Analysis of Anesthetic Consultation in Elective Surgical Patients (정규수술환자 중 마취통증의학과로 협진 의뢰한 환자의 분석)

  • Background: This study was carried out to contribute to effective management of operated patients by analyzing cooperative details about patients who were taken elective operation and consulted to the department of anesthesiology. Patients and Methods: One thousand patients who have been consulted to department of Anesthesiology, Chonnam National University Hospital from 2008. November to 2009. March were analyzed. The gender, age, cooperative departments, cooperative reason, the number of cancelled cases and cancelled reason were evaluated. Results: Among 1000 patients, there were 470 females and 530 males. After adjusting for age, middle-aged people which range 45 to 64 year-old are 39.9% and over 65 year-old people are 30.3%. Cooperative department is distributed to orthopedics which is highest occupying 20.6%, neurosurgery, otorhinolaryngology, obstetrics and gynecology in order. Dentistry occupied 2.0%. By specifying reasons of cooperation, cardiovascular diseases distribute to 38.4% which is the highest, the respiratory diseases, 17.5% and the endocrine diseases including diabetes 13.2%. Arranging details, hypertension is 23.9%, ECG abnormality 14.5%, chronic bronchitis in chest radiographic interpretation 14.2%. Of 1,000 consulted patients, delayed or cancelled cases are 58. And the reasons were 26 cases of need of appropriate the blood-sugar level. 26 cases needed appropriate treatment for comorbidities and others 6. Conclusion: Thoroughly evaluating patients before operation and maintenace of active cooperative system between operative department and anesthesiology department can contribute to reduction of cancellation rate and effective management of both hospitalizing and operating rooms.

  • Purpose - The purpose of this study is to compare and analyze the ordinances of other local governments to propose implications and improvement plans for the ordinances of Chuncheon City. To this end, cases of overseas legislation related to youth labor rights, domestic laws and major policies of the central government were reviewed. Design/methodology/approach - This study mainly utilized literature research methods. In addition, it was based on foreign laws and case analysis, domestic laws and ordinances, and preceding research and data related to government policies. Findings - Chuncheon city ordinances are superior to those of other local governments. However, the details of the establishment of the implementation plan, the actual condition survey, and the establishment of the consultation system are included in the project contents of Article 6, so it is not concrete and clear. And there were no articles related to the labor environment check. Research implications or Originality - It is proposed to make the contents of the establishment of an implementation plan, survey, and consultation system as independent provisions. It is also proposed to establish a new article related to the youth labor environment check. In addition, it is desirable to establish detailed regulations by establishing the ordinance enforcement regulations.

An Analysis of the Trends of Korean National Health Insurance Administrative Cost (건강보험 관리운영비 추이 분석)

  • Social solidarity, equity in financing, and efficiency in administration have been core issues in the development of Korean health insurance reformation since 1988. This study is to investigate the trend of administrative cost in Korean National Health Insurance from various aspects. For the analysis of administrative cost, the expenditures of each insurance society and the National Health Insurance Corporation are divided into 4 items of (1) insurance benefit, (2) administrative cost, (3) an agency provision accounts, (4) other expenses, and then they are reorganized. The analyses based on 5 types of the health insurance administrative cost showed that efficiency in administration has been improved generally. We, however, should consider qualitative aspects such as customer's satisfaction with health insurance administration, prompt service, control of unjust expenditure (unjust claims), and provision of medical service including health consultation in assessing efficiency of administration. And, in order to connect the administrative costs of health insurance with efficiency, we need to give a fundamentally new definition, which can contain elaborateness of expenditure in details including the structure and evaluation method of administrative costs. It may be necessary to develop new indicators or analyzing methods hereafter.

