돼지 하악골의 micro-CT영상에서 추출한 3차원 골미세구조와 골강도 사이의 상관관계 (Relationship between trabecular strength and three-dimensional architecture in the pig mandible using microcomputed tomography)

  • Purpose : To investigate the relationship between three-dimensional (3D) bone imaging parameters and trabecular strength in the mandible. Materials and Methods : Bone specimens were obtained from the mandibles of five male pigs weighing around 110 kg each. Of those, 43 samples were selected for 3D analysis and measured by micro-computed tomography. The five morphometric parameters were trabecular thickness (Tb.Th), bone specific surface (BS/BV), percent bone volume (BV/TV), structure model index (SMI) and degree of anisotropy (DA). Through destructive mechanical testing, strength parameters were obtained. Results : BV/TV, SMI, BS/BV, and Tb.Th showed significant correlations with strength parameters. DA did not show any correlation with the other parameters. In multiple linear regression analysis, BV/TV alone explained $43\%$ of the variance in Young's modulus. By stepwise inclusion of SMI, the variance in the Young's modulus was better explained up to $52\%$. Conclusions : Predicting trabecular strength in the mandible through architectural analysis would be possible. Further study is needed to establish the tendency and variety of trabecular architecture and strength according to the locations within the mandible.

Cephalometric Angular Measurements of the Mandible Using Three-Dimensional Computed Tomography Scans in Koreans

  • Background We conducted this study to analyze the values of the key cephalometric angular measurements of the mandible using 3-dimensional (3D) computed tomography scans. Methods In the 106 enrolled patients, a 3D cephalometric analysis was performed to measure the angular variables of the mandible. These values were compared between the two sides and between the two sexes. Results The frontal measurements revealed that the mandibular body curve angle was larger on the left (Lt) side (right [Rt], $141.24{\pm}7.54$; Lt, $142.68{\pm}6.94$; P=0.002) and the gonial angle was larger on the right side (Rt, $134.37{\pm}8.44$; Lt, $131.54{\pm}7.14$; P<0.001). The sagittal measurements showed that the gonial angle was larger on the right side (Rt, $134.37{\pm}8.44$; Lt, $131.54{\pm}7.14$; P>0.05). Further, the transverse measurements revealed that the mandibular body curve angle was larger on the right side (Rt, $140.28{\pm}7.05$; Lt, $137.56{\pm}6.23$; P<0.001). Conclusions These results provide an average of the mandibular angular measurements for the Korean population, establishing a standard for determining surgical patient groups and outcome evaluations in the field of mandible contour surgery.

완전 편측성 구순구개열자의 하악골 형태와 위치에 관한 측모 두부 방사선학적 연구 (Cephalometric Study of the Morphology and Position of Mandible in Complete Unilateral Cleft Lip and Palate Patients)

  • This study was designed to evaluate the morphology and the position of the mandible in the complete unilateral cleft lip and palate patients, Craniofacial skeletal morphology pattern was analyzed on the lateral cephalometric radiographs of the 50 subjects of complete unilateral cleft lip and palate, the 50 normal and 50 class III, Each group was divided into child and adult sub-groups, All the data were tested statistically. The results were as follows: I, In the comparison with the normal group, complete cleft group showed smaller angular, condylar length, clockwisely rotated mandible and larger NMe/SGo(p<0.01). 2, In the comparison with the class III group, the complete cleft group showed significantly smaller angular, condylar, ramal, body length of the mandible(p<0,01). 3. As for the position of the mandibular condyle to the cranial base, the class III group was the most anterior, the normal group was the most posterior and the complete cleft group was in the middle(p<0.05). 4. In the comparison with child group, the normal adult group showed smaller mandibular angle and mandibular plane angle, but not the other two groups. And the complete cleft group and the class III group showed the similar change. The normal and class III group showed increased XiCd/XiPog, but not the complete cleft group(p<0.01).

