추간판탈출증 환자에서 발생한 기능성 소화불량에 대한 한방통합치료 호전사례 (Improvement of Functional Dyspepsia with a Herniated Intervertebral Disk through Korean Medical Treatment: A Case Report)

  • Objective: This study investigated the efficacy of using Korean medical treatments on a patient with functional dyspepsia. Methods: A 61-year-old female patient with functional dyspepsia and C/L-spine HIVD was treated with herbal medicine (Gamihyangsayukgunja-tang), acupuncture, pharmacopuncture, chuna manual therapy, and cupping therapy for 18 days. The treatment effects on the functional dyspepsia were evaluated using the Numerical Rating Scale score, Gastrointestinal Symptom Score, and Bristol Stool Scale results. Results: Following treatment, the patient showed decreases on the Numerical Rating Scale and Gastrointestinal Symptom Score and improvement on the Bristol Stool Scale. Conclusions: Korean medical treatment improved the condition of a patient with functional dyspepsia.

사회 불안 장애에 대한 한의복합치료의 효과: 후향적 관찰 연구 (A Retrospective Study on the Effect of Complex Korean Medicine Treatment on Social Anxiety Disorder)

  • Objectives: This study was conducted to observe the progression of symptoms according to the treatment period of patients with social anxiety disorder who received complex Korean medicine treatment. Methods: The medical records of 25 patients who were diagnosed with social anxiety disorder according to the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, and received complex Korean medicine treatment (herbal medicine, acupuncture, and Korean psychotherapy) for 12 weeks were analyzed. The State-Trait Anxiety Inventory (STAI), the Beck Depression Inventory-II (BDI-II), the Beck Anxiety Inventory (BAI), the Korean-Social Avoidance and Distress scale (K-SAD), and the Korean-Fear of Negative Evaluation (K-FNE) were measured at the initial hospital visit and during the 4, 8, and 12 weeks of treatment to evaluate the effectiveness of treatment. Missing values were replaced with the average evaluation index value at that time. Results: 1) Statistically significant changes in STAI-X1, STAI-X2, BDI-II, BAI, K-SAD, and K-FNE scores were seen according to the time of treatment. 2) When scores were compared between each treatment time point, STAI-X2 and BDI-II showed statistically significant changes between the initial visit and four weeks of treatment, the initial visit and eight weeks of treatment, and the initial visit and 12 weeks of treatment. STAI-X1 and K-FNE showed statistically significant changes between the initial visit and eight weeks of treatment and the initial visit and 12 weeks of treatment. There was a statistically significant change in BAI scores between the initial visit and the 12th week of treatment. Conclusions: Complex Korean medicine treatment alleviated anxiety, depression, fear, and avoidance of social situation symptoms in patients with social anxiety disorder. The longer the treatment period, the more symptoms tended to be alleviated. However, the evidence should be supplemented with prospective, controlled research.

동결견 환자에 대한 한의복합치료 효과: 후향적 차트 리뷰 (Effect of Complex Korean Medical Treatment in Patients with Frozen Shoulder: A Retrospective Chart Review)

  • Objectives Through this study, we investigated the effects of Korean medicine treatments on range of motion recovery and pain reduction in patients with Frozen shoulder. Methods In this study, the medical records of 26 patients diagnosed with frozen shoulder were retrospectively observed and analyzed through the IBM SPSS Statistics 25 program. We used range of motion to observe the patient's motion recovery and numeric rating scale to evaluate pain reduction. Results The shoulder range of motion increased statistically significantly from 160.0 (123.5~170.0) to 170.0 (160.0~180.0) (flexion), 115.0 (90.0~135.0) to 167.5 (130.0~178.8) (abduction), 40.8±22.1 to 58.3±16.0 (external rotation) (p<0.001). The average of numeric rating scale reduced statistically significantly from 6.7±2.0 to 3.2±1.4 (p<0.001). Depending on the period, the range of motion increased statistically significantly when visited within 3 months, also the pain score decreased 7.0 (5.4~8.0) to 3.0 (1.8~3.6) statistically significantly when visited within 3 months. In the treatment intervention, the increase in the range of motion and the decrease in pain were statistically significant, especially when the bee venom acupuncture and chuna manaul therapy were used together. Conclusions Korean medicine treatments was effective in improving the range of motion and reducing pain in frozen shoulders. In particular, when treated within 3 months of onset and when bee venom acupuncture and chuna manual therapy were used together, it was effective in recovering the range of motion and reducing pain.

