토끼의 방사선 촬영상과 초음파 촬영상에 나타난 신장 크기의 비교평가 (Comparative evaluation of renal size on radiography and ultrasonography in rabbits)

  • 토끼에서 신장의 크기(길이, 폭 및 높이)를 측정하고 이에 대한 기본자료로 삼고자 53마리의 토끼(Newzealand White)의 신장을 방사선 촬영과 초음파 신장촬영 그리고 신장을 적출하여 실제 길이를 재고 이들간의 측정치를 비교하였다. 방사선 촬영에서는 우측신장은 T13-L2에서 관찰되었고 좌측신장은 L2-L4에서 관찰되었다. 초음파 신장촬영상에서는 신장 피질은 작고 균등하며 주변조직에 비하여 저에코상을 보였고 신장의 신우는 무에코 내지는 약간의 저에코상을 보였다. 토끼의 좌측신장에서 길이, 폭 및 높이는 각각 $35.84{\pm}3.12(mean{\pm}SD)$, $23.52{\pm}3.21$$15.11{\pm}2.58cm$ 였으며 우측신장에서는 $36.02{\pm}3.42$, $23.69{\pm}3.50$$14.13{\pm}3.55cm$ 이었다. 방사선 사진상에서 양쪽 신장의 길이와 폭은 실제 크기에 비하여 약간 확대되었으며(102~104%), 초음파신장 촬영상에서 양쪽 신장의 길이, 폭 및 높이는 실제 크기에 비하여 적게 축소된 상을 보였다(70~96%). 복배상 방사선 사진에서 신장의 길이와 폭의 제2요추 길이의 대비에서 1.85와 1.25배로 나타났다. 체중과 신장의 각 측정치와의 상관관계에서는 서로간에 유의치 있는 상관관계가 있었다.

족관절 골절 환자의 골밀도 분석 (Analysis of Bone Mineral Density of Ankle Fracture Patients)

  • 목적: 족관절 골절과 골다공증의 상관관계를 알아보기 위하여 족관절 골절 환자의 요추체와 대퇴골의 골밀도 값을 분석하였다. 대상 및 방법: 2002년 4월부터 2014년 7월까지 족관절 골절로 진단받고, 수상 후 1년 이내 골밀도 검사를 시행했던 100명의 환자를 분석하였다. 수상 당시 연령에 따라 50세 미만, 50세 이상 70세 미만, 70세 이상의 세 군으로 나누었고, 골절의 유형은 단과골절, 양과골절, 삼과골절로 분류하였다. 골밀도는 요추체(요추 1-4번 평균치), 대퇴골 경부, 대퇴골 전자부, 대퇴골 전체의 T점수, Z점수 및 골밀도 절대값(g/cm2)을 분석하였다. 결과: 족관절 골절 환자는 여자의 수가 남자보다 3.2배 많았다. 연령별 군에 따른 골다공증의 유병률은 50세 미만 군에서 0%, 50-69세 군에서 24.2%, 70세 이상 군에서 15%였고, 이중 50-69세 군에서 삼과골절 환자의 30%가 골다공증으로 나타났다. 전체 환자 군에서 연령이 낮을수록 단과 골절의 빈도가 높게 나타났다. 골밀도 값과 골절 유형과의 관계는 요추체의 T점수와 골밀도 절대값(g/cm2) 및 대퇴골 경부의 Z점수가 낮을수록 삼과 골절의 빈도가 증가하였으나, 이외의 다른 지표는 골절의 유형과 통계적으로 뚜렷한 상관관계를 보이지 않았다. 결론: 100명의 족관절 골절 환자를 분석한 결과, 여자의 수가 남자보다 많았으며 골다공증의 정도가 심하지 않을수록 삼과골절보다는 단과골절의 발생 빈도가 높은 경향을 보였지만, 족관절 골절과 대퇴골 및 요추의 골밀도 값과의 상관관계는 통계적으로 의미있게 나타나지 않았다.