A Study on the Preparation of Distribution Table for Quantitative Evaluation of Small-Scale Environmental Impact Assessment

  • Purpose: The small-scale environmental impact assessment has been in operation since its implementation in August 2000, and is a system that meets the purpose of sustainable development in consideration of the environment in areas requiring conservation. However, when preparing a small environmental impact assessment report, the contents of the reduction measures are too qualitative, or reports are prepared to simply list the compliance stipulated by individual laws, and the contents of consultations prepared by consultative agencies and review agencies are not much different. In addition, the direction of consultation on development projects for similar locations of the same project type is frequently changed by reflecting the subjective judgment of the consultative officer of the Ministry of Environment (Environment Agency). Therefore, this study attempted to improve the establishment of measures to reduce the existing simple listing of qualitative contents and the inconsistent presentation of review opinions by consultative agencies and review agencies. Research design, data and methodology: The research method extracted absolute evaluation items and relative evaluation items among small environmental impact assessment items, analyzed and presented detailed items, and prepared a distribution table for each section according to the details of the relative evaluation items, and presented them as a table. Results: This study was conducted to derive uniform results with objective indicators in the preparation and consultation process of a small-scale environmental impact assessment. Conclusions: Once a quantitative evaluation is established, the consultant can objectively determine and process the environmental impact.

Risk Assessment Method for Activities of Apartment Construction (공동주택 건축공사 단위작업의 위험성 평가 방법)

  • Recently, the disaster rate of the construction industry has increased with high-rise, and complexity of the building, unlike the decrease in the disaster rate in other industries. Although risk assessment is performed to reduce the occurrence of disasters, it is difficult to estimate the risks accurately due to activity in which no disaster has occurred, and inconsistencies in the level of details of work. In this study, in order to evaluate the risk of the major activity for the apartment construction work, the activity was identified by referring to the risk assessment model of construction industry type by the KOSHA. The construction work types and activities were consistently organized in level of work into nine work types and 82 activities were through experts consultation. Analyzing the disaster types that occurred during work through KOSHA disaster cases, calculating the probability of disaster occurrence according to the type of disaster, and combining the probability of disaster with the severity of disaster to estimate the risk assessment method was presented. Using the daily report of the construction site of the apartment, the results of a case study confirmed the validity of the risk calculation method presented in this study.

  • The Environmental Impact Assessment(EIA) follow-up is integral phase of the EIA system. The EIA is complemented and completed by the EIA follow-up and the experience and data accumulated during the process contributes to the advancement of the EIA system. In Korea, institutional foundation of EIA is acknowledged as having been settled. Therefore, eyes are now on the qualitative growth of the EIA system including the normalization of the EIA follow-up management. In relevant prior studies there are many suggestions to make advancement of the EIA system. Nevertheless, it is difficult to apply the real system because most suggestions need prior preparation. Given the context, we propose a practical step-by-step strategies for the vitalization and advancement of the EIA follow-up system while recommending the direction for the development in this study.

Derivation of Success Elements for the Sustainability of Landscape Agreements - A Case Study on Ongjin-gun Mungab Island and Suwon Gobuk Market - (경관협정의 지속성을 위한 성공요소 도출 - 옹진군 문갑도와 수원시 거북시장길 사례분석을 통하여 -)

  • This study shows that the role of residents in landscape management is becoming increasingly important. The purpose of this study is to suggest elements that can continue the operation of landscape agreements and directions for promoting them. Therefore, 1)the operational elements considering the sustainability of the landscape agreements were proposed by way of literature research, expert interviews, and surveys. 2)The sustainable operation of elements of the landscape agreement were applied and best practices were developed through interviews with participants and literature analysis. 3)The final plan operational elements considering the sustainability of the landscape agreements and the directions for implementation were presented. As for the results, it was first presented that the elements of continuous operation of the landscape agreement, consisted of 3 major categories, 10 subcategories, and 25 details. These include resident awareness, practical applicability, effectiveness of administrative means, securing the budget, maintenance, public relations, expert support, dedicated support organization, sustainability of participation, and resident participation and communication methods. It is a detailed list of items that should be considered in the preparation phase, maintenance phase, and conclusion phase. Second, it suggested the direction for the sustainable operation of the landscape agreements be highly backed by the residents, and after reaching consensus on a landscape agreement, it is necessary that the agreement is based on contents that the residents can execute themselves. In addition, it was found that there is a need for a system to prepare the basis for securing the budget for the continuity of work, preparation of the landscape agreements, and consultation and activity costs during the maintenance phase. In addition, continuous exchanges and capacity building among residents have signed landscape agreements, and step-by-step support from experts in accordance with the level of involvement of residents is necessary. Third, even if a landscape agreement is concluded in connection with public projects, it is understood that the residents have the capacity to participate and can continue to support the administration and experts to enable the continued operation of the landscape agreement.
