저빈도 rTMS의 적용이 만성 뇌졸중환자의 인지기능에 미치는 영향 (Effect of the application of low-frequency rTMS on cognitive function in chronic stroke patients)

  • 본 연구는 저빈도 반복경두개자기자극(rTMS)이 만성 뇌졸중 환자의 인지기능에 미치는 영향을 알아보기 위해 실시하였다. MMSE-K와 한국어판 BCRS-K를 통해서 선별된 만성 뇌졸중 환자 30명을 대상으로 3개의 군으로 무작위로 나누었다. 실험군 I(n=10)은 소리만 동일시간 적용하게 하였고, 실험군 II(n=10)은 손상 측에 1 Hz rTMS를, 실험군 III(n=10)은 손상반대측에 1 Hz rTMS를 적용하여 주 5회, 1일 20분, 2주간 중재하였다. 인지기능의 변화를 알아보기 위해서 중재 전, 1주 후, 2주 후, 3개월 후 시점에서 CERAD-K 점수를 측정하였다. 연구 결과 즉각 기억단어는 실험군 I에 비교하여 실험군 II와 실험군 III에서 2주 후와 3개월 후에 통계학적으로 유의한 차이를 보였고(p<.05), 재인 기억단어에서는 실험군 III에서 2주 후와 3개월 후에 통계학적으로 유의한 차이가 나타났다(p<.05). 지연 기억단어는 실험군 I에 비해 실험군 III에서 3개월 후에 통계학적으로 유의한 차이를 보였다(p<.05). 따라서 본 연구결과를 통해서 뇌졸중 환자에서 저빈도 rTMS의 적용이 인지재활에 긍정적인 영향을 줄 수 있을 것으로 생각된다.

릴레이 경주 중에 발생한 대퇴직근 기시부의 완전 파열 - 증례보고 - (Complete Rupture of the Origins of Rectus Femoris Occurred During Relay Race - A Case Report -)

  • 대퇴직근은 슬관절을 신전하고 고관절을 굴곡하는 근육이며, 빠르고 강한 수축을 하는 제 2형 근섬유로 이루어져 있으며 두개의 관절을 거치므로, 운동 경기 중에 근위부 대퇴직근의 손상이 흔히 발생할 수 있다. 소아에서 발생하는 전하장골극에서의 대퇴직근 견열 골절이 가끔 보고가 되고 있고, 성인에서의 대퇴직근 근위부의 근-건 결합부 완전 파열은 반복적인 운동에 의해 드물게 발생하며, 외상과 관련이 없는 경우 대퇴부에서의 연부조직 종양으로 오인될 수 있다. 그러나, 릴레이 경주 중에 대퇴직근 근위부의 기시부가 급성으로 완전 파열된 예는 아직까지 보고된 바가 없다. 이에 저자들은 릴레이 경주 중에 발생한 중년 남성의 대퇴직근 기시부의 급성 완전 파열에 대하여 자기공명영상을 통하여 확인한 후 수술적 치료를 시행하였으며, 이를 문헌 고찰과 함께 보고하고자 한다.

스퀴드 심자도 장치를 이용한 심방성 부정맥의 측정 (Detection of Rapid Atrial Arrhythmias in SQUID Magnetocardiography)

  • We propose a method to measure atrial arrhythmias (AA) such as atrial fibrillation (Afb) and atrial flutter (Afl) with a SQUID magnetocardiograph (MCG) system. To detect AA is one of challenging topics in MCG. As the AA generally have irregular rhythm and atrio-ventricular conduction, the MCG signal cannot be improved by QRS averaging; therefore a SQUID MCG system having a high SNR is required to measure informative atrial excitation with a single scan. In the case of Afb, diminished f waves are much smaller than normal P waves because the sources are usually located on the posterior wall of the heart. In this study, we utilize an MCG system measuring tangential field components, which is known to be more sensitive to a deeper current source. The average noise spectral density of the whole system in a magnetic shielded room was $10\;fT/{\surd}Hz(a)\;1\;Hz\;and\;5\;fT/{\surd}Hz\;(a)\;100\;Hz$. We measured the MCG signals of patients with chronic Afb and Afl. Before the AA measurement, the comparison between the measurements in supine and prone positions for P waves has been conducted and the experiment gave a result that the supine position is more suitable to measure the atrial excitation. Therefore, the AA was measured in subject's supine position. Clinical potential of AA measurement in MCG is to find an aspect of a reentry circuit and to localize the abnormal stimulation noninvasively. To give useful information about the abnormal excitation, we have developed a method, separative synthetic aperture magnetometry (sSAM). The basic idea of sSAM is to visualize current source distribution corresponding to the atrial excitation, which are separated from the ventricular excitation and the Gaussian sensor noises. By using sSAM, we localized the source of an Afl successfully.