강직성척추염 환자의 1례 증례보고 (A Case Report on the Ankylosing Spondylitis Patient)

  • Objectives : Ankylosing Spondylitis, inflamatic disease, appeared symostosis, pain and dyscinesia in spine and sacroilic joint. It repeated an improvement and a depravation. We observed one patient with Ankylosing Spondylitis. Methods : This patient was juged incurable in points of western medical's View. But he was improved by treating oriental medical therapy. The therapies which used in this patient was acupunture, bee venom, korean chuna therapy and kinesiatrics etc. We measured the R.O.M, special test and degree of pain every day. Results : The R.O.M of extension in lumbar ${vertebrae(0_{\circ}\;{\rightarrow}5_{\circ}\;)\;and\;patrick\;sign(++/-\;{\rightarrow}\;-/-)}$ was improved a little. Degree of pain was improved better than admission. Conclusions : This case has demonstrated that the Ankylosing Spondylitis treated by oriental medical therapies proved good results.

Fracture analysis of wild mammals in South Korea

  • This study was conducted to evaluate and analyze fractures types in wild mammals in Korea. Wild mammals treated at 3 wildlife centers in Korea were included. Of the 63 cases included in this study, water deer was the most common species encountered with fractures. We determined 85 different fracture sites, with maximum lumber vertebral fractures followed by femoral, tibial and pelvic fractures. External skeletal fixator (ESF)-intramedullary (IM) pin (IM+ESF tie-in fixation) was the most common method applied for the surgical treatment of mammals. Lumbar vertebrae and femur were the most common bones involved. The findings of this study are expected to provide information for veterinarians regarding the frequency and characteristics of fractures in wild mammals and serve as a basic database for the types and outcomes of fractures in wild mammals.

A Study on Asymmetric Lifting Capacity Due to Spine Deformity

  • Scoliosis can be biomechanically described as a three dimensional deformity of the spine, with deviations from the physiologic curves in the sagittal and frontal planes, usually combined with intervertebral rotation. Various factors are suspected such as genetic defects, uneven growth of the vertebrae, hormonal effects, abnormal muscular activity, postural problems, or a mix of some of these elements, but its initial cause is known in only 15-20% cases. The screening test for diagnosing scoliosis is called the Adams Forward Bend Test. During the experiment, the subjects were asked to bend over, with arms dangling, until a curve could be observed. The Scoliometer was placed on the back of the subjects and used to measure the difference between the left and right apex of the curve in the thoracic, thoracolumbar and lumbar area. Then, the subjects were asked to perform Maximum Voluntary Contractions (MVCs) using the digital back muscle dynamometer in three different postures: (1) 0o (sagittally symmetric); (2) 30o from the mid-sagittal plane (clockwise); and (3) 30o from the mid-sagittal plane (counterclockwise). In addition to the experimental data, subject-dependent variables including Body Mass Index (BMI), percentage of body fat and muscle mass of left/right arms and legs were employed to reveal the cause of difference among three MVC conditions. All those variables were tested using statistical methods.

후두에 원발한 악성 흑색종 1례 (Primary Laryngeal Malignant Melanoma: Report of a Case with Review of Literatures)

  • Mucosal melanoma of the head and neck is a rare and usually lethal disease. Primary laryngeal malignant melanoma(LMM) are exceedingly rare tumors that morphologically are readily confused with more common types of laryngeal cancer. Treatment of choice for LMM is complete surgical excision and elective lymph node dissection is usually not recommended. The use of radiation or chemotherapy is generally thought to have no effect on local or distant disease and currently used as adjuvant therapy. The prognosis is extremely poor. We have experienced a 61 year old male patient with symptoms of foreign body and lump sense in throat. A dark pigmented polypoid mass was found on the right aryepiglottic folds with normal mobility of vocal cord. Total laryngectomy was performed under the diagnosis of malignant melanoma. Bone scan revealed multiple bony metastasis on ribs and lumbar vertebrae after 5 months of operation. There have been no evidence of recurrence at primary area. The patient died after 8 months of operation.

