상악견치(上顎犬齒)의 맹출로예측(萌出路豫測) 및 매복(埋伏)에 관(關)한 누년적(累年的) 연구(硏究) (A LONGITUDINAL STUDY ON PREDICTION OF ERUPTIVE PATH AND IMPACTION OF MAXILLARY CANINE)

  • To predict eruptive path of maxillary canine, 13 male and 11 female malocclusions were longitudinally studied for 4 years. And to study frequency and distribution of impaction of maxillary canine, 1500 malocclusions were studied. The path, velocity and duration of maxillary canine eruption were determined by periodic angular and linear measurement using periodic orthopantomograms and cephalograms. The following results were obtained. 1. Maxillary canine was erupted with $14.5^{\circ}$ distal tipping from initial stage $98.1^{\circ}$ to final stage $83.6^{\circ}$ of axial inclination on orthopantomogram. 2. Eruptive velocity of maxillary canine was fastest on stage 4, and mean eruptive velocity was 10.5mm per year on stage 4. 3. Eruption of maxillary canine was completed 12 year 5 months in male and 11 years 8 months in female. 4. To predict the duration for eruption completion by position of maxillary canine on cephalogram, regression equation was obtained. 5. Frequency of impaction of maxillary canine was 1.47% in malocclusion and more frequent in male. Distribution of buccal and palatal, right and left impaction was no different, but unilateral impaction was more frequent.

  • Maxillary canine is the most common impacted tooth except third molars. In Asian populations, labial impaction is more common than palatal impaction, but palatal impaction is three times more common than labial impaction in Caucasion. The incidence of maxillary canine impaction is known as 1~3%, and 12% of these cases are involved in the root resorption of the adjacent lateral incisor. In children, early diagnosis is important to prevent the impaction of maxillary canine through clinical and radiographic examinations. In addition, preventive measures should be considered at the proper time. These measures include the removal of deciduous canines and the expansion of the maxillary arch.

Minimally traumatic extraction of fractured bilateral maxillary canine teeth using a piezoelectric surgical unit in an African lion (Panthera leo)

  • Importance: Canine extraction of large carnivores can pose significant risk due to extensive tissue damage during aggressive bone reduction. This report highlights a rare instance in which the use of a piezoelectric surgical unit (PSU) for maxillary canine extraction in a large carnivore resulted in successful outcomes with minimal tissue damage. Case presentation: A 10-year-old male African lion presented with decreased appetite because of bilateral maxillary canine fractures. Intraoral radiographs revealed enlarged root canals and periapical radiolucency of the fractured canines, leading to a diagnosis of periapical periodontitis and pulpitis. To extract the right maxillary canine, conventional method using hand instrument failed to achieve adequate luxation, necessitating the use of the flat blade of the PSU to sever the periodontal ligament. The left maxillary canine was extracted using PSU from the beginning, and the extraction time was markedly shortened by using PSU without additional alveolar bone damage or bleeding. Conclusion and Relevance: This case demonstrated that utilizing PSU for canine extraction in a lion resulted in periodontal ligament separation, reducing damage to the alveolar bone and shortening surgical time. It suggests the promising application of PSU in tooth extraction for large wild animals, indicating its potential significance in veterinary dentistry.

매복 상악 견치의 처치 (Management of Maxillary Impacted Canines)

  • 상악 견치는 기능과 외모에 있어서 중요한 역할을 하지만, 맹출 장애로 인해 문제를 일으키는 경우가 많다. 상악 견치의 매복은 비교적 흔하며 구조적 장애, 발달 이상, 공간 부족, 유전과 같은 다양한 요인에 의해 발생할 수 있다. 상악 견치의 구개측 매복은 서양인에게서 흔한 것으로 보고되지만 한국인에게는 순측 매복이 더 흔한 것으로 관찰된다. 순측으로 변위된 상악 견치의 경우, 치아 총생 및 골격성 부정교합이 보일 수 있으며, 측절치의 선천적 상실이나 과잉 측절치는 상악 견치의 구개측 매복 가능성을 높일 수 있다. 상악 견치 매복의 조기 발견 및 적절한 처치는 조화로운 치열 발달과 정상적인 치아 기능을 위해 중요하다. 본 종설을 통해 매복 상악 견치의 다양한 처치법을 제안하고 그 원리를 설명하고자 한다.