뇌내출혈 이후 발생한 혈관성 치매 환자에 대한 귀비탕가미방 치험 1례 (A Case Report of a Patient with Vascular Dementia Caused by Intracerebral Hemorrhage Treated with Modified Guibi-tang)

  • Objectives: The aim of this study is to describe the effects of traditional Korean medicine treatment on a patient with vascular dementia caused by an intracerebral hemorrhage. Methods: The patient was treated with herbal medicine (modified Guibi-tang) and acupuncture. The effects of these treatments were evaluated using the Korean Mini Mental Status Examination (K-MMSE-2), Cognitive Dementia Rating (CDR), Manual Muscle Test (MMT), Korean version of Modified Barthel Index (K-MBI), and Videofluoroscopic Swallow Study (VFSS). Results: After treatment, the K-MMSE-2 score, CDR score, MMT, K-MBI, and VFSS improved. Conclusion: The results suggest that traditional Korean medicine may be effective for the symptoms of vascular dementia caused by intracerebral hemorrhage.

통규활혈탕가감방을 포함한 한의복합치료로 뇌동맥류 파열에 의한 뇌실내출혈을 동반한 지주막하출혈 환자의 사지마비에 개선을 보인 증례보고 1례 (A Case Report of Quadriparesis due to Subarachnoid Hemorrhage with Intraventricular Hemorrhage due to Cerebral Aneurysm Rupture Improved after Treated with Korean Medicine Treatment Including Tonggyuhwalhyeol-tang-gagambang)

  • This study reported the case of a patient with quadriparesis due to cerebral aneurysm rupture, subarachnoid hemorrhage, and intraventricular hemorrhage (IVH) treated with Korean medicine. The patient was treated with acupuncture, herbal medicine (mainly Tonggyuhwalhyeol-tang-gagam), Western medicine, moxibustion, cupping, and rehabilitative therapy for 75 days, and improved after administration. After treatment, the Manual Muscle Test grade improved from 4/4-/3+/3 to 4+/4+/4+/4+, the Korean version of the modified Barthel Index score improved from 9 to 100, the National Institute of Health's Stroke Scale score improved from 2 to 0, the Global Deterioration Scale score improved from 3 to 2, and the Korean version of the Mini-Mental State Examination score improved from 22 to 30. During administration, the patient did not show seizures, shock, or loss of consciousness, and the vital signs were stable in the normal range. We followed up the brain computed tomography findings for 2 times and found that there was no definite evidence of intracranial hemorrhage or IVH or re-rupture or rebleeding after Korean medicine treatment. This study suggests that Korean medicine treatment with blood-invigorating and stasis-removing herbs could be a safe and effective intervention option for improving quadriparesis due to cerebral aneurysm rupture and subarachnoid hemorrhage.

자음건비탕가녹용을 비롯한 한방치료로 호전된 편마비를 동반한 뇌출혈 환자 1례 (A Case Report of Intercranial Hemorrhage with Hemiplegia Improved by Jayumkeonbi-tang with Deer Antler and Other Korean Medicine Treatments)

  • Objective: This study reports on the effectiveness of Korean medicine treatment in a patient with intracerebral hemorrhages with symptoms of hemiplegia, personality change, and aphasia. Case presentation: The patient was treated using Korean medicine, including Jayumkeonbi-tang with deer antler as herbal medicine, acupuncture, moxibustion, and a limb activation exercise. To evaluate the effect on clinical symptoms, a manual muscle test, the Modified Functional Ambulation Classification, and the Korean version of the Modified Barthel Index were used. After 84 days of treatment, the MMT grade of the right upper and lower limbs improved from Gr. 2+/2- to Gr. 4/3~4 on average, and personality change and aphasia were gone. Conclusion: A complex treatment that includes Jaeumkunbi-tang, acupuncture, moxibustion, and limb activation exercises can be effective following an intracerebral hemorrhage resulting in hemiplegia and personality change. However, more research is needed.

