BENIGN LYMPHOEPITHELIAL LESION ARISING IN THE PAROTID GLAND (이하선에 발생한 양성 림프상피성 병소)

  • The authors diagnosed a 33 years old female as benign lymphoepithelial lesion after undergoing clinical, radiological and histopathological examinations and the characteristics were as follows : 1. Clinically, the patient complained of painless bilateral swelling of the parotid glands and dryness of the palate. Rheumatoid factor was detected in her serum. 2. Sialograms showed punctate or globular collections of contrast media distributed evenly throughout the parotid glands in so-called 'cherry blossom' or 'leafless fruit-laden tree' appearance. 3. A salivary gland scan showed no uptake of radioisotopes by the parotid glands. 4. At Tl-weighted imaging of PNS MRI, the lesions had the same signal intensity as the rest of the gland. At TI-weighted imaging, the lesions could be seen as high signal intensity 1.3 cm and 2.1 cm in diameter in the left and the right parotid gland respectively. 5. Ultrasonogram showed sonolucent lesions 20×15mm and 17×14mm in size in the lower part of the left parotid gland and another 18×11mm in size in the lower part of the parotid gland as well as many other small sonolucent lesions. 6. Histopathologically, lymphocytic infiltration replacing the normal acini and lymphoid follicles containing germinal centers could be seen. Epimyoepithelial islands were scattered throughout the lesion and benign lymphoepithelial cysts were also observed.

Capillary Hemangioma in the Posterior Mediastinum -A case report- (후종격동에 발생한 모세혈관상 혈관종 -1예 보고-)

  • Hemangiomas of mediastinum are rare lesions comprising less than 0.5% of all mediastinal masses. Posterior mediastinal hemangiomas are less common than anterior mediastinal. A 21 year old female was refered to our hospital because of abnormal mediastinal shadow in simple chest X-ray. Chest CT scan and T-spine MRI revealed a posterior mediastinal dumbbell-shaped mass with extradural extension. Surgical excision was performed and pathologic diagnosis was confirmed as 3${\times}$4${\times}$2 cm sized capillary hemangioma.

Case Report of Brain Metastsis of Primary Cardiac Chondrosarcoma without Primary Recurrence (원발성 심장 연골육종 증례보고)

  • Because of paucity of primary cardiac tumors, primary cardiac tumors are of much interest to the pathologist, cardiologist and cardiac surgeon. A cardiac chondrosarcoma, which very rare, is presented as follows: This 37 year-old female patient was admitted under the diagnosis of a left atrial tumor(myxoma) on July 3, 1993. Using the CPB, she underwent complete removal of tumor including the atrial septum. And final pathology report was chondrosarcoma. The adjuvant irradiation with total dose of 5040 cGy(28 fractiations for 2 months) and chemotherapy(VP-16 and ifosfamide) was completed. She had no evidence of recurrence until May 2, 1995. when she was admitted due to brain metastasis, which was detected by MRI scan with a tumor free interval for 2 years. A tumor, located around the frontoparital cortex was removed neurosurgern on Sep. 13, 1995 and the final pathology report was also a chondrosarcoma on pathologic exam. The postoperative course was uneventful.

Unifocal Langerhans Cell Histiocytosis of Frontal Bone in a Child (소아 전두골에 발생한 랑게르한스세포 조직구증)

  • Lateral eyebrow mass with primary skull lesion are rare in pediatric population. Although epidermoid cyst and dermoid cyst are the most commonly encountered skull lesions in pediatric population, Langerhans cell histiocytosis (LCH) is rarely reported. We report a case of LCH arising from the lateral eyebrow with osteolytic lesion involving the frontal bone. A 5-year-old boy was presented with a hard, fixed mass in his lateral eyebrow. Contrast magnetic resonance imaging revealed inhomogeneous enhancement of the mass with direct invasion of the frontal bone and adjacent dura mater. Under general anesthesia, linear incision at the lateral eyebrow region was made. Intraoperative evaluation revealed hard, fixed and well-defined soft tissue mass. The final extirpated mass was $2.5{\times}2.4cm$ in size, and was accompanied by a $1{\times}1cm$ sized defect on the frontal bone with intact dura mater. The surgical wound was closed primarily by a layer-by-layer fashion. Histologic examination was later performed for definite diagnosis. The histologic examination revealed abnormal proliferation of Langerhans cell with granuloma formation. Radionuclide bone scan and positron emission tomography was taken and revealed free of multi-organ involvement. At 3 months after surgery, natural looking contour at the lateral eyebrow region was observed with no tumor recurrence. Differential diagnosis of the hard and fixed mass at the lateral eyebrow region affecting the primary skull lesion from pediatric population includes epidermoid cyst, dermoid cyst and LCH. Generally, brief physical examination with plain X-ray view can be performed for clinical evaluation, but for a definite diagnosis, contrast MRI may be helpful.

Hepatic Pseudolymphoma Mimicking a Hypervascular Tumor: A Case Report (과혈관성 종양으로 오인된 간의 가성림프종: 증례보고)

  • Hepatic pseudolymphoma is a rare benign liver mass that is characterized by proliferation of non-neoplastic lymphocytes extranodally. To the best of our knowledge, only 46 cases have been reported in the English literature. We described the case of a 75-year-old woman with hepatic pseudolymphoma mimicking a hypervascular tumor. After the histological confirmation of the rectal neuroendocrine tumor, CT scan revealed a 1.0 cm-sized, poorly-defined and low-density nodule in the liver. On MRI, the hepatic nodule showed an arterial enhancement and a low-signal intensity on the hepatobiliary phase. On diffusion-weighted imaging, the hepatic nodule showed a high signal intensity on a high b-value. On fluorodeoxyglucose (FDG) positron emission tomography (PET)/CT, it revealed a high standardized uptake value nodule. The US showed the hypoechoic nodule and the US-guided biopsy confirmed the hepatic pseudolymphoma.

