Treatment Protocol for Cleft Lip and/or Palate Children in Kyushu University Hospital

  • Our Team Approach consists of following five stages; (1) Peri-natal care until lip repair After ultrasound diagnosis, some obstetricians recommend the mother with CL/P fetus to undergo prenatal counseling in our CLP clinic. On the day the CL/P baby was born, our oral surgeon, nurse, and pedodontist visit the maternity clinic, and take counseling and take impression for a feeding plate. The cheiloplasty is performed in three months old. (2) From lip repair to palatal repair At one year of age, Otorhinolaryngologist checks middle-ear disease. Palatoplasty is carried out at 1.5 - 2 years old. (3) In deciduous and early mixed dentitions Speech is the most important issue in social life for the CL/P subjects, therefore the training of velopharyngeal function is essential. Orthodontist monitors dentofacial development from 5 years of age. In the case of severe maxillary under-growth or severe collapse, maxillary protractor or lateral expansion is indicative, respectively. In early mixed dentition, upper central incisor on the cleft area erupts with some torsion, and then the traumatic occlusion with tooth torsion must be corrected. (4) In mixed dentition Right before the eruption of upper canines, secondary bone grafting is performed. One year prior to the operation, maxillary fan-type expansion is carried out to correct the collapse of maxillary segments. Following the surgical operation, the erupted canine will be moved into the transplanted bone to avoid alveolar resorption. (5) In permanent dentition Final tooth alignment is carried out after eruption of second molars. Some cases may require orthognathic surgery after physical maturation. Prosthetic oral rehabilitation including the dental-implant is carried out after age eighteen.

  • 구강악안면부위의 선천성 기형은 복잡한 발생학적 과정과 연관이 있으며 비정상적인 기형을 적절한 시기에 치료하지 못할 경우 이로 인한 이차적인 기형을 유발하여 치료를 더욱 어렵게 할 수 있다. 저자 등은 우측 상악돌기 후외하방에 중복 발생된 상악돌기와 과잉치로 인하여 저작장애를 보이며, 안와, 관골 및 상하악골의 심한 안모 비대칭 소견을 보이는 악안면기형환자를 골절제와 발치, 교정치료, 관골과 안와재건술, 악교정수술 및 반흔교정술 등의 4회에 걸친 수술을 통해 심미적, 기능적으로 만족스러운 결과를 얻었다. 악안면영역의 선천성 기형의 치료는 매우 난이하며 수차례의 수술을 요하고 수술 결과가 만족스럽지 못한 경우가 많으므로 사전 철저한 분석이 요구되며, 환자의 심리적 부담을 덜어주기 위한 특별한 배려가 요구된다.

Effectiveness of the Invisalign Mandibular Advancement Appliance in Children with Class II Division 1 Malocclusion

  • Background: This study aimed to determine the skeletal and dental effects in pediatric and adolescent Korean patients with Class II Division 1 malocclusion treated using the Invisalign Mandibular Advancement (MA®) appliance. Methods: The study included patients aged 6 to 18 years who received orthodontic treatment with the MA® appliance for Class II Division 1 malocclusion at the Department of Pediatric Dentistry, Wonkwnag University Daejeon Dental Hospital, between July 1, 2018, and December 31, 2021. The treatment group consisted of 20 patients, 10 boys and 10 girls. The control participants were also 10 boys and 10 girls. Lateral cephalometric radiographs were taken before and after treatment, and 41 measurements of skeletal and dental changes were measured and analyzed using the V-CephTM 8.0 (Osstem Implant). All analyses were performed using SPSS software (IBM SPSS for Windows, ver 26.0; IBM Corp.), and statistical significance was tested using paired and independent samples t-tests for within-group and between-group comparisons, respectively. Results: The patients in the treatment group showed significant decreases in ANB (A point, Nasion, B point), maxillary protrusion, maxillary anterior incisor labial inclination, and maxillary protrusion after treatment. However, when compared with the growth changes observed in the control group, only ANB and maxillary protrusion decreased, with no significant differences in SNA, SNB, and mandibular length. Conclusion: Collectively, the results of this study confirm that the use of MA® appliance in pediatric and adolescent Korean patients with Class II Division 1 malocclusion results in a reduction of anteroposterior skeletal and dental disharmony.

