The Usefulness of $^{99m}Tc$-Sestamibi Scan in Preoperative Localization in Primary Hyperparathyroidism (원발성 부갑상선 기능 항진증의 수술전 병소확인에서 $^{99m}Tc$-Sestamibi 스캔의 유용성)

  • An accurate preoperative localization in patients with primary hyperparathyroidism is important for successful surgical intervention. There are many methods for the localization of the primary hyperparathyroidism such as ultrasonography, computerized tomography(CT), magnetic resonance imaging(MRI), angiography and $^{201}T1/^{99m}Tc$ subtraction scan. Among them $^{201}T1/^{99m}Tc$ subtraction scan is known as the most accurate tool for preoperative localization. Recently $^{99m}Tc$-Sestamibi has been used for parathyroid gland imaging. We experienced 19 primary hyperparathyroidisms accurately localized with $^{201}T1/^{99m}Tc$ subtraction imaging and $^{99m}Tc$-Sestamibi. The sensitivities of the $^{99m}Tc$-Sestanibi, $^{201}T1/^{99m}Tc$ subtraction imaging, CT and US were 100%(5/5), 89.5%(17/19) and 61.7(12/19) respectively. With the combination of $^{99m}Tc$-Sestamibi and $^{201}T1/^{99m}Tc$ subtraction imaging we could localize with 100% accuracy. Although the case number is small, this study suggests that the $^{99m}Tc$-Sestamibi parathyroid scan is very useful and easy to use for preoperative localization in primary hyperparathyroidism.

Real-Time fMRI-Guided Functional MR Spectroscopy: The Lactate Peaks at the Motor Cortex during Hand-Grasping Tasks (실시간 기능적 자기공명영상 유도 기능적 자기공명분광법: 손운동 수행중 운동피질의 젖산변화 관찰)

  • Purpose : There is debate concerning the observation of metabolite changes on MRS at the designated cortex during some tasks. The purpose of this study is to assess the change of the lactate content at the motor cortex during hand-grasping tasks with performing real-time fMRI-guided fMRS. Materials and Methods : Seven healthy volunteers (23-28 years old) underwent realtime fMRI during right hand grasping tasks with using a 1.5 T system. After confirming the activating area, single voxel MRS was preformed at 1) the baseline, 2) during the task and 3) after the task on the activating cortex. The three consecutive spectra were compared for observing the changes of the lactate content by the tasks. The Cho/Cr, NAA/Cr and Lac/Cr ratios were calculated manually from those spectra. Results : MRS during the tasks revealed the lactate peaks at the 1.33 ppm resonance frequency with great conspicuity at the activated area, which was identified on the real-time fMRI. After the task scan, the lactate peaks completely disappeared and the spectra recovered to the values of the baseline scan in all volunteers. At baseline, during the task and after the task, the Cho/Cr ratios were 0.81, 0.76 and 0.77, respectively, and the NAA/Cr ratios were 1.68, 1.65 and 1.72, respectively, and the Lac/Cr ratios were 0.28, 0.41 and 0.30, respectively. During the task, Lac was significantly increased by 46%. Conclusion : We observed prominent lactate peaks on MRS during hand-grasping tasks at the activated area, as was shown on the real-time fMRI. We suggest that fMRS can be used as a sensitive tool for observing the metabolite changes of the functioning brain.

Lateral Medullary Syndrome Caused by Prone Position for Spine Surgery

  • We report a rare case of Wallenberg's lateral medullary syndrome caused by prone position for spine surgery. A 48-year old man developed Wallenberg's syndrome characterized by involuntary myoclonic movements, ataxia on his left side, hyperalgia and cold sensation on his right side after prone position for general anesthesia for the spinal stenosis L3-L4, L4-L5. Brain computed tomography scan was immediately performed and showed negative findings, but magnetic resonance image [MRI] demonstrated brain infarction on the left medulla. Emergent heparinization was performed and his motor power and sensation returned to normal and discharged with stable and satisfactory recovery after 16 days.

  • In orthopedics, hip arthroplasty is the operation that replaces damaged hip joint to artificial joint. In hip arthroplasty, quite better result can be achieved if robot is applied to machine cavity in bone, especially when cementless stem is used. So several kinds of robots were introduced for hip arthroplasty, but they used MRI, CT Scan, vision analysis and real time tracking of bone position for registration of robot. To overcome shortage of conventional robot surgery, gauge based registration method was proposed and small robot was designed. In this method, small robot is mounted on femur, and its position is determined by gauge registration method. Operation procedure was performed on model femur and result was analyzed. This robotic hip surgery system is expected to more adaptable in operation room.

