 0.1 second in the control entrance, control room window, entrance of radiography, adjacent site. The leakage dose per week was which the control entrance was 0.11 mR/week, control room window was 0.15 mR/week, entrance of radiography was 0.12 mR/week and adjacent site was 0.06 mR/week with X-ray unit the mean And the leakage mean dose was 0.11 mR/week. Diagnostic X-ray tubes must ensure that the leakage radiation in the maximum leakage dose in week emitted by the tube outside the useful beam does not exceed certain levels provided by standards.
