가토 적출 폐장의 장시간 보존에 관한 실험적 연구 (The Experimental Study for Isolated Rabbit Lung Preservation)

  • We have modified the isolated perfused working rabbit lung model [IPWL] by perfusing the isolated lung with a hollow fiber membrane deoxygenator.For assessment the stored lung was ventilated with FIO2 0.4 and perfused with 37$^{\circ}$C deoxygenated circulating blood at a rate 5ml/kg/min for several hours until lung failure.We chose to compare our developing solution which contained low potassium and pentastarch with the modified Euro-Collins solution .Experiments were divided into four groups[n=6] based on the type of flushing preservation solution and preservation time.The flushed lungs were then preserved into same solution at 8~10$^{\circ}$C with 100% O2 inflated condition for 1 or 20 hours.These following results were obtained.The IPWL model requires only one animal per experiment and allows for the continuous assessment of aerodynamic performance. This should therefore be used as screening test in lung preservation.One hour preservation groups, there were no significant difference in recovery rates of PaO2, PAP and Paw. Survival time in the one hour preservation groups were very significant long in the Group II[LPPS, p<0.01]. Twenty hours preservation groups, there were no significant difference in the recovery rates of PAP and Paw between Group III[m-ECS] and Group IV[NS], but PaO2 was significantly worse at onset of reperfusion in Group III when compared with Group IV [p<0.05]. And also survival time in the 20 hours preservation groups were significant long in the Group IV [p<0.05].

  • Hypercoagulability disorders are commonly encountered in clinical situations in patients with a variety of cancers. However, several hypercoagulability disorders presenting as first symptoms or signs in cancer patients have rarely been reported. We herein described a case of a woman with adenocarcinoma of the lung presenting with deep vein thrombosis, nonbacterial thrombotic endocarditis, recurrent cerebral embolic infarction, and heart failure.

폐암의 임상적 고찰 (A Clinical Study on Primary Lung Cancer)

  • A clinical study was made on 72 cases of primary lung cancer operated in the department of thoracic & cardiovascular surgery, Kyungpook national university hospital from January 1975 to May 1985. The ratio of male to female was 13.4: I and mean age was 53. Histologically, squamous cell carcinoma comprised 72.2% of 72 operated cases. Before admission to our hospital, the erroneous diagnoses were made at local clinics on the 43 cases[59.7%] and a large percentage of them was diagnosed as pulmonary tuberculosis. And then, total of 56 cases received inadequate treatment or delayed the operation. For the location of the tumor, right to left ratio is 1.2:1 and the right upper lobe was most often involved [23.6%]. Operation was performed on the 72 cases and resection on the 59 cases[8.2%]. Postsurgical staging showed that stage III was found most frequently [59.4%] and T,N,M, was 28% of total cases. Two common surgical complications were bleeding in 7 and acute respiratory failure in 6 cases, and these 6 cases of acute respiratory failure were all died. On the basis of these experiences, we conclude that aggressive effort is needed for the early accurate diagnosis and adequate treatment of lung cancer.

비소세포 폐암의 방사선 치료 (Radiotherapy of Non-Small Cell Lung Cancer)

  • 131 Patients of non·small cell lung cancer treated with irradiation at Yonsei Canter Center from Jan. 1971 to Dec. 1980 were retrospectively analysed. Overall 5 year survival rate was $7\%$ in 117 cases, treated with radiotherapy alone and $33\%$ in 14 Cases, treated with surgery & postoperative ratiotherapy. Their median survival was 9.6 months in the former, while 11.1 months in the latter. The patients treated with radical aim achieved $10\%$ in 5 YSR and with palliative aim, $0\%$. Also, survival according to histolocial classification and staging was obtained. Treatment failure was mostly distant failure (40 cases/49 cases), and within 6 months (34cases/49cases).

Flow-Dependent Friction Loss in an Implantable Artificial Lung

  • The goal of this work is to design and build an implantable artificial lung that can be inserted as a whole into a large vein in the body with the least effect on cardiovascular hemodynamics. The experimental results demonstrate that the pressure drop is not entirely related to viscosity effects. The friction factor decreases with an increase in the number of tied-hollow fibers at a constant Reynolds number A uniform flow pattern without stagnation is observed at all numbers of tied hollow fibers tested. The tied hollow fiber module, built in this study with 3 cm of outer diameter of module. 380 m of outer diameter of tied hollow fiber, and 700 number of tied hollow fiber with length of 60 cm, which shows a pressure drop of 13-16 mmHg, satisfies the required pressure drop qualifying 15 mmHg as an intravascular artificial lung.

