A study on Phantom image of Mammography in some area (일부지역 유방촬영술의 팬톰영상에 관한 연구)

  • Purpose : The goal of this study was to survey the quality control of mammographic phantom images in some area. Methods : Between May and June, 2005, I have got a total of 47 mammographic phantom images collected from 49 hospitals and were analized about phantom score of fibrous, speck, tumor with enlargement scope. By critic level of experimental mammographic phantom case images, classified through the pass case that fibrous score 4, speck score 3, tumor score 3 over. Result : It passed 33case (70.3%), failed 14 case (29.7%) about 47 case. Passing case images showed tube voltage $23{\sim}30\;kVp$, tube current $20{\sim}60\;mAs$, photographic density $0.48{\sim}1.77$ Conclusion : It passed case about 70.3% of mammographic phantom image in some area. Optimum range of tube voltage, tube current, photographic density have to use to get fine quality of mammographic images.

  • We studied radiation dose in mammography through 34-46 kv range using acryl phantom. The obtained results were as follows: 1. Incident radiation was maximum with high kvp and thin added filtration. 2. Transmitted radiation by acryl phantom and its thickness were in reciprocal relationship. 3. The acryl thickness to produce comparable film density with soft tissue of breast was 6 cm. 4. The X-ray exposure for comparable density radiographs increased mammographic film more than medical x-ray film and the amount of x-ray exposure was directly proportional to the added filtration of x-ray beam. 5. The surface dose of x-ray exposure needed to produce film density of 1.0 for 6cm acryl phantom was 1,084-1,575mR in mammographic film and 476-625 mR in medical x-ray film.

  • Quantitative analysis was performed to confirm that moire artifact was removed without loss of image when using grid suppression software in the diagnosis of micro lesions. we showed that grid suppression images can be morphologically different from original images as they are significantly lower than those of the optimized grid in the similarity analysis with reference images in mammographic phantom images. We were confirmed that images of microcalcification with smaller signal than noise were lost because the pixel values of all lesions increased significantly after the grid suppression, The change in contrast using the NORMI 13 X-ray test phantom was reduced to 30% of the reference image, This result was significantly lower than the 90% when using the optimized grid. In conclusion, the use of grid suppression software in clinical images should be carefully considered because of the possibility of misdiagnosis due to micro lesion loss and morphological changes.

Usefulness Evaluation of Mammographic Image (유방촬영 영상의 유용성 평가)

  • Purpose : To valuate the usefulness of mammographic image by using phantom 1,2 to controls the enhancement of image quality. Procedure : Set up same equipments for experiment (all the qualification must be the same). Control group and 4 other experiment the developing mammographys by using the image system with film/intensifying screen and distribute marks on each test objects of phantom 1, 2. Result : The results of the experiment using phantom 1, 2 on control group and 4 others are : 1. phantom 1's valuation with 3 items. - control group received 29 out of 32 - group A received 25 - group B received 16 - group C received 11.5 - group D received 28.5 The evaluation shows that the system has proved to display excellent image quality except group B and C. 2. Phantom 2's valuation with 4 items. - control group received 38 out of 38 - group A received 30 - group B received 16 - group C received 12.5 - group D received 38 Even with phantom 2, group B and C has displayed poor image quality. Conclusion : Both experiments using phantom 1 and 2 have shown that the most strong factor that control and influence the image quality are the filming equipments. Especially, the developing system has most powerful influence above all.

Monte Carlo Simulation-Based Mammographic Anti-Scatter Grids to Evaluate Performance of Digital Mammography Detector (디지털 맘모 디텍터 성능평가를 위한 몬테카를로용 산란선 제거 그리드 작성에 관한 연구)

  • In Recent years, there has been a noticeable increase in the global incidence of breast cancer, with approximately 2.3 million cases of female breast cancer reported worldwide in 2020. Numerous studies are currently underway to enhance the accuracy of breast cancer diagnosis through the development of digital mammography detectors. This study aims to create Monte Carlo simulation-based mammographic anti-scatter grids and investigate their utility in evaluating the performance of digital mammography detector. Two types of mammographic anti-scatter grids, MAM-CP and Senographe 600T HF, were created using Monte Carlo simulation software (MCNPX 2.7.0), with grid ratios of 3.7 : 1 and 5 : 1, respectively. The grid physical characteristics (sensitivity, exposure factor, contrast improvement ratio) were calculated based on the KS C IEC60627 in the simulations using two X-ray qualities, RQA-M2 (28 kVp) and MW4 (35 kVp). As the X-ray tube voltage increased from 28 kVp to 35 kVp, sensitivity and exposure factor exhibited a decreasing trend, while contrast improvement ratio demonstrated an increasing trend. With an increase in grid ratio from 3.7 : 1 to 5 : 1, all physical characteristics showed an upward trend. Our results were consistent with a previous study that conducted measurements of physical properties using a real phantom. However, the pattern of change in the contrast improvement ratio with X-ray tube voltage differed from the previous study.

