Improved Diagnostic Accuracy in Characterization of Adnexal Masses by Detection of Choline Peak Using 1H MR Spectroscopy in Comparison to Internal Reference at 3 Tesla

  • Background: The aim of this study was to assess the role of the presence of a choline peak in 3 Tesla 1H magnetic resonance spectroscopy (MRS) for differentiating benign from malignant adnexal masses. Materials and Methods: A total of 46 adnexal masses (23 malignant and 23 benign) underwent 1H MRS study prior to surgery to assess the presence of choline peak. Results: A choline peak was detected in 16 malignant masses (69.5%) and was absent in the other 7 (30.5%). A choline peak was only detected in 6 (26%) of the benign adnexal masses. The presence of an MRS choline peak had a sensitivity of 69.5%, a specificity of 74%, a positive predictive value (PPV) of 72.7%, and a negative predictive value (NPV) of 71% for diagnosing malignant adnexal masses. A significant difference between the frequency of mean choline peaks in benign and malignant adnexal masses was observed (P value < 0.01). Conclusions: A 1H MRS choline peak is seen in malignant adnexal masses more frequently than the benign masses, and may be helpful for diagnosing malignant adnexal masses.

Quantification of Metabolic Alterations of Dorsolateral Pre-Frontal Cortex in Depression SD Rat by MR Spectroscopy

  • Purpose: Contrary to the human study, it has rarely investigated metabolic alterations in the dorsolateral prefrontal cortex (DLPFC) of depressed rats versus age and sex-matched controls using proton magnetic resonance spectroscopy (MRS). Thus, the purpose of this research was to verify the feasibility of metabolic differences between the normal rat and the depression model rat. Materials and Methods: A homogeneous group of 20 SD male rats was used for MRI and in vivo 1H MRS. To induce a depressed status in SD rats, we performed the forced swimming test (FST). Using image-guide, water suppressed in vivo 1H MRS with 4.7 T MRI/MRS system, NAA/Cr and Cho/Cr ratios were mainly measured between depressed rats and normal subjects. Results: In depressed rats, increased Cho/Cr ratio was measured versus control subjects. However, no significant group effect for NAA/Cr was observed between case-control pairs. Discussion and Conclusions: The present 1H MRS study shows significant brain metabolic alterations of dorsolateral prefrontal cortex with experimental depressed status of SD rat induced by FST compared to normal subjects. This result provides new evidence that in vivo 1 H MRS may be a useful modality for detecting localized functional neurochemical markers alterations in left DLPFC in SD rats.

  • Purpose: We Investigated to achieve high resolution magnetic resonance (MR) Imaging and spectra of human skin in vitro with using a 14.1 T MRI/MRS system, and to evaluate the hydration effect of a moisturizer by measuring the skin's water concentration. Materials and Methods: We used the Brukrer 14.1 T MRI/MRS system with a vertical standard bore that was equipped with a DMX spectrometer gradient system (200 G/cm at a maximum 40 A), RF resonators (2, 5 and 10 mm) and Para Vision software. Spin echo and fast spin echo pulse sequences were employed for obtaining the high resolution MR images. The 3D-localized point resolved spectroscopy (PRESS) method was used to acquire the MR spectra. Results: The high resolution MR images and spectra of human skin in vitro were successfully obtained on a 14.IT system. The water concentration of human skin after applying a moisturizer was higher than that before applying a moisturizer. Conclusions: The present study demonstrated that the high-resolution MR images and spectra of human skin from a high field MRS instrument could be applicable to evaluating the hydration state of the stratum corneum.

Sensitivity Assessment of Spiral RF Surface Coils for MR Microscopic Imaging and Spectroscopy (자기공명미세영상 및 분광법을 위한 나선형 RF 표면코일의 감는 횟수에 따른 민감도 평가)

  • The purpose of this study was to improve the technique so as to develop an advanced sensitive RF surface coil for investigating the sensitivities of the multi-spiral surface coils, and we eventually wanted to achieve high resolution of the microscopic MR images and MR spectra. The magnetic field inhomogeneity and shape of a surface coil were statistically estimated by simulation of the magnetic field distribution. On the basis of the experimental results with single, 3 and S-turned spiral RF surface coils, we found that the 3-turned coil had the highest sensitivity. The present study showed that the sensitivity of the RF surface coil was improved by increasing the number of spiral coil turns, and also the SNR of the RF surface coil was dependent upon the number of spiral coil turns. However, we found, rather strikingly, that the sensitivity of excessive turns of the coils was decreased due to the rise of the coil's Impedance. Thus, the present results demonstrated that the sensitivity was not proportional to the number of a spiral RF coil's turns, and the number of spiral coil turns should be optimized for obtaining the highest sensitivity and SNR.

