Maxillary protraction using customized mini-plates for anchorage in an adolescent girl with skeletal Class III malocclusion

  • The treatment of skeletal Class III malocclusion in adolescents is challenging. Maxillary protraction, particularly that using bone anchorage, has been proven to be an effective method for the stimulation of maxillary growth. However, the conventional procedure, which involves the surgical implantation of mini-plates, is traumatic and associated with a high risk. Three-dimensional (3D) digital technology offers the possibility of individualized treatment. Customized mini-plates can be designed according to the shape of the maxillary surface and the positions of the roots on cone-beam computed tomography scans; this reduces both the surgical risk and patient trauma. Here we report a case involving a 12-year-old adolescent girl with skeletal Class III malocclusion and midface deficiency that was treated in two phases. In phase 1, rapid maxillary expansion and protraction were performed using 3D-printed mini-plates for anchorage. The mini-plates exhibited better adaptation to the bone contour, and titanium screw implantation was safer because of the customized design. The orthopedic force applied to each mini-plate was approximately 400-500 g, and the plates remained stable during the maxillary protraction process, which exhibited efficacious orthopedic effects and significantly improved the facial profile and esthetics. In phase 2, fixed appliances were used for alignment and leveling of the maxillary and mandibular dentitions. The complete two-phase treatment lasted for 24 months. After 48 months of retention, the treatment outcomes remained stable.

  • Purpose: Pnumosinus dilatans is a rare disease that one or more of the paranasal sinuses are dilatated without functional alteration. The most frequently involved sites are frontal and sphenoid sinus. Facial asymmetric contour is the most common signs and nasal obstruction and pain may be combined. The purpose of reconstruction of pneumosinus dilatans is two-fold, to re-establish a permanent pressure equilibrium of sinus and to correct the possible facial deformities. Methods: We present a case of a 24-year-old female with a 7-year history of protrusion of right malar region. Plane radiography and computed tomography detailed an abnormal expansion of the right maxillary sinus without thinning of bony wall, leading to diagnosis of maxillary pneumosinus dilatans. Surgical decompression and maxilloplasty were achieved by ostectomy of anterior wall of maxillary sinus and repositioning of removed bony fragment with miniplate. Results: Post operative course was uneventful without complication and malar height became symmetric by physical and radiologic examination. Conclusion: We corrected successfully pneumosinus dilatans of maxillary sinus by surgical decompression and maxilloplasty. For this case, we reviewed literature related to this topic.

  • Fibrous dysplasia(FD) of the bone is a slowly progressive, benign disease of unknown cause where normal architectures are replaced with fibrous and osteoid tissue. FD of the maxilla usually manifests as a bony enlargement with painless swelling and bone deformity, contouring to facial asymmetry. The lesion may involve the nasal fossae, orbits, or alveolus bone, causing diverse functional disturbance. Treatment options range from shaving to total maxillectomy and reconstruction depending on the presenting symptoms. Shaving, partial maxillectomy and maxillary sinus formation was performed in 5 patients with fibrous dysplasia in the past 2 years. Follow up period ranged from 1 month to 11 months. Aesthetic appearance, CT findings, and relief from symptoms were compared. In all patients, facial asymmetry was restored to symmetry and nasal obstructive symptoms were improved. With this procedure, expansion of the lesion will be controlled until puberty, preventing the development of new functional disturbances. After puberty, no further treatment can be anticipated due to the growth arrest inherent to the disease.

  • Dentigerous cysts are the most common type of developmental odontogenic cysts. They form as a result of a separation of the follicle from around the crown of an unerupted tooth. Usually this lesion can be asymptomatic. Such cysts are often discovered accidently on inspection of x-rays. In other advanced cases, cortical bone expansion, displacement of teeth, secondary infection can be observed. The treatment of dentigerous cyst is enucleation. And according to size, location of lesion and environmental structure marsupialization can be considered. However, Marsupialization takes long treatment time and, cystic tissues are remained so secondary surgery may be needed for total removal., Risks of oroantral fistula, damage on maxillary sinus wall and infraorbital nerve can be considered as complications of conventional surgical treatment of cysts located in maxillary sinus. We treated third molar origin dentogerous cyst located in maxillary sinus removing endoscopically both the tooth and an associated dentigerous cyst. We report our clinical experience with literature review.

