This case report presents inferior alveolar nerve (IAN) repositioning as a viable approach for implant placement in the mandibular molar region, where challenges of severe alveolar bone width and height deficiencies can exist. Two patients requiring implant placement in the right mandibular molar region underwent nerve transposition and lateralization. In both cases, inadequate alveolar bone height above the IAN precluded the use of short implants. The first patient exhibited an overall low alveolar ridge from the anterior to posterior regions, with a complex relationship with adjacent implant bone level and the mental nerve, complicating vertical augmentation. In the second case, although vertical bone resorption was not severe, the high positioning of the IAN within the alveolar bone due to orthognathic surgery raised concerns regarding adequate height of the implant prosthesis. Therefore, instead of onlay bone grafting, nerve transposition and lateralization were employed for implant placement. In both cases, the follow-up results demonstrated successful osseointegration of all implants and complete recovery of postoperative numbness in the lower lip and mentum area. IAN repositioning is a valuable surgical technique that allows implant placement in severely compromised posterior mandibular regions, promoting patient comfort and successful implant placement without permanent IAN damage.
Objectives: This study was performed to evaluate course of the inferior alveolar canal in the mandibular ramus and to find safety zone when ramal bone is harvested. Patients and Methods: From January, 2009 to February, 2009, the 20 patients who visited in the Department of Oral and Maxillofacial Surgery, Sanbon Dental Hospital. Wonkwang University and the Conebeam CT was taken of various chief complaints, were selected. The patients who had left and right mandibular first molar and incisor missing, jaw fracture and bone pathology were excluded. The R point was defined as the point which occlusal plane was crossed to the mandibular anterior ramus(external oblique ridge). In the cross-sectional coronal and axial views, the inferior alveolar canal position to the R point, buccal bone width(BW), alveolar crest distance(ACD), distance from alveolar crest to occlusal plane(COD) and inferior alveolar canal to sagittal plane(CS) were measured and horizontal distance(HD), vertical distance(VD) and nearest distance(ND) were measured. Results: The inferior alveolar canal is located $6.19{\pm}1.21\;mm$ from the R point. Horizontal distance from the R point were $13.07{\pm}2.45\;mm$, vertical distance from the R point were $14.24{\pm}2.41\;mm$ and nearest distance from the R point were $10.12{\pm}1.76\;mm$. The course of the inferior alveolar canal was positioned within $0.61{\pm}0.68\;mm$. The distance from external buccal bone to the inferior alveolar canal was increased from the R point anteriorly. Conclusions: It is considered that the mandibular ramus from the R point to 10 mm anteriorly can be harvested safely at ramal bone grafting.
The purpose of this study was to determine the thickness of the cortical bone at the mandibular angle in children and adolescents. The subjects were composed of 153 subjects who were 3 to 17 years of age that had not been history of bone disease or metabolic disease and no dental caries or tooth loss on mandibular molars. The subjects were divided into three age groups, group I (preschool children) was 3 to 5 years of age and grouop II (school children) was 6 to 11 years of age and group III (adolescence) was 12 to 17 years of age. Panoramic radiographs of these patients were collected and measurements of the thickness of the cortical bone were obtained at the mandibular angle. The average thickness of the cortical bone in the group I was 1.01$\pm$0.18 mm in boys and 1.07$\pm$0.22 mm in girls. In the group II was 1.23$\pm$0.18 mm in boys and 1.32$\pm$0.18 mm in girls. In the group III was 1.60$\pm$0.23 mm in boys and 1.58$\pm$0.20 mm in girls. Statistical analyses did not reveal any significant difference between the right or left side of the mandible but difference was exist between the same age groups by sex, especially in group II (p<0.01), except in group III. The greater thickness of cortical bone in the older age group was shown to be statistically significant. The correlation coefficient between age and the thickness of the cortical bone at the mandibular angle showed a high value of r=0.76.
