Primary Ostoegenic Sarcoma on Calcaneus - One Case Report - (종골에 발생한 원발성 골육종 - 1예 보고 -)

  • Osteogenic sarcoma is the most common primary malignant tumor of bone in which tumor cells form neoplastic osteoid or bone or both. Classic osteogenic sarcoma usually involves the metaphysis of the more rapidly growing long bones (distal femur, proximal tibia). Osteogenic sarcoma of the foot is rarely noted and only a few well documented cases have been reported. Osteogenic sarcoma of foot can clinically, radiographically, and histologically mimic several benign lesions and tumor-like lesions, so it sometimes leads to late diagnosis and delayed treatment. We experienced a case of primary osteogenic sarcoma on left calcaneus in 66-years-old female and report it with a review of references.

Primary Malignant Tracheal Tumor : 3 Cases (원발성 악성 기관 종양 3례)

  • The rarity of primary tumor of the trachea, which was recently estimated in a circumscribed population to be 2.7 new cases per million per year, explains the relatively limited experience that has been acquired even by major institutions. Although there may already by a high degree of airway obstruction, tracheal tumors are usually misdiagnosed as bronchial asthma or chronic bronchitis because of its nonspecific symptoms. Surgery is the treatment of choice. Recently, the authors experienced three cases of primary tracheal malignant tumors ; one case of mucoepidermoid carcinoma and two cases of adenoid cystic carcinoma. The authors report on these cases with a review of the literature for give help in differential diagnosis and treatment planing of tracheal tumor.

ANALYSIS OF 67 MALIGNANT SALIVARY GLAND TUMORS IN KOREAN POPULATION (한국인에서 발생한 67례의 악성타액선종양에 대한 연구)

  • Malignant salivary gland tumor is rare neoplasm. In Korean population, retrospective study of malignant salivary gland tumor has not been performed. We analyzed 67 cases of malignant salivary gland tumors from 2001 to 2005 in Seoul National University Dental Hospital in Seoul, Korea. The mean age is 51.7 and the male to female ratio is 1:1.39. The most affected site is the palate. Histologically, the tumors were classified as adenoid cystic carcinoma(34.4%), mucoepidermoid carcinoma(31.3%), adenocarcinoma, NOS(11.9%), polymorphous low grade adenocarcinoma(3.0%), salivary duct carcionoma(6.0%), carcinoma ex pleomorphic adenoma(4.5%), myoepithelial carcinoma(4.5%), epithelial-myoepithelial carcinoma(1.5%), cyatadenocarcinoma(1.5%) and adenosquamous carcinoma(1.5%).

Radiation Therapy in Malignant Tumors of the Parotid Gland (이하선 악성종양에 대한 방사선 치료의 효과)

  • A retrospective analysis was performed on 55 patients with malignant parotid tumor who were treated with radiation therapy between March, 1979 and July, 1989. Of these patients, 8 patients received radiation therapy(RT) alone and 47 patients were treated with combined operation and radiation therapy(OP + RT). The follow-up period of the survivors ranged from 1 to 129 months with a median of 48 months. The common histologic types were mucoepidermoid carcinoma (25 cases), malignant mixed tumor(12 cases), adenoid cystic carcinoma(6 cases). The 5 and 10 year local control rate were 69.8% and 65.7% in all patients. In OP+RT group, prognostic factors related to local control were histologic grade, tumor size, lymph node metastasis. Resection of facial nerve did not affect the local control rate significantly(p=0.129). Distant metastasis developed in 23.6% of patients, mostly to the lung. Actuarial overall survival rate was 72.2% at 10 years and formed plateau after 5 years. Disease-free (NED) survival rate was 49.4% at 10 years and was better achieved in OP+RT group and low grade lesions. Based on our result, a well planned postoperative RT following parotidectomy is highly efficacious in controlling malignant tumors of the parotid gland and preservation of facial nerve.

Malignant Transformation of Giant Cell Tumor Not Associated with Radiotherapy (방사선 조사 없이 속발한 거대 세포종의 악성 전환)

  • Purpose : Giant cell tumors(GCT) sometimes undergo malignant transformation after the radiotherapy, but very rarely do without radiotherapy. We reviewed the clinical experiences of the malignant transformation of GCT to suggest the guidelines for diagnosis and treatment of them. Materials and Methods : We examined four patients of pathologically proven malignant transformation of GCT, which occurred after the operative treatment alone without radiation, from September 1985 to January 2001. The mean follow-up period after the malignant transformation was 2.4 years(range, 1.3~4 years). Results : The mean time-interval from the initial diagnosis to the malignant transformation was 6.9 years(range, 2.2~13.5 years). The locations of tumors were soft tissues of proximal upper arm, proximal femur, distal femur and proximal tibia. The histology of malignant GCT was osteosarcoma in 3 cases and malignant fibrous histiocytoma in 1 case. Local recurrence developed in 1 patient and the pulmonary metastasis developed in 3 patients which transformed to osteosarcoma. Conclusion : Thorough sampling of the surgical specimen appears to be a very important factor for diagnosing the malignant transformation of GCT. In case of suspicion of malignancy in radiographs, the incisional biopsy should be followed by definite treatment rather than the improper resection.

