Analysis of uroflow sound for BPH Diagnosis using log-log plot methode (전립선비대증 진단을 위한 요류음 신호의 log-log plot 분석)

  • The aim of this study is to acquire useful information of lower urinary tract symptom(LUTS) diagnosis through urophonogaphy signal as noninvasive method. The system was implemented to measure urophonography signal, urinary volume and uroflow. The slope of the log-log plot of power spectrum density changed rapidly from $2.01{\sim}2.07$ to over 3.32 when the occlusion rate was 2 mm to 3.8 mm. This information combined with the uroflow parameters could give a useful prediction of lower urinary tract symptoms.

Factors associated with Lower Urinary Tract Symptoms for Patients with Parkinson's Disease (파킨슨병 환자의 하부요로증상에 영향을 미치는 요인)

  • Purpose: The study was done to identify lower urinary tract symptoms (LUTS) and to evaluate the factors affecting LUTS for the people with Parkinson's disease. Methods: The research design was a cross-sectional study with interviews using a structured questionnaire. The participants were 72 patients with Parkinson's disease who were seen in the Neurology clinic of a university hospital from September to November 2005. Results: Mean score of LUTS for the participants was 10.11. In each symptom score of LUTS (range 0-5), weak stream was the highest 2.06, followed by nocturia 1.71, and urgency 1.61. The severity of LUTS was moderate to severe group for 51%. LUTS were significantly different by regular exercise. Positive correlations were observed between Hoehn and Yahr stage (stage of disease severity) and frequencyand between Hoehn and Yahr stage and urgency (r=.280, p=.018; r=.328, p=.005). LUTS were significantly predicted by regular exercise (p=.001) which explained 15.0% of the variance in LUTS. Conclusion: Regular exercise was found to be a very important factor associated with LUTS for patients with Parkinson's disease.

Quality of Life and its Related Factors in Patients with Benign Prostatic Hyperplasia in One General Hospital (일개 상급전문종합병원 전립샘비대증 환자들의 삶의 질 영향 요인)

  • Purpose: This study was to identify the quality of life and its related factors in patients with benign prostatic hyperplasia. Methods: A cross-sectional survey design was utilized. Data were collected using questionnaires from 128 patients with benign prostatic hyperplasia who visited an outpatient department at one general hospital in 2016. Data were analyzed using Mann-Whitney U test, Kruskal Wallis test, Pearson's correlation coefficient and hierarchical multiple regression analysis. Results: Mean age of the participant was $67.81{\pm}6.94$. Mean years since diagnosis was $6.41{\pm}5.20$. The mean score of quality of life was $0.82{\pm}0.09$, indicating that QOL was relatively low. Lower urinary tract symptoms (p=.029), anxiety, depression, Activities of daily living were significantly correlated to with quality of life (p<.001). Activities of daily living accounted for 54% (p<.001) of the variance in quality of life as a result of hierarchical multiple regression analysis. Conclusion: The results of the study showed that nursing intervention may improve the quality of life of patients with benign prostatic hyperplasia by increasing their Activities of daily living.

The Effects of Electrical Stimulation Therapy with Microchip for the Treatment of Bladder Irritability Symptoms in Stress Urinary Incontinent Women (방광 자극증상을 호소하는 복압성 요실금 환자에서 마이크로칩을 이용한 전기자극치료의 효과)

  • Purpose: This study was carried out to evaluate the efficacy of intravaginal pelvic floor electrical stimulation (FES) therapy on bladder irritability symptoms in stress urinary incontinent women. Materials and Methods: The evaluation before and after treatment included the Blaivas's female Bladder Questionnaire, the quality of life questionnaires and the overall satisfaction with present voiding state using visual analogue test (VAT). All patient were treated for 20 minutes, twice a week for 6 weeks, using a new intravaginal electrical stimulator with microchip (PIC16C74). Results: After the FES therapy, the overall success rate of the SUI was 50.4.% at 9 months. The bladder irritability symptoms such as frequency, nocturia, urgency, residual urine sensation and lower abdominal discomfort were improved. In particular, the symptoms of frequency, urge incontinence, dysuria were significantly improved after the FES therapy at 9 months. The VAT score of the overall satisfaction with the present voiding state was significantly lower after the FES therapy. 73.7% of patients were satisfied with the FES therapy and complications such as hemorrhage, vaginitis, urinary tract infection and pain were not encountered. Conclusion: These results suggest that FES therapy with microchip improves some bladder irritability in SUI women. Therefore, electrical stimulation therapy should be considered as a first line therapy in SUI women with bladder irritability symptoms.

