Implant overdenture of mandible with severe unilateral atrophy: Report of two cases (심한 편측 하악 치조골 흡수를 보이는 환자에서의 임플란트 피개의치 수복 증례)

  • There are several unfavorable conditions regarding alveolar bone condition that may compromise the denture patient's satisfaction. Chewing efficiency may not be satisfactory when alveolar bone is deficient, and the denture stability could hardly be achieved when alveolar bone shape is irregular. Implant overdenture can be useful to provide satisfactory denture experience compared to conventional denture. The attachment for implant overdenture can be classified into bar attachment and solitary attachment. When the positions of the implants are in the mandibular anterior region, bar attachment may be favorable to obtain a rigid support of the entire denture. When implants are distributed both on anterior and posterior region, a solitary attachment could be considered for ease of removal and maintenance. This report presents implant overdenture cases with the patients that had unilateral mandibular alveolar bone atrophy conditions. Different abutments were chosen based on the individual patient's mandibular alveolar bone condition and the treatments were successful in terms of patient satisfaction.

Concentration of As Component in Pb Dross by Magnetic Separation (자력선별에 의한 납드로스 중 비소성분의 농축)

  • Pb dross has been generated from recycling processes of waste acid lead batteries, and proper treatment of Pb dross was required because it contains As component, which has been found to be toxic. This study is aimed at concentrating As component by magnetic separation of ground product obtained from ball and mixer milling of Pb dross. No arsenic component was detected in the non-magnetic product of 10000 G magnetic separation using ground product with $-150{\mu}m$ by ball milling, and As could be concentrated upto 18.87 % by further 2000 G magnetic separation. The ball mill ground product with over $300{\mu}m$ was ground again by mixer mill to less than $150{\mu}m$, and then magnetic-separated by 4000 G followed by 2000 G magnet. The As component was concentrated upto 21.021 % in the magnetic fraction of 2000 G. It was confirmed that As component exsit as $Fe_2As$ by XRD measrument. These results indicate that As component could be concentrated from 0.6 % in the Pb dross to 21.021 % in the magnetic fraction by milling followed by magnetic separation.

  • The installation and performance test of the ISOL (Isotope Separation On Line) system for the generation and separation of Rare Isotopes (RI) beams is in progress at the Rare Isotope Science Project (RISP), Institute for Basic Science (IBS). The various RI beams generated by the ISOL target/ion source go through the beam lines and separators, and only the RI beam desired by the user is selected and transmitted to the superconducting linear accelerator at the downstream of the ISOL. In the ISOL system, two separators are installed to separate a specific RI beam, and control is performed by the Experimental Physics and Industrial Control System (EPICS). In this study, an EPICS IOC (Input-Output Control) was developed to measure the magnetic field of a dipole magnet for mass separation of a multivalent (n+) RI beam in the A/Q separator, which is one of the ISOL RI beam separators. The operational stability of the A/Q separator was tested through a magnetic field measurement using a Hall probe.

  • The power generation method using expensive diesel has operation problems such as high cost diesel generator and a lack of reserved power due to increase of power demand in some islands, requiring expansion of power generation facilities. To solve this problems, it is necessary to improve the efficiency of power generation facilities through an ORC(Organic Rankin Cycle) system application that uses waste heat as a heat source. Therefore, localized application technology of price competitive and highly reliable ORC power generation system is needed, and optimization technology of generators is having great effect, so this study performed two generator designs to get a high-efficiency generator with an optimized 30kW output. The comparison of simulation data for two designed models showed that a generator with SPM factor of 46.2% had an efficiency of 92.1% and a power ouput of about 23.2kW based on 12,000rpm, a generator with SPM factor of 44.46%, had a power output of 27.9kW and efficiency of 93.6% based on above rpm. For the verification of improved design model with SPM factor of 44.46%, the prototype test system with 110kW motor dynamometer was installed and got to the efficiency of 92.08% with conditions of the rated capacity 25kW at 12,000rpm, the test results of prototype generator showed the validity of generator design.

  • The control technology of animatronics is an interesting topic explored in various fields, including engineering, medicine, and art, with ongoing research efforts. The conventional method for controlling the movement of animatronics is to use electric motors installed inside the body. However, this method is difficult to apply when expressing a narrow space inside the body. In this study, a method of using external forces instead of installing mechanical devices inside the body was proposed to control the movement of a thin and long tentacle organism. Specifically, in this study, the joint body of animatronics was made of magnetic metal material so that it could be affected by the force of an externally installed electromagnet. The strength of the electromagnet was controlled by a PID controller to enable real-time control of the position of the animatronics body. In addition, the magnet was made to rotate, and the speed of rotation was changed to create various movements. Through virtual environment simulations, our experiments demonstrate the superiority of the proposed method, showcasing real-time control by users and the creation of animations in various styles.

