욕창방지방석용 공기셀의 설계요소에 따른 체압 분포 특성 분석 (Analysis of Body Pressure Distribution Characteristics According to the Design Factors of the Air-Cell Mattress for Preventing Decubitus Ulcer)

  • A finite element simulation model was developed for the performance optimization of a closed type air-cell mattress used for the ulcer prevention. An H-model with material properties of human flesh and kinematic joints were used for the calculation of the body contact pressure. The material property of rubber air-cell was evaluated by tensile test of standard specimen. We evaluated the body contact pressure distribution after laying human model on the inflated air-cell mattress. It was found that the body contact pressure was dependent on cell height. but hardly affected by the thickness of the rubber in a cell.

  • 본 연구에서는 Spragus-Dawley준 흰쥐의 서혜하부의 대퇴동맥을 수술현미경하에서는 대조군은 단순봉합방법으로, 실험군은 수평연차봉합 방법으로 미세분합하여 그 문합시간을 측정하고, 일정시간이 경과한 후 개존율과 조직소견을 비교 관찰하여 얻은 결과를 요약하면 다음과 같다. 문합시간에 있어서 대조군과 실험군에서 각각 17분 25초와 15분 49초로 통계학적으로 유의한 차이가 있었으며, 미세혈관문합에서 수평연차봉합이 유리함을 알 수 있었다. 미세혈관문합 후 개존율은 대조군과 실험군에서 술 후 3일까지는 비슷하였으나, 술 후 3주째는 실험군에서 더 높았다. 조직학적으로는 대조군과 실험군에서 공히 내피가 손상을 입고, 중막이 얇아지면서 공히 혈관 강이 좁아지고 술 후 3주째는 내피가 재생되는 등의 비슷한 변화를 보였다.

호안용 매트리스내 채움재의 한계_허용 전단응력에 관한 연구 (Study on Critical_Allowable Shear Stress of Filling Rocks With Mattress Revetment)

  • 본 논문은 호안용 매트리스내 채움재의 한계_허용 전단응력을 제시하였다. 먼저, 매트리스가 설치된 수로에서 유수력이 작용할 때 각 지점에 대한 유효전단응력을 산정하였다. 다음으로, 입자들의 한계운동을 결정하는데 주로 사용되는 Shields 계수와 전단응력을 이용한 채움재의 평균입경을 산정하였다. 마지막으로, 산정된 각 인자들을 근거로 철망내 개별 암석의 안정조건을 만족시키는 한계_허용 전단응력의 범위를 결정하였다. 또한 매트리스와 사석의 비교를 통해 매트리스의 거동에 관한 안정성을 규명하면서 안정적인 수리구조물임을 입증하였다. 따라서 동일 한계전단 응력에 대한 소요 평균입경은 사석이 매트리스보다 대략 3배 정도 더 크다. 이와 반대로 동일 크기의 암석이 주어질 때 매트리스는 사석보다 3배 정도 더 큰 전단응력에도 저항할 수 있는 것으로 분석되었다.

건축구조물 기초로서 셀룰러 기초 매트리스의 설계 및 시공 (Design and Construction of Cellular Foundation Mattress as Foundations of Building Structures)

  • 국내 중소규모 건축구조물 기초에 적용 사례가 없는 고밀도 폴리에틸렌 신소재 재료로 구성된 셀룰러 기초 매트리스가 건축구조물 기초 공법의 대안으로 정착되기 위해서는 국내 지반 및 현장 조건에 맞게 개량된 공법 개발 및 검증이 필요하다. 본 연구를 통해서 셀룰러 기초 매트리스의 기초적인 설계 및 시공 방법을 제시하였다. 셀룰러 기초 매트리스의 기초보강효과는 연약지반 대상으로 안정성 평가와 검증이 되어야 하기에 국내 중소규모 건축구조물 기초에 적용 실적이 풍부한 소일백공법과 성능 비교 평가를 수행하였다. 매트리스 보강 후 평판재하시험 결과, 원지반과 비교하여 침하량은 38.4% 감소, 지지력은 159% 증가하였으며, 소일백공법과는 동일한 지반보강효과 및 지반안정성을 확인하였다.

수면시 침상기후와 인체생리반응에 관한 연구 -온돌환경을 중심으로- (A Study on the Bed Climate and the Physiological Responses in Sleep. - On Ondol Environment -)

