의료수준의 향상과 더불어 환자들의 첨단의료장비에 대한 기대수준이 증가하고 있으며 특히 자기공명영상(Magnetic Resonance Image : MRI)은 현재 모든 임상 분야에서 가장 핵심적인 영상진단 도구로서 사용되고 있다. 그러나 검사 중에 발생하는 심각한 소음으로 많은 환자가 심리적인 불안을 경험한다고 한다. 이에 본 연구에서는 자기공명영상검사실의 기존 헤드셋 흡음재에서 차음재를 추가한 헤드셋의 소음저감평가와 차음재별 영상 아티팩트(artifact) 유무를 알아보고자 하였다. 3D 프린팅한 헤드셋 내부에 흡음재(스펀지)와 차음재(아크릴판, 구리판, 3D copper plate)를 교차 배열하여 MRI 검사소음을 녹음하여 스피커로 같은 dB 값의 소음을 발생시키며 3D 프린팅 된 두부모형의 내부에 소음측정기로 dB 값을 측정하여 정량분석을 하며 자체 제작한 헤드셋을 물팬텀에 밀착시킨 후 MRI영상 아티팩트 유무를 검사한다. 드셋의 정량평가를 한 결과, 헤드셋 평균 dB 값은 81.8 dB로 나타났으며, 차음재를 추가한 헤드셋에서 가장 방음효과가 뛰어난 재료조합(구리, 아크릴판, 스펀지, 스펀지) 헤드셋의 평균 dB 값은 70.4 dB 값이 측정되었지만 MRI 시뮬레이션 결과 구리가 반자성체이기 때문에 아티팩트가 나타나 배제하였고 두 번째로 방음효과가 뛰어난 (스펀지, 아크릴판, e-copper plate, 스펀지) 헤드셋의 평균 dB 값은 70.6 dB 값이 측정되었고 MRI 시뮬레이션 결과 인공물 나타나지 않았다. 구리분말이 약 40%가 포함된 e-copper PLA로 출력한 재료를 동일하게 시뮬레이션을 한 결과 인공물 나타나지 않았으므로 3D 프린팅 재료의 사용이 적합하였고 구리보다 경제성이 우수하며 가공이 용이하므로 적합한 재료로 선정하였다. MRI관련 연구에 있어 3D 프린팅을 이용한 상호발전이 매우 기대된다.
본 연구는 MRI 검사로 인하여 방사선사가 노출되는 소음의 양을 평가하여 소음저감 시설의 필요성과 제도 마련을 제안하고자 하였다. 소음측정은 대전광역시 S 종합병원의 1.5 Tesla MRI 장비(7개 검사)와 3.0 Tesla MRI 장비(16개 검사)를 대상으로 하였고, 소음측정기는 SC-804를 사용하였다. 소음측정 거리는 MRI 검사실 방음문에서 검사자의 업무 위치까지 100cm 이며, 측정 높이는 업무 시 검사자의 귀 높이 100cm 이다. 검사별 소음측정은 각 검사의 시퀀스(Sequence)마다 발생되는 소음 수치를 관측하여 20초마다 기록하였고 검사별 3회씩 측정하여 평균값을 제시하였다. 연구결과 방사선사가 노출되는 소음의 최댓값은 73.3 dB(A)로 3.0 Tesla 장비에서 시행한 MRCP 검사, 검사별 평균소음의 최댓값은 66.9(3.1) dB(A)로 역시 3.0 Tesla 장비에서 시행한 Myelogram 검사이다. 장비별 평균소음은 3.0 Tesla 장비가 61.9(4.1) dB(A), 1.5 Tesla 장비가 52.0(3.1) dB(A)로 3.0 Tesla MRI 장비가 약 10 dB(A) 정도 높았다(p<0.001). 방사선사가 노출되는 소음의 양은 청력에 영향을 미치는 수준은 아니지만 비청력적영향이 발생할 수 있는 수준이다. 소음을 저감하기 위해 MRI 조정실 후면에 커튼을 설치하여 반사음을 제거할 수 있지만, 제도 마련이 선행되어야 할 것이다.
최근 MRI와 CT와 같은 의료영상에서의 진보한 기술은 의사들에게 상세한 해부학적 정보를 제공하게 하여 그들의 진단 능력을 향상시키고 있다. 일반적으로 보다 상세한 정보를 얻기 위해서는 많은 수의 MRI이미지를 필요로 한다. 그러나 일반 병원에서 접하는 MRI 기계의 성능이 우수하지 않은 경우가 많고 따라서 획득되는 이미지의 수가 적다. 결과적으로 적은 수의 슬라이스를 이용해 3D surface를 재구성하게 되면 퀄리티가 낮아지는 문제가 발생한다. 본 논문에서는 적은 수의 슬라이스를 이용하여 높은 퀄리티의 3D surface를 얻는 방법을 제안한다. 이를 위한 알고리즘은 먼저 원하는 영역의 경계를 찾아서 그 경계선들의 점을 찾는다. 이러한 점들로부터 Radial Basis Function을 이용해서 점들을 모두 지나는 음함수 곡면 수식을 생성한다. 생성된 음함수 곡면수식으로부터 Marching cube 알고리즘을 이용하여 렌더링 한다.
