Clinical observations were performed on 373 cases of chest trauma, those were admitted and treated at the Department of surrgery, Korea University Hospital, during the past 15 years period from August 1965 to June 1980. 1. The ratio of male to female patient of chest trauma was 4:1 in male predominence and age from 10 to 50 occupied 87.4 % of the total cases. 2. The most common cause of chest trauma was traffic accident in this series. One hundred and eight one cases (48.5%) were injuried by traffic accident and total cases due to blunt trauma (non-penetrating injury) were 282 cases (75.6%) including the cases with traffic accident, and remaining 91 cases (24.4%) were due to penetrating injury including 73 cases (19.6%) of stab wounds. 3. hemopneumothorax were observed in 49% (182 cases) of the total cases, and etiologic distribution revealed 72% due to non-penetrating trauma and 28% due to penetrating injury. 4. Rib fracture was found in 44.8% of cases. common injuries associated with rib fracture were lung, brain and liver. 5. Most common symptom was chest pain and respiratory difficulty, and common sign associated with chest injury was decreased respiratory sound and subcutaneous sound. 6. conservative non-operative treatment was performed in 281 cases (75.4%) and 92 cases (24.6%) were treated with operative treatment including 33 cases (8.9%) with open thoracotomy. 7. Overall mortality was 5.6% (21 cases) and most common cause of death were due to brain edema, cardiogenic shock, asphyxia.
During the 35 month period from November 1966 to November 1967 and from June 1971 to March 1973 I had experienced 127 cases of non fatal wounds of chest in Viet-Nam. .Among these 127 cases, 62[45.4%] were gun shot wounds, 49[35.8%] were shrapnel wounds and the other were traffic accident. stab wounds and miscellanous. Approximately 21% of gun shot wound were perforating and 79% were penetrating but all cases of shrapnel wounds were penetrating. Of these 127 cases. 90% evacuated to hospital within 6 hours and average time 2.5 hours. The tranfusion requirement of these cases ranged from zero to 36 pints of whole blood with an average of 2.600cc. Initial intrathoracic findings were hemopneumothorax and hemothorax mostly. and the incidence of open thoracotomy was 9.5%[12cases] and closed thoracotomy was 82.8%[104cases], which were contrast to the reports from Korean conflict. I had experienced 24 cases with complication, such as large hematoma in lung parenchyme[8 cases], atelectasis[4 cases], pyothorax [3 cases], pneumonia [3 cases], fibrothorax [3 cases], pleural effusion [2 cases] and wound infection [2 cases]. Mortality rate for entire group was 4.7% but the cases associated with brain injury was 100%, with spinal cord injury was 50%, with large vessel 50%, and abdominal injury was 33.3%, and nobody died solely of thoracic injury.

Ventilator-associated pneumonia (VAP) is a lung infection that develops in patients receiving mechanical ventilation. VAP contributes to about 50% of hospital-acquired pneumonia in ICU settings. One of the recommendation of the Institute of for Healthcare Improvement ventilator bundle is HOB elevation. HOB elevation affects shearing forces and makes higher risk for pressure injury development. Pressure injury (PI) is localized damage to the skin over a bony prominence. PI prevention guidelines recommend that HOB positioning should be lower to reduce risk for PI development which contradicts VAP prevention guidelines for the HOB between 30 and 45 degrees for ICU patients. This presents a care dilemma and tension. The purpose of this study was to perform a secondary data analysis using cumulative electronic health record data in order to determine the association of HOB elevation with VAP and PI in ICU patients. A secondary data analysis was conducted to determine whether HOB elevation is associated with VAP and PI. HOB elevation was not likely to be associated with VAP prevention whereas it was likely to be related to PI development. This is somewhat contrary to popular data and publications. Prospective cohort study is desired to inform us in an evidence-based fashion what actually is optimal HOB elevation for ventilated patients in ICU settings.

The ratio of male to female patient of chest trauma was 3.4:1 in male predominence and age from 20 to 50 occupied 62% of the total cases. 2. The most common cause of chest trauma was traffic accident in this series. Eighty-one cases[54%] were injured by traffic accident and total cases due to blunt trauma [non-penetrating injury] were 113 cases[75.4%]including the cases with traffic accident, and remaining 37 cases[24.6%] were due to penetrating injury including 25[16.6%] cases of stab wounds. 3. Hemopneumothorax were observed in 645/[96 cases] of the total cases, and etiologic distribution revealed 78.1% due to non-penetrating trauma and 20.8% due to penetrating injury. 4. Rib fracture was found in 50% of cases. Common injuries associated with rib fracture were lung, brain and liver. 5. Most common symptom was chest pain and respiratory difficulty, and common sign associated with chest injury was decreased respiratory sound and subcutaneous emphysema. 6. Conservative non-operative treatment was performed in 94 cases[62.6%] and 56 cases [37.4%] were treated with operative treatment including 31 cases[20.6%] with open thoracotomy. 7. Overall mortality was 9.3%[14 cases] and most common causes of death were due to brain edema,asphyxia and shock.