이마관자엽 치매 환자에서 나타난 하지 근력 저하와 복합부위 통증증후군에 대한 증례 보고 (Lower Motor Weakness and Complex Regional Pain Syndrome of Lower Limb in the Patient of Frontotemporal Dementia: A Case Report)

  • 이마관자엽 치매는 조기 발현형 치매 중 두 번째로 흔한 형태로 행동, 언어, 인지 장애를 보이는 신경퇴행성 질환이다. 이마관자엽 치매에서 운동 기능 이상이 동반되는 경우는 운동 신경원 질환과 파킨슨증, 진행성 핵상 마비 등으로 대표되나, 다른 동반 질환 없이 이마관자엽 치매가 중추신경계의 운동신경영역을 직접 침범하여 나타난 운동 기능 이상은 보고된 바가 없다. 또한, 임상적 치매 집단과 복합부위 통증 증후군 사이의 연관성은 보고된 바가 없다. 저자들은 이마관자엽 치매환자에서 나타난 중추신경계 원인의 하지 근력 저하를 뇌 자기공명영상과 전기진단학적 검사를 통해 진단하였고, 동반된 복합부위 통증증후군을 삼상 골주사 검사를 통해 진단하였으며, 이에 따른 임상적 치료를 시행하였다. 스테로이드를 이용한 복합부위 통증증후군 치료 후에 환자의 통증은 호전되었고, 입원 상태에서 하지 근육에 대한 기능적 전기 자극 치료, 근력 강화 운동 및 보행 훈련을 포함한 포괄적 재활치료를 시행한 후에 저명한 기능적 호전을 보였다. 이마관자엽 치매에서 관찰된 중추신경계 원인의 근력 저하에 대한 재활 치료는 전반적 기능의 향상에 효과적일 것으로 추정된다.

신맥(申脈) 혈위자극이 fMRI상 뇌활성화 변화에 미치는 영향 연구 (fMRI Study on the Brain Activity Induced by Manual Acpuncture at BL62)

  • Objectives: Recently, the effect of acupuncture has been approved not only in the East but also in the West, so the interest on acupuncture was greatly improved. Especially, functional magnetic resonance imaging(fMRI) was embossed as the study tool for the mechanism of acupuncture noninvasively and many studies on the mechanism of acupuncture using fMRI were carried out. We archived the fMRI study on the brain activity induced by manual acupuncture at BL62(申脈). Methods: The study was the acupuncture at BL62(申脈) and we acquired 9 fMRI results from 6 persons$(age\;20{\sim}30,\;4\;male\;and\;2\;female)$. These studies employed The block design for mapping brain activity and acupuncture was perfomed at BL62(申脈) on the left foot. Results: The brain related motor function was cerebellum, basal ganglia and cerebral cortex and thalamus connected these elements. In the result of this study, the regions of significant activation in the cerebellum was centered on the spinocerebellum in the anterior lobe, so we presumed that this result showed the input of stimulation by the acupuncture on BL62(申脈). But basal ganglia and cerebral cortex showed the regions of significant activation in the left larger than the right and regions of the cerebral cortex was the motor and sensory cortex. Such a result explained that acupuncture at BL62(申脈) could have influence the motor function and acupuncture at left BL62(申脈) could affect the right side through the activation of the left basal ganglia and cerebral cortex. Conclusions: In the theory of crossing needling at collaterals(繆刺論), it the pathogenic factor invaded in the Yang Heel channel(陽?脈) that was one of the eight Extra meridians(奇經八脈), we recognized the disease of the collateral channel and used contralateral BL62(申脈) for treatment of the Yang Heel channel(陽?脈). Moreover the result of this study could bear the construction that acupuncture at the left BL62(申脈) treats the contralateral lesion and this construction is related to the theory of crossing needling at collaterals(繆刺論).

  • 열작용에 의한 전자파의 안전성을 생각하는데 있어서는 , 이 30${\sim}$300MHz 부근의 주파수대가 가장 중요하고 기립 신장 방향의 공진 상태에서는 목, 슬 관절, 발목 등 가는 부분이 강하게 가열되는 위험이 있다. 이처럼 자기자극기는 시간적으로 변화하는 자계를 인체내부에 전계를 유도하여 무접촉성, 무침습적으로 깊고, 넓은 부위의 자극이 가능한 이점으로 말초신경, 뇌질환, 뇌신경, 근력, 요실금, 노인성, 근질환, 재활치료 등까지 근래에 와서 활발하게 연구가 활발히 진행 중이다. 마지막으로 초미니 자기신경 치료기를 말초신경계 질환치료에 적용하기 위하여 연구한 결과, 첫째로 자기루프 객체의 실린더 형태로 설계하고 동작전원과 제어장치를 구현하였고, 둘째로 루프 객체 실린더형이 프로브의 특성과 치료영역 범위와 치료펄스의 거리에 대한 유효영역을 구하였고, 셋째로 프로브의 형태에 따른 펄스포밍과 에너지 값을 구현하여, 병변에 따른 치료펄스를 구현할 수가 있었다. 특히, 특성실험을 통해 댐핑펄스 형태와 치료펄스 형태 등을 다양하게 구하게 되었다. 최근에 의학 발달로 무자극, 무침습에 따른 질환치료에 조금이라도 도움이 되고자 구현 하였다. 본 연구에서는 고진폭 치료펄스(전통적인 magneto-therapy)의 크게 대단한 강렬)는 짧은 시간에 치료효과를 극대화 하고, 미래에는 수요가 크게 확대 될 것으로 예측된다.

