Management of Elderly Patients with Intracranial Aneurysm (고령군 뇌동맥류 환자의 치료)

  • Objectives : A clinical analysis was performed to provide management strategy and to improve management outcome of elderly patients with intracranial aneurysm. Patients and Methods : We reviewed medical records of 746 consecutive patients with intracranial aneurysm who were admitted from July 1991 to December 1996. They were divided into two age groups : elderly(120 patients aged 65 years or older) and non-elderly(626 patients aged 64 years or younger). We investigated the differences between the two groups in clinical characteristics, management outcome and surgical results. Results : Female(80.0%), internal carotid artery aneurysm(48.9%), poor clinical grade(Hunt and Hess Grade IV, V : 39.8%), postoperative subdural fluid collection(38.2%), and postoperative hydrocephalus(39.7%) were more frequent in the elderly patients. There were no significant differences in the incidence of hypertension, multiple aneurysm, unruptured aneurysm, rebleeding, delayed ischemic neurological deficits, postoperative hemorrhage, and low density on the postoperative brain CT scan. In some cases, surgical clipping of ruptured aneurysm could not be performed due to moribund state or refusal of surgery by the elderly patient's family. Both management outcome and surgical results in elderly aneurysm patients at 3 months after rupture were worse than those of the non-elderly group. The most common reason of unfavorable outcome was poor clinical grade in both groups, while serious medical illness causing unfavorable outcome was more common in the elderly group. Conclusion : Surgical treatment of a ruptured aneurysm should not be avoided in elderly patient solely on the basis of advanced age. If the patients are in good clinical grade, early aneurysm surgery followed by early ambulation should be recommended. Further improvements in outcome may be achieved by thorough knowledge of poor resilience of brain, CSF flow dynamics, and diminished cardiopulmonary reserve in elderly patients with intracranial aneurysm.

The Improvement and Completion of Outcome index: A new assessment system for quality of orthodontic treatment

  • Objective: Given the considerable disagreement between the Peer Assessment Rating (PAR) index and the American Board of Orthodontics Cast-Radiograph Evaluation, we aimed to develop a novel assessment system-the Improvement and Completion of Outcome (ICO) index-to evaluate the outcome of orthodontic treatment. Methods: Sixteen criteria from 4 major categories were established to represent the pretreatment malocclusion status, as well as the degree of improvement and level of completion of outcome during/after treatment: dental relationship (arch length discrepancy, irregularity, U1-SN, and IMPA); anteroposterior relationship (overjet, right and left molar position, ANB); vertical relationship (anterior overbite, anterior open-bite, lateral open-bite, SN-MP); and transverse relationship (dental midline discrepancy, chin point deviation, posterior cross-bite, occlusal plane cant). The score for each criterion was defined from 0 or -1 (worst) to 5 (ideal value or normal occlusion) in gradations of 1. The sum of the scores in each category indicates the area and extent of the problems. Improvement and completion percentages were estimated based on the pre- and post-treatment total scores and the maximum total score. If the completion percentage exceeded 80%, treatment outcome was considered successful. Results: Two cases, Class I malocclusion and skeletal Class III malocclusion, are presented to represent the assessment procedure using the ICO index. The difference in the level of improvement and completion of treatment outcome can be clearly explained by using 2 percentage values. Conclusions: Thus, the ICO index enables the evaluation of the quality of orthodontic treatment objectively and consecutively throughout the entire treatment process.

Exploring the Development Directions of Learning Outcome in Higher Education through the Analysis of Popular Tools: A Case of University K (주요 고등교육 학습성과 분석 도구 분석을 통한 발전 방향 모색: K대학 사례 연구)

  • In recent times, there has been a shift towards student-centered higher education policies, leading to a growing interest among universities to improve students' learning outcomes. To aid in this endeavor, this study aims to provide guidance for University K to enhance their learning outcome management by comparing and analyzing their learning outcome indicators with those of other domestic and foreign universities. The study examined detailed measurement questions from major learning outcome measurement tools such as AHELO, NSSE, and CLA+. Upon comparison and analysis of University K's major learning outcome indicators with those of other universities, it was found that most of the indicators overlapped. However, some indicators such as student support/facilities for learning, instructor quality, and communication were absent from University K. Therefore, it is crucial to decide whether to add these indicators to the existing learning outcomes or to confirm them through other surveys. Moreover, even for the same indicator, some indicators with different measurement need to consider changing the measurement.

  • On this research, recognizing that ship inspection and audit services are closely related in Improving ship safety, the importance of active quality managements of ship inspection and audit services have been notified. For this research, based on previous research results, Quality managements of ship inspection and audit services were divided into the following aspects; the source of quality managements, execution of quality managements and outcome of quality managements. The results of this study is as follows. Overall, it is clearly shown that the elements of quality managements execution such as management responsibilities, resource management., product realization, measurement and analyzation, and improvement had also acted as a mediator between the relation of quality managements source and outcome.

  • Recently, social enterprises are expanded quantitatively and continuously, so it is timely to suggest effective management for sustainability of social enterprises. Therefore, this study was conducted to explore various organizational factors which influence on social enterprises' economic and social outcomes. This study investigated the effects of strategic factors, management system, external environment and organizational culture on the outcomes. Strategic factors emerged as the most critical factors for both outcomes. Human resource management and innovative organizational culture affected economic outcomes. Among external environment, community support has significant effect on social outcomes. Based on the results, implications were suggested for effective management and further research.

