Lymphoscintigraphy for Intraopertive Sentinel Node Biopsy of Skin and Soft Tissue Malignancy (Lymphoscintigraphy와 전초 림프절 절제술을 이용한 피부 악성종양의 치험례)

  • Sentinel lymphnode biopsy is widely performed in the management of malignant melanoma and breast cancer. The sentinel lymphnode is the prime site of draining from the malignant lesion and of metastasis. The aim of this study was to evaluate a usefulness of lymphoscintigraphy in conjunction with a removal of sentinel lymphnodes of skin and soft tissue malignancy. We studied 11 patients selected between January, 2003 and November, 2004. Clinically sentinel lymphnodes free of metastasis were examined with lymphoscintigraphy, gamma detection probe and vital dye staining, and we reviewed histopathologic findings and inert status of the nodes and the results fo treatment. Nine cases were malignant melanoma, one was squamous cell carcinoma on the left hand and another one leiomyosarcoma. Sentinel lymphnodes were identified in all cases. Three cases of malignant melanoma had positive sentinel lymphnodes on histological examination. All patients with positive sentinel lymphnodes were treated with therapeutic regional lymphadectomy, chemotherapy and adjuvant regimen. Four patients underwent PET scanning and followed sentinel lymphnode biopsy. Two had no metastasis signs on PET scanning. Therapeutic lymphnode dissection was carried out upon the patients whose sentinel lymphnode was positive on PET scanning. We contend that lymphoscintigraphy and sentinel lymphnode biopsy are reliable to confirm regional lymphnode metastasis of the skin and soft tissue malignancy, and blind extensive lymphnode dissection can be spared.

Serotype and antimicrobial susceptibility of Salmonella spp. isolated from pigs and cattle (소와 돼지유래 Salmonella속 균의 혈청형 및 약제감수성)

  • At the present study, it was aimed to explore the states of antimicrobial resistant Salmonella spp. isolates from 3,850 pigs (2,732 ileocecocolic lymphnodes and 1,118 cecal contents) and 1,764 cattle (965 cecal lymphnodes and 799 cecal contents) slaughtered in Busan province from December 2000 to November 2001. Among 5,614 samples, 457 of Salmonella spp. were isolated from pig lymphnodes (13.5%), pig cecal contents (4.4%), cattle lymphnodes (3.5%) and cattle cecal contents (0.5%). Salmonella spp. were showed different isolation ratio, that was 10.8% in summer, 9.0% in autumn, 8.4% in spring and 5.0% in winter. As a result of serotyping, B group (65.4%) were identified as the most common in pigs and cattle, in order of $C_1$ (14.0%), $D_1$ (5.5%), $C_2$ (4.2%), $E_1$ (4.2%) and L (3.5%). 34 serotypes were found, among them, Salmonella Typhimurium (S. Typhimurium) (21.0%) was the most common serotype from pigs and cattle. The major serotypes were in order of S. Derby (15.3%), S. Schwarzengrund (14.7%), S. Typhimurium var Copenhagen (9.2%), S. Mbandaka (5.7%), S. Enteritidis (5.5%) and S. Ruiru (3.5%). The most common serotype was S. Typhimurium in pigs, and S. Ruiru in cattle. S. Ruiru was firstly isolated from pigs and cattle in Korea. In antimicrobial susceptibility test, all the isolates were demonstrated susceptibility to norfloxacin and ofloxacin. But the isolates were showed resistance other antibiotics in order of doxycycline (68.3%), tetracycline (67.8%), penicillin (54.5%) and streptomycin (52.5%). S. Typhimurium were exhibited resistance to ampicillin (34.8%), chloramphenicol (36.2%), streptomycin (94.9%), sulfamethoxazole/trimethoprim (34.8%) and tetracycline (97.8%). There were 53 strains (38.4%) which had multi drug resistant (MDR) isolates, resistant to more than 6 antimicrobial agents. The most common resistance patterns of MDR isolates were ampicillin, chloramphenicol, carbenicillin, doxycycline, nalidixic acid, penicillin, streptomycin, sulfamethoxazole/trimethoprim and tetracycline (ACCbDNaPSSuT).