  • The purpose of urodynamic investigation is to determine information on the function of the urinary system. One of the most frequently used measurement procedures in urodynamics is filling and voiding cystometry using invasive method. But in this method transurethral catheter is use and it makes patients uncomfortable. The aim of this study was to implement the system that could evaluate the function of urinary tract with noninvasive and comfortable method. Therefor in this study, a sensor and measuring system were implemented to measure uroflow, urophonography and noninvasive bladder pressure signal during urination for diagnosing the LUTS(lower urinary tract symptoms) using noninvasive method. The implemented system compose of the sensor parts, signal conditioning parts, system control parts using FPGA and PC monitoring program. For the evaluation of the implemented system, the simulation of system's control part was performed and the model system for the lower urinary system was designed. From the evaluation of the model system, the mean error rate of the uroflow measurement part was 1.08% and coefficient of variation was 1,48. And the mean error rate of the noninvasive bladder pressure measurement part was 2.41% and coefficient of variation was 2.81. urophongraphy signal analysis was accomplished in a time domain and frequency domain. Average RMS power was used in a time domain analysis, and MF was used in a frequency domain analysis. From the evaluation of the model system average RMS power and MF was dependent on the occlusion degree significantly and median frequency range of $60{\sim}160Hz$ was correlated with the occlusion.

Transplantation of Brain-Derived Neurotrophic Factor-Expressing Mesenchymal Stem Cells Improves Lower Urinary Tract Symptoms in a Rat Model (뇌유래신경영양인자 발현 중간엽 줄기세포의 하부요로증상 개선 효과)

  • This study aimed to explore the effects of brain-derived neurotrophic factor (BDNF), produced by engineered immortalized mesenchymal stem cells (imMSC), on lower urinary tract symptoms (LUTS) in a rat model with neurogenic bladder (NB). Forty-eight Sprague-Dawley (SD) rats were randomly divided into the following groups: Sham control, LUTS, LUTS+imMSC (treated with immortalized MSC), and LUTS+BDNF-eMSC (treated with BDNF-expressing MSC) groups. LUTS was induced by a crush injury to the major pelvic ganglion (MPG). Bladder function was tested under anesthesia, and bladder tissue strips were collected thereafter for contractility test and western blot analysis. Western blot results showed that the expression of both Angiopoietin 1 (Ang 1) and platelet-derived growth factor (PDGF) increased with MSC injection. The effect of treatment with BDNF-eMSC on LUTS was also evaluated, and the results were found to be better than those with imMSC (P<0.05). BDNF-eMSC prevented fibrosis in the bladder tissue and significantly reduced caspase-3 levels. In conclusion, high expression of BDNF in vivo resulted in recovery of bladder function and contractility, along with the inhibition of apoptosis in a rat model.

  • Objective : The aim of this study was to rivew systemically literature and clinical trials in the treatment of urinary incontinence or lower urinary tract syndrome(LUTS). Methods : Computerized literature searches were carried out on two electronic database, and computerized searching on some korea oriental medicine journals in library of Kyung-Hee Medical center. Results : 1. Three reports of review study, six reports of experimental study and fourteen reports of clinical trials were collected and reviewed. Three reports of review study were all printed in the korea oriental medicine journal. From 2000, researches and studies have been increased in quantity and improved in quality. 2. Urinary disturbance include variable symptoms of lower urinary tract symptoms, urinary incontinence, in theaspect of Oriental medicine these symptoms are anurin, dysuria, urinary incontinence, nochumal enuresis, uracratia and so on. 3. Roughly physiological procedure of Acupuncture in Treatment of Urianry Disturbance may be that effect of acupuncture stimulation for parasympathetic nerve, sleep-arousal system in cerebrum, pontine/spinal urination center and pudendal/pelvic nerve affect bladder in expansion of bladder capacity, inhibition of urinary contraction and affection in periurethral muscle by continuous excitement of spinal annular circuit and synapse of neuron. 4. Clinical result for acupuncture treatment in urinary disturbance is summarized that acupuncture treatment in urianation disturbance of Neurogenic Bladder, Incontinence, Cycitis, Nocturnal Enuresis, Prostatitis/Pelvic Pain Syndrom and so on is significant clinical trials and technique. Conclusion : Hereafter, in the old age society these variable urinary disturbance patients are increased and desire of treatment may be also increased. So study of various and formal treatment and tecnnique is needed.