Differences in the panoramic appearance of cleft alveolus patients with or without a cleft palate

  • Purpose: The purpose of this study was to clarify the panoramic image differences of cleft alveolus patients with or without a cleft palate, with emphases on the visibility of the line formed by the junction between the nasal septum and nasal floor(the upper line) and the appearances of the maxillary lateral incisor. Materials and Methods: Panoramic radiographs of 238 patients with cleft alveolus were analyzed for the visibility of the upper line, including clear, obscure or invisible, and the appearances of the maxillary lateral incisor, regarding congenital absence, incomplete growth, delayed eruption and medial inclination. Differences in the distribution ratio of these visibility and appearances were verified between the patients with and without a cleft palate using the chi-square test. Results: There was a significant difference in the visibility distribution of the upper line between the patients with and without a cleft palate (p<0.05). In most of the patients with a cleft palate, the upper line was not observed. In the unilateral cleft alveolus patients, the medial inclination of the maxillary lateral incisor was more frequently observed in patients with a cleft palate than in patients without a cleft palate. Conclusion: Two differences were identified in panoramic appearances. The first was the disappearance (invisible appearance) of the upper line in patients with a cleft palate, and the second was a change in the medial inclination on the affected side maxillary lateral incisor in unilateral cleft alveolus patients with a cleft palate.

편측성 완전 구순구개열 환자의 포괄적 치료 (COMPREHENSIVE TREATMENT OF UNILATERAL COMPLETE CLEFT LIP AND PALATE)

  • Cleft lip and palate is one of the congenital anomalies which need comprehensive and multidisciplinary treatment plan because 1) oral cavity is an important organ with masticatory function as a start of digestive tract, 2) anatomic symmetry and balance is esthetically important in midfacial area, and 3) it is also important to prevent psycho-social problems by adequate restoration of normal facial appearance. There are many different protocols in the treatment of cleft lip and palate, but our department has adopted and modified the $Z{\"{u}}rich$ protocol, as published in the Journal of Korean Cleft Lip and Palate Association in 1998. The first challenge is feeding. Type of feeding aid ranges from simple obturators to active orthopedic appliances. In our department we use passive-type plate made up of soft and hard acrylic resin which permits normal maxillary growth. We use Millard's method to restore normal appearance and function of unilateral complete cleft lip. In consideration of both maxillary growth and phonetic problems, we first close soft palate at 18 months of age and delay the hard palate palatoplasty until 4 to 5 years of age. When soft palate is closed, posterior third of the hard palate is intentionally not denuded to allow normal maxillary growth. In hard palate palatoplasty the mucoperiosteum of affected site is not mobilized to permit residual growth of the maxilla. We have treated a patient with unilateral complete cleft lip and palate by Ajou protocol, which is a kind of modified $Z{\"{u}}rich$ protocol. It is as follows: Infantile orthopedics with passive-type plate such as Hotz plate, cheiloplasty with Millard's rotation-advancement flap, and two stage palatoplasty. It is followed by orthodontic treatment and secondary osteoplasty to augment cleft alveolus, orthognathic surgery, and finally rehabilitation with conventional prosthodontic treatment or implant installation. The result was good up to now, but we are later to investigate the final result with longitudinal follow-up study according to master plan by Ajou protocol.