Analysis of the Mandibular Movements in Patients with Internal Derangement of the Temporomandibular Joint According to Diagnostic Subgroups (측두하악관절내장 환자의 진단분류에 따른 하악운동 특성의 분석)

  • The purpose of this study was analyse the mandibular movements in patients with internal derangement of the temporomandibular joint according to diagnostic subgroups. The author classified patients with internal derangement of the temporomandibular joint into 4 diagnostic subgroups by means of the magnet resonance imagings, and evaluated the clinical signs and the mandibular movements with Mandibular Kinesiograph(MKG) in each subgroups. The mandibular movements, measured in this study, were the types of movement in frontal and sagittal plane, velocities in opening and closing movement, and the opening and closing movement, and the opening and closing velocity pattern. The data were compared between the 5 groups including the normal group. The results were as follows : 1. Pain was more frequently observed in the anterior disc displacement without reduction group than in the anterior disc displacement with reduction group. Sound of joint was more frequently observed in the anterior disc displacement with reduction group, and limitation of mandibular opening movement was more frequently observed in the anterior disc displacement without reduction group. Duration of the anterior disc displacement without reduction group was significantly short compared to that of the anterior disc displacement with reduction group, and duration of the unilateral anterior disc displacement without reduction group was shortest in the experimental group. The frequency of Angle's classifications had not significant correlations between the experimental groups. 2. Active and passive range of the opening movement, maximum protrusive movement, maximum lateral movement toward left side were significantly decreased in the experimental groups compared to the control group, but there was no significant difference in the range of the maximum lateral movement toward right side between the control and experiment groups. In unilateral anterior disc displacement without reduction group, the range of maximum lateral movement toward unaffected side was no significant difference in the range of the maximum lateral movement between toward affected side and toward unaffected side. 3. Maximum opening velocity, maximum closing velocity, average opening velocity, average closing velocity and maximum velocity of terminal tooth contact were significantly decreased in the experimental groups compared to control group. There was no significant difference in maximum opening velocity and maximum velocity of Terminal tooth contact between the subgroups of the experimental group each other, but there was significant difference in maximum closing velocity, average opening velocity and average closing velocity between the subgroups each other. 4. In the frontal plane of the MKG, the frequency of complex deviation type(F-2)pattern was significantly increased in the anterior disc displacement with out reduction group compared to the anterior disc displacement with reduction group and the control group. In the sagittal plane, the frequency of coincident type(S-1)was decreased in the same group. 5. In the maximum opening velocity pattern, the frequency of no-peak type (OV-3)in the unilateral anterior disc displacement with reduction group was significantly increased compared to the control group. The frequency of 1-peak type (OV-1) and 2-peak type (OV-2) was decreased in the anterior disc displacement with out reduction group, but the frequency of no-peak type (OV-3)was increased in the same group. In the maximum closing velocity pattern, the frequency of no-peak type (CV-3) was significantly increased in the anterior disc displacement without reduction group. Compared to the anterior disc displacement with reduction group and the control group. The frequency of 1-peak type (CV-1) and 2-peak type (CV-2) in the anterior disc displacement with reduction group was decreased than that in the control group.

A Study on Compensation for Imaging Qualities Having Artifact with the Change of the Center Frequency Adjustment and Transmission Gain Values at 1.5 Tesla MRI (1.5 Tesla 기기에서 중심주파수 조정과 송 신호강도(Transmission Gain)값 변화에 따른 인공물이 있는 자기공명영상의 질 보상에 관한 연구)

  • The purpose of this study is to compensate for susceptibility and a ferromagnetic body artifact using CFA and TGV on MR Imaging. A total of 30 patients (15 men and 15 women, mean age: 45 years) were performed on head and neck diseases. MR Unit used a 1.5T superconducting magnet (GE medical system, High Density). This study have investigated by changing with CFA and TGV (70, 90, 110, 130, 150) searching for compensation values about susceptibility and a ferromagnetic body artifact in 60 kg standards of body weight (p<0.05). As a quality results, Image qualities were obtained at different score from CFA and TGV (70, 90, 110, 130, $150=3.23{\pm}0.35$, $4.31{\pm}0.02$ $4.23{\pm}0.21$, $5.12{\pm}0.25$, $7.13{\pm}0.72$, $8.31{\pm}0.01$, $5.21{\pm}0.15$, $6.14{\pm}0.08$, $5.23{\pm}0.72$, $5.91{\pm}0.06$, p<0.05). Absolute CNRs (TG, CNRpre, CNRpost) were acquired with (70:$-1.44{\pm}0.11$, $-2.7{\pm}0.04$, 90:$-2.18{\pm}0.42$, $-4.41{\pm}0.43$, 110:$-2.89{\pm}0.43$, $-5.23{\pm}0.02$, 130:$-2.34{\pm}0.05$, $-5.26{\pm}0.01$, 150: $-2.09{\pm}0.08$, $-3.87{\pm}0.12$, p<0.05). In conclusions, this study could be compensated for metal and flow artifacts surrounding the tissues having artifact by changing CFA and TGV.

