과잉치에 관한 X선학적 연구 (A RADIOGRAPHIC STUDY OF SUPERNUMERARY TEETH)

  • The purpose of this study was to evaluate the incidence and radiographic features of 1,300 cases of supernumerary teeth by means of the analysis of periapical radiograms and/or pantomograms in 69,442 persons visited the Department of Oral Radiology, School of Dentistry, Kyung Hee University during January 1980 to December 1989. This study of supernumerary teeth revealed the following features: 1. The incidence of supernumerary tooth was revealed to be 1.5% in total examined persons, and there was a higher incidence in males (74.0%) than in females (26.0%). 2. The supernumerary teeth were most frequently occurred in the 1 st decade (42.7%), followed by the 2nd decades (20.2%), the 3rd decades (14.1 %), and the 5th decades (7.2%). 3. There was a higher incidence in the maxilla (98.7%) than in the mandible (1.3%), and mesiodens (90.1 %) was the most frequently occurred. The maxillary lateral incisor region (3.1%) was next in order of frequency followed by maxillary distomolar (2.2%), and maxillary paramolar (1.5%). 4. There was a higher incidence of the impaction (82.7%) than that of the eruption (17.3%), and supernumerary tooth in lateral incisor region (87.8%) was the most frequently impacted one. 5. The inverted impaction was occurred in 63.1%, the vertical impaction in 18.1%, and the angulated impaction in 18.8%. In localization, a palatally (lingually) impacted supernumerary tooth was occurred in 85.0%, and middle impacted and buccally (labially) impacted supernumerary tooth showed the same incidence (7.5%). 6. A supplemental tooth was occurred in 3.2%, an accessory tooth in 96.8%, and a supplemental tooth was the most frequently occurred in maxillary lateral incisor and mandibular premolar region. 7. In effects of supernumerary tooth on adjacent tooth, crowding was occurred in 0.4%, diastema in 10.6%, rotation in 5.4%, eruption disturbance in 4.5%, root resorption in 2.5%, and dentigerous cyst in 2.2%.

Primary Mandibular Tuberculous Osteomyelitis Mimicking Ameloblastoma: A Case Report and Literature Review of Mandibular Tuberculous Osteomyelitis

  • Primary tuberculous osteomyelitis involving the mandible represents less than 2% of skeletal locations. In this paper, we report a case of mandibular tuberculosis (TB) detected after histopathological analysis of the surgically resected specimen during surgical management of a suspected case of ameloblastoma. A 14-year-old male patient presented to us with history of right-sided chin swelling. The clinical examination revealed a swelling, involving right body and parasymphysis of mandible, measuring approximately 6 cm in length and 2 cm in width, extending from right lateral incisor till the first molar. Radiological scans revealed a large multiloculated osteolytic expansive lesion measuring 52 × 20 × 18 mm. Excision of the lesion was performed and reconstruction was done with iliac bone grafting. The histopathological findings revealed a granulomatous lesion, suggestive of tuberculous osteomyelitis. The patient was successfully treated with standard multidrug therapy. One year after completion of therapy, there were no signs of recurrence. Primary mandibular TB is an extremely rare entity. Its clinical presentation is not specific. Radiologically, TB has no characteristic appearance. The positive diagnosis is based on histology. Primary mandibular TB is rare and should be kept among differential diagnoses in susceptible population and in endemic areas.

  • Surveying the calcification degree of permanent tooth crown in 719 Korean children (Male 387, female 332) from 2 to 10 years old by orthopantomograph, the author got the following results. 1. Female was earlier than male in calcification of permanent teeth. 2. The results of the complete calcification of the permanent tooth crown were as follows. 3. The completion of calcification in the mandibular crown was seen earlier than that of the maxilla. 4. The order of calcification in permanent tooth crowns was as follows : 1 st molar, central incisor, lateral incisor, canine, 1st premolar, 2nd premolar, and 2nd molar. 5. The completion of calcification of the permanent crowns in Korean children was slightly retarded comparing with the Japanese and the American children.

