미성숙 영구치의 치근 파절 시, 치수 및 치근 치유에 영향 미치는 요인들에 대한 분석 (Factors Affecting the Pulp and Root Healing of Root Fractures in Immature Permanent Teeth)

  • 본 연구는 미성숙 영구치에서 치근 파절 시 치수와 치근 치유에 영향을 주는 요인을 평가하기 위해 치근 파절위치, 치근단공의 크기, 파절편 간격을 측정하였고, 치수와 치근 치유 양상과의 연관성을 통계적으로 분석하여 치근 파절시 예후를 알아보고자 하였다. 51개의 상악 전치부 치근 파절 증례를 대상으로 치근 파절위치, 치근단공의 크기, 파절편 간격을 방사선 사진에서 길이를 측정하였다. 측정된 수치는 독립 T검증, 로지스틱 회귀분석을 통해 평가하였다. 본 연구의 결과, 치근 파절의 위치에 따른 치수 치유의 예후의 차이는 없었다. 그러나 치근 파절의 위치가 치근단에 위치할수록 치근 치유는 잘 일어났으며(p < 0.05), 정복 후 파절편 간격이 좁을수록 치수 치유가 잘 일어남을 확인하였다(p < 0.05).

A comparative evaluation of fracture resistance of endodontically treated teeth restored with different post core systems - an in-vitro study

  • PURPOSE. To compare the fracture resistance and the mode of failure of endodontically treated teeth restored with different post-core systems. MATERIALS AND METHODS. Root canal treatment was performed on 40 maxillary incisors and the samples were divided into four groups of 10 each. For three experimental groups post space preparation was done and teeth were restored with cast post-core (Group B), stainless steel post with composite core (Group C) and glass fiber post with composite core using adhesive resin cement (Group D). Control group (A) samples were selected with intact coronal structure. All the samples were prepared for ideal abutment preparation. All the samples were subjected to a load of 0.5 mm/min at $130^{circ}$.until fracture occurred using the universal testing machine. The fracture resistance was measured and the data were analyzed statistically. The fracture above the embedded resin was considered to be favorable and the fracture below the level was considered as unfavorable. The statistical analysis of fracture resistance between different groups was carried out with t-test. For the mode of failure the statistical analysis was carried out by Kruskal-Wallis test and Chi-Square test. RESULTS. For experimental group Vs control group the fracture resistance values showed significant differences (P<.05). For the mode of failure the chi-square value is 16.1610, which means highly significant (P=.0009) statistically. CONCLUSION. Endodontically treated teeth without post core system showed the least fracture resistance demonstrating the need to reinforce the tooth. Stainless steel post with composite core showed the highest fracture resistance among all the experimental groups. Teeth restored with the Glass fiber post showed the most favorable fractures making them more amenable to the re-treatment.

  • Purpose: The purpose of this study is to evaluate the effectiveness of internal fixation method of $Medpor^{(R)}$ implant with $BioSorb^{TM}FX$ screw which is used for prevention of enophthalmos in posteriorly extended large orbital floor fracture. Methods: From Jun. 1997 to Dec. 2007, 21 patients who were diagnosed with posteriorly extended large orbital floor fractures were classified into two groups. One group(n=11) had undergone reduction surgery with regular $Medpor^{(R)}$ sheets without any fixation method, while the other group(n=10) had their $Medpor^{(R)}$ sheets fixed with the $BioSorb^{TM}FX$ screws. The two groups were evaluated by comparison of their enophthalmos degree and effectiveness. Results: In the non-fixation group, six patients had enophthalmos preoperatively and three of them showed persistent enophthalmos postoperatively. In postoperative CT examination, displacement of $Medpor^{(R)}$ implant with soft tissue impaction into maxillary sinus was observed in the patients. In the screw fixation group, three patients had enophthalmos preoperatively, but none of them suffer from complication such as residual enophthalmos, soft tissue impaction, muscle entrapment or optic nerve compression postoperatively. Conclusion: Internal fixation method of $Medpor^{(R)}$ implant with $BioSorb^{TM}FX$ screw on the medial surface of orbital floor provides firm stabilization of implants and surrounding soft tissues and can be an effective option especially when postoperative implant displacement or malposition was expected.

