흉부둔상에 의한 외상성 가성 폐기낭 (Traumatic Pseudocyst of Lung by Blunt Trauma)

  • Pseudocysts of lung by blunt chest trauma are rare lesion with 1 to 2 % incidence. The symptoms are nonspecific and misled or confused with lung abscess or congenital cystic disease of lung occasionally. Diagnosis is not difficult by radiographic findings and history of blunt chest trauma. There courses are benign and spontaneously absorbed and require no specific treatment mostly. Recently, we had the one case of 8 year-old female with traumatic lung cyst and its cavities filled the entire right lower lobe. The destruction of lung was severe, so, right lower lobectomy was done and recovered, discharged without event.

Brain Abscesses Associated with Asymptomatic Pulmonary Arteriovenous Fistulas

  • Brain abscess commonly occurs secondary to an adjacent infection (mostly in the middle ear or paranasal sinuses) or due to hematogenous spread from a distant infection or trauma. Pulmonary arteriovenous fistulas (AVFs) are abnormal direct communications between the pulmonary artery and vein. We present two cases of brain abscess associated with asymptomatic pulmonary AVF. A 65-year-old woman was admitted with a headache and cognitive impairment that aggravated 10 days prior. An magnetic resonance (MR) imaging revealed a brain abscess with severe edema in the right frontal lobe. We performed a craniotomy and abscess removal. Bacteriological culture proved negative. Her chest computed tomography (CT) showed multiple AVFs. Therapeutic embolization of multiple pulmonary AVFs was performed and antibiotics were administered for 8 weeks. A 45-year-old woman presented with a 7-day history of progressive left hemiparesis. She had no remarkable past medical history or family history. On admission, blood examination showed a white blood cell count of 6290 cells/uL and a high sensitive C-reactive protein of 2.62 mg/L. CT and MR imaging with MR spectroscopy revealed an enhancing lesion involving the right motor and sensory cortex with marked perilesional edema that suggested a brain abscess. A chest CT revealed a pulmonary AVF in the right upper lung. The pulmonary AVF was obliterated with embolization. There needs to consider pulmonary AVF as an etiology of cerebral abscess when routine investigations fail to detect a source.

폐 Actinomycosis 2례 보 (Lung actinomycosis: a report of 2 cases)

  • These two pulmonary actinomycosis cases had undergone right upper lobectomy and left lower lobectomy individually under the diagnosis of lung cancer and lung abscess. Recently pulmonary actinomycosis is very rare by the widespread use of antibiotics. In addition to clinical similarity to lung cancer and pulmonary tuberculosis, lack of suspicion and difficulty in obtaining adequate culture material also hamper the precise preoperative diagnosis. The purpose of this report is to review of our experiences and to enhance consideration of pulmonary actinomycosis.

폐실질내 기형종 -1례 보고- (Intrapulmonary teratoma -Report of a case-)

  • This paper reports a case in a 16-year-old female of intrapulmonary teratoma located in the right upper and middle lobes of the lung. The initial symptoms were high spiking fever, cough and chest pain. Initial chest X-ray revealed large homogenous mass in the right upper and middle lung fields. So intercostal tube drainage was done under the impression of the lung abscess. But mass density was remained on the follow-up chest X-ray, the patient had resection of the right upper and middle lobes of the lung. The gross and microscopic findings of the resected specimen revealed characteristic findings of the intrapulmonary teratoma. The patient is asymtomatic at present and living a normal life.

폐농양의 치료에 Pig-tail 도관을 통한 경피적 배농 (Percutaneous Pig-tail Catheter Drainage in the Management of Lung Abscesses)

  • 연구배경 : 폐농양의 환자들중 1주일이상 항생제와 고식요법에 반응하지 않으며 수술 또한 용이하지 않은 환자들을 대상으로 pig-tail 도관을 이용한 경피적 배농 치료를 하였다. 방법 : 형광투시하에 국소마취후 6~10Fr.의 pig-tail 도관을 삽입하여 엑스-선상 공동이 소실되고 도관을 통한 농이 배출이 안될 때 도관을 제거하였다. 결과: 1) Pig-tail 도관을 이용한 경피적 배농 직후 모든 환자에서 엑스-선상 공기-수평선은 소실되었으며 시술후 3일 이내에 체온도 정상화되었다. 2) 폐농양의 경피적 배농에 따른 임상적으로 유의있는 합병증은 없었다. 3) 폐농양의 평균 크기는 $5.2{\times}6.4$ cm이며 배출된 옹의 양은 4~200ml로 다양하였다. 4) 1명의 환자에서 경피적 배농후 16일째 심한 폐출혈로 사망하였는데 출혈의 원인은 기저의 폐결핵, 폐농양 혹은 시술과 연관있는지 확실하지 않았다. 결론 : 경피적 배농은 내과적인 치료에 잘 반응하지 않거나 수술이 용이하지 않거나 술후 재활에 장애가 있을 것으로 사료되는 폐농양 환자에게 별 어려움없이 시행할 수 있는 치료 방법이다.

