무치악 환자에게 임플란트 지지 고정성 보철수복 치료는 그 효용성이 검증되었으나, 복잡한 치료과정이 수반된다. 반면, 현대 치의학에서 디지털 기술력은 나날이 발전하여 현재 무치악 환자의 임플란트 수복치료까지 그 범위를 확장하였다. 본 증례에서는 66세 하악 무치악 환자의 임플란트 고정성 보철 수복에 대하여 진단부터 수술, 임시수복물 및 최종수복물의 디자인과 제작까지 디지털 시스템을 모든 단계에 이용하였다. 수술 전 진단단계에서 구강스캐너를 이용한 점막스캔과 환자의 안정적인 기존 총의치 교합을 토대로 임시수복물을 디자인하였다. 수술가이드를 이용하여 무절개 임플란트 식립 후에 사전제작한 임시수복물을 임플란트와 연결하여 즉시 임시수복물로써 사용하였다. 디지털 기술을 이용하여 안정적인 교합이 형성된 임시수복물의 수직고경과 중심위를 이전하여 최종수복물을 디자인 및 제작 하였다. 디지털 기술을 활용하여 환자의 수직고경과 교합을 보전하는 방법을 사용함으로써 무치악환자에서 복잡할 수 있는 임플란트 치료의 간단한 프로토콜을 정립하였기에 보고하는 바이다.
치과의사는 악간관계기록 오류로 인해 발생하는 교합오차를 줄이기 위해 진료실재부착을 시행한다. 진료실재부착으로 수정된 의치를 사용하면 일반적으로 통증이 감소하고 의치 사용이 편해지며 더 편하게 저작할 수 있다고 한다. 하지만 이러한 평가는 환자의 주관적인 평가이므로 본 연구에서는 실험적인 지표를 통해 진료실재부착의 효과를 확인하고자 하였다. 7명의 환자를 대상으로 전치부에서 1 - 2 mm의 공간이 있도록 중심위 악간관계를 채득하여 진료실재부착을 시행한 후 미끄러짐 유무를 확인하였으며, 압력감지센서로 교합력을 측정하였고 실리콘인기재로 교합면적을 측정하였다. 진료실재부착후 교합압의 변화는 없었으나 교합면적 증가와 함께 접촉 후 미끄러짐이 줄어드는 것을 확인할 수 있었다. 진료실재부착을 시행하면 조기접촉이 감소하고 교합면적이 증가하므로 시행하는 것이 필요하다.
연구목적: 캔틸레버의 위치와 길이는 임플란트와 보철물 또한 주위 골조직의 응력분포에 중요한 영향을 미친다. 하악 무치악의 경우 기존에는 양측 이공사이에 4-6개의 임플란트를 식립하고 상부보철물을 캔틸레버형으로 제작해왔는데 캔틸레버 부위에 무리한 하중이 작용하게 되면 응력의 집중과 굽힘 현상으로 인하여 최후방 임플란트 부위의 지지골 파괴와 임플란트 및 상부 보철물의 파절을 초래했다. 이러한 캔틸레버의 약점을 보완하기 위해 1992년 McCartney가 Rest implant 개념을 2003년에는 $Mal{\acute{o}}$ 등이 All-on-Four implant 개념을 소개하여 기존 보철물의 캔틸레버 길이를 줄이려고 노력하였다. 재료 및 방법: 기존의 캔틸레버형 보철물과 rest implant, All-on-Four implant 시스템을 삼차원 모델링하여 하중을 제 1대구치 부위에 수직으로 300 N, 수평으로 설측에서 협측으로 75 N을 가하여 지지골과 임플란트, 상부보철물에 발생하는 응력의 크기와 분포 및 분산양상을 유한요소 해석 프로그램인 ANSYS (Ver. 10.0, Swanson Analysis System Inc., USA)를 이용하여 분석하였다. 결과: 1. 레스트 임플란트 및 All-on-Four 임플란트법은 기존 방법에 비해 하악골과 상부 보철물의 응력 분산에 크게 영향을 미치는 것으로 나타났다. 2. 지지골, 임플란트, 상부 보철물에서의 응력분산은 레스트 임플란트가 가장 우수한 것으로 나타났다. 3. 같은 개수의 임플란트인 경우 후방 임플란트를 경사시켜 캔틸레버의 양을 줄이는 것이 기존 방식에 비해 저작압 분산에 유리하다.
Purpose: The aim of this work is to evaluate a surgical technique for mobilization of mal posed dental implant in anterior area. Methods: A 38-year-old patient consulted our unit for esthetic dissatisfaction with the implant treatment of a central incisor. An implant was observed in 11 and 21, where 11 was 3 mm above the ideal limit, with excessive vestibular angulation. The choice was made to perform a segmental osteotomy and mobilize the bone block and the implant down and forward; a bone block extracted from the mandibular ramus was installed between the implant block and the bed to stabilize the segment. Results: After 4 months, a conventional fixed prosthesis was created and the esthetic result achieved was close to what the patient wanted, with no need for further surgery. The surgical condition was stabilized and maintained for the long-time and no complications how necrosis, infection or bone defects was present. Conclusions: It was concluded that the procedure is efficient, and the biological arguments in favor of the procedure are discussed.
