Effects of Joint Mobilization on The Range of Motion and Pain of Patient with Chronic Low Back Pain (관절가동술이 만성요통 환자의 기동범위와 통증에 미치는 영향)

  • Purpose : The purpose of this study was to investigate the effects of joint mobilization on the rang of motion and pain of patient with chronic low back pain. Methods : The subjects were consisted of thirty patients with chronic low back pain(19 females, 11 males ; mean aged 59.93) from 50 to 71. All subjects randomly assigned to the modalities treatment group, joint mobilization group. Modalities treatment group received hot pack used thermal therapy for 20minutes and ICT used electrical therapy for 15minutes, joint mobilization group received modalities treatment with sustained natural apophyseal glides(SNAGS) techniques of Mulligan for 10minutes per day and three times a week during 2 weeks period. Visual Analogue Scale(VAS) was used to measure subjective pain level. Remodified Schober test(RST) was used to measure range of motion of lumbar spine. All measurements of each patients were measured at pre-treatment and 2 weeks post-treatment. Results : The results of this study were summarized as follows : 1. VAS was joint mobilization group showed significantly decreased more than modalities treatment group (p<.05). 2. The lumbar flexion range of motion was joint mobilization group showed significantly decreased more than modalities treatment group(p<.05). 3. The lumbar extension range of motion was joint mobilization group showed significantly decreased more than modalities treatment group(p<.05). 4. The lumbar left lateral flexion range of motion was joint mobilization group showed significantly decreased more than modalities treatment group(p<.05). 5. The lumbar right lateral flexion range of motion was joint mobilization group showed significantly decreased more than modalities treatment group(p<.05). Conclusion : These data suggests that SNAGS of Mulligan is beneficial treatment for chronic low back pain.

Comparison of Radiography Findings and Magnetic Resonance Image Findings of Lumbar Spine Instability Patients (요추 불안정 환자에서 단순방사선 소견과 자기공명영상 소견의 비교)

  • Purpose: This study was to investigate how dose the radiography findings are to magnetic resonance (MR) image findings in the L5-S1 instability patients. The subjects of this study were comprised of eleven males and fifteen females, who had Lumbago and agreed with this research. Methods: Radiography and MR images of Lumbar spine were acquired respectively from subjects in conditions of maximum flexion and extension. The horizontal and angular displacements in lumabosacral spine radiography were used to assess the instability of lumbar spine. MR images were also used to evaluate the intervertebral disc abnormalities and change of bone marrow. Results: The results are as follows. 1. In the case of flexion transitional displacement proposed by Dupuis et al, the specificity and negative predictive value were good accuracy ($0.7{\sim}0.8$), and the negative predictive value was in average. In the case of extension displacement, the negative predictive value was about average ($0.6{\sim}0.7$), but the sensitivity, specificity and positive predictive value were below the poor (<0.6). On the other side, the specificity was about average but other things were below in the case of angular displacement. 2. In the case of flexion transitional displacement proposed by Dupuis et al., compared with the intervertebral disc abnormalities, the negative prediction value was excellent, the sensitivity good, and the specificity about average. In the case of extension, the negative prediction value was about average, but the other things were poor. On the other side the specificity and negative predictive value had good accuracy and the sensitivity and positive prediction value were below average in the case of angular displacement. Conclusion: The above results show that the radiography finding is sufficiently helpful to find the lumbar spine instability as an economic point of view.

Changes in the Cervical and Lumber Flexion-Relaxation Ratio, Range of Motion, Pressure Pain Threshold, and Perceived Comfort Following the Wearing of a Trunk Brace during Smartphone Watching (스마트폰 시청 시 몸통 교정기 착용에 따른 목, 허리 굽힘-이완 비율, 관절가동범위, 압통, 착용감 변화)

  • Purpose: This study was conducted to investigate changes in the cervical and lumbar flexion-relaxation ratio, range of motion, pressure pain threshold, and perceived comfort following the wearing of a trunk brace during smartphone watching. Methods: To calculate the number of subjects for this study, an analysis with G*Power was performed at a statistical power of 0.8, an effect size of 0.5, and a significance level of 0.05, based on the results of a preliminary experiment on five subjects. In total, 27 adult men and women were recruited who had been informed of the study's purpose and process and had agreed to participate. All subjects watched content on a smartphone for 20 minutes in the same posture and conditions while wearing and not wearing a trunk brace, and then their cervical and lumbar flexion-relaxation ratio, range of motion, pressure pain threshold, and perceived comfort were measured. Results: Compared to the non-wearing of a trunk brace, the wearing of a trunk brace resulted in a statistically significant smaller decline in cervical extension and right-side cervical rotation (p<0.05). When the subjects wore a trunk brace, their right- and left-side cervical and right-side lumbar pressure pain statistically significantly improved when compared to not wearing a trunk brace (p<0.05). They also perceived a significantly lowered level of comfort 20 minutes after wearing a trunk brace compared to immediately after wearing it (p<0.05). Conclusion: The trunk brace was effective in reducing declines in right-side cervical rotation and the occurrence of left- and right-side cervical and right-side lumbar pressure pain. The findings indicate the need to improve the perceived comfort of trunk braces.

