Rapidly Calcified Epidural Hematoma in a Neonate

  • We report a very rare case of a rapidly calcified chronic epidural hematoma (EDH) in a neonate. A 26-day-old female infant was referred to us from a regional hospital because of drowsy mentality and a seizure attack. She was delivered through caesarian section because normal spontaneous vaginal delivery was prolonged and failed. At birth, mild scalp swelling was found on the right frontal area. Scalp swelling was spontaneously resolved and she was discharged without any problems. On the 25th day after her birth, the baby presented with drowsiness and hypotonia following a generalized tonic-clonic seizure. Magnetic resonance imaging (MRI) and a computed tomography (CT) scan revealed a chronic EDH that had a thick layer of calcification. A small burr-hole trephination was performed and a single silastic drainage catheter was inserted. After the operation, a total of 12 ml of liquefied hematoma was drained, and the patient's mentality improved from drowsiness to alertness. The patient was asymptomatic when discharged.

A Clinical Study on the Functional Outcome in Stroke (중풍 환자의 기능 회복에 관한 임상적 고찰)

  • In the oriental medicine field, researches have been in progress regarding oriental medical factors. I also carried out a clinical study on stroke, from which I discovered meaningful results about important factors that impact on the prognosis of stroke. We studied 132 patients after diagnosis of stroke through a Brain-CT scan and MRI sea, to the oriental internal medicine department at the Hospital affiliated to Oriental Medical College, Dongeui University. All the patients showed meaningful improvement in the examination of their symptoms after four weeks, compared with the first week. Between strokes involving meridian system and strokes involving internal organs, the symptom of paralysis caused by strokes involving meridian system, which patients were in favorable initial condition, had improved significantly and that by strokes involving internal organs had not. The group whose initial bowel movement since the stroke was delayed, whose coating on the tongue was thick or the tongue was dry, whose pulse was tachycardiac, or who had a history of hypertension or diabetics showed a significantly worse symptom after 4 weeks than that of groups in the opposite cases.

Granuloma Resulting from the Cervical Metallic Plate, Masquerading as Posterior Pharyngeal Wall Cancer (하인두 후벽암으로 오인된 경추 내고정 금속판 삽입 후 발생한 육아종 1예)

  • The anterior approach to the cervical spine surgery is associated with possible complications such as dysphagia, hoarseness and granuloma formation etc. Because of orthopedic metallic plates, the increasing or focal uptake pattern may be demonstrated in $^{18}F$-FDG PET/CT scan. A 67-year-old-man came to our department, complained of dysphagia during the 4 months. He underwent cervical spine surgery three years ago. The CT and MRI findings mimicked typical posterior pharyngeal wall cancer with cervical metastasis. Furthermore, the SUV in $^{18}F$-FDG PET/CT was 10.3. But he was finally diagnosed as a granuloma resulting from the metallic cervical implants. The clinical correlation and medical history should be taken into account to avoid false-positive findings in PET/CT and to avoid many erroneous diagnostic pathways.

Spontaneous Spinal Epidural Hematoma with Intraoperative Acute Massive Bleeding - Case Report - (수술중 급성 대량 출혈을 동반한 자발성 척추경막외혈종 - 증 례 보 고 -)

  • Spontaneous spinal epidural hematoma is rare disease and usually presents with a progressive neurological syndrome for which surgical decompression is usually indicated. The cause of bleeding in epidural hematoma remains unknown in most of the cases. The most frequently identified risk factor is coagulopathy or treatment with anticoagulants. Recently, authors experienced a case of spontaneous spinal epidural hematoma with intraoperative profuse bleeding at the cervicothoracic location. Laboratory examination showed no evidence of coagulopathy or hepatic disease. On neurologic examination, left hemiparesis(Grade : II) and left side sensory change were noted. On MRI scan, there was a mass of high signal intensity in T2WI and isosignal intensity in T1WI compressing the cord to left side. The patients had good surgical outcome after decompressive laminectomy and hematoma removal.

  • 뇌종양 치료성적은 점차 향상되고 있지만 백혈병 등 다른 소아암에 비해 향상 속도가 느리다. 하지만, 근래 MRI, PET scan 등 neuro-imaging 기술의 발달, 뇌종양의 분자유전학적 연구, 외과 수술 방법의 진전, 치료방사선요법의 다양화 등 많은 분야에서 꾸준한 발전을 보이고 있다. 그리고 여러 가지 신약개발에 의한 제 1, 제 2상의 약제시험, antiangiogenesis 약제의 임상시험, gene therapy 등의 연구가 활발하게 진행되고 있으므로, 이에 따른 환자의 치료 성적도 향상될 것으로 기대하고 있다. 외국에서는 여러 대학이 함께 참여하여 작성한 공동의 치료 protocol에 의한 치료가 활발하게 진행되어 생존율을 높이는데 반해, 우리나라에서는 아직까지 각 기관의 협조체제가 구축되지 못한 형편이다. 하지만 금년에 처음으로 여러 대학에서 임상각과가 참석하는 한국소아 뇌종양연구회가 탄생되어 우리나라 뇌종양 환아들의 치료 protocol 개발에 기여할 것을 기대하고 있다. 뇌종양 치료 후 생존하는 아이들을 위한 정밀한 추적관찰이 필수사항인데 여기에 소아과 의사들의 역할은 클 것으로 사료된다. 지능장애, 부적절한 대인관계, 사회적응의 어려움, 치료 후 발생한 내분비계통의 후유증 그리고 2차 암의 발생 등의 정확한 진단 및 치료가 소아과 의사들의 몫으로 남는다. 또한, 이러한 장애가 있는 환아들이 사회에 잘 적응하도록 하기 위해서는 재활교육에 대한 사회적인 관심과 그에 적절한 지원제도의 확립이 절실히 요구된다. 이 점에 있어서도 소아과 의사들의 관심과 지원이 필요하다.