안와하벽 파열 골절에 대한 내시경적 접근법만을 이용한 재건술 (Orbital Floor Reconstruction through Endoscopic Transnasal Approach Alone)

  • Purpose: Many surgical methods for reconstruction of orbital floor fracture have been reported, which include subciliary approach, transconjunctival approach, transantral and transnasal endoscopic approach, etc. The purpose of this study is to demonstrate a surgical technique and analyze the results of transnasal endoscopic approach with Foley catheter ballooning without implantation of artificial surgical material through subciliary approach. Methods: Between February 2007 and November 2010, 29 orbital floor fracture patients, who had no herniated muscles through bone fragments, were treated through transnasal endoscopic approach with Foley catheter ballooning. Under the endoscopic view, the operator identified the opening of maxillary sinus. After widening of the opening using forceps, the operator reduced the fragmented bone with curved suction tip. Thereafter, 18-Fr Foley catheter was inserted. Four weeks after the operation, the catheter was removed. Results: Preoperatively, 6 patients had diplopias, 4 patients had limitations of extraocular motions and 3 patients had enophthalmos. After removal of the Foley catheter 4 weeks after the operation, 2 patients had diplopias, 1 patient had a limitation of extraocular motion, 1 patient has an enophthalmos and 1 patient had numbness on the cheek. These symptoms were resolved about 6 months after the surgery. Conclusion: The operative technique of Foley catheter ballooning through transnasal endoscopic approach without implantation of the artificial surgical material through subciliary approach can be considered one of the appropriate techniques for orbital floor fracture.

Effect of core design on fracture resistance of zirconia-lithium disilicate anterior bilayered crowns

  • PURPOSE. The effect of core design on the fracture resistance of zirconia-lithium disilicate (LS2) bilayered crowns for anterior teeth is evaluated by comparing with that of metal-ceramic crowns. MATERIALS AND METHODS. Forty customized titanium abutments for maxillary central incisor were prepared. Each group of 10 units was constructed using the same veneer form of designs A and B, which covered labial surface to approximately one third of the incisal and cervical palatal surface, respectively. LS2 pressed-on-zirconia (POZ) and porcelain-fused-to-metal (PFM) crowns were divided into "POZ_A," "POZ_B," "PFM_A," and "PFM_B" groups, and 6000 thermal cycles (5/55 ℃) were performed after 24 h storage in distilled water at 37 ℃. All specimens were prepared using a single type of self-adhesive resin cement. The fracture resistance was measured using a universal testing machine. Failure mode and elemental analyses of the bonding interface were performed. The data were analyzed using Welch's t-test and the Games-Howell exact test. RESULTS. The PFM_B (1376. 8 ± 93.3 N) group demonstrated significantly higher fracture strength than the PFM_A (915.8 ± 206.3 N) and POZ_B (963.8 ± 316.2 N) groups (P<.05). There was no statistically significant difference in fracture resistance between the POZ_A (1184.4 ± 319.6 N) and POZ_B groups (P>.05). Regardless of the design differences of the zirconia cores, fractures involving cores occurred in all specimens of the POZ groups. CONCLUSION. The bilayered anterior POZ crowns showed different fracture resistance and fracture pattern according to the core design compared to PFM.

Microsurgical re-treatment of an endodontically treated tooth with an apically located incomplete vertical root fracture: a clinical case report

  • Although it is challenging, the early diagnosis of a vertical root fracture (VRF) is crucial in order to ensure tooth preservation. The purpose of this clinical case report was to describe reparative surgery performed to treat a tooth affected by an incomplete VRF. A 26 year old male patient was suspected to have a VRF in a maxillary left central incisor, and an exploratory flap was performed in order to confirm the diagnosis. After detecting the fracture, the lesion was surgically treated, the fracture and the infected root-end were removed, and a platelet-rich plasma membrane was used to cover the defect in order to prevent bacterial migration. A 24 month clinical and radiological follow-up examination showed that the tooth was asymptomatic and that the healing process was in progress. The surgical approach described here may be considered an effective treatment for a combined endodontic-periodontal lesion originating from an incomplete VRF and a recurrent periapical lesion.