간농양에 속발한 기관지늑막루의 치험 6례 (Broncho-pleural Fistula due to Liver Abscess: A Review of 6 Cases)

  • Six cases of broncho-pleural fistula due to complicated liver abscess were experienced at the department of chest surgery, Seoul National University Hospital from October 1967 to March 1970. Amebic liver abscess was the primary cause in the 5 cases and the remaining one case was due to pyogenic liver abscess. Involved lung was right side in all case. The clinical manifestation was fever,chill,cough, sputum, dyspnea, chest pain, hemoptysis and shoulder pain. The methods of the treatment employed were closed thoracotomy [1], thoracotomy & drainage [2],decortication [1],and right lower lobectomy[3]. The average duration of the post-operative course was 19.6 days. There was no operative mortality.

폐 Actinomycosis 치험 1례 (Thoracic (Bronchopulmonary) Actinomycosis : A Case Report)

  • Actinomycosis is a chronic suppurative granulomatous disease due to Actinomyces israelii characterized by multiple abscess and sinus tract formation with dense fibrous scarring. This disease, especially thoracic infection, is very rare in Korea so we are not famiIliar with to make diagnosis and treatment. Otherwise the unspecificity of the clinical symptoms and the lack of adequate examination recedure (as anaerobic fungus culture) are the causes of misdiagnosis. Thoracic actinomycosis is very similar to chronic infectious disease of the lung and chest or thoracic neoplasm. Recently we experienced a case of thoracic actinomycosis (bronchopulmonary) which had been confused with chronic lung abscess and pathologically confirmed as broncho-pulmonary actinomycosis. The purpose of this report is to review our experience more thorouly to enhance consideration of Artinomycosis.

폐암 환자에서 통증치료중 발생한 하지 마비 -증례 보고- (Lower Extremity Paralysis Developed during Pain Control in Lung Cancer Patient -A case report-)

  • Continuous epidural analgesia has been used widely for chronic pain control, especially in cancer patients. As one of the complications, paraplegia developed during continuous epidural analgesia may be caused by epidural abscess, epidural hematoma, neural damage, chronic adhesive arachnoiditis, anterior spinal artery syndrome, delayed migration of extradural catheter into subdural space or subarachnoid space and preexisting disease. A 55-years-old male with lung cancer was implanted with continuous thoracic epidural catheter for pain control. Twenty days after catheterization, moderate back pain, weakness of lower extremity and urinary difficulty were developed. We suspected epidural abscess at first and made differential diagnosis with MRI which showed metastatic cancer at T2-4 spine, And compressed spinal cord was the main cause of the lower extremity paralysis.

A Multi-organ Abscesses Including Brain Caused by a Congenital Pulmonary Arteriovenous Fistula

  • In cases of brain or other organ abscess, the causative etiology or disease are not always definable. We report a case of brain, renal, and possibly lung abscesses in a middle aged woman. After close, stepwise surveillance of possible etiologic factors, we covered out a small solitary pulmonary arteriovenous fistula without any pulmonary symptoms and successfully occluded the fistula via endovascular approach. The congenital pulmonary arteriovenous fistula should be bear in mind as a cause of repeated, multiple systemic infective source spray and be pursued despite of negative initial baseline studies.

기도내 이물의 외과적 처치에 관한 연구 (The Surgical Management of Aspirated Foreign Bodies in Airways)

  • The inhalation of a foreign body into the tracheobronchial tree is a child is a life-threatening accident. Forgotten foreign bodies In the airways cause chronic pulmonary infections, allergic asthma, bronchiectatic changes, and lung abscess Foreign bodies that cannot be grasped by bronchoscopic forceps should be removed by thoracotomy and bronchotomy. This report describes our experience In 16 patients who were treated to remove inhaled foreign bodies from the airways after repeated bronchoscopies had been unsuccessful from 1963 to 1981 at the department thoracic surgery of Seoul National University Hospital. The surgical procedures are as follows: 12 patients bronchotomy, 2 patient pneumonectomy, 2 patients lobectomy. In these cases, the foreign bodies are 8 metallic material, 3 plastic material, 1 bean, 1 black snail, and 3 cases of no records. Bronchotomy must be performed as soon as possible in order to avoid more advanced pathologic changes In the Involved lung, such as bronchiectasis, fibrosis or abscess.