Evaluation of MTF Image by Target/Filter Combined of X-ray Tube Using Mammography (유방촬영용 X선관 target/filter 조합에 따른 MTF영상평가에 관한 고찰)

  • It is important to consider the contrast of object in Mammography because an absorption gap between tissues of body and breast in breast is low. This study is to evaluate MTF image with resolution chart according to change of combination of target and filter. The results were as follows : 1. There were significant differences in X-ray energy according to combination of filter(Mo/Mo, Mo/Rh. Mo/Al, Rh/Rh, Rh/Al) and acrylic thickness(2 cm, 3 cm, 4 cm). 2. The value of lp/mm on MTF to 0.5 showed that the sharpness in MTF curve was 2.4 compared to Mo/Mo and 2cm acryl, 2.63 in Mo/Rh and 4 cm acryl, and 2.9 in Rh/Rh and 6cm acryl. 3. The value of lp/mm on MTF showed that the resolution in MTF curve was 6.0 compared to Mo/Mo and 2 cm acryl, 4.60 in Rh/Al and 4cm acryl, and 6.03 in Rh/Al and 6 cm acryl. 4. The value of MTF on 2.5 lp/mm distinguishable visually was 0.48 compared to Mo/Mo and 2 cm acryl, 0.53 in Mo/Rh and 4cm acryl, and 0.59 in Rh/Rh and 6cm acryl. 5. For the evaluation of an image of the mammo-phantom, the score of Mo/Mo was 12 points, Mo/Rh 11, Rh/Rh 10.5, Mo/Al 10, Rh/Al 9.0, respectively.

Actual condition on accuracy control of mammography equipment in Kyeongsangbuk-do (경상북도 유방촬영장비의 정도관리에 대한 실태조사)

  • The breast cancer has the highest occurrence rate among the female cancers, and as the living style changes, the occurrence is increasing gradually. For the breast cancer test among women, who comprises up to 50% of the total population, the mammography is mainly used as the screening test, and the accuracy control is the most important aspect of the testing. Therefore this research divided the northern part of Kyeongsangbuk-do into 4 regions and investigated the accordance ratio of examination field and light examination field, the total focus using the optical density and compression rate, and the overall maintenance of mammography within the regions. The equipments of 11 hospitals were investigated, and the 7 hospitals passed the standard level of the accordance ratio of examination field. 6 hospitals passed the standard optical density, and 7 hospitals had the passing performance in the compression rate. Fibers, group of specks, and masses within the Mammographic Accreditation Phantom scored 10, being within the standard range. However, only 3 hospitals were equipped with private development processor and illumination. The result reflects the fact that the image quality of breast is not correctly being maintained. Moreover, only 27.27% satisfied all the three categories of compression fitting, accordance ratio of examination field, and phantom image evaluation at the same time. The accuracy control must be maintained more precisely for the accurate diagnosis of breast cancer.

Material Decomposition through Weighted Image Subtraction in Dual-energy Spectral Mammography with an Energy-resolved Photon-counting Detector using Monte Carlo Simulation (몬테카를로 시뮬레이션을 이용한 광자계수검출기 기반 이중에너지 스펙트럼 유방촬영에서 가중 영상 감산법을 통한 물질분리)

  • Mammography is commonly used for screening early breast cancer. However, mammographic images, which depend on the physical properties of breast components, are limited to provide information about whether a lesion is malignant or benign. Although a dual-energy subtraction technique decomposes a certain material from a mixture, it increases radiation dose and degrades the accuracy of material decomposition. In this study, we simulated a breast phantom using attenuation characteristics, and we proposed a technique to enable the accurate material decomposition by applying weighting factors for the dual-energy mammography based on a photon-counting detector using a Monte Carlo simulation tool. We also evaluated the contrast and noise of simulated breast images for validating the proposed technique. As a result, the contrast for a malignant tumor in the dual-energy weighted subtraction technique was 0.98 and 1.06 times similar than those in the general mammography and dual-energy subtraction techniques, respectively. However the contrast between malignant and benign tumors dramatically increased 13.54 times due to the low contrast of a benign tumor. Therefore, the proposed technique can increase the material decomposition accuracy for malignant tumor and improve the diagnostic accuracy of mammography.
