PURPOSE. This study aimed to compare the bite force (BF) between complete dentures and implant overdentures (IODs) retained by two mandibular implants. Additionally, we evaluated the quality of life (QoL) and patient satisfaction among individuals using IODs. In addition, the effects of demographic parameters such as age and sex, and clinical parameters such as implant length, implant diameter, attachment height, attachment color, and interimplant distance on BF, QoL, and patient satisfaction were evaluated. MATERIALS AND METHODS. A total of 51 edentulous patients rehabilitated with the maxillary complete dentures and mandibular IODs retained by two implants were included in this study. BF was measured using a force meter pre- and post-implant in the same patients. Oral health-related quality of life (OHRQoL) was assessed with the Oral Health Impact Profile (OHIP-14) and patient satisfaction was assessed with the Visual Analog Scale (VAS) questionnaires. RESULTS. It was found that BF values were statistically higher for IODs than complete dentures (P < .001). In terms of attachment height of the OHIP scores, there was a significant difference in the psychological disability and social disability domains (P < .05). When examining the change in patient satisfaction as a function of sex, it was found that mandibular retention satisfaction differed significantly by sex (P < .05), but there was no significant difference in the other domains. CONCLUSION. Within the limitations of this study, it was observed that the BF increased after the use of IODs. Several factors, including age, interimplant distance, attachment height, and attachment color, were found to impact OHRQoL. Sex and implant diameter were identified as factors affecting patient satisfaction.

SFI-bar is prefabricated bar system and can be assembled at chairside without soldering or welding, thus reducing bone loss, costs and time. A 53-year-old male patient, who had severely absorbed mandible, hoped to wear a stable mandiblular denture. Four implants were placed in the extraction site of canine and 1st molar. Early loaded temporary denture with solitary type attachment was delivered 3 weeks after surgery. 3 month later, SFI-bar was connected and adjusted at chairside. Then, implant overdenture using SFI-bar was delivered. This case report showed that a satisfactory clinical result was achieved by 4-implant-supported overdenture using the SFI-Bar system in a mandibular edentulous patient.

For edentulous patients with unstable mandibular movements and abnormal facial features due to condylar fractures and morphological abnormalities, it is important to find a stable mandibular position. In this case, the patient's facial improvement, mandibular movement stability, and denture stability were improved by using flat table treatment dentures. In addition, computer-aided design/manufacturing (CAD-CAM) technology was used in denture fabrication to maintain the vertical dimension and lip support of flat table treatment dentures, we report good results in improving the patient's discomfort.

Compared with traditional full dentures, mandibular implant overdentures have the advantage in that good support and retention can be obtained even with two implants. When manufacturing a mandibular implant overdenture using two implants, it is important to place the implant in the correct position. The long-term prognosis of overdenture is good when two implants are placed in the direction vertical to the occlusal plane and parallel to each other at the canine position. However, it is difficult to place two implants in the correct position in edentulous patients, and if you use surgical guides in these cases, you can get help in placing the implants in a prosthetically advantageous position. This case, a 57-year-old male patient, came to our hospital with all upper and lower teeth removed due to periodontal disease. Therefore, the maxilla was restored with a traditional full denture and the mandible was restored with an implant overdenture using two implants, which resulted in satisfactory functional and esthetic results.

Suction dentures enhance retention and support by forming negative pressure temporarily at the internal surface of denture base at times of swallowing and chewing because the areas surrounding the denture flanges are sealed by mobile mucosa. In this case, an 81-year-old male visited for new dentures. Considering the high expectations for retention and masticatory efficiency of dentures, fabricating complete dentures with suction dentures was planned. Preliminary impression was taken without applying pressure on retromolar pad area and diagnostic cast was fabricated. Afterwards, individual tray was made and final impression was taken, at the same time, gothic arch tracing was done to acquire centric relation and vertical dimension. Then, anatomic teeth were placed on maxilla and non-anatomic teeth were placed on mandible forming lingualized occlusion. Consequently, restoring a complete edentulous patient with complete dentures using mandibular suction denture resulted in recovering satisfying retention and function.

In patients with severely atrophic mandibular posterior area, implant placement is a problematic surgical procedure. Inferior alveolar nerve transpositioning technique offers better initial stability of implant and reduce the risk of permanent nerve damages. In this case report, the patient has bilateral atrophic mandibular posterior edentulous area. We placed 3 implants on each mandibular posterior area in conjunction with bilateral inferior alveolar nerve transpositioning to achieve sufficient bone height. The patient complained of paresthesia in lower lip and chin area and ptosis of lower lip after surgery. Neurosensory function was normal in 10 weeks after surgery.

Conventional denture impression techniques have limitations for edentulous patients with severe alveolar bone resorption and can cause problems from excessive border extension. Especially when a patient has movable tissue it is difficult to make accurate impression, thus might interrupt stable seating of complete denture. Fabrication of complete denture using closed mouth technique for edentulous patient with severe ridge resorption is thought to provide better stability and retention. In this case, an 86-year-old patient had both edentulous jaws with epulis fissuratum on maxillary anterior ridge and severe mandibular ridge resorption. Thus, tentative vertical dimension was determined by using Centric trayand individual tray attached with gothic arch tracer was fabricated. Complete denture was fabricated using closed mouth technique and the patient was satisfied with better stability and retention of the complete denture.

In the case of edentulous patients, the total amount of occlusal force is dispersed by the keratinized gingiva during mastication, in result, causing lower masticatory and chewing efficiency. In particular, the mandibular area has more side effects such as pain than the maxilla has. It gets worse when the patient has more absorption of alveolar bone, but the implant treatment is often interrupted due to the existence of the inferior alveolar nerve. In this case, a patient treated with the all-on-4 method by placing the implant in the anterior part of mandible and with the conventional complete denture for the maxilla has maintained without complications and was satisfied with the restoration both functionally and esthetically.

Flabby ridges commonly occur in edentulous patients. Inadequate retention and stability of a complete denture are the often encountered problems in these patients. A liquid supported denture due to its flexible tissue surface allows better distribution of stress and hence provides an alternate treatment modality in such cases. This case report presents the use of a liquid supported denture in a patient with completely edentulous maxillary arch with flabby tissue in anterior region opposing a partially edentulous mandibular arch.

BACKGROUND. In order to restore severely attrited teeth properly, surgical intervention in the form of a crown-lengthening procedure may be required. And also, proper diagnosis and treatment sequencing is critical to obtain a successful results. Adequate diagnostic wax-up ensures good esthetics and healthy periodontal tissue. CASE DESCRIPTION. This clinical case report describes a diagnostically based protocol for restoration on mandibular anterior teeth with crown lengthening procedure and the treatment of partially edentulous mandible combined with an edentulous maxilla. In addition, the effort to prevent the combination syndrome was described. CLINICAL IMPLICATION. An interdisciplinary diagnosis and examination through visualization of the desired results ensure conservative and more predictable outcome.