Primary sinonasal mucosal melanoma simulated as cystic lesions: a case report

  • Sinonasal mucosal melanoma (SNMM) in the maxillary sinus is a rare disease condition. Compared to oral mucosal melanoma, SNMM has a bulky, exophytic, and polypoid appearance, is weakly pigmented, and associated with unspecific symptoms. Due to these features, SNMM in the maxillary sinus has been misdiagnosed as nasal polyps and chronic sinusitis. In this case report, we described SNMM occurring in the right maxillary sinus simulated as a cystic or benign lesion. Cortical bone thinning and expansion were observed around the mass. The excised soft mass was encapsulated and weakly pigmented. The mass was clearly excised and covered with a pedicled buccal fat pad graft. Diagnosis using immunohistochemistry with S-100 and homatropine methylbromide-45 (HMB-45) is critical for proper treatment.

A case of canine maxillary gingival acanthomatous epulis

  • Acanthomatous epulis is a malignant form of gingival tumor. This is a case of epulis affecting a four-year old, intact, female Maltese dog raised as indoor pet in Iksan city. Clinical examination showed an enlarged palpable mass along the rostral maxillary gingiva. Section of the excised mass showed epithelial overgrowth and expansion from the basal portions through the tissue stroma. Post-surgical radio-graph showed displacement of the first and second maxillary incisors. Histochemical staining showed strongly positive reaction to epithelial marker cytokeratin and moderately positive reaction to connective tissue marker vimentin. This case was diagnosed as a form of maxillary gingival acanthomatous epulis. Routine clinical oral examination is suggested.

Sphenoid bone changes in rapid maxillary expansion assessed with cone-beam computed tomography

  • Objective: Rapid maxillary expansion (RME) is used to expand the maxilla and increase arch perimeter; yet, there are few reports on its effects on the sphenoid bone. With cone-beam computed topography (CBCT), it is possible to visualize sphenoid bone changes. The purpose of this study was to investigate sphenoid bone changes observed in conjunction with RME treatments, using CBCT. Methods: Sixty patients (34 women and 26 men, aged 11-17 years) underwent RME as part of their orthodontic treatment. Patients were randomly assigned to one of three groups: a tooth-anchored group, a bone-anchored group, or a control group. Initial CBCT scans were performed preceding the RME treatment ($T_1$) and again directly after the completion of expansion ($T_2$). Statistical analysis included ANOVA, descriptive statistics, and the intraclass correlation coefficient (ICC). Results: The reliability of the landmark location was at least 0.783, and the largest ICC mean measurement error was 2.32 mm. With regard to distances, the largest change was 0.78 mm, which was not statistically significant (p > 0.05). Statistical significance was established in patient groups of the same sex and treatment type for the following distance measurements: right anterior lateral pterygoid plate to the right edge of the hypophyseal fossa ($d_2$), anterior distance between the medial pterygoid plates ($d_4$), and anterior distance between the left medial and lateral plates ($d_8$). Conclusions: In this study, there were no clinically significant changes in the sphenoid bone due to RME treatments regardless of sex or treatment type.

