This case report presents the orthodontic treatment of a 25-year-old patient with skeletal Class II and severe maxillary arch crowding, moderate mandibular arch crowding, anterior crossbite, and a missing lower incisor. He was treated with molar distalization using a modified C-palatal plate and temporary anchorage devices to create sufficient space for retraction. The total treatment duration was 21 months. After treatment, his occlusion and smile esthetics showed significant improvement. The modified C-palatal plate represents a treatment modality that enhances the prospects of non-extraction treatment and reduces the need for extraction.
The purpose of this study was to identify the difference of vertical movement of mandible according to Angle's molar relationship and by skeletal factors affect to vertical movement of mandible. 172(age ranged from 20 to 30) subjects who go to college within territory of Kwangju city without any experience of temporomandibular disorder, extraction and orthodontic treatment. were selected for this study. The subjects were classified into class I(male:30, female:49), class II(male:18, female:24) and class III(male:18, female:33) according to Angle's molar relationship. The distance was measured between incisal edge of maxillary and mandibular central incisor and between bottom of central fossa of maxillary and mandibular 1st molar with ruler. The arch length and width were measured on the diagnostic cast. Cephalometrics were taken and then traced. Landmarks were identified and analyzed. 1. Maximal interincisal opening of male is larger than that of female in class I, class II and class III. Among each group maximal interincisal distance is the largest in class III. Maximal intermolar distance of male is superior to that of female in class I, class II, and class III, but there is no siginficant difference among them. 2. On maximal opening movement of Angle's classification class I and class II, total mandibular length, mandibular ramal length, madibular inferior border length and upper arch width were important variables and facial length, upper arch length and lower arch length had negative relationship to that. On maximal opening movement of Angle's class III, the upper arch length, the lower arch length and anterior facial length were important variables especially when compared with class I and II, and upper arch width had negative relationship. These results suggest that maximal opening movement is affected by facial morphology in all classes, but each group is affected by different facial skeletal variables. Accordingly, facioskeletal variables might be considered as diagnosis and treatment to improve the amount of mouth opening.
The purpose of this study was to photoelastically visualize 4he distribution of fortes transmitted to the alveolus and surrounding structures using three different types of headgear for the distal movement of the upper molars. A photoelastic maxillary model was made and three different directional forces applied, which were high-pull, straight-pull, and cervical-pull. Stress distribution was recorded through circular polariscope, and two-dimensional photoelastic stress analysis was performed according to isochromatic fringe characteristics. The results were as follows: 1. In the case of high-pull headgear bodily movement occurred in the medium- length outer bow, stress distribution in the apical region was 1st molar, 2nd premolar, lst premolar in sequence and there was no apparent difference. 2. In the case of straight-pull headgear, bodily movement occurred in the long outer bow and stress distribution in the apical region was heavy in the 1st molar, 2nd premolar, 1st premolar in sequence. But. there were no apparent differences according to the length of the outer bow. 3. In the case of cervical- pull headgear, bodily movement also occulted in 4he long outer bow, and apical stress of the premolar region was heaviest among other cases and apical stress of the 2nd premolar was heaviest in the short outer bow. In clinical situations, to achieve bodily movement of the upper 1st molars without modifying outer bow height, applying an outer bow length as long as the inner bow length in high-pull headgear and applying an outer bow length longer than the inner bow length in straight-pull, cervical-pull headgear are recommended.

Purpose: Aims to analyze the occlusal wear patterns in maxillary posterior teeth with palatal side abfractions and study the association between occlusal force and abfractions. Materials and Methods: This study was conducted in a total of 308 teeth from 148 patients with palatal side abfractions in maxillary posterior teeth. The occlusal wears in maxillary premolars and molars with palatal side abfractions were classified and recorded. The classification was done by type of teeth, age, and gender, and in order to evaluate the statistical significance between groups, chi-square test was conducted (${\alpha}=0.05$). Results: Palatal side abfractions in maxillary posterior teeth were observed at the highest frequency in the 1st molars, and in all teeth with palatal side abfractions, more than one occlusal wear was observed. In classification by type of teeth, by age, and by gender, the occlusal wears in teeth with palatal side abfractions were observed at high frequency in cuspal inclined plane, central fossa, and marginal ridge, and there was a statistical significance (P < 0.05). Conclusion: Palatal side abfractions were observed at the highest frequency in maxillary 1st molars, and in all maxillary posterior teeth where palatal side abfractions were found, the occlusal wears were observed. And the occlusal wears were observed at high frequency in cuspal inclined plane, central fossa and marginal ridge. Such results show that abfraction is associated with occlusal force.