한국인 여학생 상악 치조골의 성장 양상 분석 (AN ANALYSIS OF THE GROWTH PATTERN OF MAXILLARY ALVEOLAR BONE OF THE KOREAN GIRLS)

  • 상악 치조골은 상악골의 일부로 치근을 감싸고 지자하는 역할을 하며 치아의 결손 시 지지구조물로 치료에 중요한 역할을 하게 된다. 지금까지 치조골의 성장에 관한 연구는 교정적 치료의 관점에서 이루어져 영구치 결손을 위한 치료법을 위한 자료로 사용하기에는 한계가 있다. 따라서 본 연구에서는 성장기에 있는 한국인 여학생들의 측모두부규격방사선사진들을 연도 별로 수집하여 상악 치조골의 성장 양상을 분석하였다. 한국인 여학생들의 만 8세에서 만 14세까지 2년 간격으로 측모두부규격방사선 사진, 키, 역연령을 수집하였다. 여섯 항목을 계측하여 각 항목의 평균 최소값 및 최대값을 구하고 각 예측 항목과 역연령, 키 사이의 상관관계 및 동일 항목에서 나이 군사이의 상관관계를 분석하였으며 성장량 예측을 위한 선형회귀분석을 시행하였다. 상악 치조골은 14세까지 계속 성장하였으나 성장속도는 점차 감소하여 키의 성장 양상과 차이를 보였다. 역연령 및 키와 치조골 높이 사이의 상관 계수는 차이를 보이지 않았으며, 역연령 군 사이의 상관계수는 매우 높게 나타났다. 따라서 역연령을 기초로 하는 선형회귀분석을 시행하여 14세 때의 치조골 높이 예측 공식을 작성하였다. 이러한 결과는 소아 상악 치조골의 성장 양상 이해와 성장량을 예측을 가능하게 하여 임플란트 시술과 같은 적극적 치료에 도움을 줄 수 있을 것으로 기대된다.

골격성 제III급 부정교합 환자에 대한 상악골 전방견인 장치의 치료효과 (TREATMENT EFFECT OF PROTRACTION HEAD GEAR ON SKELETAL CLASS III III MALOCCLUSION)

  • Before 1970, mandibular overgrowth was known as main cause of skeletal Class III malocclusion in growing children ; however, recent study reports that many skeletal Class III malocclusion patients also show maxillary deficiency. Since 1972, when Delaire re-accommodated Protraction Head Gear (P.H.G.), many researchers have reported that skeletal Class III discrepancies could be corrected through use of P.H.G., which induces anterior movement of maxilla and change in mandibular growth pattern into infero-posterior direction ; nevertheless, it is very difficult to predict resultant changes of orofacial region. The purpose of this study was to find out what treatment effect P.H.G. has on different study samples. Author divided 51 skeletal Class III malocclusion patients with maxillary deficiency who were treated with P.H.G. into different study groups depending on sex, treatment beginning age, intraoral appliance, and facial growth pattern. By doing so, following results were obtained. 1. Treatment beginning age and Sex Four age groups (5.8 to 8 year-old, 8 to 10 year-old, 10 to 12 year-old, 12 to 14 year-old) were compared, and no significant difference was observed. (p<0.05) There was no significant difference between the sex groups, either. (p<0.05). 2. Intraoral appliance Treatment effects of study groups that used R.P.E.(mean age of 10.2) and Labio-Lingual appliance(mean age of 8.9) were compared. There was no significant difference depending on the type of intraoral appliance that was used. (p<0.05) 3. Facial growth pattern 1) Amounts of SNB and ANB corrections were smaller in clockwise growth pattern group than those in normal or counterclockwise growth pattern group. (p<0.05) 2) Amounts of increase in Wits appraisal and mandibular plane angle were greater in counterclockwise growth pattern group than those in normal or clockwise growth pattern group. (p<0.05) 3)Amounts of increase in articular angle were greater in counter lockwise growth pattern group than those in clockwise growth pattern group. (p<0.05)

쏠종개, Plotosus lineatus Thunberg 수염의 상대성장 (Relative Growth of Barbels in Striped Sea Catfish, Plotosus lineatus Thunberg)

