하지동맥 재건술 후 폐쇄성 합병증에 대한 임상적 고찰 (Occlusive Complications after Lower Limb Arterial Bypass Surgery)

  • 배경: 동맥 재건술 후에 발생하는 폐쇄성 합병증은 치료가 어렵고 재발 가능성이 높은 것으로 알려져 있다. 저자들은 동맥 재수술에 관하여 임상양상 및 수술방법, 사용된 이식편, 술 후 결과들을 분석하여 이식편 폐쇄에 영향을 미치는 인자를 알아보고 효율적인 치료 방침을 세우는 데 도움을 얻고자 하였다. 대상 및 방법: 1998년 1월부터 2002년 12월까지 부산대학교병원 흉부외과에서 시행된 173명의 동맥재건술 후 재수술을 받은 33명(55예)을 대상으로 조사하여 임상적 특성과 치초 수술의 유형, 재수술시까지의 경과한 시간, 사용한 이식편, 치료의 결과 등을 후향적으로 분석하였다. 결과: 33명 중 32명이 남자였고 재수술 당시의 평균연령은 63.5세였다. 첫 수술에서 재수술까지의 기간은 11.9개월이었고 원인질환은 동맥경화증이 28예, 버거씨병 5예였다. 동반된 질환은 고혈압 19예, 당뇨병 11예, 심부전 6예 등의 순이었다. 한 환자당 평균 1.67회의 재수술이 시행되었고 치초의 수술유형은 대퇴동맥-슬와동맥 우회술이 19예로 가장 많았다. 수술에 사용되었던 이식편은 PTFE가 25예, Dacron이 6예였고 이식편에 따른 재수술률은 차이가 없었다. 재수술은 혈전제거술이 20예, 혈관 성형술 18예, 우회로 재조성술이 13예, 요부 교감신경절제술이 4예에서 시행되었다. 재수술의 결과는 하지의 기능회복이 15명, 하지보존 7명, 슬상부절단 5명, 슬하부 절단 3명, 사망 3명이었다. 걸론· 하지 동맥 재건술 후 발생하는 폐쇄성 합병증은 유입 동맥이나 유출동맥의 폐쇄가 발생하며 생기는 경우가 많았다 치료의 방침은 최초의 수술방법과 사용된 이식편에 따라 다를 수 있고 무엇보다도 이식편의 감시가 중요한데 이차 수술의 시행빈도가 가장 높은 시기인 술 후 1년 이내의 이식편 감시가 중요하다고 생각한다. 또한 재우회로술 뿐만 아니라 방사선학적 중재술 및 교감신경 절제술과 같은 부가적인 치료도 적극적으로 시행하여야 하지 보존율의 향상을 기대할 수 있을 것이다.

척추경 나사못 고정술을 이용한 단일 흉추 청소년기 특발성 척추 측만증의 치료: 스테인리스강과 티타늄 합금 기기의 비교 (Correction of Single Thoracic Adolescent Idiopathic Scoliosis Using Pedicle Screw Instrumentation: Comparison of Stainless Steel to Titanium Alloy Instruments)