Orthodontists often treat cases which are difficult to treat with conventional orthodontics. In such cases, it could be treated with surgical procedures with the help of an oral surgeon. Especially, transverse deficiency of the mandible can be corrected by widening the transverse width of mandibular symphysis, using distraction osteogenesis. Transverse widening of mandibular sympysis is known as a safe treatment but still complications could occur during the treatment. We are reporting some complications of cases that mandibular symphysis transverse widening were applied. Some cases showed complications because of the inappropriate osteotomy line. Since straight vertical osteotomy line was inclined to the left, only the left bony segment was likely to expand. According to bio-mechanical considerations, it will be better to perform a step osteotomy, cutting the eccentric area of the alveolar crest and the centric area of the basal symphyseal area. Complications could also occur by the failure of the distraction device. The tooth borne distraction device was attached on the lingual side of the tooth with composite resin. During the distraction period, it was impossible to obtain appropriate distraction speed and rhythm because of frequent fall off of the distraction device. Therefore, distraction device should be attached firmly with orthodontic band or bone screw, etc. Tooth mobility increasement could also occur as a complication. 'Walking teeth phenomenon' was observed during the distraction period, showing severe teeth mobility and pain during mastication. These symptoms fade out during the consolidation period. Since the patient could feel insecure and uncomfortable, it should be notified to the patient before the procedure. Finally, alveolar crestal bone loss could occur. Alveolar crestal bone loss occurred because of lack of distraction device firmness and teeth trauma caused by lower lip biting habit. Therefore, adequate firmness of the distraction device and habit control will be needed.
The purpose of this study was to investigate the functional adaptation of the tongue, the hyoid bone, the digastric muscle and the upper airway and the variables predicting postoperative stability following orthognathic surgery in patients with mandibular prognathism. 18 patients were selected(8 men and 10 women) for this study, who had received orthognathic surgery. Their lateral cephalograms, those were taken preoperatively, immediate postoperatively and over 6 months follow-up, were traced and analysed. The results were as follows : 1. The downward displacement of the hyoid bone and extended head posture were recognized, right after operation. Statistically significant correlations were found between the changes of the mandibular position and the digastric muscle and the change of head posture during operation. 2. The tongue was displaced downward following the hyoid displacement postoperatively. Statistically significant correlations were found between the change of the mandibular position and the change of the distance of the tongue and hard palate, and between the change of head posture and the changes of the upper airway sizes, the digastric muscle and the hyoid position during over 6 month's follow-up. 3. The change of the distance of the tongue and hard palate was the most significant factor for prognosis during over 6 month's follow-up. 4. There were no variables before operation to predict the postoperative stability and the constriction of the upper airway.
This is a retrospective study on facial bone fractures of Koreans. This study was based on a series of 110 patients who had been treated for facial bone fractures as in-patient at Guro Hospital, School of Medicine, Korea University for the period of September, 1984. to August, 1988. The results were obtained as follows : 1. 2nd decade(42.7%) was the highest age group in incidence, and age range was 2 years to 72 years, and the ration of Male/Female was 3.7 : 1. 2. Falling was most frequent cause of facial bone fractures(40.9%) 3. The frequent location of facial bone fractures were mandibule(67.1%), zygoma & zygomatic arch(11.7%), maxilla(10.9%), and others in order. 4. In mandibular fractures, one site fractures showed 30.1% and two site fractures showed 58.8% and triple site fractures showed 11.1%. The most frequent site of mandibular fracture was symphysis(32.7%) 5. The most frequent month of facial bone fracture was September.

Objectives: This study determined the effect of platelet-rich fibrin (PRF) on extraction socket bone regeneration and assessed the patterns and determinants of bone regeneration after the surgical extraction of impacted mandibular third molars. Materials and Methods: This prospective study randomly allocated 90 patients into two treatment groups: A PRF group (intervention group) and a non-PRF group (control group). After surgical extractions, the PRF group had PRF placed in the extraction socket and the socket was sutured, while the socket was only sutured in the non-PRF group. At postoperative weeks 1, 4, 8, and 12, periapical radiographs were obtained and HLImage software was used to determine the region of newly formed bone (RNFB) and the pattern of bone formation. The determinants of bone regeneration were assessed. Statistical significance was set at P<0.05. Results: The percentage RNFB (RNFB%) was not significantly higher in the PRF group when compared with the non-PRF group at postoperative weeks 1, 4, 8, and 12 (P=0.188, 0.155, 0.132, and 0.219, respectively). Within the non-PRF group, the middle third consistently exhibited the highest bone formation while the least amount of bone formation was consistently observed in the cervical third. In the PRF group, the middle third had the highest bone formation, while bone formation at the apical third was smaller compared to the cervical third at the 8th week with this difference widening at the 12th week. The sex of the patient, type of impaction, and duration of surgery was significantly associated with percentage bone formation (P=0.041, 0.043, and 0.018, respectively). Conclusion: Placement of PRF in extraction sockets increased socket bone regeneration. However, this finding was not statistically significant. The patient's sex, type of impaction, and duration of surgery significantly influenced the percentage of bone formation.