아말감 와동의 파절에 관한 2차원 유한요소법적 연구 I : 와동 폭의 변화 (A STUDY ON AMALGAM CAVITY FRACTURE WITH TWO DIMENSIONAL FINITE ELEMENT METHOD I : VARIATION OF THE WIDTH OF CAVITY)

  • Restorative procedures can lead to weakening tooth due to reduction and alteration of tooth structure. It is essential to prevent fractures to conserve tooth. Among the several parameters in cavity designs, cavity isthmus is very important. In this study, amalgam 0 cavity was prepared on maxillary first premolar. Two dimensional finite element models were made by serial photographic method and isthmus(1/4, 1/3, 1/2, 2/3 of intercuspal distance) were varied. Three or four-nodal mesh were used for the two dimensional finite element models. The periodontal ligament and alveolar bone surrounding the tooth were excluded in these models. 1S model was sound tooth with no amalgam cavity. B model was assumed perfect bonding between the restoration and cavity wall. Both compressive and tensile forces were distributed directly to the adjacent regions. A load of 500N was applied vertically at the first node from the lingual slope of the buccal cusp tip. This study analysed von Mises stress, 1 and 2 directional normal stress and Y and Z axis translation with FEM software Super SAPII Version 5.2 (Algor Interactive System Co.) and hardware 486 DX2 PC. The results were as :follows : 1. 1S model was slightly different with 1B model in stress distibution. 1S, 2B, 3B, 4B models showed similiar stress distribution. 2. 1S model and four B models showed similiar pattern in Y axis and Z axis translation. 3. 1S model and four B models showed the bending phenomenon in the translation. 4. As increasing of the width of the cavity, experimental group was similiar with the control group in stress distribution. 5. As increasing of the width of the cavity, experimental group was similiar with the control group in Y and Z axis tranlation.

외상성 intrusion 치아의 교정적 견인시기에 관한 실험적 연구 (THE ONSET OF ANKYLOSIS FOLLOWING INTRUSIVE LUXATION INJURIES)

  • Orthodontic traction has been suggested as the treatment of choice for intrusive luxation injuries. Prior research has shown orthodontic forces to be ineffective in the presence of ankylosis or in cases with zero mobility following the injury. If orthodontic traction is to be effective, it must be initiated prior to the onset of ankylosis. The purpose of this study was to describe the effects of intrusive luxation at various times following the injury, and to determine the time of the onset of ankylosis, and to examine what effect immediate partial luxation has on the onset of ankylosis. Eight young mongrel dogs were utilized for this study. Intrusive luxation was produced with an axial impact using a gravity hammer and a specially designed holding device on 4 teeth (2 max. and 2 man. first premolars) in each dog. The teeth were intruded approximately 3-4mm in an axial direction. One maxillary and one mandibular premolars were partially luxated with the other two teeth being untouched. Pre and posttrauma tooth position was documented with plaster models and radiographs taken with an individualized X-ray jig. Dogs were sacrificed immediately following the injury and at 1, 2, 4, 7, 10, 14 and 21 days respectively. Tetracycline was administered as a vital bone marker 24 hours before sacrifice. Block sections of the tooth and alveolus were prepared for decalcified and non decalcified histologic sections. The effects of traumatic intrusion were analyzed by means of model casts, radiographs, tetracycline bone marking and histologic preparations. The results obtained were as follows: 1. The animal sacrificed immediately following the injury displayed alveolar fractures, torn periodontal ligaments, and areas of direct tooth-bone contact. 2. The odontoblastic layer of the pulp was disorganized as early as 24 hours after the injury. 3. Bony remodeling was noted at 4 days along with active surface resorption. 4. Ankylosis was first seen 7 days after the injury. 5. Osteogenesis in the dentin (thick tetracycline bands) was observed 7 days after the injury. 6. There was no progressive root resorption and ankylosis where the periodontal ligament has been healed. 7. The Luxated group showed significantly more root resolution and ankylosis than the Nonluxated group with increased observation periods. The results suggest that ankylosis may occur within the first week following the injury, and hence orthodontic traction should be initiated as soon after the injury as possible.