  • 목적: 단일 흉추 청소년기 특발성 척추 측만증의 수술적 치료로 척추경 나사못 고정술이 시행된 경우에 서로 다른 재질인 스테인레스강과 티타늄 합금 기기의 결과를 비교해 보고자 하였다. 대상 및 방법: 척추경 나사못 고정술과 선택적 흉추 유합술을 이용하여 수술을 시행하고 최소 2년 이상 추시가 가능하였던 단일 흉추 청소년기 특발성 척추 측만증 환자 141명을 후향적으로 조사하였다. 주 흉추 만곡이 40°-75°인 환자를 대상으로 하였으며, 기기의 재질에 따라 스테인레스강 기기가 사용된 경우는 S군(90명)으로, 티타늄 합금 기기가 사용된 경우는 T군(51명)으로 나누었다. S군의 강봉 직경은 7.0 mm였고 T군의 강봉 직경은 6.35 mm나 6.0 mm였다. 수술 전, 수술 직후와 술 후 2년에 촬영한 기립성 전 척추 방사선 사진을 이용하여 방사선적 측정을 시행하였다. 술 전 관상면과 시상면상 만곡의 측정값에서 두 군 간에 유의한 차이는 없었다. 결과: S군에서 술 전 51.3°±8.4°의 주 흉추 만곡은 술 후 2년에 19.0°±7.6° (63.1% 교정)로 감소되었고, 술 전 32.3°±8.4°의 요추 만곡은 술 후 2년에 12.7°±8.2° (62.9% 교정)로 감소되었다. T군에서는 술 전 49.5°±8.4°의 주 흉추 만곡과 30.3°±8.9°의 요추 만곡은 술 후 2년에 각각 18.8°±7.4° (62.2% 교정)와 11.3°±5.4° (63.3% 교정)로 감소되었다. 관상면상 만곡의 교정은 두 군 간에 통계적으로 유의한 차이는 없었다(p>0.05). 흉추 후만은 S군에서 술 전 16.8°±8.5°에서 술 후 2년에 24.3°±6.1°로, T군에서는 19.6°±11.2°에서 26.6°±8.5°로 증가되었다. 유합 분절수, 사용된 척추경 나사못의 개수 및 술 후 2년의 흉추 후만, 요추 전만, 관상면과 시상면 균형에도 두 군 간에 유의한 차이는 없었다(p>0.05). 결론: 척추경 나사못 고정술을 이용한 단일 흉추 청소년기 특발성 척추 측만증 수술에서 스테인레스강과 티타늄 합금 기기와 강봉은 관상면과 시상면에서 의미 있는 차이 없이 비슷한 교정을 보였다.

Anatomical and Functional Recovery of Neurotized Remnant Rectus Abdominis Muscle in Muscle-Sparing Pedicled Transverse Rectus Abdominis Musculocutaneous Flap

  • Background Pedicled transverse rectus abdominis musculocutaneous flaps typically sacrifice the entire muscle. In our experience, the lateral strip of the rectus abdominis muscle can be spared in an attempt to maintain function and reduce morbidity. When the intercostal nerves are injured, muscle atrophy appears with time. The severed intercostal nerve was reinserted into the remnant lateral strip of the rectus abdominis muscle to reduce muscle atrophy. Methods The authors retrospectively reviewed 9 neurotized cases and 10 non-neurotized cases. Abdominal computed tomography was performed to determine the area of the rectus muscles. Electromyography (EMG) was performed to check contractile function of the remnant muscle. A single investigator measured the mean areas of randomly selected locations (second lumbar spine) using ImageJ software in a series of 10 cross-sectional slices. We compared the Hounsfield unit (HU) pre- and postoperatively to evaluate regeneration quality. Results In the neurotization group, 7 of 9 cases maintained the mass of remnant muscle. However, in the non-neurotization group, 8 of 10 lost their mass. The number of totally atrophied muscles in each of the two groups was significantly different (P=0.027). All of the remnant muscles showed contractile function on EMG. The 9 remaining remnant rectus abdominis muscles showed declined the HU value after surgery but also within a normal range of muscle. Conclusions Neurotization was found to be effective in maintaining the mass of remnant muscle. Neurotized remnant muscle had contractile function on EMG and no fatty degeneration by HU value.

Unilateral Isthmus Resection for Elderly Foraminal Stenosis

  • We present an elderly patient with unilateral foraminal stenosis treated by isthmus resection. An 83-year-old female could not walk due to severe leg pain along right L5 sensory dermatome. Despite the laminotomy for spinal stenosis on the right side at the L4-5 level, her leg pain did not improve. Careful review of computed tomography scans and coronal source images of magnetic resonance myelography revealed foraminal stenosis on the right side at the L5 vertebra. Because of medical problem, she underwent isthmus resection on the right side at the L5 level instead of total facetectomy and fusion. After surgery, her leg pain was markedly improved. Isthmus resection showed successful result for this medically compromised elderly patient with unilateral foraminal stenosis.

Chronic Spinal Epidural Hematoma Related to Kummell's Disease

  • Chronic spinal epidural hematoma related to Kummell's disease is extremely rare. An 82-year-old woman who had been managed conservatively for seven weeks with the diagnosis of a multi-level osteoporotic compression fracture was transferred to our institute. Lumbar spine magnetic resonance images revealed vertebral body collapse with the formation of a cavitary lesion at L1, and a chronic spinal epidural hematoma extending from L1 to L3. Because of intractable back pain, a percutaneous vertebroplasty was performed. The pain improved dramatically and follow-up magnetic resonance imaging obtained three days after the procedure showed a nearly complete resolution of the hematoma. Here, we present the rare case of a chronic spinal epidural hematoma associated with Kummell's disease and discuss the possible mechanism.

