Developing Improvement Plans for National Defense Safety Directive to Align with the Serious Accident Punishment Act

  • To ensure a systematic and integrated approach to defense safety management, individual safety management regulations have been consolidated into the National Defense Safety Directive. However, despite being enacted after the enforcement of the Severe Accident Punishment Act, the National Defense Safety Directive does not incorporate the contents of the Serious Accident Punishment Act. This omission is likely to cause confusion in safety management. In this paper, a PDCA analysis of the Severe Accident Punishment Act and the National Defense Safety Directive was conducted to identify area for improvement and supplementation in the Directive. Chapter 3 proposes amendments to clearly define the scope and responsibilities of safety management, implement serious accident prevention measures and inspections, and establish the penalties for those involved. These amendments aim to ensure faithful compliance with the Severe Accident Punishment Act. Chapter 4 emphasizes the implementation and inspection of risk assessments to enhance the effectiveness of safety accident prevention and preparation, thereby ensuring the completeness of the PDCA cycle.

Compression of the Variables Classifying Domestic Marine Accident Data

  • Maritime accidents result in enormous economic loss and loss of life; thus, such accidents must be prevented, and risks must be managed to prevent these occurrences Risk management must be based on statistical evidence such as variables. Because calculating when variables increase statistically can be difficult, compressing the designated variables is necessary to use the maritime accident data in Korea. Thus, in this study, variables of marine accident data are compressed using statistical methods. The date, ship type, and marine accident type included in all maritime accident data were extracted, the number of optimal variables was confirmed using the hierarchical clustering analysis method, and the data were compressed. For the compressed variables, the validity of the data use was statistically confirmed using analysis of variance, and the data of the variables identified using the variable compression method were designated. Consequently, among the monthly and yearly data, statistical significance was confirmed in yearly data, and compression was possible. The significance of the data was confirmed in six and eight types of ships and accidents, respectively, and these were compressed. These results can be directly used for prevention or prediction based on past maritime accident data. Additionally, the data range extracted from past maritime accidents and the number of applicable data will be studied in the future.

소형선박 운용자의 냉각수 관리에 관한 고찰 (A Study on the Management of Engine Coolant in Small Fishing Vessels)

  • Majority of marine accidents that occur on fishing vessels are engine accidents. This comprises more than 26 % of the total annual fishing vessel marine accident cases. Large numbers of engine accidents happen in the cooling water system, which are mostly caused by negligence on regular check-up and repair. Notably, small-sized ships have higher engine accidents occurrence rate compared to medium-and large-sized ships. Based on the Report of the Korea Ship Safety Technology Authority, engine accident cases reached 3,032 out of the total 3,081 cases. This study researches on the differences between the small-sized ship pilot, an operator of a vessel engine of less than 200 tons, and a 6th level marine engineer, in terms of the relationship between management forms and what causes the marine accidents in association with the cooling water system. It also studies and analyzes the differences in frequency of the accident occurrence between the two groups. ${\chi}^2$ qualification was imposed through the SPSS statistical analysis program and it got qualified at the significance level of 5%. The research shall be utilized as one of the base line data for the reduction of marine accidents.

연근해어선 안전관리체제 도입에 관한 기초 연구 (A basic study on the introduction of safety management system for the costal/offshore fishing vessels in Korea)

  • This study analyzed the condition of marine accident and disaster rate of crew of Korean fishing vessels by vessel type, and checked the Australian safety management system and relevant regulation in Korea to suggest measures to build coastal/offshore fishing vessel safety management system in Korean coastal/offshore fishery. As a result, the power and production amount of Korean coastal/offshore fishing vessel is consistently decreasing while marine accident is increasing. Disaster rate of crew was higher than land industry, especially, the disaster rate of coastal/offshore fishing vessel was very high (100.0‰). Australia applies safety management system differed by vessel type and operation waters which fully considers the characteristics of pertinent vessel. The average accident rate of fishing vessels with gross tonnage over 20 tons among the Korean coastal/offshore fishing vessel was 13.6%, which was significantly higher than fishing vessels with gross tonnage below 20 tons (1.4%). Such result indicates it is urgent to implement safety management system to fishing vessel with gross tonnage over 20 tons and introduce fishing vessel safety supervisor. Establishing safety management system of coastal/offshore fishing vessel will contribute to consistent industrial development by achieving the general goal of reducing marine accident and spreading safety culture.

