Interspinous spacers have been developed as an alternative surgical treatment for laminectomy or fusion with pedicle screws and rods for the treatment of lumbar spinal stenosis. However, its biomedical efficacies are well not known. In this study, we evaluated kinematic behaviors of the surgical and the adjacent levels before and after inserting interspinous spacers. Three porcine lumbar spines were prepared. On each specimen, an interspinous spacer was inserted at the L4-L5. Flexion-extension moments (0, 2.5, 5.0, 7.5, 10Nm) were applied. A stereophotogrammetric set-up with DLT algorithm was used to assess the three-dimensional motions of the specimen where three markers $({\square}0.8mm)$ were attached to each vertebra. Results showed that extension motion decreased by $15-24\%$ at the surgical level (L4-L5) after insertion of interspinous spacer. At the adjacent levels, the range of motion remained unchanged. In flexion, no significant changes in motion were observed regardless of levels. Therefore, our experimental results demonstrated the interspinous spacer is very effective in limiting the extension motion that may cause narrowing of the spinal canal and vertebral foramen while maintaining kinematic behaviors at the adjacent levels. Further, these results suggested that the use of interspinous spacer may be able to prevent lower back pain at the surgical level and to lower the incidence of degenerative changes at the adjacent levels.
Objectives : This study was performed to evaluate the effects of Chuna treatment (Saban-method technique) on acute lumbar sprain patients with limited range of motion(LOM). Methods : Three patients suffered from acute lumbar sprain with LOM, were treated with Chuna therapy(Saban-method technique), acupuncture, herbal medicine, physical therapy and measured by VAS(Visual Analogue Scale) and ODI(Oswestry Disability Index). Results : After Chuna treatment and oriental medical treatments, we found out a recovery from three patients suffering from acute low back pain with LOM. Conclusions : Through this study, we suggest that Chuna treatment(Saban-method technique) and oriental medical treatments was effective to cure acute lumbar sprain patient with LOM.

Lumber spine range of motion has been used to measure of physical and functional impairment by various tools from a ruler to 3D kinematic devices. However, pre-existing tools have problems in either movement or accuracy and reliability limitations. Accurate devices are limited by fixed space whereas simple devices are limited in measuring complex movements with less accuracy. In order to solve the location, movement and accuracy limitations at once, we have developed a novice measurement device equipped with accelerometer sensor and gyroscope sensor for getting three-dimensional information of motion. Furthermore, Kalman filter was applied to the algorithm to improve accuracy. In addition, RF wireless communication was added for the user to conveniently check measured data in real time. Finally, the measurement method was improved by considering the movement by a reference point. An experiment was conducted to test the accuracy and reliability of the device by conducting a test-retest reliability test. Further modification will be conducted to used the device in various joints range of motion in clinical settings in the future.
Many recent studies suggest that the posterior dynamic stabilization(PDS) can be a more physiologically-relevant alternative to the rigid fixation for the patients suffering from low back pain. However, its biomechanical effects or clinically proven efficacies still remain unknown. In this study, we evaluated kinematic behaviors of the lower lumbar spine with the PDS system and then compared to those of the rigid fixation system using finite element (FE) analysis. A validated FE model of intact lumbar spine(L2-L5) was developed. The implanted model was then constructed after modification from the intact to simulate two kinds of pedicle screw systems (PDS and the rigid fixation). Hybrid protocol was used to flex, extend, laterally bend and axially rotate the FE model. Results showed that the PDS systems are more flexible than rigid fixation systems, yet not flexible enough to preserve motion. PDS system allowed $16.2{\sim}42.2%$ more intersegmental rotation than the rigid fixation at the implanted level. One the other hand, at the adjacent level it allowed more range of motion ($2.0%{\sim}8.3%$) than the rigid fixation. The center of rotation of the PDS model remained closer to that of the intact spine. These results suggest that the PDS system could be able to prevent excessive motion at the adjacent levels and restore the spinal kinematics.

