성대 용종 환자의 후두미세수술 전후 음성 평가에서 OperaVOXTM와 Multi-Dimensional Voice Program 간의 신뢰도 연구 (Reliability of OperaVOXTM against Multi-Dimensional Voice Program to Assess Voice Quality before and after Laryngeal Microsurgery in Patient with Vocal Polyp)

  • Background and Objectives OperaVOXTM (Oxford Wave Research Ltd.) is a portable voice analysis software package designed for use with iOS devices. As a relatively cheap, portable and easily accessible form of acoustic analysis, OperaVOXTM may be more clinically useful than laboratory-based software in many situations. The aim of this study was to evaluate the agreement between OperaVOXTM and Multi-Dimensional Voice Program (MDVP; Computerized Speech Lab) to assess voice quality before and after laryngeal microsurgery in patient with vocal polyp. Materials and Method Twenty patients who had undergone laryngeal microsurgery for vocal polyp were enrolled in this study. Preoperative and postoperative voices were assessed by acoustic analysis using MDVP and OperaVOXTM. A five-seconds recording of vowel /a/ was used to measure fundamental frequency (F0), jitter, shimmer and noise-to-harmonic ratio (NHR). Results Several acoustic parameters of MDVP and OperaVOXTM related to short-term variability showed significant improvement. While pre-operative value of F0, jitter, shimmer, NHR was 155.75 Hz (male: 125.37 Hz, female: 183.37 Hz), 2.20%, 6.28%, 0.16, post-operative values of these parameter was 164.34 Hz (male: 129.42 Hz, female: 199.26 Hz), 2.15%, 5.18%, 0.14 Hz in MDVP. While pre-operative value of F0, jitter, shimmer, NHR was 168.26 Hz (male: 135.16 Hz, female: 201.37 Hz), 2.27%, 6.95%, 0.26, post-operative values of these parameters was 162.72 Hz (male: 128.267 Hz, female: 197.18 Hz), 1.71%, 5.36%, 0.20 in OperaVOXTM. There was high intersoftware agreement for F0, jitter, shimmer with intraclass correlation coefficient. Conclusion Our results showed that the short-term variability of acoustic parameters in both MDVP and OperaVOXTM were useful for the objective assessment of voice quality in patients who received laryngeal microsurgery. OperaVOXTM is comparable to MDVP and has high intersoftware reliability with MDVP in measuring the F0, jitter, and shimmer

  • Many people suffer from various laryngeal diseases. Since we can notice voice change easily, acoustic analysis can be helpful to diagnose the diseases. Several attempts have been made to clarify the relation between the parameters and the state of sick vocal folds but any decisive parameters are not found yet. The purpose of this study was to select and develop those parameters useful for diagnosing and differentiating laryngeal diseases. We examined eight MDVP parameters, and two additional MFCC and LPC parameters obtained from the production of an open vowel by 252 subjects with or without laryngeal diseases. Using a statistical procedure through the artificial neural networks, we attempted to differentiate laryngeal disease groups. Results showed that the LPC parameters indicated the highest differentiating rate by the networks followed by the MFCC and the MDVP parameters. In addition, Jita, Shim and NHR among the MDVP parameters came out better parameters in diagnosing laryngeal diseases.

일측성 성대마비 환자 평가에서 Cepstral Peak Prominence의 유용성 (Usefulness of Cepstral Peak Prominence (CPP) in Unilateral Vocal Fold Paralysis Dysphonia Evaluation)

  • Background and Objectives : The purpose of this study was to compare the usefulness of Cepstral peak prominence (CPP) with parameter of Multiple Dimensional Voice Program (MDVP) in evaluating unilateral vocal fold paraylsis patients with subjective voice impairment. Materials and Methods : From July 2014 to August 2016, 37 patients with unilateral vocal fold paralysis who had been diagnosed with unilateral vocal fold paralysis and had received two or more voice tests before and after the diagnosis were evaluated for maximum phonation time (MPT), MDVP and CPP. Respectively. Voice tests were performed with short vowel /a/ and paragraph reading. Results : The CPP-a (CPP with vowel /a/) and CPP-s (CPP with paragraph reading) of the Cepstrum were statistically negatively correlated with G, R, B, and A before the voice therapy. Jitter, Shimmer, and NHR of MDVP were positively correlated with G, R, B. Jitter, Shimmer, and NHR of the MDVP were significantly correlated with the Cepstrum index. G, B, A and CPP-a and CPP-s showed a statistically significant negative correlation and a somewhat higher correlation coefficient between 0.5 and 0.78. On the other hand, in MDVP index, there was a positive correlation with G and B only with Jitter of 0.4. Conclusion : CPP can be an important evaluation tool in the evaluation of speech in the unilateral vocal cord paralysis when speech energy changes or the cycle is not constant during speech.

