Aggravation and Relief after Surgical Resection of Post Infectious Pneumatocele in Very Low Birth Weight Infant

  • Pneumatocele (PC) is a thin-walled cyst of the lung that can occur at all ages and with various etiologies. However, there is no fully accepted consensus for the management of PC in a neonatal intensive care unit. Although the management of PC is generally expectant, it is difficult to decide how long conservative management should be maintained, especially under Korea's medical care environment and the parents' worry and anxiety. We report a male neonate, born at $27^{+5}weeks$ gestation, weighing 1,000 g, who had a post infectious PC caused by methicillin-resistant Staphylococcus aureus sepsis. We treated conservatively for about 100 days (roughly 14 weeks), but unfortunately after a few days of chest retraction, acute exacerbation occurred, video assisted thoracoscopic surgery (VATS) was deemed necessary and performed. The purpose of this publication is to describe the clinical course, aggravation and relief after VATS management with a review of the literature.

Renal artery stenosis presenting as congenital nephrotic syndrome with hyponatremic hypertensive syndrome in a 2-month-old infant: a case report

  • Here, we present the case of a 2-month-old male infant with hyponatremic hypertensive syndrome resulting from stenosis of the right proximal and mid-renal arteries. The patient exhibited nephrotic-range proteinuria, low serum albumin, increased serum creatinine, and elevated renin and aldosterone levels. Doppler ultrasonography and computed tomography angiography revealed decreased vascular flow in the small right renal artery. Following a successful percutaneous balloon angioplasty, the patient experienced a decrease in blood pressure and normalization of serum electrolyte levels within a few days. However, it took 3 months for the proteinuria to resolve completely. This case is significant as it represents the first reported instance of a neonate presenting with clinical features resembling congenital nephrotic syndrome caused by renal artery stenosis that was successfully treated with percutaneous renal angioplasty.

Early neurodevelopment in very low birth weight infants with mild intraventricular hemorrhage or those without intraventricular hemorrhage

  • Purpose: This study aimed to assess early development in very low birth weight (VLBW) infants with mild intraventricular hemorrhage (IVH) or those without IVH and to identify the perinatal morbidities affecting early neurodevelopmental outcome. Methods: Bayley Scales of Infant Development-II was used for assessing neurological development in 49 infants with a birth weight <1,500 g and with low grade IVH (${\leq}$grade II) or those without IVH at a corrected age of 12 months. Results: Among the 49 infants, 19 infants (38.8%) showed normal development and 14 (28.6%) showed abnormal mental and psychomotor development. Infants with abnormal mental development (n=14) were mostly male and had a longer hospitalization, a higher prevalence of patent ductus arteriosus (PDA) and bronchopulmonary dysplasia (BPD), and were under more frequent postnatal systemic steroid treatment compared with infants with normal mental development (n=35, P<0.05). Infants with abnormal psychomotor development (n=29) had a longer hospitalization and more associated PDA compared to infants with normal psychomotor development (n=20, P<0.05). Infants with abnormal mental and psychomotor development were mostly male and had a longer hospitalization and a higher prevalence of PDA and BPD compared to infants with normal mental and psychomotor development (n=19, P<0.05). Using multiple logistic regression analysis, a longer duration of hospitalization and male gender were found to be significant risk factors. Conclusion: Approximately 62% of VLBW infants with low grade IVH or those without IVH had impaired early development.

A Study on the effects of one's blood type on emotional character of children (유아들의 혈액형과 정서적 성향간의 관계 연구)

  • This study is focused on the effects of one's blood type on emotional character of children, by comparing each individual's brain wave. Observed records are from those kids who volunteered for KRIJUS(Korea Research Institute of Jungshin Science)'s brain wave measurement from September 2005 to December 2007. The whole 3018 kids(4-7) group consists of 1322 female and 1696 male. Blood types are A>B>O>AB dominately ordered. The relations between depressive attitude and blood type of an infant couldn't be found among female. However, male type B had the most numbers of activeness character while type O shown largest rate of depressed condition. Also, the result amongst infant had shown male type B had the most active-positive rate while type AB took large for depressed-negative. The result had shown no relative connection for activity inclination. Third, among the kids type O had highest awareness rate while type A had lowest. This study revealed meaningful relation between blood types and emotional character among several categories, while suggesting follow-up researches figuring out more detailed explanations for its effect and genetic role.