거골 골연골병변 치료 동향: 대한족부족관절학회 회원 설문조사 분석 (Current Trends in the Treatment of Osteochondral Lesion of the Talus: Analysis of the Korean Foot and Ankle Society (KFAS) Member Survey)

  • Purpose: Given the lack of definite evidence-based guidelines in clinical practice, there may be a wide variation in treatment protocols for osteochondral lesions of the talus (OLT). Based on the Korean Foot and Ankle Society (KFAS) member survey, this study aimed to report the current trends in the management of OLT. Materials and Methods: A web-based questionnaire containing 30 questions was sent to all KFAS members in September 2021. The questions were mainly related to clinical experience and preferences in diagnosis, conservative, and surgical treatments for patients with OLT. Answers with a prevalence of ≥50% of respondents were considered a tendency. Results: Sixty-two (11.3%) of the 550 surgeons queried responded to the survey. The responses to 9 (30.0%) of the total of 30 questions established a tendency. Answers exhibiting a tendency were as follows; additional diagnostic tools except for plain radiograph (magnetic resonance imaging), most common conservative treatment method (oral medication, rest), most important radiological factor in decision making for surgical treatment and method (size of the lesion, ankle instability, loose bodies), most important patient factors in decision making for surgical treatment and method (age, activity or occupation), infrequently requiring posterior arthroscopy (less than 3%), most common revision surgery for failed bone marrow stimulation procedure (osteochondral autograft transplantation [OAT]), not requiring additional procedure for donor site in OAT, the main reason for unsatisfactory result after OAT (persistent pain without radiological abnormality), no generalization of autologous chondrocyte implantation or chondrogenesis using stem cells. Conclusion: This study presents updated information on current trends in the management of OLT in Korea. Both consensus and variations in the approach to patients with OLT were revealed through this survey. Since recent biologic efforts to regenerate cartilage have been unsuccessful, further studies to identify clinical evidence would be needed.

섬유근통 환자에 대한 Milnacipran과 Pregabalin 약물치료에 대한 기능적 자기공명영상에서의 후속 영향 비교 (Comparison of the Medication Effects between Milnacipran and Pregabalin in Fibromyalgia Syndrome Using a Functional MRI: a Follow-up Study)

  • 목적 : 섬유근통 증후군의 치료에 주로 쓰이는 두 계열의 약물인 Milnacipran (이하 MLN)과 Pregabalin (이하 PGB)의 환자 투여에 따른 약물 치료 효과를 뇌 활성도 변화의 비교 분석을 통해 비교하였다. 대상 및 방법 : 섬유근통 증후군 환자 20명을 대상으로 진행하였으며 20명의 환자 중 약물 치료 군에 따라 MLN 치료군 10명과 PGB 치료군 7명으로 분류하였다. 모든 환자군은 섬유근통 증후군 이외의 요인을 배제하기 위해 정신건강 의학과적 평가를 통해 선별된 후 섬유근통 진단을 위한 임상적 평가가 진행 되었다. 기능적 자기공명영상의 촬영 시 엄지손가락에 블록 형태의 압통 자극을 가해 주었고 영상은 약물 치료 전과 약물 치료 후에 각각 획득하였다. 영상 획득 후 일련의 전처리 과정을 거쳐 약물 치료 전후의 자기공명 혈중산소치의존 (Blood Oxygen Level Dependent, 이하 BOLD) 신호 비교를 위한 대응표본 t-검정과 두 표본 t-검정을 실시 하였다. 결과 : 임상적 평가에 있어 약물 치료 전에는 두 그룹간에 유의한 차이가 나지 않았으며 약물 치료 후 전신통증지수 (Widespread Pain Index, WPI)와 대상자 스스로 피로도를 평가하는 검사 (Brief Fatigue Inventory, BFI)에서 PGB군에 유의한 수준으로 낮게 나타났다. 기능적 영상 분석에 있어 약물 치료 후의 영상 비교 결과 앞 띠이랑과 대뇌섬을 포함한 영역에서 PGB군이 높은 활성도를 보였다. 또한 약물 치료 전후 효과 비교에서는 MLN군에서 대뇌섬, 시상을 포함한 영역에서 치료 후 BOLD 신호가 감소하는 경향을 볼 수 있었지만 PGB군에서는 MLN에 비해 감소된 영역의 수와 크기가 비교적 작게 나타났다. 결론 : 전체 결과에서 두 치료군 모두 증상이 호전되는 경향을 나타냈으나 임상적 평가와 기능적 영상 평가에서 서로 다른 경향을 나타내었다. 이는 두 약물 치료 기전과 세부 치료 목적이 다르고, 전체 환자 수의 부족으로 인한 임상적 평가와의 낮은 상관관계에 의한 영향으로 보이며 이러한 영향을 최소화 시킨다면 두 약물 간의 정확한 치료 효과 비교를 할 수 있을 것으로 예상된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases.