Congenital transmission of Theileria sergenti in cattle verified by immunohistochemistry (소 Theileria sergenti의 태반감염에 대한 면역세포화학적 증명)

  • The spleen and placenta from the aborted fetuses as well as lymphnodes and placenta of the corresponding dams naturally infected with T sergenti were used to localize the parasite antigens by immunohistochemical staining for the possible congenital transmission of theileriosis. Parasite-specific antigens were detected immunohistochemically by incubating the sections with specific monoclonal antibody prepared against 34KD surface antigen of T sergenti and visualized via the avidin biotin complex(ABC) method. Specific T sergenti antigen was detected in the sections of formalin or acetone-fixed fetal spleens and placenta. Similar antigens were also demonstrated in lymphnodes and placentas of the corresponding dams. It is concluded that this technique will eventually play an important role in specialized diagnostic laboratories in the verification/evaluation of congenital infection with T sergenti.

  • A 3-year old male Maltese, which had enlarged and ballooned superficial lymph-nodes, was admitted to the Veterinary Teaching Hospital, College of Veterinary Medicine, Kyung-pook National University. Radiographic findings indicated cranial mediastinal mass, interstitial pneumonitis, conspicuous splenomegaly and liver enlargement. The patient showed anorexia, depression and anemia. It was autopsied following its death 2 days after admittance. There were copious hemoperitoneum, lymphomatous mass in the spleen, torsion of the mesenterium and stenosis of the duodenum. On the microscopic observation, there was the increase of the amount of lymphocytic cytoplasm and numbers of nucleus-devided lymphocytes followed by their diffused infiltration in lymph nodule. Based on the physical, histological and pathological findings, this case was diagnosed as a multicentric form of lymphosarcoma.

  • Sixty-three Salmonella Cultures were isolated from 600 msenteric lymphnodes and 600 rectal contents of apprently healthy pigs slaughtered at abattoris in Hapchun, changnyong, Keochang. The result observed were summaried as follows ; 1. Fifty-nine out of sixty-three cultures were serotyped and Salmonella enteritis(20 cultures) 2. Was found to be the most predominant serotpye. The remainder comprised of 15 serotypes were ; S. typhimuriu, S. derby, S. wagenia, S. infantis, S. thompson, S. tennessee, S. mission, S. anatum, S. give, S. nuwlands, S. newingtion, S. newhaw, S. neumuenstes, S. senftenberg, S. gbana 3. The antibiotic susceptibility of 9 were ; Baytril, Streptomycin, Cephalothin, Ampicillin, Tetracycline, Sulfadimethoxine, Rifampicin, Gentamicin, Kanamycln.

  • Soft tissue sarcoma of the head and neck is not frequent neoplasm, accounting for less than 1% of all malignant neoplasm in the region. The histological varieties include osteogenic sarcoma, malignant fibrous histiocytoma, rhabdomyosarcoma, fibrosarcoma, tenosynovial sarcoma, angiosarcoma and chondrosarcoma. Rhabdomyosarcomas of the head and neck usually occur in children under the age of 10 years (over 70%) and rarely develop in adults over the age of 20 years. The prevalent sites of involvement include the orbit, nasal cavity, external ear, paranasal sinus and soft tissue of mouth and the primary location of tumor is considered to be one of the important prognostic factors. Before the 1960s, when surgical resection was the only method of treatment, the 5-year survival rate was less than 20%, but recently it has been greatly improved by the multimodality treatment, combining surgery with chemotherapy and radiation therapy. Here we treated a rhabdomyosarcoma woman with three cycles of high dose chemotherapy followed by radiation therapy. After the, completion of preoperative treatments, successful result of more than partial response was achieved. Three months later total maxillectomy and radical neck dissection was performed. There was no evidence of tumor infiltration in the resected tumor and regional lymphnodes but metastasized tumor cells in cervical lymphnodes were detected. Tumor cell infiltration was also found on the bone marrow biopsy to evaluate the pancytopenia which occurred during postoperative recovery. Two months later she died of secondary bone marrow failure. We think that this multimodality treatment combining pre-operative chemotherapy, radiotherapy and surgery might play an important role in curative resection and eyeball preservation in patients with rhabdomyosarcoma involving the eyeball.