Comparison of the Effects of Abdominal Bracing Exercises and Abdominal Hollowing Exercises on Lumbar Flexibility and Pulmonary Function in Healthy Adults

  • Background: Abdominal bracing exercise (ABE) and abdominal hollowing exercise (AHE) improve the lumbar flexibility and pulmonary function in various patients, yet the efficacy of ABE or AHE have not yet been evaluated. Objects: The purpose of this study was to compare the lumbar flexibility and pulmonary function during both ABE and AHE in healthy adults. Methods: The study included 40 healthy adults, who were randomly divided into the experimental group and control group, each with 20 subjects. All subjects performed ABE (experimental group) and AHE (control group). The lumbar flexibility such as trunk flexion test (sitting and standing position) and schober test and pulmonary function such as the spirometer including forced vital capacity (FVC) and force expiratory volume in one second ($FEV_1$) and chest circumference measurement (middle and lower chest) were measured, respectively. Two-way repeated analysis of variance was used to compare the lumbar flexibility and pulmonary function, respectively. Results: No significant effects of lumbar flexibility were observed on trunk flexion test from the sitting position (P=.478) and standing position (P=.096) in the ABE than in the AHE. However, the length of ABE was longer significantly than it of AHE (P=.024). No significant effects of lung function were observed on the FVC (P=.410) and $FEV_1$ (P=.072) in the ABE group than in the AHE group. And also, no significant effects of chest circumference measurement were observed on the inspiration (P=.468) and expiration (P=.563) in middle chest circumference and inspiration (P=.104) and expiration (P=.346) in lower chest circumference. Conclusion: This study indicated that the ABE is only more effective in lumbar flexibility by lumbar length difference than AHE in healthy adults.

Change of forward Head Posture in Cervical Flexion Positions while Watching Video on a Smartphone (스마트폰을 이용한 비디오 시청 시, 목뼈 굽힘 각도에 따른 앞쪽 머리 자세의 변화)

  • Background: Flexion of cervical and lumbar joints is required when viewing a smartphone screen. Thus, these joints are overused, together with the surrounding joints and muscles. Long-term use of smartphones will cause changes in cervical and lumbar posture. The effect on forward head posture will vary, depending on the angle of cervical flexion start position in relation to the smartphone. This study investigated how forward head posture changes over time when using a smartphone at 20° and 40° cervical flexion start positions. Design: Cross-sectional study. Methods: Twenty-five subjects with a forward head posture angle of 35° or less participated in the study. A Forward Head Posture app on the participants' smartphones measured forward head posture 5 and 10 minutes after watching videos on their smartphones. Cervical range of motion was used to set a smartphone watching start posture of 20° and 40° of cervical flexion. Results: There was no significant difference in forward head posture, irrespective of cervical flexion start position, but the angle of forward head posture increased more at cervical flexion of 40° than at cervical flexion of 20°. There was no significant difference in what according to smartphone video viewing times, but the angle of forward head posture increased over time. Conclusion: An increase in forward head posture over time with smartphone usage poses a potential risk of neck and shoulder pain. Therefore, smartphone users should avoid prolonged screen time.

The effect of stabilization exercise and movement with mobilization on flexibility and pain of patients with acute low back pain (관절 가동술과 안정화 운동이 급성 요통 환자의 유연성과 통증에 미치는 영향)

  • Purpose : The purpose of this study was to investigate effects of movement with mobilization(MWM) and stabilization exercise on pain and range of motion of patients with acute low back pain. Methods : The subjects were consisted of 24 patients who had non specific acute low back pain. All subjects randomly assigned to the MWM group and the stabilization exercise group. The MWM group received sustained natural apophyseal glides(SNAGs) with modality treatment and stabilization exercise group received stabilization exercise with modality treatment. The remodified schober test(RST) was used to measure forward flexion and lateral flexion range of motion of lumbar segment. Visual Analogue Scale(VAS) was used to measure subjective pain level of the patients. The Oswestry Low Back Pain Disability Scale was used to measure functional disability level of the patients. Results : The lumbar flexion range of motion of MWM group was significantly increased compared with stabilization exercise group(p<.01). The range of motion of lumbar segment of MWM group was significantly decreased compared with stabilization exercise group(p<.01). The left lateral flexion range of motion of lumbar of MWM group was significantly decreased compared with stabilization exercise group(p<.05). The right lateral flexion range of motion of lumbar of MWM group was significantly decreased compared with stabilization exercise group(p<.05). The VAS of both MWM group and stabilization exercise group was significantly decreased(p<.001). The Oswestry Low Back Pain Disability Scale of stabilization exercise group was significantly increased compared with mobilization group(p<.001). Conclusion : In the result of this study, mobilization with movement and stabilization exercise had significant difference on pain and flexibility of patients with acute low back pain.