이번 연구의 목적은 골격성 III급 부정교합자의 악교정 수술 전과 수술 후의 하악전치부 치조골의 형태 변화를 평가하는 것이다 : 하악지 시상골분할절단술만을 시행받은 30명의 성인 골격성 III급 부정교합자를 대상으로 하였다. 초진시와 수술 전과 수술 후 3개월이 경과하였을 때의 측모두부규격방사선 사진을 비교하였다 골격과 하악이부의 계측치를 비교하였으며 이들 사이의 상관성 분석을 시행하여서 다음과 같은 결과를 얻었다. 1. 술전교정후군과 악교정수술후군에서 치료전군에 비해 하악전치부의 순측과 설측의 치조골 높이가 감소하였다. 2. 치조골 기저부의 전후방적인 폭경은 두개안면골격의 수직계측항목과 역상관관계를, IMPA와는 순상관관계를 보였다(P<0.01). 3. 하악이부 계측항목중 하악이부의 장경과 폭경은 두개안면골격과 상관관계를 보이지 않았다. 4. 순측의 치조골 높이와 설측의 치조골 높이는 순상관관계를 나타냈지만(p<0.001), 설측의 치조정의 폭경과는 역상관관계를 보였다(p<0.01). 순측과 설측의 치조정의 폭경은 역상관관계를 보였다(p<0.05). 하악이부의 장경과 폭경은 서로 순상관관계를 보였다(p<0.01). 5. IMPA와 LISA는 순, 설측치조골의 장경과 설측의 치조골 폭경과는 역상관관계를 보였지만, 순측치조골폭경과는 순상관관계를 보였다

Distraction osteogenesis was pioneered by Ilizarov in the treatment of injured extremities. Its subsequent application to Oral and Maxillofacial Surgery has opened a new chapter in the treatment of facial deformity. Careful monitoring of the period of distraction and consolidation of the callus is important and has been well described. Complications, such as infection, haematoma and premature ossification, are difficult to diagnose and can compromise the outcome after the surgery. Too slow rate of distraction results in premature fusion of cortices and too rapid information of bone 'cyst' within the callus distraction. I experienced 2 patients of congenital unilateral mandibular hypoplasia, so called Hemifacial Microsomia. After distraction surgery, I evaluated callus formation of mandibular distraction with Ultrasound Sonography during distraction and consolidation period. Plain radiography, although it enables accurate measurement of the distraction gap, did not give sufficient detail to allow assessment of early stages of bone formation, But, ultrasound monitoring could enable continuous monitoring of the distraction gap without exposure to ionizing radiation and allow detection of fine detail, which may influence manipulation of the callus.
Craniometaphyseal dysplasia (CMD) is a rare hereditary disorder characterized by hyperostosis of the craniofacial bones and flared metaphyses of the long bones. Although some reports have described the dentomaxillofacial characteristics of CMD, including increased density of the jaw, malocclusion, and delayed eruption of the permanent teeth, only a few studies have reported the distinct imaging features of CMD on panoramic radiography. This report presents 2 cases of confirmed CMD patients with an emphasis on panoramic imaging features. The patients' images revealed hyperostosis and sclerosis of the maxilla and mandibular alveolar bone, but there was no change in the mandibular basal bone. In both cases, the mandibular condyle heads exhibited a short clubbed shape with hyperplasia of the coronoid process. For patients without clear otorhinolaryngological symptoms, common radiologic features of CMD could be visualized by routinely-taken panoramic radiographs, and further medical examinations and treatment can be recommended.
