  • Postoperative imaging of the rotator cuff may be performed routinely, even if pain or disability develops after surgery or if there are no symptoms. Postoperative images are obtained through MRI or US, and the purpose is to confirm the integrity of the restored tendon in general. Postoperative MRI has a relatively poor diagnostic accuracy compared to that of preoperative images because various materials used in surgeries deteriorate the image quality. US can dynamically check the condition of the restored tendon and avoid artifacts from the surgical instruments used for recovery. Although imaging findings are not always consistent with the clinical symptoms or prognosis, sub-deltoid fluid retention is more important for pain and functional recovery than the thickness of the reconstructed tendon. Strain elastography can also be a useful method for predicting the prognosis.

  • The acute lumbar pain patients who were unavoidable to take MRI examination were made to take altered lateral recumbent position. they were also not able to control their painfulness and to be in supine position. In this study, it is supposed to increase success rate of the MRI examination through taking the altered lateral recumbent position in using spine and body matrix coil. This altered position made relaxing lumber pain and fatigue for the patients who suffered from sever lumber pain. In these reasons, it decreases the motion artifacts through correcting uncomfortable posture. As a result, qualitative analysis for the image quality was estimated to have average points of supine position A group(lateral recumbent position) of normal candidates and B group(lateral recumbent position) of the abnormal candidates who have sever pain at 4.64, 3.44, and 3.40, respectively. In conclusion, while qualitative analysis in the examination with supine position of the normal patients had significantly high points, the qualitative analysis in the examination with lateral recumbent position of the normal patients and abnormal patients who had sever lumbar pain was almost same. In addition, it was judged that radiologists' imagery interpretation had no other problems in the image evaluation of B group who had acute lumbar pain of abnormal patients. Hereupon, if this technique becomes generalized for the patients who suffer from supine position in the examination, it is supposed to be useful in medical field.

A Study on Optimized MRI Fat-Saturation Technique for Brachial Plexus Patients : Focused on SPAIR and STIR Fat-Saturation (상완신경총 환자에서 최적의 자기공명영상 지방소거 기법에 관한 연구 : SPAIR, STIR 지방소거 중심으로)

  • The purpose of this study is to know optimized fat suppression techniques for brachial plexus compared with STIR and SPAIR T1, T2 techniques. A total of 30 normal volunteers without brachial plexus disease were studied on a 3.0 T MRI scanner. As an analytical method, SNR, CNR, 4-point grading scale were evaluated by using three pulse sequences. As a quantitative analysis, the SNR, CNR for SPAIR T1 technique provided high value in branchial plexus roots (03.07, -2.25), branchial plexus trunks(06.70, 36.31)(p<0.05). As a qualitative Analysis, The visibility for delineation of brachial plexus, fat suppression, artifact was significantly better on SPAIR T1(3.2, 3.6, 3.4) technique(p<0.05).

Application of the Orally Inserted Guiding Device for the Improved Motion Artifacts of the Cervical Spine MRI (경추부 자기공명영상의 움직임 인공물 개선을 위한 구강내 삽입 유도 기구의 적용)

  • This study aims to suggest and test methods using an orally inserted guiding device in order to improve a motion artifact by involuntary oral motor such as removing one's dentures and swallowing saliva clinically structured cervical spine scan and to make the optimal image by minimizing motion artifact. A cervical spine test was conducted with 30 patients who wore dentures among those who had a cervical spinal disease from January 1, 2014 through June 30, 2014. As for testing methods, after removing denture, T1-TSE-Sagittal, T2-TSE-Sagittal, T1-TSE-Axial and T2-TSE-Axial were obtained in a normal position and a supine position; the orally inserted guiding device was inserted in patients' mouth; and then T1-TSE-Axial and T2-TSE-Axial were retested. As a result, in SNR, T1-TSE-Axial before inserting an orally inserted guiding device was $22.33{\pm}8.59$; T1-TSE-Axial after inserting the orally inserted guiding device was $25.21{\pm}7.93$; T2-TSE-Axial before inserting the orally inserted guiding device was $14.49{\pm}5.74$; and T2-TSE-Axial after inserting the orally inserted guiding device was $16.61{\pm}6.72$. In CNR, T1-TSE-Axial was measured at $0.23{\pm}0.01$ while T2-TSE-Axial at $0.21{\pm}0.01$. As a result of the qualitative analysis, T1-TSE-Axial before inserting the orally inserted guiding device was $3.49{\pm}0.11$; T1-TSE-Axial after inserting the orally inserted guiding device was $3.95{\pm}0.14$; T2-TSE-Axial before inserting the orally inserted guiding device was $3.25{\pm}0.18$; and T2-TSE-Axial after inserting the orally inserted guiding device was $3.68{\pm}0.09$. As a result of using an orally inserted guiding device, the resolution and contrast of the images improved as the patients' involuntary artifact decreased because of removing dentures and swallowing saliva, and it was found that the interpretation of the images and identification of the diseases improved.