Objectives: The purpose of this study was to investigate the effect of intensive rehabilitation programs on pain, range of motion (ROM), lumbar muscle strength, core muscle endurance, disability, and depression in patients with traumatic low back injuries and to compare the efficacy of this therapy with that of conventional rehabilitation therapy. Methods: The study was performed with a retrospective medical chart review of patients with traumatic low back injury referred to the rehabilitation center at the Daegu Hospital of the Korean Workers Compensation and Welfare Service. Forty-four patients were allocated to either the conventional rehabilitation group (CRG; n = 22) or the intensive rehabilitation group (IRG; n = 22). The CRG group patients, who received 30-min therapist-supervised physical therapy and modality therapy five times per week for four weeks, were compared with the IRG group patients, who received 60-min therapist-supervised physical therapy, 30-min therapist-patient 1:1 matching rehabilitation therapy, and modality therapy five times per week for four weeks. Outcome measures were a numerical rating scale, ROM, lumbar muscle strength, lumbar core muscle endurance, thickness of lumbar deep focal core muscle (transverse abdominis and lumbar multifidus), Oswestry disability index (ODI), and depression (Korean version patient health questionnaire-9). Results: There were statistically significant improvements after treatment in all outcome measures in both groups (p < 0.05). In the intergroup comparison, NRS scores on the activity and thickness of lumbar deep focal core muscles increased significantly more in the IRG than in the CRG (p < 0.05). There were no statistically significant intergroup differences in NRS scores on resting, ROM except left lateral bending, lumbar muscle strength, core muscle endurance, ODI, and depression. Conclusions: We could confirm the superior effectiveness of an intensive rehabilitation program compared to conventional rehabilitation therapy in patients with traumatic low back injuries.

Purpose : The purpose of this study was to compare differences in thoracolumbar ranges of motions by comparing ratios among 4 type diagnosis for patient with chronic low back pain. Methods : The subjects were 58 chronic low back pain patients. A motion analysis program (Global Postural System) was used after photography for posture measurement. To analyze differences in mobility percentages and ratios of thoracolumbar ranges of motion, one-way ANOVA was used. Results : Regional difference spinal posture and movement were found to diagnosis patients with chronic low back pain. Comparison of thoracolumbar ranges of motion revealed significant differences in the thoracic region during forward-bending of the trunk (p<.05). In the upper thoracic region, the herniated intervertebral lumbar disc (HILD) group was significantly larger than the spinal stenosis/herniated intervertebral lumbar disc (SS/HILD) group (p<.05). In the lower thoracic region, chronic sprains (CS) were significantly greater than in the spinal stenosis (SS) group and in the (SS/HJLD) group (p<.05). Comparative analysis of thoracic/lumbar mobility ratio showed the CS group's ratio during forward bending was largest: 1.66, while the HILD group's ratio was smallest: 84 a significant difference between the two groups (p<.01). Diagnosis was not associated with difference in thoracolumbar backward-bending range of motion (p>.05). Conclusion : Theses results indicate the clinical efficacy of diagnosing for chronic low back pain by evaluating spinal mobility.
The purpose of this study is to understand motion characteristics of older drivers during reaching seat belt compared to young drivers and to provide design guidelines in order to reduce discomfort for the elderly. The whole body kinematics of each subject was captured using 12-camera motion analysis system. Subjective ratings on discomfort levels were obtained simultaneously using a questionnaire. This paper first presents the result of motion characteristics of elderly drivers' reach motion to seat belt. Compared to young drivers, older drivers performed seat belt reach motions less efficiently and moved slower due to mostly the movement error. Older drivers also made use of reduced joint range of motion in cervical left rotation, lumbar left rotation and right shoulder adduction, which can be explained by their reduced active range of motions (AROMs). To compensate for their reduced joint range of motion, older drivers rotated pelvis more.