한국인(韓國人) 영구치(永久齒) 맹출시기(萌出時期)에 관(關)한 통계학적(統計學的) 연구(硏究) (STATISTIC STUDY ON ERUPTION TIME OF PERMANENT TEETH IN KOREA)

  • The author has sought to determine the time and the sequence of permanent teeth eruption in Korean children. The study group consisted of 15,671 healthy children (male 8,015 ; female 7,656) aged 5-13 years old who lived in Seoul or Cheongju. The results were as follows : 1. The eruption times of permanent teeth were 0.45 years earlier in females than in males. 2. The ages corresponding to $ER_{50}$ of permanent teeth were as follows : In Maxilla 1) central incisor was 7.37 yrs 2) lateral incisor was 8.50 yrs 3) canine was 10.83 yrs 4) 1st premolar was 10.30 yrs 5) 2nd premolar was 11.09 yrs 6) 1st molar was 6.49 yrs 7) 2nd molar was 12.79 yrs In Mandible 1) central incisor was 6.40 yrs 2) lateral incisor was 7.41 yrs 3) canine was 10.18 yrs 4) 1st premolar was 10.26 yrs 5) 2nd premolar was 11.15 yrs 6) 1st molar was 6.32 yrs 7) 2nd molar was 12.05 yrs 3. The eruption sequence of permanent teeth by Z-test was as follow: In Male 1st : Mandibular 1st molar, and Mandibular central incisor 2nd : Maxillary 1st molar 3rd : Maxillary central incisor 4th : Mandibular lateral incisor 5th : Maxillary lateral incisor 6th : Mandibular canine, Maxillary and Mandibular 1st premolar 7th : Maxillary canine 8th : Maxillary and Mandibular 2nd premolar 9th : Mandibular 2nd molar 10th : Maxillary 2nd molar In Female 1st : Mandibular 1st molar, and Mandibular central incisor 2nd : Maxillary 1st molar 3rd : Mandibular lateral incisor, Maxillary central incisor 4th : Maxillary lateral incisor 5th : Mandibular canine, Maxillary and Mandibular 1st premolar 6th : Maxillary canine 7th : Maxillary and Mandibular 2nd premolar 8th : Mandibular 2nd molar 9th : Maxillary 2nd molar 4. The corresponding permanent teeth in the mandible generally erupted earlier than the corresponding permanent teeth in the maxilla by an average of 0.73 years, but the mean eruption time of mandibular 1st premolars was almost the same as those of maxillary 1st premolars, and the mean eruption time of mandibular 2nd premolars was 0.06 years later than those of maxillary 2nd premolars. 5. There is no significant difference between left and right arch in the eruption time and sequence. 6. Generally, the ages of permanent teeth eruption tended to be earlier than those of Dr. Cha's data from 1963.

견치유도군에서 견치치주인대의 국소마취가 저작근활성도에 미치는 영향 (The Influence of Local Anesthesia of Canine Periodontal Ligament on Electromyographic Activity of Jaw Elevator Muscles during Canine Guidance)

  • The author studied maximum clenching EMG activities of temporalis anterior and masseter muscle during canine guidance and centric occlusion. It was performed before and after anesthesia of maxillary and mandibular canines. The 22 normal subjects (15 males and 7 females) who had healthy maxillary and mandibular canines were selected. Their occlusal contact scheme was canine guidance during movement and they did not have temporomandibular disorder. The results were as follows : 1. The maximum clenching EMG activities of temporalis anterior and masseter muscle during guidance were less than those during centric occlusion. 2. After left maxillary and mandibular canines were anesthetized with 2% lidocanine with 1:100,000 epinephrine, the maximum clenching EMG acivities of temporalis anterior and masseter muscle during left canine guidance were greater than those before anesthesia of left maxillary and mandibular canines(p<0.01) 3. The maximum clenching EMG activities of temporalis anterior and masseter muscle during right guidance were not significantly different between before and after anesthesia of left maxillary and mandibular canines(p>0.05).