Foeniculum vulgare Mill. (fennel) is used to flavor food, in cosmetics, as an antioxidant, and to treat microbial, diabetic and common inflammation. No study to date, however, has assessed the anti-inflammatory effects of fennel in experimental models of inflammation. The aims of this study were to investigate the anti-inflammatory effects of fennel in model of lipopolysaccharide (LPS)-induced acute lung injury. Mice were randomly assigned to seven groups (n=7~10). In five groups, the mice were intraperitoneally injected with 1% Tween 80-saline (vehicle), fennel (125, 250, $500{\mu}l/kg$), or dexamethasone (1 mg/kg), followed 1 h later by intratracheal instillation of LPS (1.5 mg/kg). In two groups, the mice were intraperitoneally injected with vehicle or fennel ($250{\mu}l/kg$), followed 1 h later by intratracheal instillation of sterile saline. Mice were sacrificed 4 h later, and bronchoalveolar lavage fluid (BALF) and lung tissues were obtained. Fennel significantly and dose-dependently reduced LDH activity and immune cell numbers in LPS treated mice. In addition fennel effectively suppressed the LPS-induced increases in the production of the inflammatory cytokines interleukin-6 and tumor necrosis factor-alpha, with $500{\mu}l/kg$ fennel showing maximal reduction. Fennel also significantly and dose-dependently reduced the activity of the proinflammatory mediator matrix metalloproteinase 9 and the immune modulator nitric oxide (NO). Assessments of the involvement of the MAPK signaling pathway showed that fennel significantly decreased the LPS-induced phosphorylation of ERK. Fennel effectively blocked the inflammatory processes induced by LPS, by regulating pro-inflammatory cytokine production, transcription factors, and NO.

Materials and Methods : The extract of SCE was treated to A549 cells and and LPS-induced COPD mouse model. Then, various parameters such as cell-based cyto-protective activity and histopathological finding were analyzed. Results : SCE showed a protective effect on LPS-induced cytotoxicity in A549 cells. This effect was correlated with analysis for caspase 3 levels, elastin contents, protein levels of cyclin B1, Cdc2, and phospho-Erk1/2, and gene expression of TNF-${\alpha}$ and IL-$1{\beta}$ in A549 cells. SCE treatment also revealed a protective effect on LPS-induced lung injury in COPD mouse model. This effect was evidenced via histopathological findings including immunofluorescence stains against elastin and caspase 3, and protein levels of cyclin B1, Cdc2, and Erk1/2 in lung tissue. Conclusions : These data suggest that SCE has pharmaceutical properties on lung injury. This study thus provides scientific evidence for the efficacy of SCE for clinical application to patients with COPD.