THE FRACTURE CHARACTERISTICS OF GLASS FIBER POST AND CORE ON USING DIFFERENT TYPES OF CORE RESIN MATERIALS

  • Statement of problem. Glass fiber post is one of recent developments to accommodate esthetic restoration for endodontically treated teeth. This has many advantages over conventional post system in physical properties, esthetic factor, risk of root and restoration fracture, adhesion to core, radiopacity, removal and retrievabilty, biocompatibility and chemical stability. Purpose. This in vitro study was to evaluate the most suitable type of resin core for the glass fiber post through surveying the fracture modes and the maximum load that fractures the tooth. Material and methods. 50 sound maxillary premolars restored with glass fiber posts($ParaPost^{(R)}$ Fiber White) and different types of resin cores(ParaCore, $Z100^{TM}$, $Rebilda^{(R)}$ and $Admira^{(R)}$) were prepared and loaded to faiure in a universal test machine. The maximum fracture load and fracture mode were investigated in the specimens that were restored with resin and those of metal cast and core. With the data, Wilcoxon rank sum test was used to validate the significance between the test groups, and Tukey' s studentized range test was used to check if there is any significant statistical difference between each test group. Every analysis was approved with 95% reliance. Results. On measuring the maximum fracture load of teeth specimens, there was a significant difference between the maximum fracture loads of the tooth specimens. ParaCore showed the highest mean maximum fracture load followed by $Z100^{TM}$. And, the distribution of fracture mode of tooth specimens showed generally Type D, the three parted fracture of the core around the post was mostly seen(62.5%), and specifically, ParaCore showed 90% and $Z100^{TM}$ showed 100% Type D fracture. Conclusion. Referring to the values of maximum fracture load and mean compressive fracture load, ParaCore and $Z100^{TM}$ had high values and are recommended as tooth colored resin core material for glass fiber post. CLINICAL IMPLICATIONS. This study was carried out intending to be of aid in selecting the appropriate resin core for the glass fiber post. The dual cure type composite resin ParaCore and light cure type composite resin $Z100^{TM}$ have good properties and are recommended as tooth colored resin core material for glass fiber post.

In vitro study of the fracture resistance of monolithic lithium disilicate, monolithic zirconia, and lithium disilicate pressed on zirconia for three-unit fixed dental prostheses

  • PURPOSE. The purpose of this study was to determine fracture resistance and failure modes of three-unit fixed dental prostheses (FDPs) made of lithium disilicate pressed on zirconia (LZ), monolithic lithium disilicate (ML), and monolithic zirconia (MZ). MATERIALS AND METHODS. Co-Cr alloy three-unit metal FDPs model with maxillary first premolar and first molar abutments was fabricated. Three different FDPs groups, LZ, ML, and MZ, were prepared (n = 5 per group). The three-unit FDPs designs were identical for all specimens and cemented with resin cement on the prepared metal model. The region of pontic in FDPs was given 50,000 times of cyclic preloading at 2 Hz via dental chewing simulator and received a static load until fracture with universal testing machine fixed at $10^{\circ}$. The fracture resistance and mode of failure were recorded. Statistical analyses were performed using the Kruskal-Wallis test and Mann-Whitney U test with Bonferroni's correction (${\alpha}=0.05/3=0.017$). RESULTS. A significant difference in fracture resistance was found between LZ ($4943.87{\pm}1243.70N$) and ML ($2872.61{\pm}658.78N$) groups, as well as between ML and MZ ($4948.02{\pm}974.51N$) groups (P<.05), but no significant difference was found between LZ and MZ groups (P>.05). With regard to fracture pattern, there were three cases of veneer chipping and two interfacial fractures in LZ group, and complete fracture was observed in all the specimens of ML and MZ groups. CONCLUSION. Compared to monolithic lithium disilicate FDPs, monolithic zirconia FDPs and lithium disilicate glass ceramics pressed on zirconia-based FDPs showed superior fracture resistance while they manifested comparable fracture resistances.