The Relationship of Quality of Sleep, Depression, Late-life Function and Disability (LLFDI) in Community-Dwelling Older Women with Urinary Incontinence (노년기 요실금 여성의 수면의 질, 일상생활 수행능력과 우울)

  • Purpose: The purpose of this study was to investigate the relationships among quality of sleep, depression, late-life function and disability in community-dwelling older women with urinary incontinence. Methods: A stratified random sampling method was conducted to recruit participants from May 10 to August 17, 2007. Data were collected by questionnaires, which were constructed to include lower urinary tract symptoms, quality of sleep, depression, and late-life function and disability in 128 community-dwelling older women. Results: The major findings of this study were as follow: 1) 56.3% of participants belonged to urinary incontinence group. 2) There were significant relationships between depression and sleep latency, sleep duration, daytime dysfunction, quality of sleep, function component, frequency dimension, and limitation dimension. 3) Depression was significantly associated with frequency dimension, limitation dimension in capability which explained 44% of variance in depression. Conclusion: These results may contribute to a better understanding of sleep quality, depression, late-life function and disability in the community-dwelling older women with urinary incontinence. Therefore, health programs for prompting older women's health should be planned based on results of the study.

Study on Bladder Dysfunction in Elderly Women by the BFLUT Questionnaire and Bladder Scanner: Frequency and Residual Urine (질문지법과 초음파 방광용적진단기를 이용한 여성노인의 배뇨장애연구 -빈뇨, 잔뇨를 중심으로)

  • Purpose: This study aimed to describe bladder dysfunction in elderly women such as frequency, nocturia, and residual urine. Methods: One hundred elderly women aged 60 and over. The Bristol Female Lower Urinary Symptoms (BFLUT) was used to evaluate the bladder function and to measure the residual urine amount by using a bladder scanner. Data was analyzed with the differences between voiding dysfunction by age group and life habits by t-test, ANOVA and correlation by Pearson correlation coefficient. Results: the mean daytime frequency was 6.8 times and night-time frequency 2.7 times. Sixty three percent of subjects had urgency and 41% had urgent incontinence. Over half of subjects had problem in voiding function. There were significant differences in frequency by age groups and constipation, but not in daytime frequency and residual urine. Lastly, there were significant positive relations between daytime frequency and night-time frequency. Also results indicate that more frequency in daytime equaled to a less residual urine amount. Conclusion: We know many elderly women have lower urinary tract symptoms. Specially women over 75 years have more daytime frequency and night-time frequency. This suggests further research needed in order to understand the relation of voiding patterns and life habits and its influence on quality of life.

Animal Models for the Study of Post-Micturition Dribble in Aged Male (고령 남성의 배뇨 후 요실금 연구를 위한 동물 모델)

  • Since attaining middle age, many patients suffer from the post-micturition dribble (PMD) syndrome but do not receive proper treatment. This is because the etiology and treatment mechanisms of PMD differ from conventional lower urinary tract symptoms. Several pathophysiologies have been proposed, but the mechanisms we observed are the weakening of the bulbocavernosus muscle and dilation of the urethra due to atrophy of the cavernosal tissue. This study investigates the mechanism of PMD by observing anatomical changes in the corpus cavernosum and urethra. SD male rats were categorized by age into the young rats group (8 weeks old) and the old rats group (24 months or more). The ratio area of the bulbous urethra and corpus cavernosum to the total penis was statistically significant between both groups (P<0.05). Significant differences were obtained between the two groups for restored smooth muscle contents and several other parameters related to PMD mechanisms (nNOS, α-SMA) (P<0.05). The changes in size and parameters of the urethra and corpus cavernosum in young and old rats were confirmed. The results from this study are expected to contribute to the study of PMD in the future.