  • The purpose of this study were to investigate the bed climate and the physiological responses in sleep on the traditional Korean floor heating system called Ondol. Ondol has been the most widely used heating system in Korea, yet there has been a no systematic studies examined its environmental effects on human body. Experimental room was constructed to match the typical thermal environment of Ondol. (floor surface temperature; $32\~33^{\circ}C$, air temperature; $22.5{\pm}1.0^{\circ}C$, relative humidity; $64.0{\pm}4.0\%$RH, air velocity; 0.25 m/sec.) Three different combinations of bedclothes were chosen for the experiment based on the study results showing that they were the most widely used types in Korea. Type 1 was the combination of a cotton-padded mattress with a cotton-padded Korean style blanket. Type 2 was a cotton-padded mattress with a cotton-guilted Korean style blanket. Type 3 was a cotton-padded matless with a polyacryl blanket. Thermal resistance of each of these combina-tions in the bedclothes was measured using thermal manikin. Two adult female was chosen for the seven hour sleeping experiment which was known to be the average sleeping hours of Korean adult female. The bed climate was measured with the temperature under the mattress, the surface temper-ature of the mattress, and the air temperature and the relative humidity of the space between the mattress and blanket. The skin temperature, rectal temperature of the subjects and the bed climate were measured eight times, one hour before the experiment and every hour during the experiment. The weight loss and the subjective sensation were measured for the each subjects before and after the experiment. The procedure was repeated twice with two subjects and three types of bedclothes, yielding twelve combinations of results. The results were as follows; 1. With the surface temperature of $32\~33^{\circ}C$ of Ondol, air temperature was $22.5{\pm}1.0^{\circ}C$ with $64.0{\pm}4.0\%$ RH. The bed climates were $39.2{\pm}40.8^{\circ}C$ under the mattress, $35.3\~36.2^{\circ}C$ on the mattress, and $26.9\~32.0^{\circ}C$ with $56.0\~71.3\%$ RH between the mattress and the blanket. 2. Mean skim temperature during sleep was 34.2"C with local skin temperature of $34.0\~35.5^{\circ}C$. The skin temperatures of abdomen, thigh, foot were higher than the other parts of the body. 3. The skin temperature of chest, thigh, leg and back varied significantly according to the combinations of bedclothes. With the cotton-padded blanket, the skin temperature was the highest, while with the cotton-guilted blanket showed lowest. 4. Examining the relationship between the mean skin temperature and the local skin temper-ature, the chest temperature showed the highest correlation with the former. Therefore, the chest temperature can be recommended to represent the skin temperature in measuring the bed climate. 5. The subjective bed climates were $39.0\~40.4^{\circ}C$ under the mattress, $35.2\~35.9^{\circ}C$ on the mattress, $29.8\~31.6^{\circ}C$ with $56.8\~68.4\%$ RH between the mattress and blanket. In sum, from this experiment we not only obtained the reliable value of bed climates on Ondol, but also showed that the bed climates and the physiological responses were affected differently according to the materials of bed clothes.

하상보호용 매트리스의 수리학적 특성 연구 (A Study on the Hydraulic Characteristics for River Bed Revetments using Mattress)

  • 하상보호용 사석은 초기 거동조건의 범위를 넘어 한계조건에 도달하면 흐름에 의해서 각각의 자갈 또는 석재가 분리되면서 점진적으로 파괴된다. 이와 반대로 하상보호용 매트리스의 경우는 비록 초기 거동조건의 범위를 넘어 한계조건에 도달할지라도 매트리스 철망에 의해 각각의 자갈 또는 석재의 분리가 억제가 됨을 알 수 있었다. 비록 변형된 하상보호용 매트리스일지라도 새로운 형태의 평형을 얻을 수 있으며, 더 이상의 변형과 나쁜 조건에서도 저항할 수 있음을 확인하였다. 동일한 수리학적 거동조건에서 하상보효용 매트리스의 채움재로 사용된 자갈 또는 석재의 평균 크기가 하상보호용 사석의 1/2정도이며, 하상보호용 매트릭스에서 Shield 계수는 하상보호용 사석의 약 2배임을 알 수 있었다. 또한 동일한 크기의 자갈 또는 석재가 채움재로 사용될 때 허용유속에 대해서 하상보호용 매트리스의 경우 하상보호용 사석보다 2배이상 심지어 3∼4까지 견딜 수 있음을 알 수 있었다.

두피 봉합기로 봉합하기 어려운 두피 열상에 시행한 잠금 수평 매트리스 봉합법의 유용성 관찰 연구 (Locking horizontal mattress suture as the alternative closure method for scalp lacerations difficult to suture with staple)

  • Objective: This paper reports the possibility of using of a locking horizontal mattress suture technique in repairing lacerations that are difficult to suture with staples. Methods: Data were collected retrospectively over a 6-month period regarding the routine repair of scalp lacerations: those in areas injured by a high energy blunt mechanism, continued to bleed after pressure, nonlinear or damaged skin repaired with a locking horizontal mattress technique, and simple interrupted technique. The effects of the two techniques used to repair scalp lacerations on wound healing, complication rate, and patient satisfaction were examined. The categorical variables are expressed as the number and percent. A Mann-Whitney-Wilcoxon test was used for statistical analysis. A P-value less than 0.05 was considered significant. Results: Thirty-seven consecutive patients with scalp lacerations presented for care. Wound closure was accomplished with the locking horizontal mattress sutures in 40.5% (n=15) (median length, 5.0 cm; interquartile range [IQR], 4.0-7.0 cm). Simple interrupted sutures (median length, 4 cm; IQR, 3.0-5.0 cm) were used in 59.5% (n=22) (P=0.015). The frequency of additional bandage compression (P=0.008), frequency of exudative hemorrhage (P=0.018), and suture mark frequency at suture removal (P=0.047) were significantly lower in the locking horizontal mattress group. Conclusion: The locking horizontal mattress suture, which has the advantage of a horizontal mattress suture, may be one of the ways that can be used alternatively to treat scalp lacerations that difficult to suture with staples.

침대에 부착된 용량성 전극 배열을 이용한 수면 중의 심전도 측정 (ECG Measurement Method during Sleep with Array of Capacitive Electrodes Attached to Bed)

  • In order to measure ECG in daily life, a new ECG measurement method on bed was developed. The provided method does not require any direct conductive contact between the instrument and bare skin, so that it does not cause the uncomfortable feel of touch and the possible skin trouble which are typical shortcomings of the conventional conductive contact ECG measurement. The provided method utilized an array of high-input-impedance active electrodes fixed on the mattress and an indirect-skin-contact ground made of a large conductive textile sheet and laid on lower area of the mattress. A thin cotton bedcover covered the mattress, the electrodes, and the conductive textile and subjects lay on the mattress over the bedcover. ECG was obtained successfully. However its signal quality is lower and the motion artifact is larger than direct-contact measurement. Careful measurement setup was needed to reduce the motion artifact originated from variation in static electricity. From the ECG obtained by the provided method, R-peak could be discriminated easily and the information about the position and the posture of the subject could be obtained.