Objective: The purpose of this study was to investigate the effect of spinal mobilization with leg movement (SMWLM) and neural mobilization (NM) in patients with lumbar disc herniation (LDH) accompanied by radiating pain. Design: Three-group pre-test-post-test control group design. Methods: We enrolled 48 participants, whom we randomly assigned to three groups. The SMWLM group (n=16) underwent 20 min of conventional physical therapy (CT) and 20 min of SMWLM. The NM group (n=16) underwent 20 min of CT and 20 min of NM. The control group (n=16) underwent 20 min of CT. These interventions in all the groups were performed three times a week for 4 weeks. Numeric pain rating score (NPRS), body grid chart score (BGCS), passive straight leg raise (PSLR), active lumbar flexion range of motion (ALFROM), korean version oswestry disability index (KODI), and korean version fear avoidance beliefs questionnaire (KFABQ) were measured pre- and post-intervention. Results: In all three groups, the NPRS, PSLR, KODI, and KFABQ scores were significantly different pre- and post-intervention (p<0.05). Significant differences were observed in BGCS and ALFROM in the SMWLM and NM groups pre- and post-intervention (p<0.05). The SMWLM group showed more improvement in the NPRS of leg pain, ALFROM, and KFABQ score than that exhibited by the NM and control groups (p<0.05). Conclusions: Both SMWLM and NM were effective for improving back and leg pain, centralization of symptoms, mechanical sensitivity, lumbar mobility, lumbar functional disability, and psychosocial functioning in patients with LDH with radiating pain.

Background: In some clinical guidelines followed in clinical practice, nonsurgical treatments are recommended as the primary intervention for patients with lumbar disc herniation (LDH). However, the effect of a therapeutic exercise program based on stabilization of the lumbar spine for treatment of multilevel LDH has not been evaluated thoroughly. Objective: To investigate the effects of therapeutic exercise on pain, physical function, and magnetic resonance imaging (MRI) findings in a patient with multilevel LDH. Design: Case Report Methods: A 43-year-old female presented with low back pain, radicular pain and multilevel LDH (L3-L4, L4-L5, L5-S1). The therapeutic exercise program was conducted. in 40-min sessions, three times a week, for 12 weeks. Low back and radicular pain, lumbar disability, and physical function were measured before and after 6 and 12 weeks of the exercise program. MRI was performed before and after 12 weeks of the program. Results: After 6 and 12 weeks of the therapeutic exercise, low back and radicular pain and lumbar disability had decreased, and lumbar range of motion (ROM) was improved bilaterally, compared with the initial values. Also improved at 6 and 12 weeks were isometric lumbar strength and endurance, and the functional movement screen score. The size of disc herniations was decreased on MRI obtained after 12 weeks of therapeutic exercise than on the pre-exercise images. Conclusions: We observed that therapeutic exercise program improved spinal ROM, muscle strength, functional capacity, and size of disc herniation in LDH patient.
The number of the subjects of this study were all sixteen including 8 male and 8 female. They were chronic low back pain patients and treated at hospital out patient. The patients took lumbar extension strength training. The results of examination and analysis isometric lumbar extension muscular strength before and after the training are as follows; 1. TFT increased after lumbar extension muscular strength exercise than before the exercise. Inspire of difference of each angle. The reciprocal action didn't happen between training and angle. 2. NMT increased after lumbar extension muscular strength exercise than the before the exercise and the difference of each angle didn't happen. The reciprocal action also didn't happen between the training and angle. 3. Comparing presented standard point with individual point, 13persons' muscle strength of first step increased, on the other and 3 person's muscle strength a little increased in a same category. 4. The grade of low back pain decreased for all subjects. Concluding this study, lumbar extension muscular strength exercise through isokinetic device decreases the grade of low back pain and increase lumbar extension muscular strength. It is that equal exercise effect happen in full range of motion, for the exercise effect doesn't show the difference of each angle.