  • The purpose of this study was to determine the correlation between the Average Fundamental Frequency, Fo-Tremor Frequency, Jitter, Shimmer, Amplitude Tremor Intensity Index, and Noise to Harmonic Ratio of MDVP and Fo, Fo Tremor, Jitter, Shimmer, Amp Tremor, HNR, and NNE of Dr. Speech. The Pearson correlation coefficient was used for analysis. The results showed that there was a strong correlation between Fo and Shimmer of both instruments. However, the remaining parameters did not show a significant correlation.

  • 음성에 대한 청각적 판단으로는 차이점과 공통점을 느낄 수 있으나, 개별적인 음향 분석에서는 차이점이나 공통점을 밝혀 내지 못한 경우가 많았다 따라서 음성에 대한 청각적 판단처럼 음향 분석 변수를 종합적으로 분석할 수 있는 MDVP와 Phonetogram을 이용하여 음성질환별로 나타나는 소견을 관찰하여 청각적 판단과 종합적인 음향학적 분석간에 연관성이 있는지 알아보고자 하였다. 음성변화를 주소로 본원에 내원하였던 환자 중 화상회선경술 검사상 양성성대질환이 관찰되었던 102명을 대상으로 하였고, 성대질환이 없었던 정상군과 비교하였다. 각 대상들은 화상회선경술 검사에서 질환의 종류를 확인하였고, 질환별 청각적 분석을 GRBAS로 확인하였다. 또한 MDVP를 시행하여 질환별로 변수들의 특징적인 변화를 종합적으로 도식화하여 확인하였고, Phonetogram을 시행하여 질환별로 감소된 부분이 있는지 확인하였다.   • Measuring phonatory stability using MDVP(Multi-dimensional voice program, Kay Elemetrics Corp., NJ, USA) are becoming popular in many Korean clinics and laboratories, yet questions about standardization and reference values have remained. The purpose of present study was to examine the effects of frequency and intensity variation on the results of acoustic analysis related to phonatory stability. Twenty young adults(ten females and ten males) were asked to sustain vowel /a/ for more than 3 seconds under 9 different pitch and loudness conditions. Using MDVP, nine voice samples were analyzed, and jitter percent, fundamental frequency variation, shimmer percent, peak amplitude variation, noise to harmonic ratio, amplitude tremor intensity index, and degree of subharmonics were compared. The results showed that intensity changes can significantly affect various phonatory stability measures, and the lowest perturbation values can be obtained from slightly louder(10dB) phonatory condition than comfortable level phonation.

변성기 일반 아동 음성의 기본주파수 연구 (Fundamental Frequencies of Normal Children's Voice in mutational Period)

  • The structure changes of the vocal folds are related to the fundamental frequencies (F0). In other words, the increasing in vocal fold length and thickness makes the result of dropping in the F0 during the mutational period. The purpose of this study was to investigate F0 of normal children's voice in mutational period. 360 children (180 boys and 180 girls) were participated in this experiment. The age was ranged from 11 to 16 years. The subjects were asked to produce sustained comer vowels (/a/ /i/ /u/) five times each and the data were analyzed using the MDVP of CSL. The result shows that the F0 are considerably decreased with age and reach to adults' F0 by 16 years in most cases. In particular, the F0 of male subjects were rapidly decreased between the ages from 12 ($226.98\;{\pm}\;19\;Hz$) to 13 years ($169.3\;{\pm}\;25\;Hz$), while the F0 of female subjects were slowly changed from the later period of 12 to 16 years old. This result may be used by the meaning of guideline and lead the basic data to differentiate between normal voice and voice disorder.

다양한 특징 파라미터와 선형변별분석을 이용한 후두암의 선별검사

  • 후두질환 감별용 음성 분석방법인 multi-dimensional voice program (MDVP)으로 분석이 불가능할 정도로 주기성이 크게 훼손된 후두암 말기의 음성 에 대하여 효과적인 감별을 하기 위하여, 몇 가지 켑스트럼(cepstrum) 파라미터를 비롯하여, 주기성 및 그 동요 정도, 영교차율(zero-crossing rate, ZCR), 스텍트럼 중심 (spectral centroid, SC) 등 다양한 특징 파라미터를 이용한 감별 실험을 수행하였다. 후두암 감별 실험을 위해 부산대학교 병원 이비인후과에서 수집한 정상 남자 음성 데이터 50개, 양성 후두질환 남자 음성 데이터 50개 및 남성 후두암 환자 음성 데이터 105개를 사용하였다. 음성 데이터는 단모음 /아/ 발성만을 사용하였고, 정상인과 양성후두질환 환자, 그리고 MDVP 분석이 가능한 후두암 환자 음성 데이터 중 2/3는 학습에, 나머지 113은 감별실험에 사용하였다. 후두암 감별을 위한 분류기로는 Gaussian Mixture Model(GMM) 분류기를 사용하였으며, 이때 모델의 복잡도를 표현하는 mixture 수는 1에서 10까지 가변시키면서 가장 좋은 성능을 나타내는 값으로 결정하였다. 또한 모든 실험에서 켑스트럼 분석의 차수는 동일하게 12차로 고정시켰다. 성대에어로빅치료법이 음성장애환자의 음성개선에 미치는 효과 (Effects of vocal aerobic treatment on voice improvement in patients with voice disorders)