Antibiotic resistance and plasmid profile of salmonella spp isolated from swine in Kyoungbuk province (경북지방 돼지유래 salmonella 속균의 약제내성과 plasmid profile)

  • This study was conducted to investigate the antibiotic resistance and plasmid profiles of 58 salmonella spp isolated from mesenteric lymphnodes of slaughter pigs in Kyoungbuk province during the period from September 1997 to June 1998. The results obtained are as follow that all isolates were susceptible to amikacin, gentamicin, ciprofloxacin and enrofloxacin, and the majority of isolates were highly susceptible to norfloxacin, colistin, nalidixic acid and apramycin while they were moderately susceptible to kanamycin, cephalothin, chloramphenicol, ampicillin, trimethoprim and penicillin. The majority of isolates were over 90% resistant rates to lincomycin, sulfadimethoxine, vancomycin, methicillin and erythromycin. The plasmid profiles of 58 salmonella spp are developed 1 to 4 fractions, 0.9 to 29.5 Kb molecular range sizes and U strains (45.5%) were showed plasmid profiles by agarose gel electrophoresis. 5 derby harbored 29.5 Kb and 7 Kb, and S schwarzengrund had 14 Kb and 0.9 Kb harboring sizes. Four of 10 S agona and 2 of 4 S typhimurium were harbored 3.1 Kb and n.5 Kb, respectively. Thirty-five untypable strains are developed variable size fractions its showed small size plasmid profile less than 6 Kb and 22 (62.8%) of them had no detectable plasmids.

Evaluation of the 7th UICC TNM Staging System of Gastric Cancer

  • Since January of 2010, the seventh edition of UICC tumor node metastasis (TNM) Classification, which has recently been revised, has been applied to almost all cases of malignant tumors. Compared to previous editions, the merits and demerits of the current revisions were analyzed. Many revisions have been made for criteria for the classification of lymph nodes. In particular, all the cases in whom the number of lymph nodes is more than 7 were classified as N3 without being differentiated. Therefore, the coverage of the N3 was broad. Owing to this, there was no consistency in predicting the prognosis of the N3 group. By determining the positive cases to a distant metastasis as TNM stage IV, the discrepancy in the TNM stage IV compared to the sixth edition was resolved. In regard to the classification system for an esophagogastric (EG) junction carcinoma, it was declared that cases of an invasion to the EG junction should follow the classification system for esophageal cancer. A review of clinical cases reported from Asian patients suggests that it would be more appropriate to follow the previous editions of the classification system for gastric cancer. In addition, in the classification of the TNM stages in the overall cases, the discrepancy in the prognosis between the different stages and the consistency in the prognosis between the same TNM stages were achieved to a lesser extent as compared to that previously. Accordingly, further revisions are needed to develop a purposive classification method where the prognosis can be predicted specifically to each variable and the mode of the overall classification can be simplified.

A Case of Infectious Mononeucleosis Associated with Pleural Effusion (흉막 삼출액을 동반한 전염성 단핵구증 1례)

  • Infectious mononucleosis(IM) is a clinical syndrome characterized by fever, exudative tonsillitis, gerneralized lymphadenopathy(90% of cases), splenomegaly(50% of cases), and hepatomegaly(10% of cases). It is mainly caused by Epstein-Barr virus(EBV) and usually recovered completely in the majority of cases. The complications of IM are splenic rupture, pancreatitis, hematologic problems such as hemolytic anemia, aplastic anemia, and thrombocytopenia, neurologic problems such as meningitis, encephalitis, and Guillian-Barr$\acute{e}$ syndrome, myocarditis, parotitis, orchitis, and interstitial pneumonitis, etc. Pulmonary involvement with EBV infection is rare condition reported frequency of 3% to 5%, in addition pleural effusion has been very rarely reported, especially in the pediatric population. We herein report a case of IM with pleural effusion in 3 years old boy with fever, cervical lymphnodes enlargement, and hepato-splenomegaly. And the pleural effusion is spontaneously resolved for a hospitalization period. A brief review of literature is included.