Lumbar Angiomatous Meningioma in a Dog (개에서 발생한 요추 혈관종성수막종 증례)

  • An 11-year-old castrated Schnauzer presented with paraplegia for the last two weeks, initially developed pelvic limb ataxia 6 months ago and left pelvic limb paresis 3 months ago. On neurological examination, paraplegia and postural reaction deficits were noted in the pelvic limbs with no deep pain sensation, and the dog showed exaggerated spinal reflexes and involuntary urination. Magnetic resonance (MR) images of thoracolumbar spinal cord were obtained with a 0.3 Tesla magnet. A mass with a broad base to the dorsal and left dural margins was identified at the level of L2, causing marked spinal cord compression. The mass showed isointensity to the spinal cord on T1-weighted (T1W) precontrast images, hyperintensity on T2-weighted images, marked homogeneous contrast enhancement with well-defined margins and the "dural tail" sign on T1W postcontrast images. An intradural-extramedullay tumor was considered, most likely, meningioma was highly suggestive. Left-sided hemilaminectomy and dorsal laminectomy were performed. An intradural mass partially adherent to dura mater was compressing the spinal cord, and the mass was completely removed with the attached dura mater. A histopathologic diagnosis of angiomatous subtype meningioma was made.

Toll-like receptor 2 expression on monocytes from patients with BCG vaccine-associated suppurative lymphadenitis (BCG 접종 후 발생한 화농성 림프절염 환자의 단핵구에서 Toll-like receptor 2의 발현)

  • Purpose : Toll-like receptor 2 (TLR2) is critical in the immune response to mycobacterial infections. The purpose of this study was to analyze TLR2 surface expressions and TLR2-mediated tumor necrosis factor-alpha ($TNF-{\alpha}$) and interleukin-6 (IL-6) production in patients with BCG vaccine-associated suppurative lymphadenitis. Methods : Peripheral monocytes were separated from 16 patients with BCG vaccine-associated suppurative lymphadenitis and 10 healthy controls using a magnet cell isolation kit. Monocytes ($1{\times}10^6$ cells/well) were incubated with a constant amount of $Pam_3CSK_4$ ($100{\mu}g/mL$) for 24 hours. TLR2 surface expression on monocytes was analyzed by FACS analysis and TLR-2 mRNA expression was determined by RT-PCR. TLR2-mediated $TNF-{\alpha}$ and IL-6 production were measured by ELISA. Results : In patients with BCG vaccine-associated suppurative lymphadenitis, low TLR2 expression on monocytes ($3.39{\pm}$1.2% versus $4.64{\pm}2.6%$) together with significantly lower TLR2 mRNA expression than in the healthy controls was seen after $Pam_3CSK_4$ stimulation. TLR2-mediated $TNF-{\alpha}$ and IL-6 production in patients with BCG vaccine-associated suppurative lymphadenitis ($TNF-{\alpha}$, $775.5{\pm}60.8pg/mL$; IL-6, $4,645.8{\pm}583.9pg/mL$) were also lesser than that in healthy controls ($TNF-{\alpha}$, $1,098.5{\pm}94.3pg/mL$; IL-6, $6,696.3{\pm}544.3pg/mL$). Conclusion : These findings suggest that low TLR2 expression on monocytes might be associated with increased susceptibility to BCG vaccine-associated suppurative lymphadenitis.

  • The effects of Wang-ttum, Magnetic Water, Magnetic field and Sibjeondaebotang on hemodynamics in sleep deprivation were studied. The results as follows; 1. As mentioned obove, the effects of Wangttum and Sibjeondaebotang on hemodynamics in sleep deprivation were observed both the impulse of SIM-YANG and mutual function of QI-HYOL. The effects of Magnetic water and Magnetic field were observed the side of mutual function of QI-HYOL.