  • 본 연구는 성대에어로빅치료법(vocal aerobic treatment, VAT)이 음성장애 환자의 음성 개선에 미치는 효과에 대해 알아보았다. 연구대상은 후두스트로보스코피, 음성검사 상 음성장애로 진단된 20명(남 13명, 여 7명)이었다. 음향학적 평가는 CSL(computerized speech lab)의 MDVP(Multi-Dimensional Voice Program)와 VRP(Voice Range Profile)를 통해 평가하였다. 공기역학적 평가는 PAS(Phonatory Aerodynamic System)를 통해 평가하였다. MDVP를 통해 치료 전 후 기본주파수(Fo), 주파수변동률(Jitter), 진폭변동률(Shimmer), 소음대배음비(NHR)의 변화를 측정하였고, VRP에서는 치료 전 후 주파수 범위(Fo range), 강도범위(Energy range)를 측정하였다. PAS에서는 치료 전 후 폐활량(FVC), 최대연장발성시간(PHOT), 평균호기류율(MEAF), 성문하압(MPAP), 음성효율성(AEFF)의 변화를 알아보았다. 후두스트로보스코피에서는 치료 전 후 양측 성대의 규칙성, 대칭성, 점막파동, 진폭 변화 소견을 알아보았다. 음성치료는 총체적 음성치료 접근법 중 하나인 VAT 프로그램을 환자별로 주 1회 실시하였다. 환자별 평균 치료 회기는 6.5회였다. 연구결과, MDVP에서는 Jitter, Shimmer, NHR이 통계적으로 유의하게 감소하였다(p<.001, p<.01, p<.05). VRP 결과, 주파수 범위에서 Hz와 Semitones이 치료 후 유의미하게 향상하였다(p<.01, p<.05). PAS 결과, FVC, PHOT에서 유의미한 향상이 나타났다(p<.01, p<.001). 후두스트로보스코피 결과 치료 후 기능적 음성장애, 인후두역류질환, 양성성대점막질환군에서 성대소견이 정상범주에 해당하였다. 따라서 VAT 프로그램은 음성장애환자의 음향학적 공기역학적 후두스트로보스코피 측면에서의 음성 개선에 효과적인 것으로 나타났다. 차후 연구에서는 동일 집단의 음성장애 환자에게 VAT 적용 연구가 필요하다고 생각된다. 또한 객관적인 음성 개선뿐만 아니라 주관적 음성 개선을 알아볼 필요가 있다. 나아가 직업적 음성사용자를 대상으로 VAT 효과에 대한 적용연구가 필요가 있다.

켑스트럼 파라미터를 이용한 후두암 검진 (Laryngeal Cancer Screening using Cepstral Parameters)

  • Background and Objectives : Laryngeal cancer discrimination using voice signals is a non-invasive method that can carry out the examination rapidly and simply without giving discomfort to the patients. n appropriate analysis parameters and classifiers are developed, this method can be used effectively in various applications including telemedicine. This study examines voice analysis parameters used for laryngeal disease discrimination to help discriminate laryngeal diseases by voice signal analysis. The study also estimates the laryngeal cancer discrimination activity of the Gaussian mixture model (GMM) classifier based on the statistical modelling of voice analysis parameters. Materials and Methods : The Multi-dimensional voice program (MDVP) parameters, which have been widely used for the analysis of laryngeal cancer voice, sometimes fail to analyze the voice of a laryngeal cancer patient whose cycle is seriously damaged. Accordingly, it is necessary to develop a new method that enables an analysis of high reliability for the voice signals that cannot be analyzed by the MDVP. To conduct the experiments of laryngeal cancer discrimination, the authors used three types of voices collected at the Department of Otorhinorlaryngology, Pusan National University Hospital. 50 normal males voice data, 50 voices of males with benign laryngeal diseases and 105 voices of males laryngeal cancer. In addition, the experiment also included 11 voices data of males with laryngeal cancer that cannot be analyzed by the MDVP, Only monosyllabic vowel /a/ was used as voice data. Since there were only 11 voices of laryngeal cancer patients that cannot be analyzed by the MDVP, those voices were used only for discrimination. This study examined the linear predictive cepstral coefficients (LPCC) and the met-frequency cepstral coefficients (MFCC) that are the two major cepstrum analysis methods in the area of acoustic recognition. Results : The results showed that this met frequency scaling process was effective in acoustic recognition but not useful for laryngeal cancer discrimination. Accordingly, the linear frequency cepstral coefficients (LFCC) that excluded the met frequency scaling from the MFCC was introduced. The LFCC showed more excellent discrimination activity rather than the MFCC in predictability of laryngeal cancer. Conclusion : In conclusion, the parameters applied in this study could discriminate accurately even the terminal laryngeal cancer whose periodicity is disturbed. Also it is thought that future studies on various classification algorithms and parameters representing pathophysiology of vocal cords will make it possible to discriminate benign laryngeal diseases as well, in addition to laryngeal cancer.
