  • Multi-detector row CT (MDCT) provides faster speed, longer coverage in conjunction with thin slices, improved spatial resolution, and ability to produce high quality muliplanar and three-dimensional (3D) images. MDCT has revolutionized the non-invasive evaluation of the central airways. Simultaneous display of axial, multiplanar, and 3D images raises precision and accuracy of the radiologic diagnosis of central airway disease. This article introduces central airway imaging with MDCT emphasizing on the emerging role of multiplanar and 3D reconstruction.

Dose Measurements using Phantoms for Tube Voltage, Tube Current, Slice Thickness in MDCT (MDCT의 관전압, 관전류, 슬라이스 두께 변화에 따른 팬텀의 선량 분포 측정)

  • The purpose of this study was to measure and evaluate radiation dose for MDCT parameters. Patient dose for various combination of MDCT parameters were experimentally measured, using MDCT (GE light speed plus 4 slice, USA), model 2026C electrometer (RADICAL 2026C, USA), standard Polymethylmethacrylate (PMMA) head and body CT dosimetry phantoms. In clinical situations, for a typical abdominal scan performed with MDCT at 120 kVp, 180 mAs, 20 mm collimation, and a pitch of 0.75 $CTDI_w,\;CTDI_{vol}$ were measured as 20.2 mGy, 26.9 mGy, respectively. When scan length is assumed as 271.3 mm, DLP and measured effective dose of the abdominal would be calculated as $729.1\;mGy{\cdot}cm$, 10.9 mSv, respectively.

The clinical usefulness of 64 channel MDCT and 128 channel DSCT in coronary CT angiography (관상동맥 전산화단층촬영에서 64 channel MDCT와 128 channel DSCT의 임상 유용성 평가)

  • This study was retrospectively to compare the exposure dose and the imaging quality in coronary CT angiography by using the 64 channel multidetector computed tomography and the 128 channel DSCT. Effective dose was calculated dose length product (DLP) by multiplied the convention factor of chest (0.017). Imaging quality was assessed by radiologists using the 5-point Likert scale. The DLP was ranged from 851 to $1277mGy{\cdot}cm$ (mean: 17.23 mSv) in the 64 channel MDCT and from 82 to $110mGy{\cdot}cm$ (mean: 1.58 mSv) in the of 128 channel DSCT, respectively. The score of imaging quality was respectively $3.31{\pm}0.62$ in 64 channel MDCT and $4.05{\pm}0.46$ in the 128 channel DSCT. The exposure dose of 128 channel DSCT has decreased 1ess 1/10. The score of imaging quality was significant difference between two modalities and the frequency (>4 good) in the 128 channel DSCT is about three times than that of the 64 channel MDCT. Therefore, the 128 channel DSCT in coronary CT angiography is clinically more effective modality for both investigators and patients.

  • Catheter fragment and embolism are both potentially serious complications associated with the use of an intravenous (IV) catheter for contrast media bolus injection, and may be followed by serious or lethal sequelae. Though catheter fragment is a rare complication of IV catheter insertion, especially in peripheral veins, CT can be used to detect residual fragment. This study demonstrates the utility of MDCT to localize a small, subtle peripheral venous catheter, which can be easily reformatted of MDCT reformations. Various 3D techniques such as MPR and MIP, volume rendering, and shaded-surface displays are currently available for reconstructing MDCT data. Advances in MDCT technology contribute substantially to the detection and accurate localization of smaller IV catheter fragment.

  • Methods and results : The suspected patient who have results of CTA and CAG examinations to evaluate coronary stenose to undergo each 16MD CT(n=93) and dual source CT(n=100). As a results of statistic, the highest rank of sensitivity, specificity, PPV, NPV and accuracy in coronary artery with using 16MDCT was displayed in LAD(73.5%), RCA(74.5%), LAD(66.7%), LCX(75%), LCX(67.7%). The mean diagnostic accuracy of dual source CT was more 17% than 16MDCT. Dual source CT was recorded 84% mean of accuracy. In addition to, segments of coronary artery did not show significant differences in all of them. However, distal segment become more and more accurate than proximal site.

Comparison of the Multidetector-row Computed Tomographic Angiography Axial and Coronal Planes' Usefulness for Detecting Thoracodorsal Artery Perforators

  • Background During the planning of a thoracodorsal artery perforator (TDAP) free flap, preoperative multidetector-row computed tomographic (MDCT) angiography is valuable for predicting the locations of perforators. However, CT-based perforator mapping of the thoracodorsal artery is not easy because of its small diameter. Thus, we evaluated 1-mm-thick MDCT images in multiple planes to search for reliable perforators accurately. Methods Between July 2010 and October 2011, 19 consecutive patients (13 males, 6 females) who underwent MDCT prior to TDAP free flap operations were enrolled in this study. Patients ranged in age from 10 to 75 years (mean, 39.3 years). MDCT images were acquired at a thickness of 1 mm in the axial, coronal, and sagittal planes. Results The thoracodorsal artery perforators were detected in all 19 cases. The reliable perforators originating from the descending branch were found in 14 cases, of which 6 had transverse branches. The former were well identified in the coronal view, and the latter in the axial view. The location of the most reliable perforators on MDCT images corresponded well with the surgical findings. Conclusions Though MDCT has been widely used in performing the abdominal perforator free flap for detecting reliable perforating vessels, it is not popular in the TDAP free flap. The results of this study suggest that multiple planes of MDCT may increase the probability of detecting the most reliable perforators, along with decreasing the probability of missing available vessels.