Genial tubercle position and genioglossus advancement in obstructive sleep apnea (OSA) treatment: a systematic review

  • Background: To systematically review the literature for methods to localize the genial tubercle as a means for performing an advancement of the genioglossus muscle. Methods: PubMed, Google Scholar, CRISP, EMBASE, CINAHL, and Scopus were searched from inception through June 16, 2015. Results: One hundred fifty-two articles were screened, and the full text versions of 12 articles were reviewed in their entirety and 7 publications reporting their methodology for localizing the genial tubercle. Based upon these measurements and the results published from radiographic imaging and cadaveric dissections of all the papers included in this study, we identified the genial tubercle as being positioned within the mandible at a point 10 mm from the incisor apex and 10 mm from the lower mandibular border. Conclusion: Based upon the results of this review, the genial tubercles were positioned within the mandible at a point 10 mm from the incisor apex and 10 mm from the lower mandible border. It may serve as an additional reference for localizing the genial tubercle and the attachment of the genioglossus muscle to the mandible, although the preoperative radiological evaluation and the palpation of the GT are recommended to accurately isolate.

한국인의 부분 치아 결손 증례와 국소의치 설계에 관한 연구 (A SURVEY OF PARTIAL EDENTULISM AND REMOVABLE PARTIAL DENTURE DESIGNS FOR PATIENTS IN KOREA)

  • There were many studies that distribute the partial edentulous states and examine the removable partial denture designs in the planning of removable partial denture treatment. This study was performed in the point of removable partial denture prescription to evaluate partial edentulism and its removable partial denture designs. The data was collected from the dental laboratory of each three dental colleges in Seoul and from two dental laboratories only for removable partial dentures as a prescription form. A total of 1411 cases with prescription form collected from dental laboratories were distributed for this study, then 788 cases were selected for this study. The case selection was done according to the contents of prescription form. The selected cases were divided into maxillary arch and mandibular and classified in terms of types of major connector and direct retainer, unbroken anterior teeth, Kennedy classification, the number of remaining teeth, and distribution of age and sex. The analyzed results were as follows : 1. The Kennedy classification I showed highest frequency both in maxilla and mandible. 2. The arch distribution of removable partial denture was 50.08% for maxilla and 49.92% for mandible. 3. The highest frequency in the distribution of direct retainer was the RPA clasp design. 4. The frequency of unbroken anterior 6 was 73.36% for maxilla and 82.30% for mandible. 5. The design of broad palatal strap and lingual bar revealed the highest prevalence in the major connector construction. 6. The mean number of remaining teeth per arch was 8.25 for maxilla and 8.37 for mandible. 7. The mean age of the patients supplied with removable partial denture was 52.25 years for men, 51.68 years for women, 52.11 years for maxilla, and 51.76 years for mandible and women showed more prevalence.

Stafne Bone Cavity of the Mandible

  • Stafne bone cavity is a rare mandibular defect that was first reported by Edward C. Stafne in 1942. It commonly presents with a well-demarcated, asymptomatic, unilateral radiolucency that indicates lingual invagination of the cortical bone. A 52-year-old female patient who with nasal bone fracture, visited the hospital. During facial bone computed tomography (CT) for facial area evaluation, a well-shaped cystic lesion was accidentally detected on the right side of the mandible. Compared to the left side, no swelling or deformity was observed in the right side of the oral lesion, and no signs of deformity caused by mucosal inflammation. 3D CT scans, and mandible series x-rays were performed, which showed a well-ossified radiolucent oval lesion. Axial CT image revealed a cortical defect containing soft tissue lesion, which has similar density as the submandibular gland on the lingual surface of the mandible. The fact that Stafne cavity is completely surrounded by the bone is the evidence to support the hypothesis that embryonic salivary gland is entrapped by the bone. In most cases, Stafne bone cavity does not require surgical treatment. We believe that the mechanical pressure from the salivary gland could have caused the defect.