Analysis of the Dental Implants MRI Artifacts by Using the ACR Phantom (ACR 팬텀을 이용한 치아 임플란트 자기공명영상 인공물 분석)

  • ACR phantom for quality control of magnetic resonance imaging equipment can evaluate magnetic resonance imaging picture quality through various structures within the phantom. In this study, percent Signal Ghosting and Slice position accuracy of imaging could be analyzed by attaching implant and the wire for correction of tooth using ACR phantom in Head coil of 3.0T equipment. In the T1 weighted imaging of the first slice and the eleventh slice of implant, the slice position accuracy appeared to be good in ingress bandwidth 300, and it was good in ingress bandwidth 130 when wire for correction was attached. Percent Signal Ghosting in the seventh slice of SE T1 weighted imaging, implant and wire for correction added all appeared to be good when ingress bandwidth was 230. It is thought that in case of implant dental prosthesis patients in brain exam using magnetic resonance imaging, optimum image can be obtained by changing ingress bandwidth.

  • Atypical teratoid rhabdoidal tumor(AT/RT) is a rare and fast-growing tumor usually diagnosed in childhood. Routine magnetic resonance imaging(MRI) assessment of AT/RT patients is done to detect metastatic tumors and recurrent lesions. The purpose of this case report is to present a case about caries treatment of a 3-year-8-month old female patient with AT/RT under general anesthesia. A 3-year-8-month old patient with AT/RT visited Seoul National University Dental Hospital for caries treatment. At the first dental visit, she was receiving chemotherapy. Multiple caries were observed from clinical and radiographic examination. The dental procedure was successfully performed under general anesthesia. Patients with brain tumor may take routine MRI to detect metastatic tumors and recurrent lesions. Dentists should take into consideration of possible dislodgement of restoration by ferromagnetic properties and artifact formation on MRI image. Ready-made zirconia crowns can be a good restoration option for such patients.

Neural Substrates of Picture Encoding: An fMRI Study (그림의 부호화 과정과 신경기제 : fMRI 연구)

  • This study is to examine brain regions that are involved in picture encoding in normal adults using fMRI methods. In Scan 1, the picture encoding was studied during a semantic categorization task in comparison with word. In Scan 2 task type effects were studied both during a picture naming task and during a semantic categorization task with pictures. Subjects were asked to make decision either by pressing a mouse button (Scan 1) or by responding subvocally (naming or saying yes/no) (Scan 2). Regardless of stimulus type, left prefrontal, bilateral occipital, and parietal activations were observed during semantic processing in comparison with fixation baseline. Processing of word stimulus relative to picture resulted in activations in prefrontal and parieto-temporal regions in the left side while that of picture stimulus relative to word resultd in activations in bilateral extrastriatal visual cortices and parahippocampal regions. In spite of the same task demands, stimulus-specific information processings were involved and mediated by different neural substrates; the word encoding was associated with more semantic/lexical processings than pictures and the picture processing associated with more perceptual and novelty related information processings than word. Activations of dorsal part of inferior prefrontal region, i.e., Broca's areas were found both during the picture naming and during the semantic tasks subvocally performed Especially, during the picture naming task, greater occipital activations were found bilaterally relative to the semantic categorization task. indicating a possibility that greater and higher visual processing was involved in retrieving the name referred by picture stimuli.