  • Wallenberg's syndrome is well known of dorsolateral medullary syndrome. A 56-year-old women was admitted because vertigo, vomitting, vertiginous ataxia, ipsilateral loss Rt. face and Lt. lower limb sense. The result of all performed laboratory tests were normal, but Sr-MRI scan was right lateral medullary infarction. And Br-MRA was focal stenosis of Lt. ICA. We present here one case of Wallenberg's syndrome, who was admitted at Kyungwon University Hospital From 25th Apr. to 21th May. 1998.

  • Artifacts can seriously degrade the quality of computed tomography (CT) images, sometimes to the point of making them diagnostically unusable. Here, we report an unusual CT artifact that could have resulted in the misdiagnosis of a hyperdense hemorrhagic lesion in a 55-year-old man. The author recommend that when hemorrhagic lesion in posterior fossa is suggested on CT, the physician should carefully consider all patient-related clinical data prior to considering surgical intervention or a biopsy. Cranial magnetic resonance imaging (MRI) can help in preventing the misdiagnosis as hemorrhage of CT scan.

  • A 49-year-old woman had thoracic back pain for several years. Chest CT scan and MRI angiography revealed descending thoracic aortic aneurysm with a maximum diameter of 69 mm. Thoracic aortography showed not only the aortic aneurysm, but also coarctation of descending thoracic aorta at the level of aortic hiatus of the diaphragm. Intercostal artery arising Adamkiewicz artery was found in descending thoracic aortic aneurysm just above the coarctation, The aneurysm with coarctation of the aorta was successfully repaired with prosthetic graft replacement under left atrio-femoral bypass.

  • Acute cyanide poisoning is usually the result of attempted suicide which is often lethal within minutes or leads to a very poor prognosis after delayed and inadequate treatment. It affects the cerebral structures with the highest oxygen requirement, such as the basal ganglia, the cerebral cortex. We experienced a-45-year-old man who ingested Potassium Cyanide. He was stuporous. In 25 minutes, respiratory arrest developed and cardiopulmonary resuscitation was done. After return of spontaneous circulation, he admitted to intensive care unit, and conservative treatment was started. The clinical status was improved by degrees, but he couldn't perform daily activity like before. Minimal limitation of movement and memory deficit were left. In magnetic resonance imaging, which taken at the 11th day after admission, there were both basal ganglia and folia of cerebellum abnormality.

Clinical Analysis of the Mediatinal Neurogenic Tumor -18 case report- (종격동에 빈발하는 신경종에 대한고찰 -18례 임상 경험-)

  • Neurogenic tumors of the mediastinum may have an intraspinal component connected by a narrowed segment of tumor in the intervertebral foramen, then have symptoms of back pain,lower extremity tingling sensation. CT scan or MRI demonstrated a Dumbbell-shaped mass density compressing spinal canal, enlargement of the foramen, erosion of bone, and intervertebral widening. We report the analysis of the 18 cases of neurogenic tumors on posterior mediastinum and Dumbbell type tumors are 3 cases among the 18 cases. The neurilemmomas were 12 cases[67%], the ganglioneuroma were 5 cases[28%], and neuroblastoma was one case[5%]. The successful removal was done in all cases, a standard thoracotomy and laminectomy was done in Dumbbell type tumors.There was no postoperative neurological complications.

  • In this paper , we propose a texture feature extraction method of reduce the massive computational time on extracting texture, features of large sized medical such as MRI, CT-scan , and an index structure, called GLTFT, to speed up the retrieval performance. For these, the original image is transformed into a compressed image by Wavelet transform , and textural features such as contrast, energy, entropy, and homogeneity of the compressed image is extracted by using GLCM(Gray Level Co-occurrence Metrix) . The proposed index structure is organized by using the textural features. The processing in compressed domain can give the solution of storage space and the reduction of computational time of feature extracting . And , by GLTFT index structure, image retrieval performance can be expected to be improved by reducing the retrieval range . Our experiment on 270 MRIs as image database shows that shows that such expectation can be got.