Maxillary alveolar bone evaluation following dentoalveolar expansion with clear aligners in adults: A cone-beam computed tomography study

  • Objective: To investigate the effects of maxillary orthodontic expansion on the alveolar bone tissue in adult patients treated with aligners by using cone-beam computed tomography. Methods: Thirty patients (22 females and 8 males; mean age: 36.3 years) were treated with Invisalign® aligners. Cone-beam computed tomography and digital models were obtained before (T0) and after (T1) upper arch expansion. The bone thicknesses in the cervical, middle, and apical areas of the incisors, canines, premolar, and first molars were buccally and palatally measured, totaling 96 areas and 2,880 measurements. The buccolingual inclinations and transverse measurements of the teeth were obtained from digital models to correlate them with the bone changes. The statistical tests used were Student's t-test, analysis of variance, and Pearson's correlation tests (p < 0.05). Results: From the 96 areas evaluated, 84 revealed an increase or stability in the alveolar bone thickness and twelve displayed significant bone loss. Bone changes did not correlate with the tooth inclination and transverse measurements. Conclusions: Within the limitation of the present study, mild levels of upper arch expansion obtained with Invisalign® aligners in adult patients did not result in any clinically significant loss of alveolar bone thickness.

상악 매복 견치의 견인 기간에 영향을 미치는 요인에 대한 분석 (Factors Influencing the Duration of Forced Eruption in Impacted Maxillary Canines)

  • 이 연구는 파노라마 방사선 영상과 Cone-Beam Computed Tomography (CBCT)를 이용하여 상악 매복 견치의 견인 치료기간에 영향을 미치는 요인을 알아보고 견인 기간을 예측하고자 하였다. 2012년 1월부터 2020년 12월까지 아주대학교 치과병원에 내원한 환자들 중 상악 견치의 매복으로 진단받아 외과적, 교정적 개입을 통한 견인 치료를 시행 받고 상악 매복 견치의 맹출까지 치료받은 만 8세 이상 만 18세 이하 환자 73명(93개의 상악 매복 견치)의 의무기록지와 방사선학적 영상을 후향적으로 분석하였다. 단계적 다중회귀분석 결과 교합평면에서 상악 매복 견치 교두정까지의 거리, 근-원심과 협-구개 위치, 환자의 나이 그리고 급속구개확장장치의 사용 유무는 견인 치료기간을 예측하는 데 통계적으로 유의미한 요인이었으며, 매복 견치의 치축이 교합평면과 이루는 각과 편측성/양측성 매복 양상은 견인 기간과 유의미한 상관관계를 보였다. 견치의 낮은 매복 깊이, 견치 교두정의 근-원심 위치가 측절치 또는 견치 영역, 협-구개 위치가 협측 또는 중앙에 위치하고, 치료시작 시 환자의 어린 나이 그리고 구개 확장 장치의 사용이 상악 매복 견치의 견인 기간을 짧게 예측하는 데 도움을 줄 수 있을 것이다.

Orthodontic and orthopedic treatment for a growing patient with Tessier number 0 cleft

  • The purpose of this case report was to introduce the concept of orthodontic and orthopedic treatment for a growing patient with Tessier number 0 cleft. A 5-year-old boy patient with Tessier number 0 cleft presented congenitally missing maxillary central incisors (MXCI), a bony defect at the premaxilla, a constricted maxillary arch, an anterior openbite, and maxillary hypoplasia. His treatment was divided into three stages: management of the bony defect at the premaxilla and the congenitally missing MXCIs using a fan-type expansion plate, iliac bone grafting, and eruption guidance of the maxillary lateral incisors into the graft area for substitution of MXCIs; management of the maxillary hypoplasia using sequential facemask therapy with conventional and skeletal anchorage; and management of the remaining occlusal problems using fixed orthodontic treatment. The total treatment duration was 15 years and 10 months. Class I canine and Class II molar relationships and normal overbite and overjet were achieved at the end of treatment. Although the long-term use of facemask therapy resulted in significant protraction of the retrusive maxilla, the patient exhibited Class III profile because of continued mandibular growth. However, the treatment result was well maintained after 2 years of retention. The findings from this case suggest that interdisciplinary and customized approaches are mandatory for successful management of maxillary hypoplasia, bony defect, and dental problems in Tessier number 0 cleft. Moreover, considering the potential of orthognathic surgery or distraction osteogenesis, meticulous monitoring of mandibular growth until growth completion is important.
