상급종합병원 임상실습 학생의 메르스 예방행위 수행 예측요인 (Predictors of MERS-related Preventive Behaviors Performance among Clinical Practice Students in a Tertiary Hospital)

  • 본 연구는 2015년 국내 메르스(중동호흡기증후군) 유행시 상급종합병원에 임상실습을 경험하는 학생의 메르스에 대한 지식, 태도 및 예방행위 수행 정도를 파악하고 메르스 예방행위 수행 예측요인을 확인하기 위해서 시도되었다. 본 연구 대상자는 3, 4학년 간호학과와 의과대학 학생 480명이었으며, 자료수집은 2015년 6월 9일부터 6월 30일까지 구조화된 설문지를 이용하였다. 자료분석은 SPSS/WIN 24.0을 이용하여 분석하였고, 메르스 예방행위 수행에 영향을 미치는 예측요인들은 위계적 회귀분석(hierachial regression)을 이용하였다. 임상실습 학생의 메르스에 대한 지식 수준은 9.56점(13점 기준)으로 보통 이상으로 높았으며, 메르스의 심각성 인지와 예방가능성에 대한 신념인 메르스에 대한 태도는 4.15점(5점 기준)으로 긍정적이었다. 메르스 예방행위 수행정도는 3.02점(5점 기준)으로 보통 수준이었다. 메르스 예방행위는 여학생인 경우, 메르스에 관한 교육 경험이 있는 경우, 지난 해 인플루엔자 접종을 한 경우, 올해 인플루엔자 접종 의도가 있는 경우, 메르스 감염에 대한 두려움이 높은 경우, 메르스에 대한 지식 수준이 높고, 태도가 긍정적일수록 수행 정도가 높게 나타났다. 이에 감염환자들을 치료하고 돌보게 될 간호학과와 의과대학 임상실습 학생의 메르스 예방행위 수행률을 높이기 위해서는 메르스 예방행위 수행에 영향을 미칠 수 있는 임상실습 학생의 특성을 고려해야 한다. 더 나아가 임상실습 학생들이 메르스를 예방하기 위하여 필수적으로 습득해야 하는 내용이 포함된 효과적인 감염예방 교육프로그램을 개발하고 실시하여 미래 보건의료인들의 메르스 관련 지식 수준을 높이고, 질환 전파를 막으려는 태도를 강화함으로써 전염성 감염질환 예방에 기여해야 한다.

메르스 환자 간호에 대한 간호사의 경험 (Nurses' Experience of Middle East Respiratory Syndrome Patients Care)

  • 본 연구의 목적은 메르스 환자 간호에 대한 간호사의 실제적인 경험을 파악하기 위함이다. 본 연구에 참여한 대상자는 S시 1개 상급종합병원에서 메르스 환자 간호에 참여한 간호사 9명이며, 자료 수집을 위해 심층 면담이 수행되었다. 자료수집 기간은 2015년 8월부터 2016년 3월까지였다. 수집된 자료는 Colaizzi의 현상학적 방법에 따라 절차에 맞게 분석하였다. 도출된 5개의 범주는 다음과 같다. '1. 신종 감염병 전염 위험성에 따른 불안과 부담', '2. 메르스 환자 간호사라는 이유로 느끼는 사회적 고립', '3. 준비되지 않은 치료환경 문제에 부딪힘', '4. 힘겨운 메르스 환자 간호과정을 이겨냄', '5. 신종 감염병 대책 마련에 대해 성찰함'이었다. 이와 같은 결과는 메르스 환자를 포함한 다양한 감염병 환자를 담당하는 간호사의 부정적 영향 뿐 아니라, 긍정적인 경험을 도출했다는데 의의가 있다. 이를 바탕으로 심리사회적 부정적 영향을 감소시키고, 긍정적인 적응을 향상시키는데 근거로 활용될 수 있을 것이며, 간호사를 포함한 보건의료 종사자가 신종 감염병 환자를 간호하기 위한 인력, 부서 및 실무지침 마련이 필요하다고 생각된다.