Alterations in Functions of Cognitive Emotion Regulation and Related Brain Regions in Maltreatment Victims (아동기 학대 경험이 인지적 정서조절 능력 및 관련 뇌영역 기능에 미치는 영향)

  • Objectives Maltreatment experiences can alter brain function related to emotion regulation, such as cognitive reappraisal. While dysregulation of emotion is an important risk factor to mental health problems in maltreated people, studies reported alterations in brain networks related to cognitive reappraisal are still lacking. Methods Twenty-seven healthy subjects were recruited in this study. The maltreatment experiences and positive reappraisal abilities were measured using the Childhood Trauma Questionnaire-Short Form and the Cognitive Emotion Regulation Questionnaire, respectively. Twelve subjects reported one or more moderate maltreatment experiences. Subjects were re-exposed to pictures after the cognitive reappraisal task using the International Affective Picture System during fMRI scan. Results The maltreatment group reported more negative feelings on negative pictures which tried cognitive reappraisal than the no-maltreatment group (p < 0.05). Activities in the right superior marginal gyrus and right middle temporal gyrus were higher in the maltreatment group (uncorrected p < 0.001, cluster size > 20). Conclusions We found that paradoxical activities in semantic networks were shown in the victims of maltreatment. Further study might be needed to clarify these aberrant functions in semantic networks related to maltreatment experiences.

Clinical Feasibility of CT Brain Perfusion in a Dog with Sellar Region Tumor

  • A 10-year-old spayed female Poodle was referred for blindness. On ophthalmic examination, loss of bilateral ocular pupil light reflex, visual loss, and right retinal detachment were confirmed at a local hospital. Magnetic resonance imaging (MRI) of the brain was performed to identify the optic nerve, optic chiasm, and brain disease. A sessile mass centered on the region of the optic chiasm was identified. The mass had iso- to hypointense on fluid-attenuated inversion recovery and T2-weighted images and mildly hypointense on T1-weighted images compared to the gray matter, with strong contrast enhancement. Peripheral edema was also identified. Computed tomography (CT) brain perfusion was performed to obtain additional hemodynamic information about the patient using a multislice CT. CT perfusion showed that the cerebral blood volume in the left temporal lobe region (13.4 ± 1.6 mL/100 g) was decreased relative to the contralateral region (19.9 ± 0.3 mL/100 g). The patient showed decreased appetite and consciousness one week after the CT scan with clinical symptoms worsened. The patient had seizure, tetraparesis, and loss of consciousness. It was euthanized one month later at the request of the owner. This report suggests that CT brain perfusion can provide additional hemodynamic information such as insufficient brain perfusion in sellar region tumor which can help assess potential complications and prognosis and plan treatment.

Dermatofibrosarcoma Protuberans on the Occipital Scalp Showed Uncommon Presentation: A Case Report (비전형적 임상양상을 보이는 후두부의 융기성 피부 섬유 육종에 대한 증례보고)

  • Dermatofibrosarcoma protuberans (DFSP) is a rare soft tissue sarcoma, with an incidence of about 0.8% to 5% per million people per year, accounting for 1% of soft tissue sarcomas. In its early stage, DFSP is typically found as a violet or pinkish macule or patch, and it can develop into a palpable mass with ulceration or bleeding. The standard treatment for DFSP is wide local excision of the tumor with a 2- to 3-cm negative margin, and radiation therapy or chemotherapy can be conducted with surgical treatment. A 35-year-old man had a palpable mass on the left side of his occipital scalp without color change, ulceration, or bleeding, which typically are present in malignancy. A magnetic resonance imaging (MRI) scan showed a 3-cm homogenous enhanced mass without adhesion between the scalp and the mass. Unexpectedly, a biopsy revealed the round mass to be DFSP. A wide excision and rotation of the scalp flap were performed. The patient recovered without any complications and received adjuvant radiotherapy at a dose of 60 Gray (Gy) for six weeks. There was no recurrence through six months of follow-up. Here we report this unique case of DFSP with atypical presentation.

Rice Bodies Presenting as Intra-Articular Masses in Pediatric Idiopathic Arthritis: A Case Report (소아기 특발성 관절염 환자에서 관절 내 종양형태로 나타난 쌀소체: 증례 보고)

  • Rice bodies are commonly detected in adults with rheumatoid arthritis and rarely occur in children. An 11-year-old female adolescent who visited our hospital for evaluation of knee pain underwent a MRI scan, which revealed an intra-articular mass. Arthroscopic examination of the mass confirmed conglomerated rice bodies. We report a case of rice bodies that clinically presented as intra-articular masses.

  • The accessory tendon of the extensor hallucis longus is a common type of extensor hallucis longus variation. This is a case of a 38-year-old female patient who initially considered conservative treatment for a suspected partial rupture, but finally underwent surgery after being diagnosed with a complete rupture of the main tendon and accessory tendon medial to the main tendon on MRI scan.