A Case of Patella Metastasis of Papillary Thyroid Carcinoma (갑상선 유두암의 슬개골 전이)

  • A 73-year-old man presented with a chief complaint of progressive left knee pain for two months. He had a history of total thyroidectomy and central lymph node dissection due to papillary thyroid carcinoma three months ago. MRI images revealed a solid mass in the left patella. A solid mass demonstrated low signal on T1 weighed image, and high signal on T2 weighed image. And whole body bone scan showed focal photon defect in same lesion of left patella. The histologic result of left knee lesion was adenocarcinoma, consistent with metastatic papillary thyroid carcinoma. Although patellar metastasis of papillary thyroid carcinoma is very rare, when knee pain and radiologic abnormality are noted, differential diagnosis of metastasis is necessary.

A Case of Multiple Cervico-Mediastinal Parathyroid Adenomatosis (다발성 경부종격동 부갑상샘 선종증 1예)

  • The most common cause of primary hyperparathyroidism is a single adenoma accounts for more than 85% and about 1-2% in multiple occurrence. The adenoma arises mainly in the neck and rarely in the mediastinum. The simultaneous occurrence is extremely rare. A 73-year-old man came to our clinic complained about sense of falling forward during last eight months. The brain MRI and vestibular function test showed non-specific findings but total calcium and intact parathyroid hormone levels were markedly elevated. Radiologic studies and sesta-MIBI scan revealed multiple masses in lower paratracheal area and superior mediastinum. We performed mass excision with transcervical approach and all of them were diagnosed as parathyroid adenoma. After surgery, intact PTH and calcium levels returned to the normal range and his symptoms were dramatically improved. We report the unique and rare disease entity with a brief literature review.

A clinical study on the prognostic analysis of stroke patients (중풍환자의 예후에 관한 임상적 고찰)

  • Objectives : This study was to investigate using FIM(functional independence measure) in 70patients with stroke and to evaluate prognosis. Methods : The subjects of this study were 70 patients who were admitted to the Dong-Eui oriental hospital within 7days after stroke. Improving rate were measured by using the FIM(at admission, after 2week and 4week). Each patients was diagnosed with sasang constitutional analysis(QSCC ll), Brain-CT, MRI scan and clinical observation. Results : There were significant results statistically between FIM(degree of improvement) and sex, age, side of hemiparesis, and hypertension. While diabetes mellitus, heart disease, hypercholesterolemia, and severity of early state were not significant meaning statistically. Conclusions : Sex, age, side of hemiparesis, and hypertension were relevant factors in predicting the functional outcome in the patients with stroke. I thought that the meaning of man-left and woman-right(男左女右) is not severity of early state but degree of improvement.

Synovial Chondromatosis in the Temporomandibular Joint: Report of Two Cases

  • Background Synocial chondromatosis(SC), a proliferative disorder of the synovial membrane. The etiology or cause of SC remains unclear. SC usually occurs in large articular joints such as knee, hip, elbow, and ankle. SC of the TMJ is very rare. It is a benign disease that mainly affects unilateral side. It can form cartilagenous and calcified loose bodies of various sizes and cause abnormal function of TMJ. Case Report In this paper, we report two cases of SC in the upper joint space of the left TMJ. One complained that "Sometimes the left jaw joint feels disoriented" and the other had no symptoms. CT scan and MRI showed left TMJ space widening, multiple tiny calcified mass. After clinical and radiographic analysis, we performed surgical removal of the lesion under genereal anesthesia. In the histologic examination, synovial chondromatosis was diagnosed in both patients. Conclusions We report two cases of synovial chondromatosis in the upper joint space of the left TMJ. We performed surgical removal of the lesion. The two patients showed good prognosis without recurrence or pain up to date.

A Heterotopic Ossification in the Subscapularis Muscle - Case Report - (견갑하근에 발생한 이소성 골화 -증례보고-)

  • We report the case of a heterotopic ossification in the subscapularis muscle. A 30-year-old woman was diagnosed as a heterotopic ossification in the right subscapularis muscle. X-rays, a CT scan and a MRI showed bone-like lesions in the muscle. The location of the ossification was both inferior and anterior to the joint. The mass nearly bridged between the proximal humerus and the inferior portion of the coracoid process. Symptoms did not respond to rest, NSAIDs for 3 months and to stretching exercises treatment for 6 months. Excision and biopsy was performed through deltopectoral approach. Disodium Etidronate was administered during the postoperative period. She regained normal range of motion postoperatively and improved in the shoulder pain after performing strengthening exercises with Therabands. We think that a cause of limitation of the shoulder was impingement between the mass and the coracoid process, and that another cause was adhesion between the mass and the joint capsule. We conclude that excision and postoperative rehabilitation exercises are good methods for a heterotopic ossification in the subscapularis muscle for those that are nonresponsive to nonoperative treatments.