간호대학생의 중동 호흡기 증후군 예방을 위한 손 씻기 수행 빈도에 영향을 미치는 요인 (Hand washing and Preventive Measures for Middle East Respiratory Syndrome Coronavirus)

  • 최근 중동 호흡기 증후군의 발현으로 손 씻기가 강조되고 있는 실정이다. 본 연구의 목적은 중동호흡기 증후군의 예방을 위하여 손 씻기의 빈도에 영향을 미치는 요인을 파악하기 위하여 기질불안, 건강통제위, 건강증진행위의 관계를 분석하였다. 314명의 간호 대학생들에게 중동 호흡기 증후군을 예방하는 방법, 손 씻는 횟수, 기질불안, 건강통제위, 건강증진행위에 대하여 설문조사하였고, 수집된 자료는 Pearson correlation, 다중회귀분석으로 분석하였다. 손 씻기 수행 빈도에 영향을 미치는 요인으로는 손 씻기 중요도 이였으며, 손 씻기 중요도에 영향을 미치는 요인은 내적통제위로 나타났다. 또한, 대부분의 간호 대학생이 손 씻기가 중동 호흡기 증후군의 가장 중요한 예방법으로 인지하였다. 이에 손 씻기의 중요성과 내적 통제위를 강화한 건강교육을 진행해야 할 것이다.

How Did South Korean Governments Respond during 2015 MERS Outbreak?: Application of the Adaptive Governance Framework

  • This study examines how South Korean governments responded to the outbreak of Middle East Respiratory Syndrome Coronavirus (MERS) using the adaptive governance framework. As of November 24, 2015, the MERS outbreak in South Korea resulted in the quarantine of about 17,000 people, 186 cases confirmed, and a death of 38. Although the national government had overall responsibility for MERS response, there is no clear understanding of how the ministries, agencies, and subnational governments take an adaptive response to the public health crisis. The paper uses the adaptive governance framework to understand how South Korean governments respond to the unexpected event regarding the following aspects: responsiveness, public learning, scientific learning, and representativeness of the decision mechanisms. The framework helps understand how joint efforts of the national and subnational governments were coordinated to the unexpected conditions. The study highlights the importance of adaptive governance for an effective response to a public-health related extreme event.

초·중·고 보건교사들의 메르스 발생 시 대응 (Elementary, Middle, and High School Health Teachers' Countermeasures Against an Outbreak of Pandemic Diseases, Including MERS)

  • Purpose: This study aims to examine how elementary, middle and high school health teachers in Gyeonggi province react to infectious disease like MERS. Methods: This is a descriptive study using a convenience sample of 1,267 school health teachers. Results: When MERS was an epidemic, school health teachers who took the 'leading' role were 92.4%. A school heath teacher's countermeasures against infectious diseases showed a high score during MERS outbreak (3.81) compared to regular days (3.59). Conclusion: Based on such results, it has been determined that schools need to keep two types of procedures manual, such as 'countermeasures on a regular basis' and 'countermeasures in the event of a pandemic disease outbreak' manual in the staff offices so that anyone can view them at any time. It is necessary to prepare regulations to strengthen their authority so that school health teachers can take the role of control tower, in a professional capacity, when communicable disease is spreading. In order to prevent any confusion and chaos, there should be a unitary reporting system at each school when dealing with an epidemic. School health teachers need to get continuous training to reinforce their abilities to react to communicable diseases.

<메르스>에 대한 은유와 이데올로기적 함축: KBS와 JTBC 뉴스 보도를 중심으로 (Metaphors for MERS and Their Ideological Meaning: Focusing on the news reports from Korean media KBS and JTBC)

  • This study has two main purposes: to establish a list of source domains in the metaphors for Middle East respiratory syndrome (MERS) and to uncover ideological meanings embedded in them in Korean news reports from KBS and JTBC. The first part of this study presents metaphors such as [MERS IS WAR], [MERS IS WAVE], [MERS IS A LIVING THING], and [MERS IS A THING], which were found in the data. The latter part of this study deals with how the two broadcasting companies use these metaphors differently according to their ideologies. In the metaphor of [MERS IS WAR], KBS tends to show less of the agents who controls the war since the war against MERS has failed which casts responsibility to the controlling agents, the government and big hospitals. In this, KBS tries to present less of the information of the responsible agents that presented in JTBC. Through the metaphor of [MERS IS WAVE], KBS presents the aftermath of MERS as something not serious. Compared to JTBC, KBS tends to suggest that the aftermath of MERS is predominantly an economic effects by metaphorically suggesting that predominantly the economic sector got hit by MERS.