Analysis of Uropathogens of Febrile Urinary Tract Infection in Infant and Relationship with Vesicoureteral Reflux

  • Purpose: This study aimed to investigate the relationship between uropathogens of infants with febrile urinary tract infection (UTI) and vesicoureteral reflux (VUR). Materials and Methods: We analyzed 308 infants hospitalized for febrile UTI between January 2010 and December 2015, and assessed the voiding cystourethrography (VCUG). The medical records, including clinical symptoms, laboratory findings, urinalysis, urine culture tests, ultrasound (US), dimercaptosuccinic acid scan, and VCUG, were retrospectively obtained. The incidences of VUR and high-grade VURs (III, IV, and V) were analyzed in 4 groups categorized by uropathogens and renal US findings. Results: The mean age of 308 infants was $3.29{\pm}2.18months$. The male-to-female ratio was 3.46:1. In urine culture tests, 267 infants (86.69%) showed single bacterial uropathogen; Escherichia coli in 241 infants (78.25%) and non-E. coli uropathogens in 26 infants (8.44%). Multiple distinctive microorganisms were identified as causative uropathogens in 41 infants (13.31%). Abnormal findings of US and VCUG were identified in 216 and 64 patients, respectively. In 308 infants, the incidences of VUR and high-grade VUR were not different among the 4 groups. In 239 male infants, the incidences of high-grade VUR were higher in patients with non-E. coli single or multiple uropathogen and with abnormal US findings (p=0.042). Conclusions: In male infants with non-E. coli uropathogen or multiple uropathogens and with abnormal US findings at febrile UTI, there was an increased chance of finding high-grade VURs on subsequent VCUG tests.

Gram-negative Septicemia after Infliximab Treatment in an Infant with Refractory Kawasaki Disease (불응성 가와사키병 환아에서 infliximab 사용 후 발생한 패혈증)

  • Kawasaki disease (KD) is an immune-mediated disease which is a leading cause of acquired cardiovascular disease in developed country. Recently, tumor necrosis factor-alpha (TNF-alpha) blocker, infliximab has been considered a promising option for patients with refractory KD. Although chronic use of a TNF-alpha blocker could increase risk of opportunistic infections, a few studies have documented that use of infliximab was safe without serious adverse effects in patients with KD. We observed serious bacterial infection after infliximab treatment in an infant with refractory KD. Our patient was a 5-month-old male infant diagnosed with KD who did not respond to repeated doses of intravenous immunoglobulin. We effectively treated him with a single infusion of infliximab (5 mg/kg), but gram-negative (Acinetobacter lwoffii) septicemia developed after infliximab infusion. Therefore, we report a case of serious septicemia after treatment with infliximab, and suggest considering the risk of severe infection when deciding whether to prescribe infliximab to an infant with refractory KD.

A Case of Hemolytic Disease of a Newborn by an Anti-$Di^a$ Antibody Treated with Intravenous Immunoglobulin (정맥용 면역글로불린 투여로 호전된 항-$Di^a$ 항체에 의한 신생아 용혈성 질환 1예)

  • Hemolytic disease in a newborn that causes early jaundice is common. It is often due to the Rh (D) and ABO incompatibility, but rarely due to unexpected antibodies. Among these unexpected antibodies, the anti-$Di^a$Dia antibody rarely occurs. The anti-$Di^a$ antibody was observed in the serum and red-cell eluate of an infant, and in the serum of his mother. The frequency of the appearance of the $Di^a$ antigen in the Korean population is estimated to be 6.4-14.5%. This paper reports a case of hemolytic disease in a newborn associated with the anti-$Di^a$ antibody. A full-term male infant was transferred to the authors' hospital due to hyperbilirubinemia the day after his birth. The laboratory data indicated a hemoglobin value of 11.6 g/dL, a reticulocyte count of 10.6%, a total bilirubin count of 14.4 mg/dL, a direct bilirubin count of 0.6 mg/dL, and a positive result in the direct Coombs' test. Due to the identification of an irregular antibody from the maternal serum, an anti-$Di^a$ antibody was detected, which was also found in the eluate made from the infant's blood. The infant had been treated with phototherapy and intravenous immunoglobulin since the second day after his birth and was discharged due to an improved condition without exchange transfusion. Therefore, in cases of iso-immune hemolytic disease in a newborn within 24 hours from birth who had a negative result in an antibody screening test, the conduct of an anti-$Di^a$ antibody identification test is recommended due to the suspicion of an anti-$Di^a$ antigen, followed by early administration of intravenous immunoglobulin.