  • Breast ultrasound readings are very important to diagnose early breast cancer. In Ultrasonic inspection, it shows a significant difference in image quality depending on the ultrasonic equipment, and there is a large difference in diagnosis depending on the experience and skill of the inspector. Therefore, objective criteria are needed for accurate diagnosis and treatment. In this study, we analyzed texture characteristics by applying GLCM (Gray Level Co-occurrence Matrix) algorithm and extracted characteristic parameters and diagnosed breast cancer using neural network classifier. Breast ultrasound images were classified into normal, benign and malignant tumors and six texture parameters were extracted. Fourteen cases of normal, malignant and benign tumor diagnosed by mammography were studied by using the extracted six parameters and learning by multi - layer perceptron neural network back propagation learning method. As a result of classification using 51 normal images, 62 benign tumor images, and 74 malignant tumor images of the learned model, the classification rate was 95.2%.

Clinical Investigations of Major Salivary Gland Tumors (주타액선 종양의 임상적 고찰)

  • Objectives: Salivary gland neoplasms are a diverse group of benign and malignant tumors with a wide range of biologic behaviors. The surgeon must understand the pathologic behavior of each tumor type to develop an appropriate treatment plan. The authors planned this study to evaluate our clinical experiences and establish a new treatment strategy. Materials and Methods: From Sep. 1997 to June 2001, 25 cases of major salivary gland tumors which were underwent surgery were evaluated retrospectively. Results: 20 cases were benign and 5 were malignant. Most(17) of benign cases were pleomorphic adenoma and they showed wide distribution in age. Also we experienced other benign such as warthin's tumor, oncocytoma. In malignant, there were 2 cases of carcinoma ex-pleomorphic adenoma, 2 cases of adenoid cystic carcinoma, and one lymphoma. In carcinoma ex-pleomorphic adenoma, one showed dismal prognosis in spite of multimodality and the other were recurrent to be salvaged. Conclusion: We concluded that salivary gland neoplasms are challenging because of their relative infrequency, inconsistent classification, and highly variable biologic behavior. We need to establish new effective strategies with the regard of factors influencing survival.

Two Cases of Auricular Salvage after Wide Excision of Malignant Tumor at Periauricular Area (이개 주변 악성종양의 광범위절제술 후 이개의 보존 2례)

  • Wide excision for curative treatment of malignant tumor on periauricular area often leads to loss of the external ear. But the auricle has rich vascular supply, Conservation of auricular contour is possible through salvage and engineering of remained auricular tissue. We experienced two cases of auricular salvage after wide excision of malignant tumor. In the first case, we performed two-staged technique. In first step, we covered soft tissue defect on periauricualr area with the anterolateral thigh free flap and remained auricle was floating over the flap after split-thickness skin graft was applied on it's posterior raw surface because it's survival was not confirmed. Second step was reposition of remained auricule on the anterolateral thigh flap. In the second case, we packed in periauricular dead space and external auditory canal with temoporalis muscle and temporoparietal fascial flap and then covered the flap with split thickness skin graft. In these two cases, there were no recurrence of tumors and we obtained cosmetically & functionally satisfactory results.

MALIGNANT PERIPHERAL NERVE SHEATH TUMOR ON PALATE: A CASE REPORT (구개골에 발생한 악성 Peripheral Nerve Sheath Tumor의 증례보고)

  • Summary: The malignant peripheral nerve sheath tumor(MPNST) is an aggressive neoplasm and can either arise independently or result from malignant change in preexisting neurofibromatosis (von Recklinghausen's disease). Its histologic characteristics remain controversial, but currently it is believed that the schwann cell is the origin of the peripheral nerve sheath tumors. MPNST is an uncommon neoplasm of the head and neck region, and its presentation in the oral cavity is quite rare. In this study, we report a patient with a rare case of a MPNST involving the maxilla. A case report: A 29-year-old female presented with a chief complaint of painless swelling with bleeding tendency on the left maxillary tuberosity area 2 months ago. Clinical examination showed a $5.0{\times}3.0cm^2$ sized, indurative swelling on the site. Conventional radiographs showed a relatively well-defined soft tissue mass involving the left maxillary sinus, and destruction of the anterior, posterolateral walls of the left maxillary sinus. Subtotal maxillectomy and split-thickness skin graft from thigh were undertaken. In histochemical and immunohistochemical studies, the specimen revealed positive reactivities to Vimentin and S-100 protein. Final diagnosis was made as MPNST.

Correlation Between Malignant Phenotypes and Changes in Overall Proteolytic Capacity of Human Cervix and Liver Cancer (인체의 자궁암과 간암조직에서의 단백질 분해활성의 변화)

  • Several proteolytic activities and the level of and-trypsin in neoplastic tissues of human cervix and liver were compared to those in normal tissues to examine if any correlation exists between malignant behavior of the tumors and the changes in overall proteolytic capacity. Proteolysis against casein and insulin in cervix tumor was increased to 2-to 3-fold while that in liver tumor was reduced to one-tenth to one-half. By contrast, the level of anti-trypsin in cervix tumor was lowered to nearly one-tenth of that in normal tissues while the level rose to about 2-fold in malignant tissues of liver. On the other hand, the activities of plasmin-like protease and plasminogen activator were enhanced 10-20% over the activities in normals. These results suggest that the changes in proteolytic capacity are at least in part due to outbalance in either of proteolytic or its inhibitory activity over the other and occur distinctively to each tumor systems for their malignant behavior.