  • This study compares the TE and UTE is to evaluate. We was programming by DWT of Matlab Tool-box for evaluation. M-program used feature value extract between TE Images and UTE Images. Two images using the extracted feature values were compared. Comparison of similar features two images phase was found to have value.

Accurate Localization of Metal Electrodes Using Magnetic Resonance Imaging (자기공명영상을 이용한 금속전극의 정확한 위치 결정)

  • Purpose : Localization using MRI is difficult due to susceptibility induced artifacts caused by metal electrodes. Here we took an advantage of the B0 pattern induced by the metal electrodes by using an oblique-view imaging method. Materials and Methods : Metal electrode models with various diameters and susceptibilities were simulated to understand the aspect of field distortion. We set localization criteria for a turbo spin-echo (TSE) sequence usingconventional ($90^{\circ}$ view) and $45^{\circ}$ oblique-view imaging method through simulation of images with various resolutions and validated the criteria usingphantom images acquired by a 3.0T clinical MRI system. For a gradient-refocused echo (GRE) sequence, which is relatively more sensitive to field inhomogeneity, we used phase images to find the center of electrode. Results : There was least field inhomogeneity along the $45^{\circ}$ line that penetrated the center of the electrode. Therefore, our criteria for the TSE sequence with $45^{\circ}$ oblique-view was coincided regardless of susceptibility. And with $45^{\circ}$ oblique-view angle images, pixel shifts were bidirectional so we can detect the location of electrodes even in low resolution. For the GRE sequence, the $45^{\circ}$ oblique-view anglemethod madethe lines where field polarity changes become coincident to the Cartesian grid so the localization of the center coordinates was more facilitated. Conclusion : We suggested the method for accurate localization of electrode using $45^{\circ}$ oblique-view angle imaging. It is expected to be a novelmethodto monitoring an electrophysiological brain study and brain neurosurgery.

Ferucarbotran-Enhanced Hepatic MRI at 3T Unit: Quantitative and Qualitative Comparison of Fast Breath-hold Imaging Sequences (간의 3T 자기공명영상에서 초상자성산화철 조영증강 급속호흡정지영상기법들간의 양적 및 질적 비교평가)

  • Purpose : To compare the relative values of various fast breath-hold imaging sequences for superparamagnetic iron-oxide (SPIO)-enhanced hepatic MRI for the assessment of solid focal lesions with a 3T MRI unit. Materials and Methods : 102 consecutive patients with one or more solid malignant hepatic lesions were evaluated by spoiled gradient echo (GRE) sequences with three different echo times (2.4 msec [GRE_2.4], 5.8 msec [GRE_5.8], and 10 msec [GRE_10]) for $T2^*$-weighted imaging in addition to T2-weighted turbo spin echo (TSE) sequence following intravenous SPIO injection. Image qualities of the hepatic contour, vascular landmarks and artifacts were rated by two independent readers using a four-point scale. For quantitative analysis, contrast-to-noise ratio (CNR) was measured in 170 solid focal lesions larger than 1 cm (107 hepatocellular carcinomas, nine cholangiocarcinomas and 54 metastases). Results : GRE_5.8 showed the highest mean points for hepatic contour, vascular anatomy and imaging artifact presence among all of the subjected sequences (p<0.001) and was comparable (p=0.414) with GRE_10 with regard to lesion conspicuity. The mean CNRs were significantly higher (p<0.001) in the following order: GRE_10 ($24.4{\pm}14.5$), GRE_5.8 ($14.8{\pm}9.4$), TSE ($9.7{\pm}6.3$), and GRE_2.4 ($7.9{\pm}6.4$). The mean CNRs of CCCs and metastases were higher than those of HCCs for all imaging sequences (p<0.05). Conclusion : Regarding overall performances, GRE using a moderate echo time of 5.8 msec can provide the most reliable data among the various fast breath-hold SPIO-enhanced hepatic MRI sequences at 3T unit despite the lower CNR of GRE_5.8 compared to that of GRE_10.