The Effect of Kinesio Taping on the Change of Muscle Strength and Endurance in Trunk Flexion and Extension in Chronic Low Back Pain(CLBP) (만성요통 환자의 테이핑 적용 시체간의 굴곡과 선전의 근력과 지구력의 변화 측정)

  • In this study, the muscle strength and endurance of the lumbar at flexion and extension were determined using an isokinetic muscle strength meter (Biodex) in patients with chronic lumbar go (10 male and 10 females), and the changes in muscle strength and endurance of the lumbar at flexion and extension after application of Y-shaped sacrospinalis muscle taping, typically used for patient with lumbar go, were studied. In addition, the sacrospinalis muscle taping of a different shape (I-shaped taping) was applied bilaterally centering on the spine, and the muscle strength and endurance of the lumbar at flexion and extension were determined and compared with those before taping. In addition, the results after application of Y-shaped taping and I-shaped taping were also compared. 1. The extensor muscle strength of the lumbar at loading of $60^{\circ}$/sec before application of kinesio Taping and after application of Y-shaped taping showed the level of significance (p<.05). The flexor muscle strength of the lumbar at loading of $60^{\circ}$/sec before application of kinesio taping and after application of Y-shaped taping did not show the level of significance. 2. The flexor muscle endurance of the lumbar at loading of $90^{\circ}$/sec before application of kinesio Taping and application of Y-shaped taping did not show the level of significance.   • Purpose : The purpose of this study was to investigate the effects of Swiss ball lumbar stabilization exercise on the strength and flexibility, balance for the subjects. 30 university students who aged an average $23.21{\pm}4.41$ years. Methods : The exercise program lasted for 4 weeks with a frequency of 3 times per week within 12 to RPE. The result was analyzed by case study were measured by sit-up for strength, and back muscle strength, and trunk flexion test and trunk extension lifting for flexibility, and also measured by balance test before and during, and after the Swiss ball exercise program. Statistical analysis was performed using SPSS/Win(15.0) version and t-test. Results : By the result of this study, we found that the number of performing sit-up strength was increased, and back muscle strength was increase, and trunk flexion test and trunk extension lifting was increased after the 4 weeks Swiss ball exercise program. Conclusion : These finding indicate that Swiss ball lumbar stabilization exercise using Swiss ball could be beneficial to the subjects. The effect of increasing strength and flexibility, balance was flexibility.

Comparison of the Activity and Proprioception of Trunk Muscles According to Different Types of Bridge Exercises in Subjects with and without Chronic Low Back Pain

  • Purpose: The purpose of the present study was to examine joint position senses and muscle activity in subjects with and without chronic low back pain and to determine the effects of different types of bridge exercises on their trunk muscle activity. Methods: Thrity-eight subjects with chronic low back pain and thrity healthy controls participated in the experiment. Joint position senses and trunk muscle activity levels were measured during the different bridge exercise methods. Results: The joint position senses of the healthy group and chronic low back pain group showed significant differences during lumbar flexion, lumbar extension, lumbar lateral flexion, and lumbar rotation. The muscle activity levels of the transversus abdominis (TrA), internal oblique (IO), and external oblique (EO) were highest in the prone bridge exercise (PBE) group, followed by the supine bridge swiss ball exercise (SBSE) group and supine bridge exercise (SBE) group in order of precedence. The muscle activity level of the erector spinae (ES) was highest in the SBSE group, followed by the SBE and PBE groups in order of precedence. Conclusion: Overall, the results suggest that chronic low back pain is associated with declines in joint position senses and that PBEs increase trunk muscle activity more than conventional bridge exercises.

The Effects of Pilates Mat Exercises on Elderly Women's Basic Physical Strength and Lumbar Muscle Strength

  • The purpose of this study is to examine the changes on lumbar muscle strength in relation to 12-week pilates mat exercise of elderly women. The participants are chosen for 17 people who is an experimental group and 14 people who is an comparison group who except wastage and pilates mat exercise program was gradually intensify applied three times a week, during 12 weeks. The peak torque of flexion was increased(p<.01) and the difference of ratio of agonist to antagonist was decreased(p<.001). The total work of flexion and extension had significant increased(p<.001) at $120^{\circ}/sec$. As a result of all these, it was judged as positive effect for body composition, basal physical fitness and lumbar muscle strength after pilates mat exercise in 12 weeks. These positive effects showed that pilates mat exercise could got conclusions that mitigated the musculoskelctal disorders, slow down the progress of disorders, helped as a functions of physical.