Vascularized iliac crest flap include bone tissue of good quality and quantity for mandible segmental defect. Even if fibular flap can contain longer bone tissue, iliac crest has esthetic shape for mandible body reconstruction and large height for implant. Conventional vascularized iliac crest osteomyocutaneous flap is too bulky for reconstruction of intraoral soft tissue defect. But modified flap can reduce soft tissue volume, so is good for functional reconstruction of oral mucosa. It takes only one month for completely replace oral mucosa. The final mucosal texture is much better than other skin paddle flap, especially for implant prosthesis. Donor site morbidity of this method looks same level or less with other modalities functionally and socially. In case of oral mucosa-mandible combined defect, vascularized iliac crest with internal oblique muscle flap shows good outcomes for hard and soft tissue.
There are some cases that dental prosthesis does not operate as properly as expected in oral mouth. The reasons are such as a distortion of the mandibular, a fault of impression taking system or an extrusion of remaining teeth. One of dental prostheses to consider in the situations is the attachment which connects segment bridge. Active discussions are managed on theoretical side of this field but few on clinical side of it, which must be considered first. Accordingly I'd like to suggest a theoretical background for connect attachment of fixed segmented bridge. 1. As a bridge gets longer, burden on dental ligament is increased and the hardness of a bridge is lessened. 2. The flexibility of a bridge increases in ratio to 3 multiplication of the length and decreases in ratio to 3 multiplication of the width of occlusal surface and base of pontic. 3. Precision rest is needed to cope with the shake of teeth and the difference of axis direction among abutments. 4. Female part of the precision rest should be on middle abutment distal and male one on mesial of pontic. 5. Segmented attachment can be efficiently used to cope with long span bridgework and also in case that one piece casting can't be done because of slant of abutment.

Three-dimensional solid model has not been widely used in surgical prediction of orthognathic surgery because frequent artifacts from occlusal restorations or prosthesis limited the usefulness of simulated surgery involving occlusion. We prepared three-dimensional(3D) solid model from CT data and integrated the 3D solid model with dental cast using a face-bow transfer technique combined with skeletal reference measurement and confirmation with cephalometric radiographs. With this simple and easy method, it was possible to predict bony interference between the proximal and distal segment of the mandible so that we can prevent condylar displacement after sagittal split ramus osteotomy of the mandible with prominent asymmetry. The method error was within 2mm and it seemed to be useful in preoperative planning for maxillofacial surgery with maxillo-mandibular occlusal change.
Statement of problem : Since the concept of osseointegrated dental implant by $Br{\aa}nemark$ et al was first applied to mandibular full edentulous patients. Recently it is considerated the first treatment option on missing teeth. A common problem associated with dental implant restorations is loosening of screws that retain the prosthesis to the abutment and the abutment to the implant fixture. Purpose : This study is to examine the influence on screw loosening of implant-abutment designs. Material and methods : External hex, cone screw, beveled hex, cam cylinder, cylinder hex by means of evaluating the loosening torques, with respect to a range of tightening torques after repeated loading. Result : 1. Cone screw, beveled hex groups are the highest initial tightening rate and cylinder hex, external hex groups are the lowest initial tightening rate (p < 0.05). 2. Cone screw groups are the highest after repeated loading tightening rate and cylinder hex groups are lowest after repeated loading tightening rate(p < 0.05). 3. Cone screw groups have the highest initial stability and anal stability. 4. All groups are decreased tightening rate after repeated loading.
Purpose: The purpose of this study is to evaluate the marginal gap of all-ceramic crowns fabricated by CEREC$^{(R)}$ in-office CAD/CAM system. Methods: The mandibular first molar was selected as the abutment for the experiments. Thirty working models were prepared. VITA Mark II(VM) and VITA Enamic(VE), LAVA Ultimate(LU) blocks were milled using CEREC MCXL with CEREC 3D system to construct 10 crowns for each groups. To measure marginal gap, milled restorations were examined under digital microscope with scale under 160x magnifications. The results were statistically analyzed using the one-way ANOVA and Tukey's HSD test(${\alpha}=0.05$). Results: There was no significant difference in the marginal gap regarding to ceramic materials(p>0.05). Conclusion: Single crowns fabricated using CEREC in-office CAD/CAM system provided clinically acceptable marginal gap. This confirmed that the type of ceramic material used does not determine the precision of fit of a prosthesis.
전악 구강 수복에서 이상적인 교합평면을 형성하기 위한 기존의 진단 및 치료계획은 각각의 과정이 복잡하고 치과의사와 기공사간의 정보교환이 주관적이므로 어려움이 있다. OP finder® system은 동공간선이 평행하지 않은 환자의 정보를 보다 객관적이고 정확하게 전달하는데 도움이 된다. 본 증례에서는 오래된 불량한 고정성 보철물과 심하게 마모된 하악 치아를 가진 환자에게 OP finder® system을 이용하며 전악수복을 시행하였고, 적절한 교합평면 설정 및 저작기능 회복 결과를 얻었기에 보고하고자 한다.