강직성 척추염에 수반된 양측성 측두하악관절 강직 (Ankylosing Spondylitis Associated with Bilateral TMJ Ankylosis)

  • A 31-year-old male with the complaint of severe limitation of mouth opening was referred to our department of Chonbuk National University Hospital. The physical status of the patient was hyposthenic. Extraoral examination showed no condylar movement of the both temporomandibular joints, no pain, no facial swelling or paresthesia. Intraoral examination showed several cervical caries on the upper anterior teeth, and gingival swelling on the whole dentition. Transcranial view showed no condylar movement, and narrowing of joint spaces. Chest P-A view showed straightening of thoracic, lumbar spine, and squaring of vertebrae of the same spines. Conventional lateral radiograph of cervical spine showed calcification of the intervertebral ligament. Computed tomograph showed extensive bone formation between temporal bone and the condylar head at both sides. Laboratory findings showed positive reaction on HLA-B27 histocompatibility antigen and increased level of IgA, IgG, ESR. Based on the clinical, radiographic, and the laboratary findings, final diagnosis was made as bony ankylosis of the both temporomandibular joints secondary to ankylosing spondylitis.

Skeletal Manifestations of Mucolipidosis II/III

  • Mucolipidosis (ML) is a kind of skeletal dysplasia. Characteristic X-ray findings of the bone may contribute to the early diagnosis and treatment of ML II/III. Skeletal radiographs show distinctive patterns at different ages: neonatal hyperparathyroidism, osteodystrophy (similar to chronic osteitis fibrosa cystica), and dysostosis multiplex. Patients with ML II/III show a mixture of osteodystrophic bone changes and atypical changes of dysostosis multiplex: proximal pointing of the metacarpals in the wrist, dysplastic changes in the lower third of the ilia, marked broadening of the ribs becoming oar-shaped, and beaking of the lower thoracic and lumbar vertebrae. In ML II, the osteodystrophy has clinical and radiographic features of neonatal hyperparathyroidism. In some neonatal subjects, chemical hyperparathyroidism is also demonstrated. After transient hyperparathyroidism in newborns, the progressive osteitis fibrosa cystica develops from 3-6 months of age. Patients with ML III show prominent skeletal involvement, particularly the destruction of vertebral bodies and the femoral heads. Intravenous pamidronate treatment is well tolerated, and it can produce clinical effects, with a reduction in bone pain and improvements in mobility in patients with ML III. In this review, the skeletal manifestations of ML II and III are investigated.

Imaging Diagnosis of Sacrocaudal Dysgenesis in a Shih-tzu Dog

  • A 10-month-old, intact female Shih-tzu dog with a pelvic limb ataxic gait, and urinary and fecal incontinence since birth, was examined by computed tomography and magnetic resonance imaging. The butterfly hemivertebra of the fourth lumbar vertebra, agenesis of the third sacral and coccygeal vertebrae, and spina bifida of the remaining sacral vertebra were observed on the computed tomography images. Magnetic resonance imaging revealed traction of the conus medullaris. The dog was diagnosed with sacrocaudal dysgenesis with presumptive tethered cord syndrome, and concurrent infectious cystitis was found by ultrasonography and urinary analysis. The Shih-tzu recovered from the cystitis, but still has the ataxic gait and urinary and fecal incontinence. This report summarizes the case of a dog with sacrocaudal dysgenesis that was identified by imaging diagnosis.

저에너지 레이저 조사가 창상치유에 미치는 영향 (The effect of low energy laser irradiation on wound healing)

  • This study was performed to determine the optimal dose of laser energy for wound healing. The skin wound with 8mm diameter was induced over the lumbar vertebrae of the rats, and wound squares, scab hardness, hematologic findings and histopathlogic findings according to irradiation of laser energy were studied. 1. Wound square was significantly reduced at Day 1 (p<0.01), 2 (p<0.01), 3 (p<0.05), 7 (p<0.01) and 8 (p<0.05), respectively in experimental groups, especially group II, compared with control group. 2. Scab hardness was significantly increased at Day 1 (p<0.01), 2 (p<0.01), 3 (p<0.05), 5 (p<0.01) and 7 (p<0.01), respectively in experimental groups, especially group II, compared with control group. 3. In hematological findings, red blood cells and white blood cells in experimental group were increased according to the lapse of days, but they were not significant. 4. In histopathologic findings, experimental groups, especially group II, revealed early scab formation, early appearance of phagocytes and fibroblast, rapid growth of granulation tissue and collagen, and promotion of wound healing in the result.