간접구 중심의 한의복합치료로 전대뇌동맥 및 중대뇌동맥 경색 환자의 중추성 통증과 하지마비에 개선을 보인 증례보고 1례 (A Case Report of Central Post-stroke Pain and Hemiparesis due to Anterior Cerebral Artery and Middle Cerebral Artery Infarction That Improved Following Treatment with Korean Medicine, Including Moxibustion)

  • This case report describes a patient diagnosed with central post-stroke pain (CPSP) and hemiparesis due to anterior cerebral artery and middle cerebral artery infarction. The patient was treated with Korean medicines, including moxibustion, acupuncture, electroacupuncture, herbal medicine, Western medicine, and rehabilitation therapy for 67 days. CPSP improved based on scores on the Numeric Rating Scale (NRS) (from 8 to 0), paresthesia NRS (7 to 0), Manual Muscle Test (4/1 to 4+/4-),4+/4-), Korean version of the modified Barthel Index (24 to 73), National Institute of Health's Stroke Scale (7 to 0), Global Deterioration Scale (1 to 1), and Korean version of the Mini-Mental State Examination (27 to 29). No seizures, shocks, recurrent ischemic stroke, and liver dysfunction were recorded during the treatment. A moxibustion-induced burn healed within 7 days. This case suggests that moxibustion, combined with other treatments, has the potential to improve CPSP, without severe side effects.

  • Multiple system atrophy is a neurodegenerative disease that causes diverse bodily dysfunctions (cerebellar, pyramidal, automatic, and urological, in any combination), as well as Parkinsonism. Patients with multiple system atrophy commonly display antecollis, a condition where the patient's head tilts forward more than 45 degrees. Despite its common occurrence in these patients, no current standardized therapies are effective for treating antecollis. In this study, Korean medicinal treatments, including Chuna manual therapy, pharmaco-acupuncture, bee venom acupuncture, acupuncture, herbal medicine, and moxibustion therapy, were administered to the patient over a 27-day period. After the treatment, assessments of the head position on the EPIS-PD scale (Part I) and at a standing position from the side (Part II) both revealed improvements. As the head flexion angle decreased, the patient's head posture improved, as determined by a decrease in angle from 80 degrees to 30 degrees in the upright, standing position. As a result, patients who previously were unable to walk without the support of walking frames could now roam freely and independently, with significant increases in both walking speed and distance. In essence, this study suggests that Korean medicine is an effective treatment for patients with multiple system atrophy who suffer from antecollis and gait disorders.

  • Objective: This case study reports improvement in a case of Guillain-Barre Syndrome by Korean medicine treatment in a patient with quadriplegia and anorexia. Methods: A 79-year-old woman diagnosed with Guillain-Barre Syndrome was treated with acupuncture, electroacupuncture, Bojungikgi-tang-gami, cupping, moxibustion, and rehabilitation. Her clinical symptoms were measured with the Manual Muscle Test (MMT), Modified Barthel Index (MBI), Functional Independence Measurement (FIM), and the EuroQol five-dimensional descriptive system (EQ-5D-5L). Results: After treatment, improvements in the MMT, MBI, FIM, and EQ-5D-5L were observed and her anorexia decreased. Conclusion: Korean medicine treatment appeared to be effective in the treatment of a patient with Guillain-Barre Syndrome with quadriplegia and anorexia.

우측 어깨의 충격증후군을 동반한 뇌졸중 후 중추성 통증 환자 치험 1례 (Case Report of Treatment with Korean Medicine on a Central Post-Stroke Pain Patient who had Impingement Syndrome of the Right Shoulder)

  • Objectives: This case study examined the effects of treatment with traditional Korean medicine on a central post-stroke pain patient, who had impingement syndrome of the right shoulder. Methods: The patient received traditional Korean medical treatments, including acupuncture, warm-needling, moxibustion, and cupping, throughout the treatment period, and extracorporeal shock-wave therapy and physical therapy for part of the treatment period. The effects of treatments were measured with a manual muscle test (MMT), numerical rating scale (NRS), range of motion (ROM), and gait staging index (self-invented). Results: The clinical symptoms of the patient were improved considerably after the complete treatment. Conclusions: Traditional Korean medicine may be effective for treatment of central post-stroke pain in patients with impingement syndrome.