Anchorage control is important in orthodontic treatment. Recently miniscrew is widly used as maximum anchorage in orthodontic treatment, and then it is important to install miniscrew safely without damaging adjacent anatomic structure. In a view of Miniscrew's stability, maxilla is unfavorable than mandible, and moreover maxillary soft buccal bone has disadvantage on stability. so palatal area comes into notice for installation area. We measured distance between palatal roots and bone thickness at midpalatal area using 3D computed tomography, and have found following results. 1. On the comparison of distance between palatal roots, the distance between 2nd premolar and 1st molar was significantly longest and the distance between premolars was significantly shortest. 2. Going toward lateral area from midpalatal suture and posterior area from zero point, bone thickness significantly became shorter and shorter. And 5.0mm palatal sagittal plane has more significance decrease of bone thickness than 2.5mm palatal sagittal plane. According to these results, we can conclude that the palatal installation of miniscrew between 2nd premolar and 1st molar is safest. And it is more safe that comes closer to midpalatal suture and to anterior area in regard to incisive canal.

In the process of assessing the children with anterior crossbite in early mixed dentition, it has frequently been detected that the stronger the skeletal pattern of the malocclusion is, the more markedly delayed the development and eruption of maxillary teeth are. If the anteroposterior characteristics of craniofacial skeleton has any relationship with dental maturation, the evaluation of dental development and eruption was thought to be able to contribute to early diagnosis of crossbite in children. This study was performed for the purpose of elucidating the relationship between dental maturation of maxillary teeth and some cephalometric values in children with anterior crossbite of maxillary undergrowth type in early mixed dentition. Among the children in Hellman dental age IIA and IIC who attended the Pediatric Dental Clinic of Pusan National University Hospital with orthodontic problems, cases with Class III malocclusion were classified and 50 cases of maxillary undergrowth type and type with normal maxilla respectively were randomly selected and studied as subjects. From their lateral cephalographs and panoramic radiographs, their anteroposterior skeletal features, the dental maturity and eruption rate were obtained of each group and data were analyzed to yield the results as follows: 1. Comparing the maturity of maxillary teeth of both groups, only the first molars of maxillary undergrowth group showed significantly slower development and eruption (p<0.05). 2. There was high correlation between maturation of maxillary 1st molar and chronological age(p<0.05). 3. Among the parameters of anteroposterior relationship of skeletal pattern in maxilla and mandible. Wits was revealed as a useful index to predict both the calcification and eruption rate of the 1st molars whereas SNA was to eruption rate(p<0.05).
Statement of the problem: Under anatomical limitations on maxillary posterior region, a poor crown-to root ratio acting on dental implants can result in undesirable stress in surrounding bone, which in turn can cause bone defects and eventual failure of implants. Purpose: The purpose is to compare stress distribution due to different crown-root ratio and effect of splinting between natural teeth and implants in maxillary molar area under different loads. Material and methods: Analysis of stress arising supporting bone of the natural teeth and the implant was made with 3-dimensional finite element method. The model simulated naturel teeth was made with 2nd premolar and 1st molar in the maxillary molar region (Model T). The model simulated implants placed on same positions with two parallel implants of Straumann Dental Implant cemented abutment (Model I). Each model was designed in different crown-root ratio (0.7:1, 1:1, 1.25:1) and set cement type gold crown to make it non-splinted or splinted. After that, 300 N force was loaded to each model in five ways (Load 1: middle of occlusal table, Load 2: middle of buccal cusp, Load 3: middle of lingual cusp, Load 4: horizontal load to buccal cusp of anterior abutment only, Load 5: horizontal load to middle of buccal cusp of each abutment), and stress distribution was analyzed. Results and conclusion: On all occasions, stress was concentrated at the cervical region of the implant. Under load 1, 2 and 3, stress was not increased even when crown-root ratio increases, but under load 4 and 5, when crown-root ratio increases, stress also increased. There was difference in stress values between natural teeth and implants when crown-root ratio gradually increases; In case of natural teeth, splinting decreased stress under vertical and horizontal loads. In case of implants, splinting decreased stress under vertical loads 1,2 and 3, but increased maximal stress under loads 2 and 3. Under horizontal loads, splinting decreased stress, however the effect of splinting decreased under load 5 than load 4. Furthermore, the stress was increased, when crown-root ratio is 1.25:1. Clinical implications: This limited finite element study suggests that the stress on supporting bone may be increased under non-axial loads and poor crown-root ratio. Under poor crown-root ratio, excessive stress was generated at the cervical region of the implant, and decreased splinting effect for stress distribution, which can be related to clinical failure.
Various methods have been used on patients with skeletal Class II division 1 malocclusion. The activator, Frankel appliance, headgear, Herbst appliance, and Twin-block appliance are some examples. The ideal treatment effect using these appliances would be to inhibit horizontal and vertical growth of the maxilla while promoting mandibular growth and obtaining optimum dentition. The Teuscher appliance has a simultaneous combined headgear effect with maxillary growth inhibition and an activator effect with mandibular growth promotion. The purpose of this study was to examine how well these effects were clinically obtained and the results are as follows. 1. The forward growth of the maxilla was effectively inhibited. 2. The downward-forward growth of the maxillary dentoalveolar complex was inhibited. 3. Growth promotion of the mandible was not observed. 4. The overjet, overbite, molar key were effectively improved. 5. The protruded upper lip and facial profile were unproved.