Update of Research on Drug Resistance in Small Cell Lung Cancer Chemotherapy

  • Small cell lung cancer (SCLC) is characterized by a short cell doubling time, rapid progression and early occurrence of blood-borne and lymph metastasis. The malignancy is the highest of all lung cancer types. Although SCLC has a relatively good initial response to chemotherapy as well as radiotherapy, relapse or disease progression may occur quickly after the initial treatment. Drug resistance, especially multi-drug resistance, is the most important cause of failure of SCLC chemotherapy. This article provides a brief update of research on mechanisms of drug resistance in SCLC and reversal strategies.

한 대학병원 내과계 중환자실로 입원한 폐암 환자들의 임상 특성 및 예후 (Clinical Characteristics and Prognosis of Lung Cancer Patients Admitted to the Medical Intensive Care Unit at a University Hospital)

  • 연구배경: 최근 중환자실에 입원하여 기계환기 등의 적극적인 치료를 받는 폐암 환자들이 증가하고 있다. 내과계 중환자실에 입원하는 폐암 환자의 임상적인 특성과 사망률에 영향을 미치는 위험 인자에 대하여 알아보고자 하였다. 방 법: 2004년 4월부터 2008년 3월까지 내과계 중환자실에 입원한 폐암 환자 88명을 후향적으로 조사하였다. 결 과: 대상 환자는 88명(평균 나이, 66세)이었으며 비소세포폐암이 71명(80.7%), 소세포폐암이 17명(19.3%)이었다. 79명(89.8%)에서 전이가 있었으며 중환자실에 입원하는 주된 이유로는 급성호흡부전(77.3%), 패혈증(11.4%), 중추신경계 기능장애(4.5%) 순이었다. 54명(61.4%)이 기계환기 치료를 받았다. APACHE II 점수, 중환자실 입원기간, 입원 경로, 기계환기 등과 중환자실 사망률은 유의한 관계가 있었다. 그러나 조직형, 전이 여부 등은 유의한 관계가 없었다. 결 론: 중환자실 사망률은 65.9%이었으며, 중환자실 사망과 유의한 관계가 있는 인자는 APACHE II 점수, 중환자실 입원 기간, 입원 경로, 기계환기 등이었다.

Biomarkers for the lung cancer diagnosis and their advances in proteomics

  • Over a last decade, intense interest has been focused on biomarker discovery and their clinical uses. This interest is accelerated by the completion of human genome project and the progress of techniques in proteomics. Especially, cancer biomarker discovery is eminent in this field due to its anticipated critical role in early diagnosis, therapy guidance, and prognosis monitoring of cancers. Among cancers, lung cancer, one of the top three major cancers, is the one showing the highest mortality because of failure in early diagnosis. Numerous potential DNA biomarkers such as hypermethylations of the promoters and mutations in K-ras, p53, and protein biomarkers; carcinoembryonic antigen (CEA), CYFRA21-1, plasma kallikrein B1 (KLKB1), Neuron-specific enolase, etc. have been discovered as lung cancer biomarkers. Despite extensive studies thus far, few are turned out to be useful in clinic. Even those used in clinic do not show enough sensitivity, specificity and reproducibility for general use. This review describes what the cancer biomarkers are for, various types of lung cancer biomarkers discovered at present and predicted future advance in lung cancer biomarker discovery with proteomics technology.

Humidifier Disinfectant-Associated Lung Injury: Six Years after the Tragic Event

  • In 2011, a cluster of peripartum patients were admitted to the intensive care unit of a tertiary hospital in Seoul with signs and symptoms of severe respiratory distress of unknown etiology. Subsequent epidemiological and animal studies suggested that humidifier disinfectant (HD) might represent the source of this pathology. Epidemiological studies, animal studies, and dose-response analysis demonstrated a strong association between HD use and lung injuries. The diagnostic criteria for HD-associated lung injury (HDALI) was defined on the basis of the clinical, pathological, and radiological attributes of the patients. The clinical spectrum of HDALI appears to range from asymptomatic to full-blown acute respiratory failure, and some patients have required actual lung transplantation for survival. The overall mortality of the exposed population was not significant, although peripartum patients and children who were admitted to the intensive care unit did show high mortality rates. Persistent clinical findings such as diffuse ill-defined centrilobular nodules and restrictive lung dysfunction were observed in some of the survivors. The findings of this review emphasize the importance of assessment of the level of toxicity of chemical inhalants utilized in a home setting, as well as the need to identify and monitor afflicted individuals after inhalational injury.