Clinical Application of $^{18}F-FDG$ PET in Epilepsy (간질에서의 $^{18}F-FDG$ PET의 임상 이용)

  • FDG PET has been used as a diagnostic tool for localization of seizure focus for last 2-3 decades. In this article, the clinical usefulness of FDG PET in the management of patients with epilepsy has been reviewed, which provided the evidences to justify the medicare reimbursement for FDG PET in management of patients with epilepsy. Literature review demonstrated that FDG PET provides an important information in localization of seizure focus and determination whether a patients is a surgical candidate or not. FDG PET has been reported to have high diagnostic performance in localization of seizure focus in neocortical epilepsy as well as temporal lobe epilepsy regardless of the presence of structural lesion on MRI. Particularly, FDG PET can provide the additional information when the results from standard diagnositic modality such as interictal or video-monitored EEG, and MRI are inconclusive or discordant, and make to avoid invasive study. Furthermore, the presence of hypometabolism and extent of metabolic extent has been reported as an important predictor for seizure free outcome. However, studies suggested that more accurate localization and better surgical outcome could be expected with multimodal approach by combination of EEG, MRI, and functional studies using FDG PET or perfusion SPECT rather than using a single diagnostic modality in management of patients with epilepsy. Complementary use of FDG PET in management of epilepsy is worth for good surgical outcome in epilepsy patients.

Relationship between Percutaneous Transluminal Coronary Anigioplasty Volume and Associated Immediate Outcome (경피적 관동맥 확장술의 시술량과 조기 시술결과의 관련성)

  • Objectives : To explore the relationship between Percutaneous Transluminal Coronary Angioplasty(PTCA) volume and the associated immediate outcome. Methods : A total of 1,379 PTCAs were peformed in 25 hospitals in Korea between October 8 and December 31 in 1997. Data from 1,317 PTCAs (95.5%) were collected through medical record abstraction. Inter-observer reliability of the data was examined using the Kappa statistic on a subsample of 110 PTCA procedures from five hospitals. Intra-observer reliability of the data was also examined. PTCA success and immediate adverse outcomes were selected as the outcome variables. A successful PTCA was defined as a case that shows less than 50% diameter stenosis and more than 20% reduction of diameter stenosis. Immediate adverse outcomes included deaths during the same hospitalization, emergency coronary artery bypass graft (CABG) within 24 hours after PTCA, and acute myocardial infarction within 24 hours after PTCA. The numbers of PTCAs performed in 1997 per hospital were used as the volume variables. Results : Without adjusting for patient risk factors that may affect outcomes, procedures at high volume hospitals ($\geq200$ cases per year) had a greater success rate (P=0.001) than low volume hospitals. There was a marginally significant difference (P=0.070) in major adverse outcome rates between high and low volume hospitals. After adjusting for risk factors, there were significant differences in procedural failure and major adverse outcome rates between high and low volume hospitals. Conclusions : After adjusting for patient clinical risk factors, the hospital volume of PTCA was associated with immediate outcomes. It is recommended that a PTCA volume per year be established in order to improve the immediate outcome of this procedure in Korea.

  • Since Research and Development has been expanded by government, It is very important to evaluate the outcome of Research and Development. Government have levied the penalty of researchers who misused research funding as time goes on. However, there is no protect law for the research before 2021. Government put new committee for the researchers to judge whether their action is legal or illegal based of Innovation Act 2021. Due to the various outcome index of research and development, many firms which is paticipating the research and development have been confused the outcome index. Also, It is difficult for government agencies for management to evaluate the outcome. Even if the committee is trying to solve dispute between researchers and the government agencies, it is not enough to solve it. Therefore, we need to consider Alternative Dispute Resolution(ADR), because the ADR has been developved detail skills for long time.

  • This study focused on analyzing the relations of empowerment recognized by a five star hotel culinary team in job satisfaction, organizational commitment, and outcome. The purpose of this survey is to show a desirable culinary organization in hotel management. This survey was shared by two groups recognized as high empowerment and low empowerment groups, for job satisfaction, success, organizational commitment, and outcome measurements. This study presented the difference between recognizable high empowerment and the effects of low empowerment groups which showed positive effects on the high empowerment group compared with the low empowerment group. This means that positive effects on the three main factors of job satisfaction, success, organizational commitment, outcome measurements, and hotel culinary organizations are required empowerment for the new organization of management.

  • It has been argued that Enterprise systems (ES) implementations are overshadowed by a high failure rate despite their promised benefits. One of the commonly cited reasons for ES implementation failures in the context of mandatory use is end-user's unwillingness or sabotage to adopt or use systems. Considering that the appropriate management of expectations may play an important role in making positive behavior toward newly implemented systems, this study examines the effect of outcome expectations on the system use activity in the mandatory use context of information systems from the Social Cognitive Theory perspective. Structural equation model analysis using LISREL 8.7 provides significant support for the proposed relationships. The empirical results suggest that outcome expectations and user satisfaction have positive effects on system use activity conceptualized by immersion, reinvention, and learning. Theoretical and practical implications of the study shed some light on how to improve system use activity in the mandatory use context of information systems.