Among the permanent teeth. the first permanent molars play the greatest role in occlusion and function. So, the congenital missing, abnormal reuption or abnormal formation of the first permanent molars in the course of arch development would inflict normal development of dental arches. Therefore, early detection of abnormal cases related to first permanent molars and understanding of current and predictable clinical problems are essential for proper occlusal guidance in children. With the aim of investigating the clinical patterns of delayed eruption of first permanent molars in children, panoramic tomograms of the childern in mixed and early permanent dentition were observed and analyzed. The results were as follows: 1. Among the delayed eruption of first permanent molars, on tooth or bilateral teeths were affected most frequently. Delayed eruption was more prevalent in maxilla than in mandible. 2. The formation of tardily erupted teeth were also delayed. 3. Delayed eruption was generally limited in first molars or molar segments. 4. Delayed eruption of first permanent molars is accompanied by abnormal position of tooth germs, for example, ectopic eruption, delayed dental age, delayed localized tooth formation and generalized congenital missing. 5. There was a tendency of delayed formation or congenital missing of second molars distal to tardily erupted 1st molars. And that was more marked in maxilla than in mandible. 6. There was reported that affected 1st molars show various size and shapes. Maxillary 1st molars showing delayed eruption showed a tendency of having 3 cusps. But, tardily erupted mandibular 1st molars showed no significant reduction in mesiodistal dimension, as reported. 7. In some cases, the delayed eruption of 1st permanent molars was associated with ectopic eruption, but their formation was not usually retarded. 8. In skeletal class III cases, there showed a tendency of mandibular 1st molars to erupt earlier than maxillary 1st molars with greater interval than in normal occlusion.

Objective: To develop a mixed dentition analysis method in consideration of the normal variation of tooth sizes. Methods: According to the tooth-size of the maxillary central incisor, maxillary 1st molar, mandibular central incisor, mandibular lateral incisor, and mandibular 1st molar, 307 normal occlusion subjects were clustered into the smaller and larger tooth-size groups. Multiple regression analyses were then performed to predict the sizes of the canine and premolars for the 2 groups and both genders separately. For a cross validation dataset, 504 malocclusion patients were assigned into the 2 groups. Then multiple regression equations were applied. Results: Our results show that the maximum errors of the predicted space for the canine, 1st and 2nd premolars were 0.71 and 0.82 mm residual standard deviation for the normal occlusion and malocclusion groups, respectively. For malocclusion patients, the prediction errors did not imply a statistically significant difference depending on the types of malocclusion nor the types of tooth-size groups. The frequency of prediction error more than 1 mm and 2 mm were 17.3% and 1.8%, respectively. The overall prediction accuracy was dramatically improved in this study compared to that of previous studies. Conclusions: The computer aided calculation method used in this study appeared to be more efficient.