생체흡수성 재료를 이용한 하악골절 치료의 결과 (The Result of Mandible Fracture Fixations with Biodegradable Materials)

  • Purpose: Traditionally, titanium miniplate has been used for rigid fixation of mandible fractures. However, the limitations of metal plate have been reported such as hypersensitivity, interference with the cranio-facial growth of growing child, secondary bone resorption around the plate, foreign body reaction, declination of primary callus formation, and bone atrophy, and so forth. Recently, biodegradable miniplate has been introduced and used as an alternative to the metal plate despite of its lower strength. This study evaluated the usefulness and stability of biodegradable plate and screw for treatment of mandible fractures. Methods: In this study, 61 patients(92 areas) diagnosed as mandible fracture in the last 2 years have been reviewed. We used titanium plate and screw in 32 patients, and biodegradable plate and screw($INION^{(R)}$) in 29 patients. Stability of plates and screws, bony healing process and its side effects were observed by clinical and radiographic assessment. Results: In the titanium material group, one of malocclusion, two of mouth opening limitation, three of pain, three of palpation were shown. The plate of six patients involved in these complications were removed. In the biodegradable group, two of mouth opening limitation, two of pain, one of localized wound infection were shown and one plate was removed secondarily. Conclusion: There was no statistical difference between two groups in bony healing and complication rates. Biodegradable implants show efficient stability during initial bone healing and low side effects in long-term follow up periods.

하악골 재건을 위한 가상수술계획 시스템 (Virtual Surgical Planning System for Mandible Reconstruction)

  • In this paper, we propose a virtual surgical planning system specialized to mandible reconstruction surgery. Mandible reconstruction surgery is one of the most difficult surgeries, even for experienced surgeons. Compared to the traditional surgical procedures, virtual surgical planning can reduce the operation time in operating room while expecting better surgical outcome with optimized planning. However, with existing software systems, it requires much time and manual operations in virtual surgical planning. To reduce preparation time and improve accuracy of virtual surgical planning, we have developed optimized functions for virtual surgical simulation of mandible reconstruction with user-friendly interface. We found that the proposed system shortened the preparation time by half compared to the existing system from the experiments. The proposed system supports surgeons to make accurate plan faster and easier. The virtually planned results are used to make surgical cutting guide by 3D printing, and this will enhance surgical performance in operating room.

Preliminary clinic study on computer assisted mandibular reconstruction: the positive role of surgical navigation technique

  • Background: The objectives of the present study were to investigate the reliability and outcomes of computer-assisted techniques in mandibular reconstruction with a fibula flap and verify whether the surgical navigation system was feasible in mandible reconstructive surgery. Methods: Eight cases were enrolled in the computer assisted surgery (CAS) group and 14 cases in the traditional group. The shaping and fixation of the fibula grafts were guided by computer assisted techniques, which could be monitored with the BrainLAB surgical navigation system. The variation of mandible configuration was evaluated by CT measurement in the Mimics software, including the variation of length, width, height and gonial angle of the mandible. The 3D facial soft tissue alteration was also analyzed in 3D chromatogram by Geomagic software. Results: All 22 fibula flaps survived. The mandibular configurations and facial contours had a better clinic result in the CAS group. The length, width, height and gonial angle of the reconstructive mandible were more similar to the original one. The Wilcoxon rank sum test analysis suggested significant differences in the measurements. The chromatographic analysis also visually showed superiority over the traditional group. Conclusions: The computer assisted surgical navigation method used in mandibular reconstruction is feasible and precise for clinical application. The contour of the reconstructed mandible and facial symmetry are improved with computer techniques.

Three-dimensional functional unit analysis of hemifacial microsomia mandible-a preliminary report

  • Background: The aim of this study was to present three-dimensional (3D) structural characteristics of the mandible in the hemifacial microsomia. The mandible has six distinct functional units, and its architecture is the sum of balanced growth of each functional unit and surrounding matrix. Methods: In order to characterize the mandibular 3D architecture of hemifacial microsomia, we analyzed the mandibular functional units of four hemifacial microsomia patients using the 3D reconstructed computed tomography (CT) images. And we compared the functional unit size between affected and non-affected side. Results: The length of condyle and angle showed significant differences between affected and non-affected sides. However, the length of mandibular body showed insignificant differences. The size differences between affected and non-affected side were observed at the condyle, angle, and body in descending order. Conclusions: This preliminary study suggests that the main etiopathogenic units are condyle and angle in the hemifacial microsomia mandible. Further investigation with the increased number of subjects will be helpful to establish treatment modality by etiopathogenic targeting of hemifacial microsomia.