The Study of the Attachment-oriented Caretaking Behaviors among the young infant's mothers (모유수유아 선발대회 참가 영아모의 애착 지향적 양육행동에 관한 연구)

  • The purpose of this study is to identify the characteristics and influencing factors of the attachment-oriented caretaking behaviors among the mothers with infants. The total number of 204 mothers participated in this survey as subjects. They all were breast-feeding mothers who had their infants joining in the semi-final round of a healthy breast-feeding infant contest. The data were collected from July 14, 1999 to May 19, 2000. And the data were analyzed by SPSSWIN 10.0 version. The result of this study was as follows ; 1. The mean ages of the subjects were 29.5 and 95.6 percents of them were no-job housewives. 57.8% of them were normal delivery. And the mean months of the infants were 6.2 and the male infants were 57.4%. The first born babies were 62.3%. 2. The mothers are affirmative enough with their attachment-oriented caretaking behaviors (the total mean was $3.58{\pm}.631$). In general, the mothers showed very attachment-oriented caretaking behaviors, but the points that indicated "recording the development characteristics of the infant and discussing them with husband" were very low(score $2.79{\pm}.982$). 3. There are Significant differences in caretaking behaviors of the mothers according to their ages(F=3.497, p=.017), the degree of morning sickness(F=2.633, p=.050) and the birth order of their infants(F=4.854, p=.003). The older mothers and mothers who experienced the less morning sickness showed the higher scores of the attachment-oriented behaviors. The mothers who experienced the more deliveries showed the higher scores of the attachment-oriented behaviors. Accordingly, the above result suggests that the systematic and practical nursing intervention to encourage the attachment-oriented behaviors is essential to the younger, primipara mother.

A Study on Status of Death in Rural Residents (일부(一部) 농촌주민(農村住民)의 사망(死亡) 실태(實態) 조사(調査))

  • A study on the status of death in rural area was conducted during the period from July '75 to August '75. 1,225 families and 8,067 population (4,124 male, 3,943 female) had been lived and 149 events of death since 5 years before study were occurred in survey area, Nammyon, Hwasoongun, Chonnam. The summarized results were as follows: 1. Quinquennial death rate was 3.7 (5.2 for male, 2.1 for female). 2. In respect of age group, the highest group was over 70 years old group (age at death, 30.8% of total death). High age groups (over 50 years old) occupied 71.1% of total death and death rate in these groups were higher in male than female. Child death (0-4 years old) occupied 7.4% of total death and infant death rate was higher in female than male. 3. Duration of sickness before die was highest in 1 to 12 months (39.6%) 4. The most frequent cause of death was disease of digestive system (12.1%). Other important causes were disease of circulatory system (10.7%), disease of respiratory system (9.4%) and infectious and parasitic disease (4.7%). Disease of digestive system was the most frequent cause of death in male (14.0%) and disease of respiratory system was the most frequent cause in female (9.5%).

  • The congenital esophageal atresia with tracheoesophageal fistula has been reported only 15 cases up to date in Korea, but this might be the first cases that was operated on successfully. A five day old male infant underwent the Haight`s operation on Nov-l,1968. The diagnosis was made by the esophagography and the operation was performed transpleurally through the right fifth intercostal space and the Stamm`s feeding gastrostomy was made on at the same time. The post-operative course has been uneventful until now. A literature review was done in this subject.