Objective: To assess the accuracy and potential bias of computed tomography (CT) ventricular volumetry using semiautomatic three-dimensional (3D) threshold-based segmentation in repaired tetralogy of Fallot, and to compare them to those of two-dimensional (2D) magnetic resonance imaging (MRI). Materials and Methods: This retrospective study evaluated 32 patients with repaired tetralogy of Fallot who had undergone both cardiac CT and MRI within 3 years. For ventricular volumetry, semiautomatic 3D threshold-based segmentation was used in CT, while a manual simplified contouring 2D method was used in MRI. The indexed ventricular volumes were compared between CT and MRI. The indexed ventricular stroke volumes were compared with the indexed arterial stroke volumes measured using phase-contrast MRI. The mean differences and degrees of agreement in the indexed ventricular and stroke volumes were evaluated using Bland-Altman analysis. Results: The indexed end-systolic (ES) volumes showed no significant difference between CT and MRI (p > 0.05), while the indexed end-diastolic (ED) volumes were significantly larger on CT than on MRI (93.6 ± 17.5 mL/m2 vs. 87.3 ± 15.5 mL/m2 for the left ventricle [p < 0.001] and 177.2 ± 39.5 mL/m2 vs. 161.7 ± 33.1 mL/m2 for the right ventricle [p < 0.001], respectively). The mean differences between CT and MRI were smaller for the indexed ES volumes (2.0-2.5 mL/m2) than for the indexed ED volumes (6.3-15.5 mL/m2). CT overestimated the stroke volumes by 14-16%. With phase-contrast MRI as a reference, CT (7.2-14.3 mL/m2) showed greater mean differences in the indexed stroke volumes than did MRI (0.8-3.3 mL/m2; p < 0.005). Conclusion: Compared to 2D MRI, CT ventricular volumetry using semiautomatic 3D threshold-based segmentation provides comparable ES volumes, but overestimates the ED and stroke volumes in patients with repaired tetralogy of Fallot.
In this paper, we propose a method to fabricate a patient-specific breast implant using MRI images and 3D scan data. Existing breast implants for breast reconstruction surgery are primarily fabricated products for shaping, and among the limited types of implants, products similar to the patient's breast have been used. In fact, the larger the difference between the shape of the breast and the implant, the more frequent the postoperative side effects and the lower the satisfaction. Previous researches on the fabrication of patient-specific breast implants have used limited information based on only MRI images or on only 3D scan data. In this paper, we propose an algorithm for the fabrication of patient-specific breast implants that combines MRI images with 3D scan data, considering anatomical suitability for external shape, volume, and pectoral muscle. Experimental results show that we can produce precise breast implants using the proposed algorithm.
This paper describes the procedure of implementing an articulatory speech simulator, in order to model the human articulatory organs and to synthesize speech from this model after. Images required to construct the vocal tract model were obtained from MRI, they were then used to construct 2D and 3D vocal tract shapes. In this paper 3D vocal tract shapes were constructed by spatially concatenating and interpolating sectional MRI images. 2D vocal tract shapes were constructed and analyzed automatically into a digital filter model. Following this speech sounds corresponding to the model were then synthesized from the filter. All procedures in this study were using MATLAB.
목적 : 기존의 시각 자극과 관련한 뇌기능 연구가 일반적인 이차원 시각 자극을 이용한 데 반하여 본 논문에서는 삼차원 스트레오스코픽 비젼에 의한 시각 자극을 이용한 뇌기능 영상 연구를 수행하였다. 본 연구를 통하여 삼차원 시각처리와 관련한 뇌의 기능 영역 및 특징을 이차원 시각 자극 결과와 비교 검토하였다. 대상 및 방법 Anaglyph(이차원 그림을 칼라 코딩한 후 왼쪽과 오른쪽에 서로 다른 색깔의 렌즈를 사용하여 스트레오스코픽 비젼을 구현)를 이용하여 삼차원 시각 자극을 주었으며, 이와 비교를 위하여 동일한 영상으로 이차원 시각 자극을 구현하였다. 건강한 volunteer들에 대하여 삼차원 및 이차원 시각 자극에 대한 뇌기능 영상을 3.0 Tesla MRI 시스템에서 얻었다. 결과 : 삼차원 시각 자극에 의하여 활성화된 뇌의 영역은 이차원 시각 자극과 동일하게 후두엽으로 나타났으며, 삼차원 시각 자극에 의한 활성영역이 이차원 시각 자극에 의한 영역 보다 약 $18\%$ 더 넓게 나타났다. 결론 : 사람은 양 눈을 사용하여 삼차원 물체를 인식한다. 삼차원 물체 인식은 스테레오스코픽 비젼에 기인하는데, anaglyph 를 이용하여 삼차원 시각 자극에 대한 뇌기능 영상을 3.0 Tesla MRI 시스템에서 수행하였다. 삼차원 시각 자극에 의하여 활성화된 뇌의 영역은 이차원 시각 자극과 동일하게 후두엽으로 나타났으나, 활성영역이 이차원 자극에 비하여 약 $18\%$ 더 넓게 나타났다. 이것은 삼차원 영상 처리가 이차원 영상 처리에 비하여 왼쪽 영상과 오른쪽 영상의 차이에서 거리 정보를 추출하는 삼차원 시각 처리 기능이 추가됨에 따라 뇌의 처리 영역이 넓어진 것으로 판단된다. 이러한 삼차원 시각 자극 뇌기능 영상은 가상현실이나 3-D 디스플레이, 3-D 멀티미디어 콘텐츠 연구 등 다양한 분야에 활용될 수 있을 것으로 기대한다.