Ligamentum Flavum Hematoma in the Adjacent Segment after a long Level Fusion

  • Ligamentum flavum hematoma (LFH) is a very rare condition of dural compression; most are observed in the mobile cervical and lumbar spine regions. A 67-year-old man who had a long level interbody fusion at L3-S1 four years ago presented with symptoms suggestive of dural compression. Magnetic resonance imaging showed a posterior semicircular mass located at the adjacent L2-L3 level. After decompression of the spinal canal and removal of the mass lesion, pathological examination of the surgical specimen revealed a hematoma within the ligamentum. The patient fully recovered to normal status after surgery. Here, we report our experience with a LFH in the adjacent segment after a long level fusion procedure and discuss the possible associated mechanisms.

Acute Spinal Epidural Hematoma Following Unilateral Laminectomy for Bilateral Decompression

  • We present a case of acute spinal epidural hematoma[EDH] following unilateral laminectomy for bilateral decompression[ULBD]. A 45-year-old male presented with severe multi-level spinal stenosis underwent ULBD on the left side at the L2-3 and L3-4 level. Five hours after operation, paraparesis developed along with severe bilateral buttock pain. The CT scan showed an acute spinal EDH at the L2-3 level. The acute spinal EDH was successfully decompressed after emergency decompressive surgery with performing an additional laminectomy on the contralateral side at the L2-3 level. Although ULBD is an effective minimally invasive surgical technique for treating spinal stenosis, the possibility of acute spinal EDH should be kept in mind, as happened in our case.

Unilateral Pedicle Fracture Accompanying Spondylolytic Spondylolisthesis

  • Unilateral pedicle stress fracture accompanying spondylolytic spondylolisthesis is rare even in the elderly. Most are associated with major trauma, previous spine surgery, or stress-related activity. Here, the authors describe an unique case of unilateral pedicle fracture associated with spondylolytic spondylolisthesis at the L5 level, which was successfully treated by posterior lumbar interbody fusion with screw fixation at the L5-S1 level. As far as the authors' knowledge, no such case has been previously reported in the literature. The pathophysiological mechanism of this uncommon entity is discussed and a review of relevant literature is included.

개의 흉·요추 디스크 질환에 대한 전침요법 시술 예 (Electroacupuncture therapy for the treatment of thoracolumbar disc disease in dogs)

  • Electroacupuncture therapy was performed in 2 dogs with thoracolumbar disc disease. Case was a 3-year-old female chihuahua with a calcified disc between the 13th thoracic and 1st lumbar vertebrae. Treatment was done by electroacupuncture at San sho yu, Shen yu, Huan tiao and Fu yang-San yin jiao with the current of 0.5~8 volts, 30 Hz for 30 min. She could pass feces and urine 2 days, and walk 9 days after the beginning of the therapy. Case 2 Was a 6-year-old male cocker spaniel with 4 calcified discs(T12-T13, T13-L1, L2-L3 and L6-L7) and spondylosis(lumbosacral junction). Treatment was done by electroacupuncture at Tian ping, Bai hui and Pung kung yu with the current of 2~4 volts, 30 Hz for 20 min. He could pass feces 6 days, urine 8 days, and stand 21 days, after the beginning of the therapy.

Spontaneous Resolution of Nontraumatic Acute Spinal Subdural Hematoma

  • Spinal subdural hematoma (SSDH) is an extremely uncommon condition. Causative factors include trauma, anticoagulant drug administration, hemostatic disorders, and vascular disorders such as arteriovenous malformations and lumbar punctures. Of SSDH cases, those that do not have any traumatic event can be considered cases of nontraumatic acute spinal subdural hematoma, which is known to have diverse clinical progress. Treatment typically consists of surgical decompression and cases in which the condition is relieved with conservative treatment are rarely reported. We report two nontraumatic acute spinal subdural hematoma patients who were successfully treated without surgery.