  • In pre-surgical evaluation of pediatric epilepsy, the combined use of multiple imaging modalities for precise localization of the epileptogenic focus is a worthwhile endeavor. Advanced neuroimaging by high field Magnetic resonance imaging (MRI), diffusion tensor images, and MR spectroscopy have the potential to identify subtle lesions. $^{18}F$-FDG positron emission tomography and single photon emission tomography provide visualization of metabolic alterations of the brain in the ictal and interictal states. These techniques may have localizing value for patients which exhibit normal MRI scans. Functional MRI is helpful for non-invasively identifying areas of eloquent cortex. These advances are improving our ability to noninvasively detect epileptogenic foci which have gone undetected in the past and whose accurate localization is crucial for a favorable outcome following surgical resection.

Polycarprolacton으로 표면 수식된 나노 산화철 조영제의 합성에 관한 연구

  • 목적: 나노 산화철 입자 및 생고분자인 PCL(Polycarprolacton)로 표면 코팅한 T2 MR 조영제를 합성하고 동물 종양 모델을 이용하여 In vivo 특성을 조사하고자 하였다. 대상 및 방법: $FeC1_2$.$4H_2$O와 $FeC1_3$.$6H_2$O을 무게비를 1:2로 정량하여 첨가하고 NaOH 혹은 TMAOH로 pH를 조절한후 PCL를 첨가하여 magnetite가 생성되는 동시에 고분자로서 코팅을 한후 증류수로 여러번 씻어준다. TEM, SEM, DLS 및 IR spectroscopy와 SQUID등을 측정하여 최종 반응물의 입자크기, 자성, 코팅 상태등을 평가하였다. 최소의 입자크기를 형성하는데 필요한 실험 조건을 찾기 위해 반응온도, 코팅할 고분자의 함량, 교반속도별로 실험하여서 최적의 조건을 찾으려 하였다. 토끼의 간에 VX2 암종을 이식한 동물 모델을 이용하여 PCL로 표면 코팅된 나노자성체의 in vivo 영상 특성을 알아보았다.

3.0T 1H MR Spectroscopy를 위한 기존 STEAM 펄스시퀀스의 최적화

  • 목적: Phase cycling과 fat suppression을 기존 STEAM 펄스시퀀스에 추가 및 최적화함으로써 3.0T 1H MRS 스펙트럼의 quality를 향상시키는 데 그 목적이 있다. 대상 및 방법: 가톨릭의대에 설치된 3.0T MRI를 이용하여 STEAM 펄스시퀀스를 최적화하였다. 우선 CHESS 펄스시퀀스를 이용하여 물 신호를 억제하고, 대사물질의 신호에 영향을 주는 fat 신호를 제거하도록 펄스시퀀스 최적화를 시험하였다. 또한 Phase cycling을 이용하여 Noise 신호를 제거하였다. 스펙트럼을 분석하기 위하여 본 사에서 개발한 MaqSpec 프로그램을 이용하였고, Post-processing을 한 후 펄스시퀀스 변경 전후의 MRS 스펙트럼을 비교하였다.

Quantitative Evaluation of Hepatic Steatosis Using Advanced Imaging Techniques: Focusing on New Quantitative Ultrasound Techniques

  • Nonalcoholic fatty liver disease, characterized by excessive accumulation of fat in the liver, is the most common chronic liver disease worldwide. The current standard for the detection of hepatic steatosis is liver biopsy; however, it is limited by invasiveness and sampling errors. Accordingly, MR spectroscopy and proton density fat fraction obtained with MRI have been accepted as non-invasive modalities for quantifying hepatic steatosis. Recently, various quantitative ultrasonography techniques have been developed and validated for the quantification of hepatic steatosis. These techniques measure various acoustic parameters, including attenuation coefficient, backscatter coefficient and speckle statistics, speed of sound, and shear wave elastography metrics. In this article, we introduce several representative quantitative ultrasonography techniques and their diagnostic value for the detection of hepatic steatosis.