Middle East Respiratory Syndrome Coronavirus-Encoded ORF8b Inhibits RIG-I-Like Receptors by a Differential Mechanism

  • Middle East respiratory syndrome coronavirus (MERS-CoV) belongs to the genus Betacoronavirus and causes severe morbidity and mortality in humans especially when infected patients have underlying diseases such as chronic obstructive pulmonary disease (COPD). Previously, we demonstrated that MERS-CoV-encoded ORF8b strongly inhibits MDA5- and RIG-I-mediated induction of the interferon beta (IFN-β) promoter activities. Here, we report that ORF8b seemed to regulate MDA5 or RIG-I differentially as protein levels of MDA5 were significantly down-regulated while those of RIG-I were largely unperturbed. In addition, ORF8b seemed to efficiently suppress phosphorylation of IRF3 at the residues of 386 and 396 in cells transfected with RIG-I while total endogenous levels of IRF3 remained largely unchanged. Furthermore, ORF8b was able to inhibit all forms of RIG-I; full-length, RIG-I-1-734, and RIG-I-1-228, the last of which contains only the CARD domains. Taken together, it is tempting to postulate that ORF8b may interfere with the CARD-CARD interactions between RIG-I and MAVS. Further detailed analysis is required to delineate the mechanisms of how ORF8b inhibits the MDA5/RIG-I receptor signaling pathway.

사업장의 감염성재난 대응체계: 2015년 메르스 유행을 중심으로 (Workplace Response System Against Infectious Disasters based on the MERS Outbreak in Korea)

  • Purpose: This study aimed to evaluate the workplace response system (WRS) against infectious disasters (IDs) based on the response attitudes and capacity of occupational health nurses (OHNs) who worked during the 2015 Middle East Respiratory Syndrome (MERS) outbreak in South Korea. Methods: Fifty-one participants who had worked as OHNs at the time of the 2015 MERS outbreak were surveyed from November 20 to December 10, 2016. Data were collected using a self-report questionnaire and analyzed using descriptive statistics and McNemaer's test. Results: According to the survey results, the following were lacking in the WRS: a dedicated ID-response team, manuals, related education programs for OHNs, and health education programs for workers. Results also confirmed that workers were vulnerable to new infectious diseases because of the lack of health checkups and support for workers before and after business trips abroad. Among the respondents, 98.0% answered affirmatively that an ID was important for health management in the workplace, but 64.7% answered that it was difficult to manage an ID. The perceived capacity items to respond to IDs ranged from 3.0 points to 3.3 points. This scores were generally high. Conclusion: As the WRS is currently insufficient, it is necessary to develop an adequate WRS to IDs by addressing the identified problems.

중소병원의 2015 MERS 위기 대응: G병원의 사례 (A Small-and-medium-sized Hospital's Crisis Management during 2015 MERS Outbreak: A Case of G Hospital)

  • Purpose: Crisis is inevitable to every organization and therefore, successful crisis management is critical to the organizations' survival and prosperity. With the understanding, this study aims to draw propositions for successful crisis management of hospitals when facing infectious disease outbreak. For the purpose, a case of a small and medium sized hospital's experience of crisis management during 2015 Middle East Respiratory Syndrome outbreak was analyzed. Methodology/Approach: The detailed internal circumstances and experiences of the hospital during the MERS outbreak were identified by in-depth interview as well as the extensive material review, and analyzed under the view of the theories of accident, error, and crisis in relation of organization management Findings: Overall, nine propositions are drawn by the phase of crisis. In pre-crisis phase, for example, 'the hospital preparedness has positive influence on the effective responding to the crisis'. In detection phase, 'the mindfulness of the hospital organizations' as well as the individuals' has positive influence on detecting the crisis signals'. In crisis phase, for example, 'improvising naturally occurs in crisis by the unknown disease, therefore, a component site supervisor coordinating such improvision is important'. Lastly, in post-crisis phase, 'successful crisis responding experience facilitates the positive hospital culture'. Practical implication: From the experience of a small and medium size hospital, it is suggested that proactive system approach oriented by safety is beneficial for effective crisis management.

Comparative study: nonsynonymous and synonymous substitution of SARS-CoV-2, SARS-CoV, and MERS-CoV genome

  • The direction of evolution can estimate based on the variation among nonsynonymous to synonymous substitution. The simulative study investigated the nucleotide sequence of closely related strains of respiratory syndrome viruses, codon-by-codon with maximum likelihood analysis, z selection, and the divergence time. The simulated results, dN/dS > 1 signify that an entire substitution model tends towards the hypothesis's positive evolution. The effect of transition/transversion proportion, Z-test of selection, and the evolution associated with these respiratory syndromes, are also analyzed. Z-test of selection for neutral and positive evolution indicates lower to positive values of dN-dS (0.012, 0.019) due to multiple substitutions in a short span. Modified Nei-Gojobori (P) statistical technique results also favor multiple substitutions with the transition/transversion rate from 1 to 7. The divergence time analysis also supports the result of dN/dS and imparts substantiating proof of evolution. Results conclude that a positive evolution model, higher dN-dS, and transition/transversion ratio significantly analyzes the evolution trend of severe acute respiratory syndrome coronavirus 2, severe acute respiratory syndrome coronavirus, and Middle East respiratory syndrome coronavirus.