상악 매복 견치의 교정학적 고찰

  • Orthodontists, in evaluating maxillary impactd canines, must take into consideration the etiology of the problem. First, the possible etiologic factors must be eliminated. Thus some cases can be treated with orthodontic treatment alone without surgical trauma following window opening etc. Now, two cases are reported, which were treated without surgical intervention. Sometimes, impacted canine can be included in a plan for tooth removal due to its severe impaction and displacement. In such case, first premolar can be substituted for canine. One case which was treated by substituting maxillary first premolar for maxillary impacted canine is reported.

  • Transposition is defined as a dental anomaly manifested by a positional interchange of 2 adjacent teeth within the same quadrant of the dental arch. Maxillary canine-first premolar [Mx4-3] transposition is the most frequent tooth transposition reported in the literature. In this case report, an orthodontic correction of a transposition of the maxillary left canine and first premolar with the help of palatally located mini-implant anchorage is described. Esthetic and occlusal evaluations suggested alignment of the transposed teeth to their correct anatomic positions in the dental arch. The clinical result at the end of the treatment was satisfactory. Alignment was obtained, and intercuspation was adequate. Nevertheless, the maxillary canine showed facial recession, probably because it was initially positioned buccally. Supporting tissue was examined after treatment and no alveolar bone damage was observed.

  • Early identification of maxillary canine impaction is very important, because of treatment duration time. Analysis used by 3-dimensional computed tomography is more important than before in treatment planning. The aim of this study was to analysis of factors associated with the treatment during time of forced eruption of the impacted maxillary canine using 3-dimensional computed tomography. 17 patients were analysed in Daejeon dental hospital of Wonkwang university(16 patients with unilateral impactions and 1 patient with bilateral impaction). In total, 18 maxillary impacted cannines were analysed by 3-dimensional computed tomography. The results obtained were as follow; 1. There was positive correlation between the age of patient and treatment duration time. 2. There was positive correlation between the distance of occlusal plane to cusp of maxillary impacted canine and treatment duration time, and negative correlation between the length of maxillary impacted canine and treatment duration time. 3. There was positive correlation between the angulation of midsagittal plane to axis of maxillary impacted canine and treatment duration time, and negative correlation between occlusal plane to axis of maxillary impacted canine and treatment duration time. In conclusion, age, tooth length, distance from the canine cusp tip to the occlusal plane, angulation of tooth axis to occlusal plane and midsagittal plane allow that we can predict treatment duration time.

Orthodontic treatment of a transposed maxillary canine and first premolar in a young patient with Class III malocclusion

  • A 12-year-old girl was referred to our clinic for evaluation of an unaesthetic dental appearance. All permanent teeth were erupted, while the deciduous maxillary right canine was retained. Cone-beam computed tomography revealed a complete transposition of the maxillary left canine and first premolar involving both the crowns and the roots. Initial cephalometric analysis showed a skeletal Class III pattern, with a slight maxillary retrusion and a compensated proclination of the upper incisors. The patient's teeth were considered to be in the correct position; therefore, we decided to attempt treatment by correcting the transposition and using only orthodontic compensation of the skeletal Class III malocclusion. After 25 months of active orthodontic treatment, the patient had a Class I molar and canine relationship on both sides, with ideal overbite and overjet values. Her profile was improved, her lips were competent, and cephalometric evaluation showed acceptable maxillary and mandibular incisor inclinations. The final panoramic radiograph showed that good root parallelism was achieved. Two-year follow-up intraoral photography showed stable results.