턱관절 디스크 검사 시 MRI는 유용한 검사법으로 디스크의 정확한 위치변화를 관찰하고자 개구상태에서 오랜 시간 검사가 필요하다. 이에 3D프린팅 기술을 이용하여 개구상태를 유지시켜 줄 고정 장치(TMJ device)를 제작하고 이를 기존 고정 장치와 비교하여 유용성을 평가하고자 하였다. 기존 TMJ device를 사용한 영상에 비해 3D 프린터로 자체 제작한 TMJ device를 사용한 영상에서 SNR이 다소 낮은 결과를 보였지만 임상적으로 사용하는데 큰 결함이 없고 환자 개인별 특화된 맞춤형 제작이 가능하고 더불어 3D 프린팅 기술을 활용하여 유사도구 제작에 기여함으로서 비용대비 편익이 상승될 것이라 사료된다.

Objective: The intra-parotid facial nerve (FN) can be visualized using three-dimensional double-echo steady-state water-excitation sequence magnetic resonance imaging (3D-DESS-WE-MRI). However, the clinical impact of FN imaging using 3D-DESS-WE-MRI before parotidectomy has not yet been explored. We compared the clinical outcomes of parotidectomy in patients with and without preoperative 3D-DESS-WE-MRI. Materials and Methods: This prospective, non-randomized, single-institution study included 296 adult patients who underwent parotidectomy for parotid tumors, excluding superficial and mobile tumors. Preoperative evaluation with 3D-DESS-WE-MRI was performed in 122 patients, and not performed in 174 patients. FN visibility and tumor location relative to FN on 3D-DESS-WE-MRI were evaluated in 120 patients. Rates of FN palsy (FNP) and operation times were compared between patients with and without 3D-DESS-WE-MRI; propensity score matching (PSM) and inverse probability of treatment weighting (IPTW) were used to adjust for surgical and tumor factors. Results: The main trunk, temporofacial branch, and cervicofacial branch of the intra-parotid FN were identified using 3D-DESS-WE-MRI in approximately 97.5% (117/120), 44.2% (53/120), and 25.0% (30/120) of cases, respectively. The tumor location relative to FN, as assessed on magnetic resonance imaging, concurred with surgical findings in 90.8% (109/120) of cases. Rates of temporary and permanent FNP did not vary between patients with and without 3D-DESS-WE-MRI according to PSM (odds ratio, 2.29 [95% confidence interval {CI} 0.64-8.25] and 2.02 [95% CI: 0.32-12.90], respectively) and IPTW (odds ratio, 1.76 [95% CI: 0.19-16.75] and 1.94 [95% CI: 0.20-18.49], respectively). Conversely, operation time for surgical identification of FN was significantly shorter with 3D-DESS-WE-MRI (median, 25 vs. 35 min for PSM and 25 vs. 30 min for IPTW, P < 0.001). Conclusion: Preoperative FN imaging with 3D-DESS-WE-MRI facilitated anatomical identification of FN and its relationship to the tumor during parotidectomy. This modality reduced operation time for FN identification, but did not significantly affect postoperative FNP rates.
The hippocampal volume atrophy is known to be linked with neuro-degenerative disorders and it is also one of the most important early biomarkers for Alzheimer's disease detection. The measurements of hippocampal pure volumes from Magnetic Resonance Imaging (MRI) is a crucial task and state-of-the-art methods require a large amount of time. In addition, the structural brain development is investigated using MRI data, where brain morphometry (e.g. cortical thickness, volume, surface area etc.) study is one of the significant parts of the analysis. In this study, we have proposed a patch-based ensemble model of 3-D convolutional neural network (CNN) to measure the hippocampal pure volume from MRI data. The 3-D patches were extracted from the volumetric MRI scans to train the proposed 3-D CNN models. The trained models are used to construct the ensemble 3-D CNN model and the aggregated model predicts the pure volume in one-step in the test phase. Our approach takes only 5 seconds to estimate the volumes from an MRI scan. The average errors for the proposed ensemble 3-D CNN model are 11.7±8.8 (error%±STD) and 12.5±12.8 (error%±STD) for the left and right hippocampi of 65 test MRI scans, respectively. The quantitative study on the predicted volumes over the ground truth volumes shows that the proposed approach can be used as a proxy.