전치부 매복치에 관한 교정학적 고찰 (ORTHODONTIC CONSIDERATION ON THE IMPACTED TEETH OF ANTERIOR SEGMENT)

  • A study on the impacted teeth of the anterior segment was carried out. The data for this study were compiled form 1739 outpatients of the Department of Orthodontics, College of Dentistry, Seoul National University. The following conclusions were obtained; 1. The frequency of the impacted teeth of the anterior segment was 2.8% and the frequency of male was 4.0%, that of female was 2.1%., respectively. 2. The order of occurrence of the impacted teeth in the anterior segment was maxillary canine mesiodens, maxillary central incisor, maxillary lateral incisor, mandibular canine. 3. The etiologic factors of the impaction were the space deficiency for eruption, cleft palate, the prolonged retention of deciduous teeth, dentigerous cyst. 4. The favorable results of treatment and prognosis were obtained from the impacted teeth of the normal shape after surgical exposure and adhered the plastic attachment and inducted them into the dental arch.

교흔감정을 위한 상하악 전치부 치열상태에 관한 연구 (Analysis of Anterior Dentition for Identification of Bite-mark Evidence)

  • A human bite-mark shows special feature according to the suspect's dentition. The teeth which most frequently give useful bite-marks are six upper and lower anteriors, while the premolar teeth somtimes give marks it is often difficult to distinguish. The author tried to classify and to analize the anterior dentition which makes the bite-mark directly by means of 672 maxillary and 691 mandibular stone model taken from Korean adult aging from 17 to 40 years old. The results were as follows : 1. There was no particular correlation between the presence of six normal-shaped and correctly positioned upper and lower anteriors and the presence of rotation of teeth. 2. Inter central incisor, inter lateral incisor, inter canine width and angles of adjecent teeth were not identical eath other in studied models. 3. The results of this analysis supported the statement that any bite-mark had no same feature.

파노라마 X선사진을 이용한 치아수 이상에 관한 연구 (A STUDY OF TOOTH NUMBER ANOMALY USING PANORAMIC RADIOGRAPHS)

  • 경북대학병원 치과에 내원한 8세에서 27세까지의 환자 6,531명의 파노라마 X선사진에서 제3 대구치를 제외한 선천성 결손치, 제3대구치의 선천성 결손, 과잉치의 발생율, 성별분포, 호발 부위 및 수에 따른 분포를 조사하여 다음의 결과를 얻었다. 1. 제3대구치를 제외한 선천성 결손치의 발생율은 10.8%였으며, 남성이 44.6%, 여성이 55.4%였다. 하악 제2소구치가 23.2 %로 가장 많았으며, 상악 측절치 18.4%, 하악 측절치 18.3%, 상악 제2소구치 15.4%의 순으로 나타났다. 결손치의 수는 1개 인 경우가 48%로 가장 많았으며, 2개인 경우가 35.4%, 3개인 경우가 6.6%로 나타났다. 2. 제3대구치 선천성 결손의 발생율은 39.7%였으며, 남성이 48.6%, 여성이 51.4%였고, 상악 60.3%, 하악이 39.7%였다. 결손치의 수는 1개인 경우가 28.7%, 2개인 경우가 37.2%, 3개인 경우가 12.5%, 4개인 경우가 21.6%로 나타났다. 3. 과잉치의 발생율은 4.2%였으며, 남성이 65.7%, 여성이 34.3%였다. 상악 중절치부가 64.8%로 가장 많았으며, 상악 측절치부 13.2%, 상악 제3대구치 후방부 8.7%의 순으로 나타났다. 과잉치의 수는 1개인 경우가 79.9%로 가장 많았고, 2개인 경우가 18.9%, 3개인 경우가 1.2%로 나타났다.

  • Purpose: The purpose of the study is to evaluate the effectiveness of anterior segmental osteotomy (ASO) in bimaxillary protrusion (BP) patients by comparison between the mandibular soft and hard tissue changes from orthodontic treatment and ASO. Methods: All 44 patients were diagnosed with BP in Kyung-Hee Medical Center. Orthodontic treatment with teeth extractions were underwent by 23 patients (Group A) and 21 patients underwent ASO (Group B). Mandibular soft and hard tissue changes were measured and evaluated, which were based on the vertical and horizontal reference line in lateral cephalometric radiographs. Statistical significance between the changes and correlation between each measurement were analyzed. Results: The amount of B point movement was lesser than that of the lower incisal tip (LIT) retraction, and LIT was tilted lingually in group A. The posterior movement discrepancy between LIT and B point was insignificant, and the inclination of lower incisor was not changed in group B. The soft tissues, including the lower lip, showed a posterior movement and reduction in the depth of mento-labial groove. According to the correlation analysis, the movement of the lower incisor was significantly related to the movement of the lower lip in group A, and the movement of the lower incisor was significantly related to that of the movement of lower lip, B point and Pog in group B. Conclusion: The orthodontic treatment in BP patients results in posterior tilting movement of the lower incisor, but ASO results in the bodily movement of the lower incisor. Consequently, ASO is more effective in BP cases because it ensures the controlled movement of the lower incisors.