A Study on the Analysis of the Safety Management System of Korea-China Car Ferries

  • The purpose of this study is to keep the safety of the car ferry passengers and vessels by investigating and analyzing vessel safety management systems in Korea and China. To this end, we investigated Korea-China car ferries and the current status and causes of global marine accidents corresponding to the sizes of the vessels from Korea and China. Furthermore, we investigated car ferries' crew management and safety management. As a result of the analysis of the ferry accident, the causes of human error and ship's age were the greatest, but the ship's companies showed a negative stance regarding the age restriction. It seems that it is necessary to utilize the near-miss accident reporting system and differentiate the management of ship's aging. Also, it was analyzed that both the ship company and the crew of the ship need to strengthen their awareness of safety management.

  • Elevator is very important transportation facility of modern society. But there have been frequent elevator accidents recently. In the view of safety management of elevator, the corresponding study must be done since accidents related elevator include not only actual accident but also possible accident in order to protect user from danger. A 3D design was developed using SolidWorks and behavior analysis using FEM Code.

  • 정부(해양수산부) 및 관계기관에서는 안전관리 취약선박의 해양안전 증진을 위한 많은 정책을 시행하고 있으나, 선박의 안전관리에 대한 위험성평가 부재로 각 제도의 효과성에 다소 아쉬움이 확인되었다. 선박의 위험성평가에 대한 국내 선행연구는 대부분 개별적 해양사고 위험성에 대한 단편적 위험성만을 취급하고 있어 포괄적 해양사고 위험성평가를 위한 새로운 방식이 필요하다. 선박의 안전관리에 대한 포괄적 위험성평가 적용사례로 항만국통제와 노르웨이 해사국의 점검대상 선정방법이 있다. 본 연구에서는 국내 해양사고 및 등록현황의 높은 비중을 차지하고 있는 연근해어선을 대상으로 포괄적 선박안전관리 위험성평가를 위한 지표를 개발하고 적용하였다. 그 결과, 각 선박별 위험성을 계량적 수치로 확인하였고, 개별적 안전관리 취약요소의 순위를 식별하였다.

우리나라 다랑어선망어선의 어선원 안전 위험요소 분석 (Analysis of safety risk factors of fishermen on the Korean tuna purse seiner)

  • Tuna purse seine fishery (TPF) constitute more than 60% of distant water fishery production in Korea based on a statistic of 2018, and 28 ships from four different companies were under operation at the western and central Pacific Ocean. On this research, common risk factors during TPF were investigated via enumeration of five years Korean fisherman's insurance payment statement, followed by some counterplans to diminish the accident rate. The accident rate of TPF on the Pacific Ocean peaked by 43.0% in 2014 and constantly decreased to 23.0% until 2018, presenting an average of 33.6%. Meanwhile, the accident rate on the Indian Ocean reached the highest point 55.1% in 2014 and declined to 11.6% in 2016, having an average of 24.7%. The average accident rate of the Indian Ocean scored 8.9% lower than the rate of the Pacific Ocean, but no statistic significance was observed. Depending on the process of operation, 'casting or hauling of net' was the most frequent part that people received an injury (40.4%). When the accidents were classified by their types, 'falling down' was the most recurrent cause of the injuries (28.5%). At the point of severity, the worst injuries were induced by crush hazard. Considering aforementioned accident frequency and severity, all the factors on the accident type list were divided into three different groups including high risk, moderate risk, and common risk. This study is expected to contribute to the reduction of occupational accidents during the work of fishermen and establishment of a safety management system for distance water fishing vessels.

A study on maritime casualty investigations combining the SHEL and Hybrid model methods

  • This paper reviews the analysis of a given scenario according to the Hybrid Model, and why accident causation models are necessary in casualty investigations. The given scenario has been analyzed according to the Hybrid Model using its main five components, fallible decisions, line management, psychological precursors to unsafe acts, unsafe acts, and inadequate defenses. In addition, the differences between the SHEL and the Hybrid Model, and the importance of a safety barrier during an accident investigation, are shown in this paper. One unit of SHEL can be linked with another unit of SHEL. However, it cannot be used for the analysis of an accident. Therefore, we must use an accident causation model, which can be a Hybrid Model. This can explain the "How" and "Why" of accident, so it is a suitable model for analyzing them. During an accident investigation, the reason we focus on a safety barrier is to create another safety barrier or to change an existing safety barrier if that barrier fails. Hence, the paper shows how a sea accident can be investigated, and we propose a preventive way of avoiding the accident through combining the methods of different models for the future.