  • To facilitate estimation of the barbel size of a striped sea catfish, Plotosus lineatus Thumberg total length (TL) and head length (HL) against body weight (BW), HL against TL, and 1st maxillary barbel $(MxBL_1)$, 2nd maxillary barbel $(MxBL_2)$, 1st mandibular barbel $(MnBL_1)$ and 2nd mandibular barbel $(MnBL_2)$ against HL were regressed. The relationship of TL and HL for BW were described by the equation $TL=50.9373BW^{0.3072}\;(r^2=0.9898)$ and $HL=11.2938BW^{0.3144}(r^2=0.9572)$, respectively. The relationship of HL for TL was described by the linear equation $HL=0.1982TL+2.1996(r^2=0.9568)$. The relationship of each barbel for HL described by the equation of $HxBL_1=0.04420HL+0.3105(r^2=0.9615),\;MxBL_2=0.4592HL+0.5321(r^2=0.9519), MnBL_l=0.4057HL+1.9824 (r^2=0.9465)\;and\;MnBL_2=0.4355HL+1.8010(r^2=0.9429)$. Knowledge of the relative growth patterns about each barbel of this species is important for the propagation of seed as stock for large-scale striped sea catfish culture.

성장기 아동에서 Activator-Headgear를 이용한 II급 부정교합의 치험례 (ACTIVATOR-HEADGEAR COMBINATION THERAPY IN CASE WITH CLASS II MALOCCLUSION CHILDREN)

  • 골격성 II급 1류 부정교합을 가지는 아동에서 상악골의 과도한 성장은 흔히 전후방적 요소 뿐 아니라 수직적 요소도 가지고 있는데, 그 이유는 상악골이 전하방으로 성장, 이동할 경우 하악골이 후하방 회전하기 때문이며, 하악골의 성장이 전방으로 이루어지는 것을 방해하게 된다. 따라서 이 경우 치료목표는 하악골이 더욱 돌출 되어 상악골과 정상적인 관계를 가지도록 성장하는 동안 상악골의 성장을 억제하는 것이며, 이를 위해 구외력을 적용하는 것이 현명한 방법으로 보고되고 있다. 조선대학교 소아치과에 내원한 골격성 II급 1류 부정교합 환자에서 C II activator와 headgear를 이용한 구외력을 적용후 양호한 결과를 얻었기에 보고하는 바이다.

편측 구순구개열 환자에서 ANS 골절단술을 이용한 코 비대칭의 교정 (ANS Repositioning for Correction of Asymmetric Nose in Unilateral Cleft Lip and Palate)

  • Patients with unilateral cleft lip and palate (UCLP) generally demonstrate the asymmetries in the lip, nose and the naso-maxillary complex. And their skeletal asymmetries are known to be derived from the displacement of nasal septum, anterior nasal spine (ANS) and the pre-maxilla toward the non-affected side during the developmental and growth period. Due to the interruption of the important facial muscles, which are critical for the symmetric growth of premaxilla, functional matrix system fails to develop and results in the displacement of the ANS toward the non-affected side and nasal asymmetry. Therefore the rhinoplasty for CLP patients is required to have inter-skeletal and muscular rearrangement in the naso-maxillary complex in order to let them recover from esthetic and physiologic imbalances. And functional cheilorhinoplasty (FCR) has been a representative treatment of choice for this concept of treatment modality. The outcome and prognosis of primary or repair FCR have been known to be definitely affected by timing of the operation as well as adequate reconstruction of naso-labial muscles. However, sometimes FCR has an ineffective treatment results for patients after the facial growth period, and the limited rhinoplasty around the nose often fails to bring satisfying results. In order to circumvent this limitation, we performed ANS osteotomy for patients with unilateral CLP showing asymmetric nose, as an alternative way for corrective rhinoplasty. We could observe that the nose was rearranged along the facial mid-line by this osteotomy design and asymmetries were evidently improved postoperatively. Here we present this osteotomy method in CLP patients.