This effect was evidenced via histopathological findings including immunofluoresence stains against elastin, collagen, and caspase 3, and protein level of Cdc2, cyclin B1, and phospho-Erk1/2 in lung tissue. Conclusion: These data suggest that SCRT has pharmaceutical properties on COPD. This study provides scientific evidence for the efficacy of SCRT for clinical application to patients with COPD.
Malaria is one of the most common infectious diseases in the world. Plasmodium falciparum, accounting for nearly all malaria mortality, kills an estimated 1 to 2 million persons yearly and has several features that make it deadlist of malarias. While cerebral malaria is the most common presentation of severe disease, acute lung injury associated with malaria is uncommon but serious and fatal complication. We report two cases of severe malaria with ARDS and multi-organ failure. All two patients traveled to foreign countries, Kenya, Papua New Guinea where choroquine-resistant malaria is distributed. The first case, which developed cerebral malaria, hypoglycemia, multi-organ failure, and ARDS, treated with quinine and mechanical ventilator, but expired due to oxygenation failure. Autopsy showed acute necrotizing infiltration, diffuse eosinophilic fibrinoid deposits along the alveolar space, and alveolar macrophage with malaria pigment The second case also developed multi-organ failure, followed by ARDS, and was treated with quinine, exchange transfusion, plasmapheresis, and mechanical ventilator. He recovered with residual restrictive lung change after treatment.
Purpose: Multiple rib fracture (MRF) and a hemopneumothorax accompany with most blunt chest traumas. We aimed to analyze the factors increasing the probability of a hemopneumothorax. In addition, other injuries accompanying MRF were analyzed. Methods: We retrospectively reviewed the medical records of 154 mutiple rib fracture patients who visited our hospital between January 2005 and December 2007. The medical records were reviewed for sex, age, mechanism of injury, location, number of fractures, distance of dislocated rib fragments, and presence of complications. We measured the distance of bony dislocations by using the PACS (Picture Archiving and Communication System). Results: The average number of rib fractures was $3.7{\pm}2.1$, and the number of rib fractures significantly influenced the incidence of a hemothorax (p<0.001). The risk of a hemothorax was increased in a bilateral MRF compared to a unilateral MRF (p=0.027). The distance of dislocated rib fragments influenced the probability of a hemothorax significantly (p=0.018), and subcutaneous emphysema and lung contusion were significantly associated with a pneumothorax (p=0.021, p=0.036). Conclusion: The number of MRFs did not influence the risk for a pneumothorax, but did influence the risk for a hemothorax. The laterality, distance of dislocation, also had an influence on the risk for a hemothorax. Also, subcutaneous emphysema and lung contusion were increased in cases with a pneumothorax. We must consider the possibility of a hemothorax even when the initial chest X-ray shows no evidence of a hemothorax. If a lung contusion is present, then an occult pneumothorax must be considered.
연구배경 : 급성폐손상의 병태생리학적 기전에는 염증세포들이 분비하는 다양한 염증성 매개물질들이 매우 중요한 역할을 한다. 이중 특히 tumor necrosis factor-$\alpha$ (TNF-$\alpha$) 는 다른 염증세포들의 화학주성 및 각종 염증성 매개물질 분비에 영향을 미치는 proin-flammatory cytokine으로 작용하는 한편, 직접적으로 세포손상을 야기시키는 세포독성 cytokine으로도 작용하는데, 급성폐손상에서 TNF-$\alpha$와 폐조직 손상과의 직접적인 관련성에 대해서는 아직 구체적으로 확인된 바가 많지 않다. 또한, 최근에 생체내 방어기전으로 스트레스 단백질에 대한 관심이 높아지면서, 단핵구에 내독소를 처치하거나, 동물에 내독소를 투여하기 전에 미리 스트레스 단백질을 합성시킨 경우, 내독소에 의한 손상을 감소시켜 준다는 연구가 보고되었지만, 내독소 자극 자체만으로 스트레스 단백질 합성이 유도되는지는 아직 분명하지 않다. 이에 저자들은 내독소 유도성 급성 폐손상에서 TNF-$\alpha$ 분비와 폐조직 손상을 포함한 일련의 염증반응과의 관계를 분석하고, 생체내 내독소 자극에 대하여 폐포대식세포에서 스트레스 단백질을 포함한 새로운 단백질 합성이 유도되는지 여부를 분석하고자 하였다. 연구방법 : 흰쥐의 기관내로 내독소를 투여한 후 시간별로 기관지폐포세척액내 TNF-$\alpha$농도, 염증세포 백분율 변화, 병리조직학적 소견을 관찰하고, 또한 각 시간대의 폐포대식세포에서 sodium dodesyl sulfate-polyacrylamide gel electrophoresis와 inducible heat stress protein72에 대한 면역화학염색을 시행하여 단백질 합성양상을 분석하는 한편, 폐포대식세포에 다양한 농도의 내독소 자극과 열처리를 가한 후, 배양상층액에서 tumor necrosis factor-a 농도를 측정하고, 폐포대식세포의 단백질 합성양상을 분석하였다. 연구결과 : 내독소 투여 후 tumor necrosis factor-$\alpha$는 첫 1시간째부터 현저하게 증가하여 (p< 0.0001) 3시간째 최고치에 이르렀고 6시간째는 감소하기 시작하여 12시간째는 정상 대조군 수준으로 감소하였다. 내독소 투여 후 염증세포 백분율의 변화는 2시간째부터 시작하여 6시간째 최고에 이르러 12시간째까지 지속하였으며, 장시간째에 정상 대조군 수준으로 회복하였다. 병리조직학적 소견상 폐손상 지표 점수는 내독소 투여후 6시간째 최고치에 이르러 24 시간째까지 지속하였다. 내독소 투여 후 분리한 폐포대식세포에서 첫 1시간째부터 장시간째까지 정상 대조군에서는 관찰할 수 없던 35kDa의 새로운 단백질 띠가 관찰되었으며, 면역화학염색상 inducible heat stress protein72는 관찰되지 않았다. 내독소 자극을 가하지 않은 정상 대조세포군에 비해 내독소 자극을 가한 세포군의 배양상층액에서 tumor necrosis factor-$\alpha$ 농도가 유의하게 높았으며 (p<0.001), 내독소 자극만 가한 세포군에 비해 열충격 전처치후 내독소 자극을 가한 세포군의 배양상층액에서 tumor necrosis factor-$\alpha$ 농도가 10 ${\mu}g/ml$ 내독소 자극군만 제외하고 모두 유의하게 감소하였다 (p<0.05). 내독소 자극만 가한 세포군은 10 ${\mu}g/ml$의 고농도에서만 35 kDa 의 단백질 띠가 합성되었고 inducible heat stress protein72는 관찰되지 않았다. 열충격 전처치후 내독소 자극을 가한 세포군은 모두 inducible heat stress protein72가 관찰되었다. 결 론 : 기관내 내독소 투여에 의한 급성 폐손상에서 tumor necrosis factor-$\alpha$는 폐손상 정도와 밀접한 관련이 있다. 또한 내독소 자극에 의해서는 폐포대식세포에서 inducible heat stress protein72 합성이 유도되지 않으며, 35 kDa의 새로운 단백질 합성이 유도되었는데, tumor necrosis factor-$\alpha$ 농도 및 병리조직소견과의 관계를 볼 때, 급성 폐손상에 있어 35 kDa 단백질이 방어적인 역할을 담당하지는 않을 것으로 보이며, 이에 대해서는 향후 더 연구가 필요할 것으로 생각된다.