하악 우측 측절치에 발생한 선양 치성 종양 (ADENOMATOID ODONTOGENIC TUMOR ASSOCIATED WITH AN IMPACTED MANDIBULAR RIGHT LATERAL INCISOR)

  • 선양 치성 종양(adenomatoid odontogenic tumor)은 악골에 드물게 발생하는 치성 기원의 종양이다. 이전에는 법랑모세포종(ameloblastoma)의 한 종류로 여겨졌으나 양성이며 재발되는 경우는 극히 드물다. 상악 견치에 호발하며 대개는 무증상이지만 크기가 천천히 커져 무통성 종창을 일으킬 수 있다. 방사선학적으로 함치성 낭종(dentigerous cyst) 및 단방성 법랑모세포종(unicystic ameloblastoma)과 매우 유사하다. 일반적으로 보존적인 소파술(curettage) 및 적출술(enucleation)을 통해 치료한다. 본 증례는 하악 우측 측절치 미맹출을 주소로 내원한 10세 여자 환아에서 방사선 사진 검사를 통해 치아의 매복 및 병소를 확인하고 적출술을 시행하였다. 치아에는 브라켓을 부착하여 교정적 정출을 유도하였다. 조직학적 검사를 통해 선양 치성 종양으로 확진되었으며 양호한 치료 결과를 보였기에 보고하는 바이다.

복잡 치아종으로 인한 매복치아의 교정적 견인 (THE ERUPTION GUIDANCE OF AN IMPACTED TOOTH ASSOCIATED WITH A COMPLEX ODONTOMA : CASE REPORT)

  • 치아종은 치성 종양 중 가장 흔한 형태로 증상이 없는 경우가 대부분이며 종종 영구치의 매복이나 맹출지연을 유발한다. 이는 치아를 구성하는 법랑질, 상아질, 백악질, 치수로 구성되며, 형태에 따라 복합 치아종과 복잡 치아종으로 구분된다. 복합 치아종은 다수의 작고 치아와 유사한 형태를 지니며 상악 전치부에 호발하고, 복잡 치아종은 무정형의 치성조직으로 구성되어 있으며 전체 치아종의 약 25%를 차지하고 하악 구치부에 호발한다. 치아종의 병인은 확실치는 않으나 국소적인 외상, 감염, 유전적 요인이 관여할 것이라 추측된다. 치료는 보존적인 외과적 적출술이 추천되며 재발은 거의 없다. 다음의 두 증례는 각각 하악 제 1대구치와 하악 측절치의 맹출 지연을 주소로 본원에 내원한 환아들로, 매복치 상방의 복잡 치아종을 외과적 적출술을 시행하고 매복치의 외과적 노출 및 교정적 견인을 시행하여 양호한 결과를 보였기에 보고하는 바이다.

Clinical effects of different prescriptions on the inclination of maxillary and mandibular incisors by using passive self-ligating brackets

  • Objective: Controlling the incisal inclination is fundamental in orthodontics. However, the relationship between the inclination prescription and its clinical outcome is not obvious, and the incisal inclination changes generated by different bracket prescriptions were investigated. Methods: Twenty-eight non-extraction dental Class II patients (15 females, 13 males; mean age = 12.9) were retrospectively analyzed. Patients were treated using passive self-ligating fixed appliances with three inclination prescriptions for maxillary incisors (high, standard, low), and two for mandibular incisors (standard, low). Clinical outcomes were compared among different prescriptions, and regression analysis was used to explain the effects of bracket prescriptions and to understand the prescription selection criteria (α = 0.05). Results: For maxillary central incisors, low and high prescriptions were related to linguoversion (p = 0.046) and labioversion (p = 0.005), respectively, while standard prescription maintained the initial dental inclination. Maxillary lateral incisors did not show significant changes. For mandibular incisors, low prescription led to linguoversion (p = 0.005 for central incisors, p = 0.010 for lateral incisors), while standard prescription led to labioversion (p = 0.045 for central incisors, p = 0.005 for lateral incisors). The factors affecting inclination changes were the imposed change and selected prescription, while prescription selection was influenced by the initial dental inclination and initial intercanine distance. Conclusions: The direction of correction of incisal inclination can be controlled by choosing a certain prescription, but the final inclination may show limited consistency with it. The amount of imposed inclination change was the most relevant predictor of the clinical outcome.