High Quality Audio Watermarking using Spread Spectrum and Psychoacoustic Model (대역확산과 심리음향 모델을 이용한 고음질 오디오 워터마킹)

  • In this paper, we proposed the high quality audio watermarking algorithm using MDCT/IMDCT (Modified DCT/Inverse Modified DCT) with psychoacoustic model. Generally, a digital audio watermark is embedding the frequency domain after frequency transform of the digital audio data but the digital audio quality is affected by watermarking. In our scheme, the digital audio data is spread with PN((Pseudo Noise) code and then audio watermark is embedded in MDCT processing that refers psychoacoustic model. In MDCT processing, according to the shape of filter bank output, the block switching selects a window sequence that has 256, 1,024 or 2,048 points interval for high quality audio. The author confirm that when watermark weight ${\alpha}$ is 2.5 below, the detection ratio of watermark is a satisfied to SDMI's(Secure Digital Music Initiative) recommendation 50% above and SM is $50{\sim}68dB$ range with mainly 4 kind of attacks(Compression, Cropping, FFT and Echo).

  • Purpose : To compare the CT numbers on 3 cone-beam CT (CBCT) images with those on multi-detector CT (MDCT) image using CT phantom and to develop linear regressive equations using CT numbers to material density for all the CT scanner each. Materials and Methods : Mini CT phantom comprised of five 1 inch thick cylindrical models with 1.125 inches diameter of materials with different densities (polyethylene, polystyrene, plastic water, nylon and acrylic) was used. It was scanned in 3 CBCTs (i-CAT, Alphard VEGA, Implagraphy SC) and 1 MDCT (Somatom Emotion). The images were saved as DICOM format and CT numbers were measured using OnDemand 3D. CT numbers obtained from CBCTs and MDCT images were compared and linear regression analysis was performed for the density, $\rho$ ($g/cm^3$), as the dependent variable in terms of the CT numbers obtained from CBCTs and MDCT images. Results : CT numbers on i-CAT and Implagraphy CBCT images were smaller than those on Somatom Emotion MDCT image (p<0.05). Linear relationship on a range of materials used for this study were $\rho$=0.001H+1.07 with $R^2$ value of 0.999 for Somatom Emotion, $\rho$=0.002H+1.09 with $R^2$ value of 0.991 for Alphard VEGA, $\rho$=0.001H+1.43 with $R^2$ value of 0.980 for i-CAT and $\rho$=0.001H+1.30 with $R^2$ value of 0.975 for Implagraphy. Conclusion: CT numbers on i-CAT and Implagraphy CBCT images were not same as those on Somatom Emotion MDCT image. The linear regressive equations to determine the density from the CT numbers with very high correlation coefficient were obtained on three CBCT and MDCT scan.

The Assessment of Scattered Ray According to the Beam Thickness of Z-axis in MDCT(Multi Detector Computed Tomography) (MDCT(다배열검출기 전산화단층촬영장치)에서 Z-축의 빔 두께에 따른 산란선의 평가)

  • The purpose of this study is to measure scattered ray which is occurred except for Z-axis range of the detector in MDCT's iso-center and present the basic data about the standard for reduction of scattered ray. The development of MDCT brings out the enlargement of beam thickness to the patient's Z-axis, which distributes to the increase in exposure dose according to the rise of scattered ray. Also MDCT brings out the increase of scattered ray about 4times more than SDCT. To evaluate scattered ray according to the change of beam thickness on MDCT, we measured scattered ray of MDCT's Z-axis beam thickness by using one 16-slice CTs and two 64-slice CTs. We used the ionization chamber 60ml 2026C as the equipment of measurement. In our results, we found out that the change of scattered ray according to the beam thickness in the same kVp has increase of scattered ray. Secondly we found out the increase of scattered ray according to the increase of kVp. Lastly we found out the decrease of scattered ray according to the increase of the distance from the ionization chamber.

3D MDCT Reformation Findings of the Radiographic Contrast Medium Extravasation (조영제 혈관외유출 현상의 3D MDCT 재구성 영상)

  • Radiographic contrast medium may cause tissue injury by extravasation during intravenous automated injection during CT examination. A large - volume extravasation (140 mL) occurred in an adult during contrast-enhanced CT The patient had a swelling and injury on the dorsum right hand of intravenous catheter region. The extravasation injury site was determined by CT scanning. The extavasation compartment syndrome case was examined using four separate display techniques. These 3D MDCT findings might help to determine the best course of treatment for patient with contrast extravasation. 3D image reconstructions provide accurate views of high-resolution and soft-tissue imaging. This paper introduces extravasation with the radiography and 3D MDCT findings.
